    Race and Income Disparities in Disaster Preparedness in Old Age

    Objective: Older adults are one of the most vulnerable populations impacted by disasters and communities continue to struggle addressing preparedness. This study investigated to what extent income status and race/ethnicity in old age interplayed with disaster preparedness. Methods: Data came from the 2010 Health and Retirement Study, a nationally representative panel survey of older Americans over 51 years old. Our sample was restricted to respondents who participated in a special survey about disaster preparedness (N=1,705). Disaster preparedness was measured as a score, which includes 13 variables related to personal, household, program, and medical preparedness. Race/ethnicity was categorized by White, Black, and Hispanic. Low income was defined as below 300% of the federal poverty line. OLS regression was used to examine the main and interaction effects of race/ethnicity and lower income status on disaster preparedness scores. Results: We found that older adults in lower income status had lower preparedness level than those in higher income (Coef.=-0.318, p\u3c.01). Hispanics tend to be less prepared compared to White and African Americans (Coef.=--0.548, p\u3c.001). Preparedness of Black elders was not significantly different from that of Whites. However, interestingly, Black elders in lower income status were significantly less prepared for disaster than other groups (Coef.=-0.520, p\u3c.05). We did not find significant interaction effects between Hispanic and lower income status on disaster preparedness. Discussion. This study identified vulnerable subgroups of older adults for disaster preparedness and suggests that preparedness programs should target minority and low income elders, particularly Hispanics and low income Black elders

    When fully implemented, the Patient Protection and Affordable Care Act, amended by the Health and Education Reconciliation Act will extend health insurance coverage to 94 percent of Americans while establishing a comprehensive set of strategies to improve care and contain costs. The central provisions of the Act – guaranteed affordable and accessible coverage – take effect January 1, 2014. Important insurance reforms aimed at improving coverage become effective before that date, as do a series of investments aimed at improving the accessibility and quality of health care. This report has several aims: 1) to examine how the laws address vaccine policy and practice; 2) to assess how access to vaccines and immunization services will be affected; and 3) to assess the extent to which health reform addresses recommendations of the National Vaccine Advisory Committee\u27s (NVAC) Vaccine Finance Working Group (VFWG) 2008

    Text messages to reduce depressive symptoms: Do they work and what makes them effective? A systematic review

    Objective: In this systematic review and meta-analysis, we aimed to quantify the effects of text messaging interventions to reduce depressive symptoms and identify variables that might influence the effectiveness of the intervention. Design: Electronic databases including EMBASE, CENTRAL, MEDLINE, CINAHL, PsycINFO and SCOPUS, as well as Clinicaltrials.gov and the World Health Organization (WHO) International Clinical Trials Registry Platform (ICTRP) were searched for randomised controlled trials that sent one or more text messages with health-related content to adults who had been identified by a healthcare provider. Results: Seven trials (nine comparisons), with 1,918 participants, were included in the review, and the pooled analysis revealed a borderline statistically significant reduction in depressive symptom scores between the text messaging intervention and control groups (standardised mean difference [SMD], −0.27; 95% confidence interval [CI], −0.54 to 0.00; p = .00) favouring intervention at the end. Statistically significant reductions were shown in important subgroups, for example, where the primary aim of the messages was to reduce depressive symptoms; in those using the Beck Depression Inventory (BDI) or 9-item Patient Health Questionnaire (PHQ-9) questionnaires; where text message content was targeted at mental well-being, mood improvement and cognitive behavioural therapy information; and when the message frequency was ⩾2 times per week. Conclusion: Text messaging has potential to reduce depressive symptoms. The results of this review should be interpreted with caution, however, due to the methodological limitations of included trials. More research is required before recommendations can be made about the routine use of text messaging for the management of depressive symptoms

Many maleidrides possess significant bioactivities, making them attractive pharmaceutical or agrochemical lead compounds. Their unusual biosynthetic pathways have fascinated scientists for decades, with recent advances in our bioinformatic and enzymatic understanding providing further insights into their construction. However, many intriguing questions remain, including exactly how the enzymatic dimerisation, which creates the diverse core structure of the maleidrides, is controlled. This review will explore the literature from the initial isolation of maleidride compounds in the 1930s, through the first full structural elucidation in the 1960s, to the most recent in vivo, in vitro, and in silico analyses

    Heterologous Production of Fungal Maleidrides Reveals the Cryptic Cyclization Involved in their Biosynthesis

    Fungal maleidrides are an important family of bioactive secondary metabolites that consist of 7, 8, or 9-membered carbocycles with one or two fused maleic anhydride moieties. The biosynthesis of byssochlamic acid (a nonadride) and agnestadride A (a heptadride) was investigated through gene disruption and heterologous expression experiments. The results reveal that the precursors for cyclization are formed by an iterative highly reducing fungal polyketide synthase supported by a hydrolase, together with two citrate-processing enzymes. The enigmatic ring formation is catalyzed by two proteins with homology to ketosteroid isomerases, and assisted by two proteins with homology to phosphatidylethanolamine-binding proteins. Ring cycle: The enzymes involved in the cyclization of the maleidride family of bioactive fungal natural products, including agnestadride A and byssochlamic acid, were identified. These previously unknown proteins show homology to ketosteroid isomerases (KI-like) and phosphatidylethanolamine-binding proteins (PEBP-like).BBSRCSyngent

