    Dysphagia rehabilitation following acquired brain injury, including cerebral palsy, across the lifespan: a scoping review protocol

    BACKGROUND: Swallowing impairment (dysphagia) following brain injury can lead to life-threatening complications such as dehydration, aspiration pneumonia and acute choking episodes. In adult therapeutic practice, there is research and clinical evidence to support the use of swallowing exercises to improve swallowing physiology in dysphagia; however, the use of these exercises in treating children with dysphagia is largely unexplored. Fundamental questions remain regarding the feasibility and effectiveness of using swallowing exercises with children. This review aims to outline the published literature on exercise-based treatment methods used in the rehabilitation of dysphagia secondary to an acquired brain injury across the lifespan. This will allow the range and effects of interventions utilised to be mapped alongside differential practices between adult and child populations to be formally documented, providing the potential for discussions with clinicians about which rehabilitative interventions might be appropriate for further trial in paediatrics. METHODS: This study will use a scoping review framework to identify and systematically review the existing literature using Joanna Briggs Institute (JBI) and Preferred Reporting Items for Systematic Reviews (PRISMA) scoping review guidelines. Electronic databases (MEDLINE, PubMed, Cumulative Index to Nursing and Allied Health Literature (CINAHL) and Allied and Complementary Medicine Database (AMED)), grey literature and the reference lists of key texts including systematic reviews will be searched. Information about the rehabilitation design, dosage and intensity of exercise programmes used as well as demographic information such as the age of participants and aetiology of dysphagia will be extracted. The number of articles in each area and the type of data source will be presented in a written and visual format. Comparison between the literature in adult and child populations will be discussed. DISCUSSION: This review is unique as it directly compares dysphagia rehabilitation in adults with that of a paediatric population in order to formally identify and discuss the therapeutic gaps in child dysphagia rehabilitation. The results will inform the next stage of research, looking into the current UK-based speech and language therapy practices when working with children with acquired dysphagia.     Multiwavelength radio observations of the compact starburst in Arp 220

    We report the first detection at multiple radio wavelengths (13, 6, and 3.6 cm) of 18 compact sources within both nuclei of the Ultra Luminous Infra-Red Galaxy (ULIRG) Arp 220. In just over half of the sources we find that the observed spectra are consistent with the standard model of powerful Type IIn supernovae interacting with their pre-explosion stellar wind. The rate of appearance of new radio sources ascribed to these supernova events suggests that a large fraction of core-collapse supernovae in Arp 220 are highly luminous, possibly implying a radically different stellar initial mass function (IMF) or stellar evolution compared to galactic disks. A second group of sources, consisting of the brightest and longest monitored sources at 18 cm, do not easily fit the radio supernova model. We propose that these are young supernova remnants that have just begun interacting with their surrounding dense ISM

    Selfish learning:the impact of self-referential encoding on children's literacy attainment

    Self-referencing (i.e., thinking about oneself during encoding) can increase attention toward to-be-encoded material, and support memory for information in adults and children. The current inquiry tested an educational application of this 'self reference effect' (SRE) on memory. A selfreferential modification of literacy tasks (vocabulary spelling) was tested in two experiments. In Experiment 1, seven- to nine-year-old children (N = 47) were asked to learn the spelling of four nonsense words by copying the vocabulary and generating sentences. Half of the children were asked to include themselves as a subject in each sentence. Results showed that children in this self-referent condition produced longer sentences and increased spelling accuracy by more than 20%, relative to those in an other-referent condition. Experiment 2 (N = 32) replicated this pattern in real-word learning. These findings demonstrate the significant potential advantages of utilizing self-referential encoding in the classroom

    Comparative population structure of <i>Plasmodium malariae</i> and <i>Plasmodium falciparum</i> under different transmission settings in Malawi

