    VirtualECare: group support in collaborative networks organizations for digital homecare

    Collaborative Work plays an important role in today’s organizations and normally in areas where decisions must be made. However, any decision that involves a collective or group of decision makers is, by itself, complex but is becoming normal in recent years. In this work we present the VirtualECare project (Figure 10), intelligent multi‐agent system able to monitor, interact and serve its customers, which are, normally, in need of care services. In the last years there has been a substantially increase in the number of people needed of intensive care, especially among the elderly, a phenomenon that is related to population ageing. However, this is not exclusive of the elderly, as diseases as obesity, diabetes, and blood pressure have been increasing among young adults. As a new reality, it has to be dealt by the health sector, and particularly by the public one. Thus, the importance of finding new and cost effective ways for health care delivery are of particular importance, especially when one want them not to be removed from their “habitat”. Following this line of thinking, the VirtualECare project will be presented, like similar ones that preceded it. Recently we have assisted to a growing interest in combining the advances in information society ‐ computing, telecommunications and presentation – in order to create Group Decision Support Systems (GDSS). Indeed, the new economy, along with increased competition in today’s complex business environments, takes the companies to seek complementarities in order to increase competitiveness and reduce risks. Under these scenarios, planning takes a major role in a company life. However, effective planning depends on the generation and analysis of ideas (innovative or not) and, as a result, the idea generation and management processes are crucial. Our objective is to apply the above presented GDSS to a new area. We believe that the use of GDSS in the healthcare arena will allow professionals to achieve better results in the analysis of one’s Electronically Clinical Profile (ECP). This achievement is vital, regarding the explosion of knowledge and skills, together with the need to use limited resources and get better results

    Multi-objective parameter CPG optimization for gait generation of a quadruped robot considering behavioral diversity

    This paper presents a gait multi-objective optimization system that combines bio-inspired Central Patterns Generators (CPGs) and a multi-objective evolutionary algorithm. CPGs are modeled as autonomous differential equations, that generate the necessary limb movement to perform the required walking gait. In order to optimize the walking gait, four conflicting objectives are considered, simultaneously: minimize the body vibration, maximize the velocity, maximize the wide stability margin and maximize the behavioral diversity. The results of NSGA-II for this multi-objective problem are discussed. The effect of the inclusion of a behavioral diversity objective in the system is also studied in terms of the walking gait achieved. The experimental results show the effectiveness of this multi-objective approach.     Este estudo apresenta a avaliação de indicadores de durabilidade de uma mistura de UHPC cuja composição foi previamente otimizada em termos de ecoeficiência e custo bem como, especialmente concebida para aplicações de reabilitação e/ou reforço com betonagem ?in-situ?. Recorrendo a técnicas de ensaio comummente implementadas, nomeadamente porosidade, resistividade elétrica, absorção de água por capilaridade, penetração de cloretos e reações expansivas, caracteriza-se a durabilidade deste novo material.     Elevação das enzimas hepáticas, persistente e assintomática, como forma de apresentação da doença de Wilson em idade pediátrica

    Introdução: A doença de Wilson é uma doença rara, autossómica recessiva, caracterizada por uma alteração no transporte do cobre no fígado, com acumulação progressiva em vários órgãos (fígado, cérebro, rins e córneas). A expressão fenotípica é muito variável desde a elevação das enzimas hepáticas, esteatose ou litíase vesicular, em doentes assintomáticos, até a cirrose ou insuficiência hepática fulminante, ou doença neuropsiquiátrica incapacitante. Objectivo: Caracterizar uma amostra de doentes com doença de Wilson. Métodos: Estudo retrospectivo, incluindo as crianças diagnosticadas entre 2002 e 2011, segundo os critérios da European Association for the Study of the Liver (2012). Variáveis analisadas: antecedentes familiares; idade, dados clínicos, bioquímicos, imagiológicos e histológicos a data do diagnóstico; estudo genético; terapêutica e efeitos colaterais; seguimento e estado actual. Resultados: Foram identificados cinco doentes, três com antecedentes familiares da doença. Todos se apresentavam assintomáticos e com elevação persistente das enzimas hepáticas. Em nenhum havia estigmas de doença hepática crónica. Uma doente tinha excesso de peso. Todos foram tratados com D-penicilamina, interrompida em dois por efeitos colaterais. Actualmente mantem-se assintomáticos, sem evidência de progressão da doença hepática, com um seguimento mediano de 5 anos e 3 meses. Conclusões: A nossa série mostra que a doença de Wilson se pode apresentar com elevação das enzimas hepáticas em crianças assintomáticas. A doente com excesso de peso alerta-nos para a necessidade de rastrear a doença nos que tem excesso de peso/obesidade quando a elevação das enzimas hepáticas e/ou esteatose persistem por mais de seis meses após a perda de peso

    The cholesterol 24-hydroxylase activates autophagy and decreases mutant huntingtin build-up in a neuroblastoma culture model of Huntington’s disease

