Hypovitaminosis D Influences the Clinical Presentation of Immune Thrombocytopenia in Children with Newly Diagnosed Disease

Immune thrombocytopenia (ITP) is an acquired autoimmune disorder characterized by isolated thrombocytopenia defined as platelet count in peripheral blood <100 × 109/L. Hypovitaminosis D is very common in children with autoimmune diseases. To analyze whether hypovitaminosis D is associated with the clinical presentation of ITP in children, medical records of 45 pediatric patients with newly diagnosed immune thrombocytopenia in the coastal region of Croatia were evaluated. The severity of bleeding was assessed using two bleeding scores. Children with lower 25-hydroxyvitamin D (25(OH)D) values had higher values of the skin-mucosa-organ-gradation (SMOG) bleeding score and respectively more severe bleeding on diagnosis of ITP. With further analysis of the main domains of that score, we found that patients with a lower 25(OH)D value had more severe bleeding in the skin and organs. When 25(OH)D and ITP Bleeding Scale (IBLS) score were analyzed, a negative correlation was found, but it was not significant. Our findings suggest that hypovitaminosis D influences the severity of the clinical presentation of ITP in children on initial diagnosis of the disease. Therefore, therapy with 25(OH)D could be a new potential option for treatment of ITP. To investigate the connection between 25(OH)D and the incidence and severity of ITP, further studies, especially randomized controlled studies, are needed

Issuance of e-passes as a new form of providing e-services

Predmet ovog rada je analiza integriranog sustava e-Propusnica u sustav e-Građani. Cilj rada je analizirati i istaknuti sve dostupnosti koje nudi nova e-usluga, odnosno integrirani sustav e-Propusnica građanima Republike Hrvatske kao odgovor na novonastalu svjetsku zdravstvenu krizu koju je prouzrokovao koronavirus „COVID-19“. Sustav e-Propusnica pokrenut je 02.04.2020. godine s ciljem stvaranja jedinstvenog mjesta pristupa izdanim propusnicama čime bi se prvenstveno smanjile mogućnosti zlouporabe i olakšao sustav kontrole izdanih propusnica, ali i kako bi se ubrzao proces povezivanja Ministarstva unutarnjih poslova, Porezne uprave, Hrvatskog zavoda za mirovinsko osiguranje, zdravstvenog sustava te Stožera civilne zaštite u obavljanju djelatnosti za vrijeme pandemije korona-virusa COVID-19. Sustav e-Propusnica osmišljen je za lakši nadzor ulaska i izlaska građana s područja prebivališta i stalnog boravka za vrijeme trajanja ograničenog kretanja građana u Republici Hrvatskoj. Valja napomenuti da su se putem sustava e-Propusnica otvorile mogućnost traženja i izdavanja digitalne e-propusnice. Navedeni sustav svojom povezanošću omogućio je provjeru valjanosti podataka, dok nadležno tijelo ili odgovorna osoba na temelju tih podataka ima mogućnost odobravanja ili odbijanja zahtjeva za izdavanjem e-Propusnice. Novi sustav, koji je integriran u sustav e-Građani, pokazao se kao odličan odgovor na sprječavanje širenja pandemije unutar granica Republike Hrvatske.The subject of this paperwork is analysis of integrated system E-passes in the E-Citizens system. The aim of this paperwork is to analyze and accentuate all availabilities which the new E-service is offering, apropos the integrated system of E-passes to the citizens of the Republic of Croatia in response to the new world crises affected by Corona virus “COVID-19”. The system of E-passes has been initiating on the 2nd April 2020 with aim of creating unique spot where access to the issued passes would be primarily less affected to the misuse and would be alleviate of the system of controlling issued passes, but also accelerate connection between Ministry of the Interior, Tax administration, Croatian Pension Agency, Health System and Civil Protection Directorate in their work during pandemic caused corona virus COVID-19. The system of E-passes was designed to make easier supervision in the entering and exiting citizens in the area of the permanent residence and temporary residence during the restricted movement in the Republic of Croatia. It also should be pointed that through the system of E-passes there is possibility to request and to be issued digital E-passes. The specified system has enabled to check validity of data, while competent authority or responsible person on the fundament of that data has opportunity to authorize or to denial the claim of issuing e-passes. New system, which has been integrated in the system of E-Citizens, has been shown like a great response for the prevention of spreading pandemic within borders in Republic of Croatia

A Croatian case of the Schinzel-Giedion syndrome

The Schinzel-Giedion syndrome is an infrequently described malformation syndrome, mainly characterized by a profound mental deficiency, a typical face including a midface hypoplasia, urogenital abnormalities, and minor radiographic features. Commode Cardia-Death by Valsalva Maneuver: A Case Series

The Valsalva maneuver is used in clinical medicine for the diagnosis and/or treatment of various cardiovascular conditions. It can also be used in activities of daily living, such as defecation. Due to the cardiovascular effects produced during the Valsalva maneuver, it may be contraindicated in certain medical conditions and could be a trigger of sudden cardiac death. The incidence and prevalence of death following Valsalva maneuver in the presence of underlying cardiovascular disease, or â commode cardia,â has not been examined. In 2012, the Wayne County Medical Examiner’s Office (Detroit, MI) investigated 21 deaths that occurred on the toilet, fourteen of which were due to cardiovascular disease. In another 31 deaths in the bathroom due to cardiovascular disease, the possibility that the decedent defecated immediately prior to death could not be excluded.
