    Oral lichenoid lesions related to contact with dental materials: A literature review

    Oral lichenoid lesions related to contact are defined as oral-cavity eruptions with an identifiable etiology, and are clinically and histologically similar to oral lichen planus. Within this group are found oral lichenoid lesions related to contact with dental materials (OLLC), the most common being those related to silver amalgam. Currently, it remains difficult to diagnose these lesions due to the clinical and histopathological similarity with oral lichen planus and other oral mucosa lesions of lichenoid characteristics. In the present paper, we carry out an updated review of the tests for, and the different characteristics of OLLC, which may aid the diagnosis. For this review, we made searches in the Pubmed® and Cochrane® databases. Among the literature we found several published papers, from which we have used review papers, case papers, cohort studies, case and control studies, and a meta-analysis study. After carrying out this review, we can conclude that the diagnosis of these lesions is still difficult and controversial. However, there are different aspects in the clinical presentation, pathological study and results obtained when replacing suspect materials, which, when taken together, may be useful when establishing the final diagnosis of OLLC

    Las reacciones alérgicas de contacto intraoral se atribuyen con frecuencia a materiales de uso odontológico. Aunque se consideran poco frecuentes, existen muchas publicaciones de reacciones a materiales de restauración, sobre todo a la amalgama. En cambio, resulta muy difícil encontrar referencias en la literatura de este tipo de reacciones a metales presentes en aleaciones metálicas de prótesis dentales. Presentamos el caso de una mujer de 60 años que presenta lesiones de tipo liquenoide en paladar duro y en reborde alveolar, todas las lesiones se hallaban bajo la base de la prótesis. Se realiza una biopsia incisional y el estudio histopatológico mostró cambios compatibles con liquen plano oral. Las pruebas epicutáneas mostraron una sensibilización a cloruro de cobalto, amonio hexacloroplatinato y cloruro de aluminio. Como medidas terapéuticas se recomendó retirar la prótesis y recambiar las obturaciones de amalgama, consiguiendo una mejoría considerable en las lesiones y la sintomatología que presentaba la paciente.Intraoral contact allergy reactions often attributed to dental materials. We report a clinical case of a 60 year-old woman with oral lichenoid lesions located in hard palate and alveolar ridge, all lesions were located under the base of the prosthesis. Incisional biopsy was made and the histopathological study revealed changes compatible with oral lichen planus. The epicutaneous test showed a sensitivity to chloride of cobalt, ammonium hexacloroplatinato and chloride of aluminium. Treatment consisted of the removal the prosthesis and amalgam fillings, obtaining a considerable improvement in lesions and symptoms of patient

    The role of relief, perceived control, and prospective intolerance of uncertainty in excessive avoidance in uncertain-threat environments

    Excessive avoidance is a key feature of pathological anxiety. However, the precise mechanisms underlying the development of excessive avoidance are still unknown. In the present study, we tested the hypothesis that excessive avoidance, especially in individuals with high Intolerance of Uncertainty (IU) is aimed at distress reduction via the enhancement of subjective perceived control in uncertain-threat environments. In our experiment, participants learned to avoid an uncertain aversive sound through a discriminated free operant procedure. In a later test phase in extinction, we manipulated the amount of avoidance responses available per trial by creating a limited and an unrestricted response condition. Nonetheless, the aversive sound could be effectively avoided in both conditions. We measured response frequency, avoidance confidence ratings and anxiety-predisposing traits such as intolerance of uncertainty, trait anxiety and distress tolerance. The degree of distress suffered during trials was inferred from post-trial relief ratings that were requested after trials in which the aversive sound had been omitted. In the avoidance acquisition phase, we found a positive association between prospective intolerance of uncertainty (P-IU) and the decline rate of distress. This relationship was not significant, however, when inhibitory intolerance of uncertainty (I-IU) was controlled for. At test, we found that the increase in avoidance responses led to distress reduction through the enhancement of avoidance confidence. Finally, we found a significant modulating role of P-IU in the effect of response limit on distress reduction that lends further support to our hypothesis. Specifically, P-IU was positively associated with the effect of response limit on distress.     El cáncer oral es un problema de salud con el que nos enfrentamos todos los días en nuestras consultas. Los dentistas formamos la primera línea de defensa ante una patología que no ha presentado una mejora de los índices de mortalidad y morbilidad en las últimas décadas. La principal arma con la que contamos es la del diagnóstico precoz de la enfermedad, y esto es mucho más complicado de lo que parece. Para ayudar a los dentistas en la visualización de lesiones de precáncer o cáncer temprano han aparecido algunas técnicas o dispositivos de screening de cáncer oral basadas en la interacción de los tejidos orales con la luz. La técnica más extendida y estudiada es la basada en la quimioluminiscencia, intentaremos poner en claro algunos aspectos analizando los estudios que hay en la literatura actual.Oral cancer is a health problem with which we face every day in our consultations. Dentists form the first line of defense against a disease that has not produced an improvement in mortality and morbidity in recent decades. The main weapon with which we have is the early diagnosis of the disease, and this is much more complicated than it seems. To assist dentists in the visualization of pre-cancer lesions or early cancer have appeared some techniques and devices for oral cancer screening based on interaction of the oral tissues with light.     La leucoplasia es definida actualmente en base a un concepto esencialmente clínico. En este sentido, se propone un diagnóstico provisional de leucoplasia basado en criterios clínicos y un diagnóstico definitivo para el que se requiere la eliminación de los factores etiológicos y, ante lesiones persistentes, el examen histopatológico. La consideración de la leucoplasia como lesión precancerosa proviene esencialmente de los estudios de seguimiento en los que se constata su transformación maligna. Pese a que el diagnóstico de displasia es esencialmente subjetivo y a la existencia de algún trabajo reciente que no encuentra relación entre ésta y la evolución posterior de las leucoplasias, la presencia de displasia epitelial en el examen histológico se considera actualmente como el indicador más importante de una mayor probabilidad de transformación maligna de la leucoplasia. Entre otros factores relacionados con un peor pronóstico se consideran el que se trate de una leucoplasia idiopática, las formas clínicas no homogéneas, la localización en cara ventrolateral de la lengua y suelo de boca y un tamaño mayor de 1 centímetro.The current definition of leukoplakia is based on an essentially clinical concept. Therefore, both a provisional diagnosis of leukoplakia based on clinical criteria and a definite diagnosis for which etiological factors need to be eliminated and, should there be persistent lesions, a histological examination is required, are accepted. Follow up studies in which a malignant transformation has been observed have resulted in leukoplakia being considered as a precancerous disorder. Despite the fact that the diagnosis of dysplasia is subjective and certain recent studies have not found any association between this and the development of leukoplakia, the presence of epithelial dysplasia in the histological examination is currently considered to be the most important indicator of a higher possibility of malignant transformation of the leukoplakia. Among other factors that may indicate a worse prognosis are: if the leukoplakia is idiopathic, if clinical forms are non-homogenous, if it is located on the ventrolateral side of the tongue and the floor of the mouth, and if it is over 1 cm.     Intermediate Molecular Phenotypes to Identify Genetic Markers of Anthracycline-Induced Cardiotoxicity Risk.

