    Swan: for conducted amplified septet, electronics, and video projections

    Swan is a multimedia work for conducted ensemble with amplified instruments, electronics and video projections. Swan is about going out: going out to the street, to the club, to a ritual, to a party or a funeral. It’s about real places with real people, but less about the realities of these places and more about their vibe. It’s about getting out of home, the studio, the institution, going to places where people connect and do things, sing, dance, laugh, cry, perform or celebrate. The music of Swan come from ‘outside.’ Swan’s aesthetic is a blend of Turkish/Islamic and pop-cultural elements. The opening section, Korridor, is a drone/ambient movement with a big trance synth part. It is ritual music. It is big, dense, heavy, and it moves slowly, like lava. Karaoke Mahshar is a Turkish Trance-Pop hybrid. It is a very melancholic, dark piece of music. The instrumental choir sing an emotional pop/“fantasy music” (a Turkish genre) melody in unison over a flamboyant electronic track. It’s the soundtrack to a club for the wasted, for emotional after-hours karaoke. The final section, Rod Modell, is a dub-techno influenced ambient movement. It is the sound of a giant, post-apocalyptic mosque - a mosque sunken in chalky waters. This section evolves to a big, stretched monophonic melody, a song from the old times, which finally cadences to an electronically processed “tilâvet”. I started composing Swan in July 2016 in Turkey, before the military coup attempt took place. The work is not programmatic, however, the sound materials I worked with, the musical references and the sonic and visual iconography it incorporates are rather influenced by and derive from the sounds, sights and emotions I experienced during my stay. By the end of my visit, a person who had been very dear to my heart, Ferhunde Köke, had passed away. I recorded the sounds of her burial accompanied by a hafız’s recitation of the Surah Al-Baqarah 2:156 from the Holy Qur’an which I edited, processed and ended this work with

    Hot tear-melt quality relationship in 3xxx aluminium alloys

    Corrosion resistance, formability and high strength is few of the major properties that makes aluminium alloys preferred choice of material for extrusion or deep drawing processes. Particularly, buildings, windows, doors are main application areas and it is growing. Twin roll casting method is used to produce such materials. One of the common problems faced in this method is called hot tearing. The effect of alloying elements is the reason for this defect. As a result, segregation causes tear and decrease process efficiency. The economic impact is immanent. Thus, it is aimed to study the hot tear tendency of 3xxx series aluminium alloys and the effect of alloying elements will be investigated in this work

    Goodbye Hartmann trial: a prospective, international, multicenter, observational study on the current use of a surgical procedure developed a century ago

    Background: Literature suggests colonic resection and primary anastomosis (RPA) instead of Hartmann's procedure (HP) for the treatment of left-sided colonic emergencies. We aim to evaluate the surgical options globally used to treat patients with acute left-sided colonic emergencies and the factors that leading to the choice of treatment, comparing HP and RPA. Methods: This is a prospective, international, multicenter, observational study registered on ClinicalTrials.gov. A total 1215 patients with left-sided colonic emergencies who required surgery were included from 204 centers during the period of March 1, 2020, to May 31, 2020. with a 1-year follow-up. Results: 564 patients (43.1%) were females. The mean age was 65.9 ± 15.6 years. HP was performed in 697 (57.3%) patients and RPA in 384 (31.6%) cases. Complicated acute diverticulitis was the most common cause of left-sided colonic emergencies (40.2%), followed by colorectal malignancy (36.6%). Severe complications (Clavien-Dindo ≥ 3b) were higher in the HP group (P < 0.001). 30-day mortality was higher in HP patients (13.7%), especially in case of bowel perforation and diffused peritonitis. 1-year follow-up showed no differences on ostomy reversal rate between HP and RPA. (P = 0.127). A backward likelihood logistic regression model showed that RPA was preferred in younger patients, having low ASA score (≤ 3), in case of large bowel obstruction, absence of colonic ischemia, longer time from admission to surgery, operating early at the day working hours, by a surgeon who performed more than 50 colorectal resections. Conclusions: After 100 years since the first Hartmann's procedure, HP remains the most common treatment for left-sided colorectal emergencies. Treatment's choice depends on patient characteristics, the time of surgery and the experience of the surgeon. RPA should be considered as the gold standard for surgery, with HP being an exception

    Infected pancreatic necrosis: outcomes and clinical predictors of mortality. A post hoc analysis of the MANCTRA-1 international study

