Prevalence and associated factors of skin cancer in aged nursing home residents: A multicenter prevalence study
Non-melanoma-skin cancer is an emerging clinical problem in the elderly, fair skinned population which predominantly affects patients aged older than 70 years. Its steady increase in incidence rates and morbidity is paralleled by related medical costs. Despite the fact that many elderly patients are in need of care and are living in nursing homes, specific data on the prevalence of skin cancer in home care and the institutional long-term care setting is currently lacking. A representative multicenter prevalence study was conducted in a random sample of ten institutional long-term care facilities in the federal state of Berlin, Germany. In total, n = 223 residents were included. Actinic keratoses, the precursor lesions of invasive cutaneous squamous cell carcinoma were the most common epithelial skin lesions (21.1%, 95% CI 16.2 to 26.9). Non-melanoma skin cancer was diagnosed in 16 residents (7.2%, 95% CI 4.5 to 11.3). None of the residents had a malignant melanoma. Only few bivariate associations were detected between non-melanoma skin cancer and demographic, biographic and functional characteristics. Male sex was significantly associated with actinic keratosis whereas female sex was associated with non-melanoma skin cancer. Smoking was associated with an increased occurrence of non-melanoma skin cancer. Regular dermatology check-ups in nursing homes would be needed but already now due to financial limitations, lack of time in daily clinical practice and limited number of practising dermatologists, it is not the current standard. With respect to the worldwide growing aging population new programs and decisions are required. Overall, primary health care professionals should play a more active role in early diagnosis of skin cancer in nursing home residents. Dermoscopy courses, web-based or smartphone-based applications and teledermatology may support health care professionals to provide elderly nursing home residents an early diagnosis of skin cancer
Modeling of grain boundary dynamics using amplitude equations
We discuss the modelling of grain boundary dynamics within an amplitude
equations description, which is derived from classical density functional
theory or the phase field crystal model. The relation between the conditions
for periodicity of the system and coincidence site lattices at grain boundaries
is investigated. Within the amplitude equations framework we recover
predictions of the geometrical model by Cahn and Taylor for coupled grain
boundary motion, and find both and
coupling. No spontaneous transition between these modes occurs due to
restrictions related to the rotational invariance of the amplitude equations.
Grain rotation due to coupled motion is also in agreement with theoretical
predictions. Whereas linear elasticity is correctly captured by the amplitude

Crucial Role of Reactive Oxygen Species (ROS) for the Proapoptotic Effects of Indirubin Derivatives in Cutaneous SCC Cells
Efficient drugs are needed for countering the worldwide high incidence of cutaneous squamous cell carcinoma (cSCC) and actinic keratosis. Indirubin derivatives represent promising candidates, but their effects in cSCC cells have not been reported before. Here, we investigated the efficacy of three indirubin derivatives (DKP-071, -073 and -184) in four cSCC cell lines. High efficacy was seen in SCL-I, SCL-II, SCC-12 and SCC-13, resulting in up to 80% loss of cell proliferation, 60% loss of cell viability and 30% induced apoptosis (10 µM). Apoptosis was further enhanced in combinations with TNF-related apoptosis-inducing ligand (TRAIL). Induction of reactive oxygen species (ROS) appeared as critical for these effects. Thus, antioxidative pretreatment completely abolished apoptosis as well as restored cell proliferation and viability. Concerning the pathways, complete activation of caspases cascades (caspases-3, -4, -6, -7, -8 and -9), loss of mitochondrial membrane potential, activation of proapoptotic PKCδ (protein kinase C delta), inhibition of STAT3 (signal transducer and activator of transcription 3), downregulation of antiapoptotic XIAP (X-linked inhibitor of apoptosis protein) and survivin as well as upregulation of the proapoptotic Bcl-2 protein Puma and the cell cycle inhibitor p21 were obtained. Importantly, all activation steps were prevented by antioxidants, thus proving ROS as a master regulator of indirubins’ antitumor effects. National-scale modelling of N leaching in organic and conventional horticultural crop rotations - policy implications
A method is presented to model N leaching in crop rotations on a national scale. Representative crop rotations for different regions and soil types are used in the cross-disciplinary, plant, soil, environment & economics model EU-Rotate_N. By comparing contrasting farming systems (organic and conventional) in the UK, their strengths and weaknesses in delivering environmental and economic sustainability can be assessed. Modelling results show that the annual leaching in different horticultural rotations and UK regions, using median weather, is within the range of 13-88 kg N/ha/year for organic and 54-130 kg N /ha/year for conventional. The weighted annual average figures are 39 kg N/ha/year for organic and 81 kg N/ha/year for conventional, respectively. It is concluded that organic horticultural rotations, with a current share of 6.1% already contribute to lower overall N losses from agriculture. However, on a UK national scale, only a large share of organic land use (e.g. >50%) has a large effect on reducing N losses. Similar reductions are also predicted by substantial cuts in conventional N inputs, giving a policy choice if pollution from agriculture steps up further on the political agenda
s-ICAM-1 and s-VCAM-1 in healthy men are strongly associated with traits of the metabolic syndrome, becoming evident in the postprandial response to a lipid-rich meal
<p>Abstract</p> <p>Background</p> <p>The importance of the postprandial state for the early stages of atherogenesis is increasingly acknowledged. We conducted assessment of association between postprandial triglycerides, insulin and glucose after ingestion of a standardized lipid-rich test meal, and soluble cellular adhesion molecules (sCAM) in young healthy subjects.</p> <p>Methods</p> <p>Metabolic parameters and sICAM-1, sVCAM-1 and E-selectin were measured before and hourly until 6 hours after ingestion of a lipid-rich meal in 30 healthy young men with fasting triglycerides <150 mg/dl and normal fasting glucose levels. Subjects were classified as either normal responders (NR) (postprandial triglyceride maxima < 260 mg/dl) or high responders (HR) (postprandial triglyceride maxima > 260 mg/dl). Levels of CAM were compared in HR and NR, and correlation with postprandial triglyceride, insulin and glucose response was assessed.</p> <p>Results</p> <p>Fasting sICAM-1 and sVCAM-1 levels were significantly higher in HR as compared to NR (p = 0.046, p = 0.03). For sE-selectin there was such a trend (p = 0.05). There was a strong positive and independent correlation between sICAM-1 and postprandial insulin maxima (r = 0.70, p < 0.001). sVCAM-1 showed significant correlation with postprandial triglycerides (AUC) (r = 0.37, p = 0.047). We found no correlation between sCAMs and fasting insulin or triglyceride concentrations.</p> <p>Conclusion</p> <p>This independent association of postprandial triglycerides with sICAM-1 may indicate a particular impact of postprandial lipid metabolism on endothelial reaction.</p
EU-Rotate_N – a decision support system – to predict environmental and economic consequences of the management of nitrogen fertiliser in crop rotations
A model has been developed which assesses the economic and environmental performance of crop rotations, in both conventional and organic cropping, for over 70 arable and horticultural crops, and a wide range of growing conditions in Europe. The model, though originally based on the N_ABLE model, has been completely rewritten and contains new routines to simulate root development, the mineralisation and release of nitrogen (N) from soil organic matter and crop residues, and water dynamics in soil. New routines have been added to estimate the effects of sub-optimal rates of N and spacing on the marketable outputs and gross margins. The model provides a mechanism for generating scenarios to represent a range of differing crop and fertiliser management strategies which can be used to evaluate their effects on yield, gross margin and losses of nitrogen through leaching. Such testing has revealed that nitrogen management can be improved and that there is potential to increase gross margins whilst reducing nitrogen losses