    Objective Compromised brain cholesterol turnover and altered regulation of brain cholesterol metabolism have been allied with some neurodegenerative diseases, including Huntington’s disease (HD). Following our previous studies in HD, in this study we aim to investigate in vitro in a neuroblastoma cellular model of HD, the effect of CYP46A1 overexpression, an essential enzyme in cholesterol metabolism, on huntingtin aggregation and levels. Results We found that CYP46A1 reduces the quantity and size of mutant huntingtin aggregates in cells, as well as the levels of mutant huntingtin protein. Additionally, our results suggest that the observed beneficial effects of CYP46A1 in HD cells are linked to the activation of autophagy.     Resultados da utilização rotineira de catéteres 4F no cateterismo diagnóstico num laboratório de hemodinâmica

    INTRODUCTION: Complications at the site of vascular access are the most common adverse events in cardiac catheterization. The use of small gauge catheters may reduce this risk and allow earlier ambulation, the main disadvantage according to some authors being inferior image quality. The aim of our study was to evaluate the safety and image quality of 4 French diagnostic catheters. METHODS: We performed a retrospective study of 1656 patients who underwent diagnostic cardiac catheterization with 4F catheters via the femoral artery between January 2006 and December 2007, and analyzed the complications during and immediately after the procedure. The quality of the films was assessed in 125 consecutive patients from this group, who were also followed up on average one month after hospital discharge. RESULTS: Cardiac catheterization with 4F catheters was technically possible in all cases. Patients were able to ambulate and were discharged from hospital on average four and six hours respectively after the procedure. Complications during or immediately after the procedure occurred in 5.8% of cases. In the subgroup with clinical follow-up, there was minor bleeding at the access site in 16.4% and hematoma in 14.4%; in the latter group, only one patient had major hematoma requiring therapeutic intervention. No other major complications were recorded and the patients resumed their daily activities on average 7 days after discharge. Image analysis revealed that most of the films were of good quality. Conclusion: The use of 4F catheters for diagnostic cardiac catheterization via the femoral approach enables rapid hemostasis and early ambulation, with a low incidence of complications at the access site. This type of catheter provides good quality images and there were no problems in their handling. 4F catheters are therefore a good option to consider for cardiac catheterization, especially when no therapeutic procedures are expected

    The challenge of long-distance over-the-air wireless links in the ocean: a survey on water-to-water and water-to-land miot communication

    Robust wireless communication networks are a cornerstone of the modern world, allowing data to be transferred quickly and reliably. Establishing such a network at sea, a Maritime Internet of Things (MIoT), would enhance services related to safety and security at sea, environmental protection, and research. However, given the remote and harsh nature of the sea, installing robust wireless communication networks with adequate data rates and low cost is a difficult endeavor. This paper reviews recent MIoT systems developed and deployed by researchers and engineers over the past few years. It contains an analysis of short-range and long-range over-the-air radio-frequency wireless communication protocols and the synergy between these two in the pursuit of an MIoT. The goal of this paper is to serve as a go-to guide for engineers and researchers that need to implement a wireless sensor network at sea.     Aqueous humor erythropoietin levels in open-angle glaucoma patients with and without TTR V30M familial amyloid polyneuropathy

    Purpose: Glaucoma is the leading cause of irreversible blindness in familial amyloidotic polyneuropathy (FAP) patients. Erythropoietin (EPO) is a cytokine that has been shown to play a role in neuroprotection and is endogenously produced in the eye. EPO levels in the aqueous humor are increased in eyes with glaucoma. In this study, we evaluated the EPO concentration in the aqueous humor of FAP and non-FAP patients, with and without glaucoma. Methods: Undiluted aqueous humor samples were obtained from 42 eyes that underwent glaucoma surgery, phacoemulsification, or vitrectomy. EPO concentration in the aqueous humor and blood were measured using the Immulite 2000 Xpi using an automatic analyzer (Siemens Healthcare Diagnostics). Results: The mean EPO concentration in the aqueous humor of non-FAP glaucoma eyes group 2 (75.73±13.25 mU/ml) was significantly higher than non-FAP cataract eyes (17.22±5.33 mU/ml; p<0.001), FAP glaucoma eyes (18.82±10.16 mU/ml; p<0.001), and FAP nonglaucoma eyes (20.62±6.22 mU/ml; p<0.001). There was no statistically significant difference between FAP nonglaucoma eyes versus non-FAP cataract eyes (p = 0.23) and FAP glaucoma eyes versus FAP nonglaucoma eyes (p = 0.29). In the glaucoma groups, there was no correlation between the aqueous humor EPO concentration and the ocular pressure (p = 0.95) and mean deviation (p = 0.41). There was no correlation between the EPO serum concentration and EPO aqueous humor concentration in our patients (p = 0.77). Conclusions: Unlike other glaucomatous patients, FAP patients with glaucoma do not show increased and potentially neuroprotective endocular EPO production in the aqueous humor and may need more aggressive glaucoma management