    : The identification of high-risk patients in the early stages of infected pancreatic necrosis (IPN) is critical, because it could help the clinicians to adopt more effective management strategies. We conducted a post hoc analysis of the MANCTRA-1 international study to assess the association between clinical risk factors and mortality among adult patients with IPN. Univariable and multivariable logistic regression models were used to identify prognostic factors of mortality. We identified 247 consecutive patients with IPN hospitalised between January 2019 and December 2020. History of uncontrolled arterial hypertension (p = 0.032; 95% CI 1.135-15.882; aOR 4.245), qSOFA (p = 0.005; 95% CI 1.359-5.879; aOR 2.828), renal failure (p = 0.022; 95% CI 1.138-5.442; aOR 2.489), and haemodynamic failure (p = 0.018; 95% CI 1.184-5.978; aOR 2.661), were identified as independent predictors of mortality in IPN patients. Cholangitis (p = 0.003; 95% CI 1.598-9.930; aOR 3.983), abdominal compartment syndrome (p = 0.032; 95% CI 1.090-6.967; aOR 2.735), and gastrointestinal/intra-abdominal bleeding (p = 0.009; 95% CI 1.286-5.712; aOR 2.710) were independently associated with the risk of mortality. Upfront open surgical necrosectomy was strongly associated with the risk of mortality (p < 0.001; 95% CI 1.912-7.442; aOR 3.772), whereas endoscopic drainage of pancreatic necrosis (p = 0.018; 95% CI 0.138-0.834; aOR 0.339) and enteral nutrition (p = 0.003; 95% CI 0.143-0.716; aOR 0.320) were found as protective factors. Organ failure, acute cholangitis, and upfront open surgical necrosectomy were the most significant predictors of mortality. Our study confirmed that, even in a subgroup of particularly ill patients such as those with IPN, upfront open surgery should be avoided as much as possible.     Reducing the environmental impact of surgery on a global scale: systematic review and co-prioritization with healthcare workers in 132 countries

    Abstract Background Healthcare cannot achieve net-zero carbon without addressing operating theatres. The aim of this study was to prioritize feasible interventions to reduce the environmental impact of operating theatres. Methods This study adopted a four-phase Delphi consensus co-prioritization methodology. In phase 1, a systematic review of published interventions and global consultation of perioperative healthcare professionals were used to longlist interventions. In phase 2, iterative thematic analysis consolidated comparable interventions into a shortlist. In phase 3, the shortlist was co-prioritized based on patient and clinician views on acceptability, feasibility, and safety. In phase 4, ranked lists of interventions were presented by their relevance to high-income countries and low–middle-income countries. Results In phase 1, 43 interventions were identified, which had low uptake in practice according to 3042 professionals globally. In phase 2, a shortlist of 15 intervention domains was generated. In phase 3, interventions were deemed acceptable for more than 90 per cent of patients except for reducing general anaesthesia (84 per cent) and re-sterilization of ‘single-use’ consumables (86 per cent). In phase 4, the top three shortlisted interventions for high-income countries were: introducing recycling; reducing use of anaesthetic gases; and appropriate clinical waste processing. In phase 4, the top three shortlisted interventions for low–middle-income countries were: introducing reusable surgical devices; reducing use of consumables; and reducing the use of general anaesthesia. Conclusion This is a step toward environmentally sustainable operating environments with actionable interventions applicable to both high– and low–middle–income countries

    Influence of Different Cross Sections on Fluidity Characteristics of A356

    Fluidity of cast metals are typically measured by the length of metal that is stopped by the solidification where the filling is carried out at a constant cross sectional mold cavity. In this work, a sand mold geometry with different section thicknesses were used to measure the fluidity characteristics of A356 alloy at various casting temperatures (700, 725, 750 A degrees C) with and without modifications. It was found that Ti modification had no significant effect on fluidity. On the other hand, the highest fluidity was achieved by Sr modification even at low pouring temperatures. The results were compared with simulation software (SolidCast), and critical fraction of liquid (CFL) was determined

    The treatment of liquid aluminum prior to casting typically consists of purging gas and/or fluxes through the melt. By the use of several chemicals during these operations, several environmental problems can occur. Therefore, in this study, the melt cleanliness of Al-4Cu secondary alloy was investigated by comparing the use of argon degassing with or without chlorine fluxing.     Fluidity of alloys are typically carried out in a spiral test mould. Due to the variation of the results, several attempts have been made to optimise the test procedure. Pouring basin has been updated by addition of a stopper and using a defined quantity of melt to be poured. However, one of the discussed issues of the spiral test mould is its fixed size cavity where the liquid is transferred. Therefore, a mould was introduced by Campbell in which a single runner bar was used to feed different thickness sections. In this study, this mould design was used to investigate the effect of temperature (700 degrees C, 725 degrees C and 750 degrees C) and modification (Ti grain refinement and Sr) on the fluidity of A356 alloy.     Statistics is an important part of our life when it comes to economy, education, sport, politics and research. Research is a primary job for the ones pursuing academic career and statistics is one of the basic components of a research. Metaphor is used as an equivalent to figurative expression (‘mecaz’ in Turkish language). Study group is composed of 19 females (age 27.7) and 40 males (age 30.85) - 59 students in total. Demographic information of participants was collected. Participants were provided with questionnaire in order to assess their metaphorical creation whereby they were asked to fill in the sentence - “statistics is like/ similar to …, because…” - with metaphor. The study has collected total of 59 metaphors in 6 categories. The most created metaphor is in ‘discovering/ revealing’ category. 42.5% of Master’s students and 25% of PhD students are in this category. Metaphors created by graduate students are representing their views about statistics course and hence their view about importance of research. The category ‘discovering/ revealing’, the one with the most created metaphor of this study, is the main concept of science. Consequently, good knowledge of statistics is needed for graduate students to conduct research