    Cardiotoxicity due to anthracyclines (CDA) affects cancer patients, but we cannot predict who may suffer from this complication. CDA is a complex trait with a polygenic component that is mainly unidentified. We propose that levels of intermediate molecular phenotypes (IMPs) in the myocardium associated with histopathological damage could explain CDA susceptibility, so variants of genes encoding these IMPs could identify patients susceptible to this complication. Thus, a genetically heterogeneous cohort of mice (n = 165) generated by backcrossing were treated with doxorubicin and docetaxel. We quantified heart fibrosis using an Ariol slide scanner and intramyocardial levels of IMPs using multiplex bead arrays and QPCR. We identified quantitative trait loci linked to IMPs (ipQTLs) and cdaQTLs via linkage analysis. In three cancer patient cohorts, CDA was quantified using echocardiography or Cardiac Magnetic Resonance. CDA behaves as a complex trait in the mouse cohort. IMP levels in the myocardium were associated with CDA. ipQTLs integrated into genetic models with cdaQTLs account for more CDA phenotypic variation than that explained by cda-QTLs alone. Allelic forms of genes encoding IMPs associated with CDA in mice, including AKT1, MAPK14, MAPK8, STAT3, CAS3, and TP53, are genetic determinants of CDA in patients. Two genetic risk scores for pediatric patients (n = 71) and women with breast cancer (n = 420) were generated using machine-learning Least Absolute Shrinkage and Selection Operator (LASSO) regression.     Diseño estratégico de vanguardia

    La integración del diseño con la vanguardia se observa natural, esto es, el diseño es una disciplina abductiva y la vanguardia persigue fines prospectivos, es decir, en ambos casos se trata de objetivos de posibilidad futura. De tal suerte, este libro, emanado de una parte de las ponencias rigurosamente arbitradas del Coloquio Internacional de Diseño 2016, está dividido en tres secciones o capítulos, a saber, el capítulo uno relacionado con la teoría y metodología para proyectos de diseño de vanguardia, el segundo sobre la tecnología, la innovación y la sostenibilidad de vanguardia de dichos proyectos, y finalmente el último capítulo, vinculado con la gestión estratégica de proyectos de vanguardia.La historia se forja de hechos e interpretaciones, de pasados construidos y de presentes en procesos constantes, estudiados en forma estricta por las ciencias. Por su parte, el futuro ostenta la posibilidad de ser indefinidamente planeado con base en las variopintas aproximaciones teóricas y empíricas que dan fundamento a este tipo de ciencia; éstas son denominadas prospectivas y sus bases vanguardias. Resulta importante señalar, que estas posibilidades sólo permiten tener una idea hipotética de lo que será la realidad y el mundo de vida de los seres vivos y su contexto, no obstante, se trata de la única manera racional que tiene el ser humano de prever ese futuro posible. Las distintas ciencias y disciplinas nos permiten construir históricamente estas posibilidades partiendo de datos, hechos, significados y un sinfín de informaciones que le dan cuerpo y sentido a tales posibilidades. En este sentido, la vanguardia, como base del conocimiento prospectivo, observa la necesidad de ser escrita, leída y discutida en los términos más estrictos con el fin de volver las predicciones más precisas. El diseño por su parte, es definido de manera sucinta como la disciplina proyectual estratégica y sistémica de la posibilidad, dirigida a procesos de significación utilitaria y simbólica para la comprensión –o interpretación– y modificación –o proyectación– de niveles de realidad (referentes y sujetos) desde diversos aparatos teóricos y empíricos –perspectivas disciplinarias–
