    An Axiomatic Setup for Algorithmic Homological Algebra and an Alternative Approach to Localization

    In this paper we develop an axiomatic setup for algorithmic homological algebra of Abelian categories. This is done by exhibiting all existential quantifiers entering the definition of an Abelian category, which for the sake of computability need to be turned into constructive ones. We do this explicitly for the often-studied example Abelian category of finitely presented modules over a so-called computable ring RR, i.e., a ring with an explicit algorithm to solve one-sided (in)homogeneous linear systems over RR. For a finitely generated maximal ideal m\mathfrak{m} in a commutative ring RR we show how solving (in)homogeneous linear systems over RmR_{\mathfrak{m}} can be reduced to solving associated systems over RR. Hence, the computability of RR implies that of RmR_{\mathfrak{m}}. As a corollary we obtain the computability of the category of finitely presented RmR_{\mathfrak{m}}-modules as an Abelian category, without the need of a Mora-like algorithm. The reduction also yields, as a by-product, a complexity estimation for the ideal membership problem over local polynomial rings. Finally, in the case of localized polynomial rings we demonstrate the computational advantage of our homologically motivated alternative approach in comparison to an existing implementation of Mora's algorithm.Comment: Fixed a typo in the proof of Lemma 4.3 spotted by Sebastian Posu

    Thomas Decomposition and Nonlinear Control Systems

    This paper applies the Thomas decomposition technique to nonlinear control systems, in particular to the study of the dependence of the system behavior on parameters. Thomas' algorithm is a symbolic method which splits a given system of nonlinear partial differential equations into a finite family of so-called simple systems which are formally integrable and define a partition of the solution set of the original differential system. Different simple systems of a Thomas decomposition describe different structural behavior of the control system in general. The paper gives an introduction to the Thomas decomposition method and shows how notions such as invertibility, observability and flat outputs can be studied. A Maple implementation of Thomas' algorithm is used to illustrate the techniques on explicit examples

    A Core Outcome Set for Efficacy of Acute Treatment of Hereditary Angioedema

    BACKGROUND: Clinical trials investigating drugs for the acute treatment of hereditary angioedema attacks have assessed many different outcomes. This heterogeneity limits the comparability of trial results and may lead to selective outcome reporting bias and a high burden on trial participants. OBJECTIVE: To achieve consensus on a core outcome set composed of key outcomes that ideally should be used in all clinical efficacy trials involving the acute treatment of hereditary angioedema attacks. METHODS: We conducted a Delphi consensus study involving all relevant parties: patients with hereditary angioedema, hereditary angioedema expert clinicians and clinical researchers, pharmaceutical companies, and regulatory bodies. Two Internetbased survey rounds were conducted. In round 1, panelists indicated the importance of individual outcomes used in clinical trials on a 9 -point Likert scale. Based on these results, a core outcome set was developed and voted on by panelists in round 2. RESULTS: A total of 58 worldwide panelists completed both rounds. The fi rst round demonstrated high importance scores and substantial agreement among the panelists. In the second round, a consensus of 90% or greater was achieved on a core outcome set consisting of fi ve key outcomes: change in overall symptom severity at one predetermined time point between 15 minutes and 4 hours after treatment, time to end of progression of all symptoms, the need for rescue medication during the entire attack, impairment of daily activities, and treatment satisfaction. CONCLUSIONS: This international study obtained a high level of consensus on a core outcome set for the acute treatment of hereditary angioedema attacks, consisting of fi ve key outcomes.     The international WAO/EAACI guideline for the management of hereditary angioedema—The 2021 revision and update

    Hereditary angioedema (HAE) is a rare and disabling disease for which early diagnosis and effective therapy are critical. This revision and update of the global WAO/EAACI guideline on the diagnosis and management of HAE provides up-to-date guidance for the management of HAE. For this update and revision of the guideline, an international panel of experts reviewed the existing evidence, developed 28 recommendations, and established consensus by an online DELPHI process. The goal of these recommendations and guideline is to help physicians and their patients in making rational decisions in the management of HAE with deficient C1 inhibitor (type 1) and HAE with dysfunctional C1 inhibitor (type 2), by providing guidance on common and important clinical issues, such as: (1) How should HAE be diagnosed? (2) When should HAE patients receive prophylactic on top of on-demand treatment and what treatments should be used? (3) What are the goals of treatment? (4) Should HAE management be different for special HAE patient groups such as children or pregnant/breast-feeding women? and (5) How should HAE patients monitor their disease activity, impact, and control? It is also the intention of this guideline to help establish global standards for the management of HAE and to encourage and facilitate the use of recommended diagnostics and therapies for all patients

    Sympatho-renal axis in chronic disease

    Essential hypertension, insulin resistance, heart failure, congestion, diuretic resistance, and functional renal disease are all characterized by excessive central sympathetic drive. The contribution of the kidney’s somatic afferent nerves, as an underlying cause of elevated central sympathetic drive, and the consequences of excessive efferent sympathetic signals to the kidney itself, as well as other organs, identify the renal sympathetic nerves as a uniquely logical therapeutic target for diseases linked by excessive central sympathetic drive. Clinical studies of renal denervation in patients with resistant hypertension using an endovascular radiofrequency ablation methodology have exposed the sympathetic link between these conditions. Renal denervation could be expected to simultaneously affect blood pressure, insulin resistance, sleep disorders, congestion in heart failure, cardiorenal syndrome and diuretic resistance. The striking epidemiologic evidence for coexistence of these disorders suggests common causal pathways. Chronic activation of the sympathetic nervous system has been associated with components of the metabolic syndrome, such as blood pressure elevation, obesity, dyslipidemia, and impaired fasting glucose with hyperinsulinemia. Over 50% of patients with essential hypertension are hyperinsulinemic, regardless of whether they are untreated or in a stable program of treatment. Insulin resistance is related to sympathetic drive via a bidirectional mechanism. In this manuscript, we review the data that suggests that selective impairment of renal somatic afferent and sympathetic efferent nerves in patients with resistant hypertension both reduces markers of central sympathetic drive and favorably impacts diseases linked through central sympathetics—insulin resistance, heart failure, congestion, diuretic resistance, and cardiorenal disorders

    Positional Income Concerns: Prevalence and Relationship with Personality and Economic Preferences

    This paper presents detailed evidence about who compares to whom in terms of relative income. We rely on representative survey data on the importance of income comparisons vis-á-vis seven reference groups, allowing us to exploit within-subject heterogeneity. We explore the prevalence and determinants of positional income concerns, investigating the role of personality and economic preferences. Our results establish robust relationships between positional income concerns and the personality traits agreeableness, conscientiousness, and neuroticism, some of which depend on the reference group. Furthermore, risk and fairness preferences are significantly correlated with positional income concerns