    &lt;b&gt;Background:&lt;/b&gt; Described here is the first population genetic study of Plasmodium malariae, the causative agent of quartan malaria. Although not as deadly as Plasmodium falciparum, P. malariae is more common than previously thought, and is frequently in sympatry and co-infection with P. falciparum, making its study increasingly important. This study compares the population parameters of the two species in two districts of Malawi with different malaria transmission patterns - one seasonal, one perennial - to explore the effects of transmission on population structures. &lt;BR/&gt; &lt;b&gt;Methods:&lt;/b&gt; Six species-specific microsatellite markers were used to analyse 257 P. malariae samples and 257 P. falciparum samples matched for age, gender and village of residence. Allele sizes were scored to within 2 bp for each locus and haplotypes were constructed from dominant alleles in multiple infections. Analysis of multiplicity of infection (MOI), population differentiation, clustering of haplotypes and linkage disequilibrium was performed for both species. Regression analyses were used to determine association of MOI measurements with clinical malaria parameters. &lt;BR/&gt; &lt;b&gt;Results:&lt;/b&gt; Multiple-genotype infections within each species were common in both districts, accounting for 86.0% of P. falciparum and 73.2% of P. malariae infections and did not differ significantly with transmission setting. Mean MOI of P. falciparum was increased under perennial transmission compared with seasonal (3.14 vs 2.59, p = 0.008) and was greater in children compared with adults. In contrast, P. malariae mean MOI was similar between transmission settings (2.12 vs 2.11) and there was no difference between children and adults. Population differentiation showed no significant differences between villages or districts for either species. There was no evidence of geographical clustering of haplotypes. Linkage disequilibrium amongst loci was found only for P. falciparum samples from the seasonal transmission setting. &lt;BR/&gt; &lt;b&gt;Conclusions:&lt;/b&gt; The extent of similarity between P. falciparum and P. malariae population structure described by the high level of multiple infection, the lack of significant population differentiation or haplotype clustering and lack of linkage disequilibrium is surprising given the differences in the biological features of these species that suggest a reduced potential for out-crossing and transmission in P. malariae. The absence of a rise in P. malariae MOI with increased transmission or a reduction in MOI with age could be explained by differences in the duration of infection or degree of immunity compared to P. falciparum

    VLBI Imaging of Luminous Infrared Galaxies: AGN Cores in Mrk231, UGC 5101 & NGC 7469

    We report 18cm VLBI continuum imaging observations at 5 mas resolution for UGC 5101, NGC 7469, and Mrk 231, all part of a sample of Luminous Infrared Galaxies which have been shown to have strong VLBI radio cores. The radio morphology of these three systems on VLBI scales is AGN-like, with well-defined ridgelines and high-brightness yet spatially resolved components. The structure and flux densities of these VLBI components are not consistent with starburst generated radio supernovae of the type found in Arp 220. On scales of 100pc the radio continuum in all three objects appears to be dominated by an AGN, not a starburst. Radio emission on larger scales may well originate in a less compact circumnuclear star-forming region. Confirming and extending VLBI imaging of Mrk 231 by Ulvestad et al. (1999), our continuum image shows a triple structure, with a core and two lobes, classifying it as a Compact Symmetric Object (CS0). If the southern (primary) lobe/hot-spot in Mrk 231 is confined by ram pressure, we estimate a lobe advance speed, va104cv_a \sim 10^{-4}c, and an age for the jet/compact source, <106yr< 10^6 yr. We have also imaged the 1667 MHz OH maser emission in Mrk 231, which is extended on scales of 50--100 milliarcsec (40--80 pc) and probably coincides with the inner region of the disk which is seen in CO emission and HI absorption. Among OH megamasers studied at high sensitivity with mas resolution, Mrk 231 is unique in the stringent upper limits placed upon the flux density of compact OH structures of the type found in Arp 220 and other LIGs. It is possible that the circumnuclear environment of Mrk 231 has been sufficiently disrupted by the emergent QSO that the cool, dense clouds necessary for such compact masers no longer exist.Comment: 43 pages, 7 figure

    The feasibility of an online platform delivery of pulmonary rehabilitation for individuals with chronic respiratory disease

