    La enfermedad de Lyme es un padecimiento infeccioso causado por la espiroqueta Borrelia burgdorferi, que se transmite por la mordedura de garrapatas del género Ixodes. En Europa y EU la seroprevalencia determinada por ELISA es de 1 a 27%. En México sólo hay reportes de casos aislados, pero recientemente se encontró prevalencia de 3 a 6% en un banco de sueros congelados del Distrito Federal y el Noreste del país. El cuadro clínico es muy variable; el eritema migratorio es el síntoma inicial, que aparece en el sitio de la mordedura; semanas más tarde surgen diversas manifestaciones clínicas, entre las cuales se encuentran artritis, fi bromialgia y fatiga crónica. En etapas tardías hay manifestaciones neurológicas y cardiovasculares. El diagnóstico se realiza por ELISA y se confirma mediante Western blot o PCR, debido a la dificultad para aislar Borrelia spp. en cultivos

    Similarities between the lipid proile of Mexican patients with lupus and the general population

    Premature cardiovascular events have been observed in systemic lupus erythematosus (SLE) patients, but the reason for this accelerated process is still debatable; although traditional risk factors are more prevalent in such patients than in the general population, the do not seem to fully explain that enhanced risk. One of the most important conditions is a proatherogenic lipid proile. There is not enough data about it in Mexican SLE patients. Objective: To establish the differences in the lipid proiles between Mexican patients with SLE and the general population. Material and methods: Observational, transversal, descriptive and comparative study, between SLE patients and age-sex-matched healthy volunteers. We performed a full lipid proile (by spectrophotometry) 14 hours of fast. The results obtained were analyzed by the statistical program SPSS® Statistics version 17. Results: We studied the full lipid proiles of 138 subjects, 69 with a diagnosis of SLE and 69 agesex- matched healthy volunteers; 95.7% were females and 4.3% males. Average age was 30 years; average body mass index (BMI) 25.96 ± 5.96 kg/m² in SLE patients and 26.72 ± 4.36 kg/m² in the control group (p = 0.396). Average of total cholesterol 156 mg/dl in the SLE patients and 169.4 mg/dl in the control group (p =0.028); average of low density lipoprotein (LDL) cholesterol 85.27 mg/dl in the SLE patients and 97.57 mg/dl in the control group (p = 0.023). Conclusions: We did not ind statistical differences in the lipid proiles among patients and healthy volunteers, which could explain increased cardiovascular morbidity and mortality observed in SLE patient

    CUBREBOCAS EN TIEMPOS DE PANDEMIA, REVISIÓN HISTÓRICA, CIENTÍFICA Y RECOMENDACIONES PRÁCTICAS

    The aim of this article is to present the medical and scientific literature (including an umbrella review) related to the use of face masks for the prevention of viral respiratory infections, to make an informed decision on the use of face masks during a pandemic. Since the beginning of the 20th Century, it was determined that the use of face masks is effective in reducing the transmission of respiratory infections. The first time face masks were used amongst the general public was during the 1918 Pandemic. Research on face masks during the 20th Century focused mainly on the effectiveness of the materials. It was determined that the best protection was achieved by using several layers of different close knit materials, preferably three. Meta-analyses showed that the use of face masks decreased the risk of contagion of respiratory viral diseases, including COVID-19, compared with not using them. Face masks protect both the user and other people in the population by limiting the exposure to SARS-CoV-2, minimizing the viral load, hence decreasing the risk of developing severe COVID-19. It has been established that any type of face mask is better than not using any mask at all. When deciding upon the use of face masks one should consider the risks and benefits. We conclude that there is sufficient evidence to support that the benefits of using face masks outweigh the risks.El objetivo de este artículo es revisar la literatura médica y científica (incluida una revisión paraguas) relacionada con el uso de cubrebocas como medida de prevención en la transmisión de enfermedades respiratorias virales para tomar decisiones informadas durante una pandemia. Desde inicios del siglo XX se estableció que el cubrebocas es efectivo para reducir infecciones respiratorias.  El uso entre la población general inició hasta la pandemia de 1918. Desde entonces, las investigaciones se han enfocado principalmente en determinar los mejores materiales para su elaboración, encontrando que la efectividad del cubrebocas aumenta con el número de las capas, siendo ideal tres, de punto cerrado y de diversos materiales. Los meta-análisis demostraron que el uso de cubrebocas disminuye el riesgo de contagio de enfermedades respiratorias virales, incluido COVID-19, comparado a no usarlo. Los cubrebocas evitan el paso de una gran proporción de partículas del virus SARS-CoV-2 protegiendo tanto al portador como al resto de la población al reducir la carga viral a la que se está expuesto, disminuyendo así el riesgo de desarrollar enfermedad grave de COVID-19. Cuando las personas están en espacios públicos es preferible elegir usar cualquier cubrebocas, a no usar ninguna protección naso bucal. Para decidir sobre el uso del cubrebocas es importante evaluar el riesgo-beneficio. Con este análisis concluimos que según la evidencia, los beneficios del uso del cubrebocas superan los riesgos, por lo que recomendamos que el uso de cubrebocas de forma generalizada

    Long-term safety of COVID vaccination in individuals with idiopathic inflammatory myopathies: results from the COVAD study