    The Spatial Extent and Distribution of Star Formation in 3D-HST Mergers at z~1.5

    We present an analysis of the spatial distribution of star formation in a sample of 60 visually identified galaxy merger candidates at z>1. Our sample, drawn from the 3D-HST survey, is flux-limited and was selected to have high star formation rates based on fits of their broad-band, low spatial resolution spectral energy distributions. It includes plausible pre-merger (close pairs) and post-merger (single objects with tidal features) systems, with total stellar masses and star formation rates derived from multi-wavelength photometry. Here we use near-infrared slitless spectra from 3D-HST which produce Halpha or [OIII] emission line maps as proxies for star-formation maps. This provides a first comprehensive high-resolution, empirical picture of where star formation occurred in galaxy mergers at the epoch of peak cosmic star formation rate. We find that detectable star formation can occur in one or both galaxy centres, or in tidal tails. The most common case (58%) is that star formation is largely concentrated in a single, compact region, coincident with the centre of (one of) the merger components. No correlations between star formation morphology and redshift, total stellar mass, or star formation rate are found. A restricted set of hydrodynamical merger simulations between similarly massive and gas-rich objects implies that star formation should be detectable in both merger components, when the gas fractions of the individual components are the same. This suggests that z~1.5 mergers typically occur between galaxies whose gas fractions, masses, and/or star formation rates are distinctly different from one another.Comment: Accepted for publication in MNRAS, 16 pages, 10 figure

    Murine ethanol ingestion promotes hepatic platelet accumulation mimicking acute Cclâ‚„ exposure

    Background: Dysregulated coagulation contributes to inflammation and fibrosis from chemical injury. Platelets are key contributors to inflammation and are primary sources of TGF-b, PDGF, and EGF that promote fibrosis, and so may contribute to hepatic fibrosis. We questioned whether platelets accumulate during chronic ethanol- or chemical-induced hepatic injury, whether platelet accumulation would occur prior to induction of hepatic fibrotic responses, and whether platelet accumulation reflects deposition of intravascular microthrombi or individual platelets intercalated into liver parenchyma.Methods: We modeled acute hepatic injury with a single injection of CCl₄, a chronic model of moderate ethanol ingestion, or a combination of the two insults. C57Bl6 mice ingested a control liquid diet or provided free access to 1% ethanol (2d), then 2% ethanol (2d, 11% calories). At day 4, mice received, or not, a single i.p. injection of CCl₄ (1 μl/g, 1:3 in olive oil), with sacrifice 72h later. Formalin-fixed livers were transversely sectioned, paraffin-embedded prior to immunohistochemistry with anti platelet integrin gpIIb (CD41), endothelial CD31, or a-smooth muscle actin (aSMA) antibodies with DAPI nuclear staining. Adherent platelets spread to micron thickness, so detection was by serial tyramide amplification (Biotium). This catalyzed reporter deposition system uses a single tyramide dye activated by HRP-derived H₂O₂ to a reactive specie that multiply ligates adjacent molecules before the antibody complex is thermally stripped prior to a subsequent tyramide labeling.Results: Our preliminary data show basal parenchymal platelet deposition with inflammation 24h after CCl₄ injection massively increasing platelet accumulation, with enhanced expression of aSMA, just below the outer Glisson’s sheath encasement, correlating to the area of highest arterial flow (DOI 10.1139/y93- 018). Platelet accumulation, but not aSMA, within liver parenchyma was modestly increased at this time. 72h after CCl₄ injection, subsurface platelet accumulation in association with endothelial cell PECAM1 remained apparent, with aSMA now extended in disordered filaments surrounding portal tracts. Ethanol ingestion alone, similar to CCl₄ exposure, revealed massive platelet accumulation just below the Glisson’s sheath liver encasement in association with endothelial cell PECAM1 without aSMA deposition. The combination of ethanol and CCl₄ presented similarly.Conclusions: We conclude ethanol ingestion promotes hepatic platelet accumulation, providing a nontranscriptional source of fibrotic growth factors, that parallels hepatic injury invoked by CCl₄ exposure

    An interdisciplinary approach to enhance children’s listening, learning, and wellbeing in the classroom : The Listen to Learn for Life (L3) Assessment Framework

    Introduction: Listening is the gateway to children learning in the mainstream classroom. However, modern classrooms are noisy and dynamic environments making listening challenging. It is therefore critical for researchers from speech and hearing, education, and health sciences to co-design and collaborate to realistically assess how children listen to learn in the classroom and to understand how listening can be improved to enhance children’s learning and wellbeing – an understanding which is currently lacking. Such highly interdisciplinary thinking demands a holistic classroom listening framework that can integrate a range of varied assessments and outcomes. Methods: An extensive review of literature into classroom listening was conducted but failed to identify a suitable framework. In this hypothesis and theory article we present a new framework that we have developed – the Listen to Learn for Life (L3) Assessment Framework. Results: The L3 Assessment Framework holistically incorporates frameworks from health, speech and hearing sciences, and education sectors. The framework accommodates a broad range of different factors that may affect listening, allowing for researchers to choose specific factors dependent on the context of use. Discussion: Selected examples of applying the framework are provided demonstrating how to assess children’s performance during different classroom activities as well as the effectiveness of a chosen intervention. For example, the framework can be used to assess the effectiveness of a wireless remote microphone intervention during group work activities for a child with autism. Conclusion: The L3 Assessment Framework provides a theoretical basis for the future development of research and practice as applied to listening in a classroom setting