    Copyright © Author(s) (or their employer(s)) 2021. Introduction SARS-CoV-2 has restricted access to face-to-face delivery of Pulmonary Rehabilitation (PR). Evidence suggests that telehealth-PR is non-inferior to outpatient PR. However, it is unknown whether patients who have been referred to face-to-face programmes can feasibly complete an online-PR programme. Methods This service evaluation used a mixed-methods approach to investigate a rapid PR service remodelling using the University of Gloucestershire eLearn Moodle platform. Quantitative baseline demographic and PR outcome data were collected from online-PR participants, and semi-structured interviews were completed with PR staff and participants. Results Twenty-five individuals were eligible from a PR waiting list. Thirteen declined participation and fourteen completed PR. Significant Pre-post online PR improvements were achieved in 1-minute sit-to-stand (Confidence interval (CI) 2.1 - 9 (p = 0.004)), Generalised Anxiety Disorder (CI -0.3 - -2.6(p = 0.023)), Primary Health Questionnaire-9 (CI -5.1 - -0.3 (p = 0.029)), Chronic Respiratory Questionnaire dyspnoea (CI 0.5 - 1.3 (p = 0.001)), Fatigue (CI 0.7 - 2 (p = 0.0004)), Emotion (CI 0.7 - 1.7 (p = 0.0002)), Mastery (CI 0.4 - 1.3(p = 0.001)). Interviews indicated that patient PR inclusion was made possible with digital support and a PR introduction session improved participant engagement and safety. Incremental progression of exercise was perceived as more successful online compared to face-to-face PR. However, perceptions were that education sessions were less successful. Online-PR required significant staff time resource. Discussion Online-PR improves patient outcomes, is feasible and acceptable for individuals referred for face-to-face PR in the context of a requirement for social distancing. Face-to-face programmes can be adapted in a rapid fashion with both staff and participants perceiving benefit.     e-MERLIN and VLBI observations of the luminous infrared galaxy IC883: a nuclear starburst and an AGN candidate revealed

    The high star formation rates of luminous infrared galaxies (LIRGs) make them ideal places for core-collapse supernova (CCSN) searches. At radio frequencies, free from dust extinction, it is possible to detect compact components within the innermost LIRG nuclear regions, such as SNe and SN remnants, as well as AGN buried deep in the LIRG nuclei. We studied the LIRG IC883 aiming at: (i) investigating its (circum-)nuclear regions using the e-EVN at 5GHz, and e-MERLIN at 6.9GHz, complemented by archival VLBI data; (ii) detecting at radio frequencies the two recently reported circumnuclear SNe 2010cu and 2011hi, which were discovered by near-IR (NIR) adaptive optics observations of IC883; and (iii) further investigating the nature of SN2011hi at NIR by means of observations with Gemini-North. The circumnuclear regions traced by e-MERLIN at 6.9GHz have an extension of ~1kpc, and show a striking double-sided structure, which very likely corresponds to a warped rotating ring, in agreement with previous studies. Our e-EVN observations at 5GHz and complementary archival VLBI data at 5GHz and 8.4GHz, reveal the presence of various milliarcsec compact components in the nucleus of IC883. A single compact source, an AGN candidate, dominates the emission at both nuclear and circumnuclear scales, as imaged with the e-EVN and e-MERLIN, respectively. The other milliarcsec components are very suggestive of ongoing nuclear CCSN activity. Our e-EVN observations also resulted in upper limits to the radio luminosity of the two SNe in IC883 recently discovered at NIR. We refine the classification of SN2011hi as a Type IIP SN according to our latest Gemini-North epoch from 2012, in agreement with a low-luminosity radio SN nature. We estimate a CCSN rate lower limit of 1.1_{-0.6}^{+1.3} yr^{-1} for the entire galaxy, based on three nuclear radio SNe and the circumnuclear SNe 2010cu and 2011hi.     Live imaging molecular changes in junctional tension upon VE-cadherin in zebrafish

    Forces play diverse roles in vascular development, homeostasis and disease. VE-cadherin at endothelial cell-cell junctions links the contractile acto-myosin cytoskeletons of adjacent cells, serving as a tension-transducer. To explore tensile changes across VE-cadherin in live zebrafish, we tailored an optical biosensor approach, originally established in vitro. We validate localization and function of a VE-cadherin tension sensor (TS) in vivo. Changes in tension across VE-cadherin observed using ratio-metric or lifetime FRET measurements reflect acto-myosin contractility within endothelial cells. Furthermore, we apply the TS to reveal biologically relevant changes in VE-cadherin tension that occur as the dorsal aorta matures and upon genetic and chemical perturbations during embryonic development