    Limited evidence on long-term COVID-19 vaccine safety in patients with idiopathic inflammatory myopathies (IIMs) continues to contribute to vaccine hesitancy. We studied delayed-onset vaccine adverse events (AEs) in patients with IIMs, other systemic autoimmune and inflammatory disorders (SAIDs), and healthy controls (HCs), using data from the second COVID-19 Vaccination in Autoimmune Diseases (COVAD) study. A validated self-reporting e-survey was circulated by the COVAD study group (157 collaborators, 106 countries) from Feb-June 2022. We collected data on demographics, comorbidities, IIM/SAID details, COVID-19 history, and vaccination details. Delayed-onset (> 7 day) AEs were analyzed using regression models. A total of 15165 respondents undertook the survey, of whom 8759 responses from vaccinated individuals [median age 46 (35-58) years, 74.4% females, 45.4% Caucasians] were analyzed. Of these, 1390 (15.9%) had IIMs, 50.6% other SAIDs, and 33.5% HCs. Among IIMs, 16.3% and 10.2% patients reported minor and major AEs, respectively, and 0.72% (n = 10) required hospitalization. Notably patients with IIMs experienced fewer minor AEs than other SAIDs, though rashes were expectedly more than HCs [OR 4.0; 95% CI 2.2-7.0, p < 0.001]. IIM patients with active disease, overlap myositis, autoimmune comorbidities, and ChadOx1 nCOV-19 (Oxford/AstraZeneca) recipients reported AEs more often, while those with inclusion body myositis, and BNT162b2 (Pfizer) recipients reported fewer AEs. Vaccination is reassuringly safe in individuals with IIMs, with AEs, hospitalizations comparable to SAIDs, and largely limited to those with autoimmune multimorbidity and active disease. These observations may inform guidelines to identify high-risk patients warranting close monitoring in the post-vaccination period

    Prediction of cardiovascular events in rheumatoid arthritis using risk age calculations: evaluation of concordance across risk age models

    Background: In younger individuals, low absolute risk of cardiovascular disease (CVD) may conceal an increased risk age and relative risk of CVD. Calculation of risk age is proposed as an adjuvant to absolute CVD risk estimation in European guidelines. We aimed to compare the discriminative ability of available risk age models in prediction of CVD in rheumatoid arthritis (RA). Secondly, we also evaluated the performance of risk age models in subgroups based on RA disease characteristics. Methods: RA patients aged 30?70 years were included from an international consortium named A Trans-Atlantic Cardiovascular Consortium for Rheumatoid Arthritis (ATACC-RA). Prior CVD and diabetes mellitus were exclusión criteria. The discriminatory ability of specific risk age models was evaluated using c-statistics and their standard errors after calculating time until fatal or non-fatal CVD or last follow-up. Results: A total of 1974 patients were included in the main analyses, and 144 events were observed during followup, the median follow-up being 5.0 years. The risk age models gave highly correlated results, demonstrating R2 values ranging from 0.87 to 0.97. However, risk age estimations differed > 5 years in 15?32% of patients. C-statistics ranged 0.68?0.72 with standard errors of approximately 0.03. Despite certain RA characteristics being associated with low c-indices, standard errors were high. Restricting analysis to European RA patients yielded similar results. Conclusions: The cardiovascular risk age and vascular age models have comparable performance in predicting CVD in RA patients. The influence of RA disease characteristics on the predictive ability of these prediction models remains inconclusive

    Obstetric Outcomes in Women with Rheumatic Disease and COVID-19 in the Context of Vaccination Status

    OBJECTIVE: To describe obstetric outcomes based on COVID-19 vaccination status, in women with rheumatic and musculoskeletal diseases (RMDs) who developed COVID-19 during pregnancy. METHODS: Data regarding pregnant women entered into the COVID-19 Global Rheumatology Alliance registry from 24 March 2020-25 February 2022 were analysed. Obstetric outcomes were stratified by number of COVID-19 vaccine doses received prior to COVID-19 infection in pregnancy. Descriptive differences between groups were tested using the chi -square or Fisher's exact test. RESULTS: There were 73 pregnancies in 73 women with RMD and COVID-19. Overall, 24.7% (18) of pregnancies were ongoing, while of the 55 completed pregnancies 90.9% (50) of pregnancies resulted in livebirths. At the time of COVID-19 diagnosis, 60.3% (n = 44) of women were unvaccinated, 4.1% (n = 3) had received one vaccine dose while 35.6% (n = 26) had two or more doses. Although 83.6% (n = 61) of women required no treatment for COVID-19, 20.5% (n = 15) required hospital admission. COVID-19 resulted in delivery in 6.8% (n = 3) of unvaccinated women and 3.8% (n = 1) of fully vaccinated women. There was a greater number of preterm births (PTB) in unvaccinated women compared with fully vaccinated 29.5% (n = 13) vs 18.2%(n = 2). CONCLUSION: In this descriptive study, unvaccinated pregnant women with RMD and COVID-19 had a greater number of PTB compared with those fully vaccinated against COVID-19. Additionally, the need for COVID-19 pharmacological treatment was uncommon in pregnant women with RMD regardless of vaccination status. These results support active promotion of COVID-19 vaccination in women with RMD who are pregnant or planning a pregnancy

    ABSTRACT Background: Only a few biomarkers are available for assessing disease activity in systemic lupus erythematosus (SLE). Mean platelet volume (MPV) has been recently studied as an inflammatory biomarker. It is currently unclear whether MPV may also play a role as a biomarker of disease activity in adult patients with SLE. Objective: We investigated the association between MPV and disease activity in adult patients with SLE. Methods: In this retrospective study, we compared two groups of adult patients divided according to disease activity (36 per group). Subjects were age- and gender-matched. Results: MPV was significantly decreased with respect to those of inactive patients (7.16 ± 1.39 vs. 8.16 ± 1.50, p = 0.005). At a cutoff level of 8.32 fL, MPV has a sensitivity of 86% and a specificity of 41% for the detection of disease activity. A modest positive correlation was found between MPV and albumin (r = 0.407, p = 0.001), which in turn is inversely associated with disease activity. Conclusions: In summary, MPV is decreased in adult patients with active lupus disease, and positively correlated with albumin, another biomarker of disease activity. Prospective studies are needed to evaluate the prognostic value of this biomarker
