    Quality of Life using AQLQ (S), ACT and GINA in patients with bronchial asthma in South India

    Asthma has been notified as a chronic illness that impacts a large number of individuals and affects their quality of life. Aim: To measure the Quality of Life in patients with Bronchial Asthma in a tertiary care setting in South India. Method: Structured face to face interviews were conducted using standardized tools i.e. Standardized version of Juniper‟s Asthma Quality of Life Questionnaire and the responses were classified under the domains of activity limitations, symptoms, emotions and exposure to environmental stimuli. The Asthma Control Test was also used categorizing respondents as demonstrating total control, well controlled or uncontrolled asthma. GINA guidelines was used to classify the patients based on severity of Asthma as intermittent, mild persistent, moderate persistent and severe persistent. Result: 200 physician diagnosed patients with Bronchial Asthma participated in the study. Majority were male (n=115) and rest female (n=85). 143 were married and many were graduates (n=52). The mean QOL of the patients was 4.83 on 7 point scale. More than half of the sample population (57% n=114) were found to experience uncontrolled asthma. The average score received in Asthma Control Test was 17 against a maximum of 25. Less than half the patients (37.5% n =75) in the study were classified as having moderate Asthma. Conclusion: The findings suggest that there is a need to control asthma and the environmental factors that trigger it. Educating patients on treatment and precautionary measures may be a potential solution to enhance the overall sense of well-being in patients with bronchial asthma

    Reconstructing Coastal Sediment Budgets From Beach‐ and Foredune‐Ridge Morphology: A Coupled Field and Modeling Approach

    Preserved beach and foredune ridges may serve as proxies for coastal change, reflecting alterations in sea level, wave energy, or past sediment fluxes. In particular, time‐varying shoreface sediment budgets have been inferred from the relative size of foredune ridges through application of radiocarbon and optically stimulated luminescence dating to these systems over the last decades. However, geochronological control requires extensive field investigation and analysis. Purely field‐based studies might also overlook relationships between the mechanics of sediment delivery to the shoreface and foredune ridges, missing insights about sensitivity to changes in sediment budget. We therefore propose a simple geomorphic model of beach/foredune‐ridge and swale morphology to quantify the magnitude of changes in cross‐shore sediment budget, employing field measurements of ridge volume, ridge spacing, elevation, and shoreline progradation. Model behaviors are constrained by the partitioning of sediment fluxes to the shoreface and foredune ridge and can be used to reproduce several cross‐shore patterns observed in nature. These include regularly spaced ridges (“washboards”), large singular ridges, and wide swales with poorly developed ridges. We evaluate our model against well‐preserved ridge and swale systems at two sites along the Virginia Eastern Shore (USA): Fishing Point, for which historical records provide a detailed history of shoreline progradation and ridge growth, and Parramore Island, for which a relatively more complex morphology developed over a poorly constrained period of prehistoric growth. Our results suggest this new model could be used to infer the sensitivity of field sites across the globe to variations in sediment delivery

    Squirrelpox virus: assessing prevalence, transmission and environmental degradation

    Red squirrels (Sciurus vulgaris) declined in Great Britain and Ireland during the last century, due to habitat loss and the introduction of grey squirrels (Sciurus carolinensis), which competitively exclude the red squirrel and act as a reservoir for squirrelpox virus (SQPV). The disease is generally fatal to red squirrels and their ecological replacement by grey squirrels is up to 25 times faster where the virus is present. We aimed to determine: (1) the seropositivity and prevalence of SQPV DNA in the invasive and native species at a regional scale; (2) possible SQPV transmission routes; and, (3) virus degradation rates under differing environmental conditions. Grey (n = 208) and red (n = 40) squirrel blood and tissues were sampled. Enzyme-linked immunosorbent assay (ELISA) and quantitative real-time polymerase chain reaction (qPCR) techniques established seropositivity and viral DNA presence, respectively. Overall 8% of squirrels sampled (both species combined) had evidence of SQPV DNA in their tissues and 22% were in possession of antibodies. SQPV prevalence in sampled red squirrels was 2.5%. Viral loads were typically low in grey squirrels by comparison to red squirrels. There was a trend for a greater number of positive samples in spring and summer than in winter. Possible transmission routes were identified through the presence of viral DNA in faeces (red squirrels only), urine and ectoparasites (both species). Virus degradation analyses suggested that, after 30 days of exposure to six combinations of environments, there were more intact virus particles in scabs kept in warm (25°C) and dry conditions than in cooler (5 and 15°C) or wet conditions. We conclude that SQPV is present at low prevalence in invasive grey squirrel populations with a lower prevalence in native red squirrels. Virus transmission could occur through urine especially during warm dry summer conditions but, more notably, via ectoparasites, which are shared by both species

    Obesity-induced insulin resistance in human skeletal muscle is characterised by defective activation of p42/p44 MAP kinase

    Insulin resistance (IR), an impaired cellular, tissue and whole body response to insulin, is a major pathophysiological defect of type 2 diabetes mellitus. Although IR is closely associated with obesity, the identity of the molecular defect(s) underlying obesity-induced IR in skeletal muscle remains controversial; reduced post-receptor signalling of the insulin receptor substrate 1 (IRS1) adaptor protein and downstream effectors such as protein kinase B (PKB) have previously been implicated. We examined expression and/or activation of a number of components of the insulin-signalling cascade in skeletal muscle of 22 healthy young men (with body mass index (BMI) range, 20–37 kg/m2). Whole body insulin sensitivity (M value) and body composition was determined by the hyperinsulinaemic (40 mU. min−1.m−2.), euglycaemic clamp and by dual energy X-ray absorptiometry (DEXA) respectively. Skeletal muscle (vastus lateralis) biopsies were taken before and after one hour of hyperinsulinaemia and the muscle insulin signalling proteins examined by western blot and immunoprecipitation assay. There was a strong inverse relationship between M-value and BMI. The most striking abnormality was significantly reduced insulin-induced activation of p42/44 MAP kinase, measured by specific assay, in the volunteers with poor insulin sensitivity. However, there was no relationship between individuals' BMI or M-value and protein expression/phosphorylation of IRS1, PKB, or p42/44 MAP kinase protein, under basal or hyperinsulinaemic conditions. In the few individuals with poor insulin sensitivity but preserved p42/44 MAP kinase activation, other signalling defects were evident. These findings implicate defective p42/44 MAP kinase signalling as a potential contributor to obesity-related IR in a non-diabetic population, although clearly multiple signalling defects underlie obesity associated IR

    X-ray radiography of cavitation in a beryllium alloy nozzle

    [EN] Making quantitative measurements of the vapor distribution in a cavitating nozzle is difficult, owing to the strong scattering of visible light at gas-liquid boundaries and wall boundaries, and the small lengths and time scales involved. The transparent models required for optical experiments are also limited in terms of maximum pressure and operating life. Over the past few years, x-ray radiography experiments at Argonne's Advanced Photon Source have demonstrated the ability to perform quantitative measurements of the line of sight projected vapor fraction in submerged, cavitating plastic nozzles. In this paper, we present the results of new radiography experiments performed on a submerged beryllium nozzle which is 520m in diameter, with a length/diameter ratio of 6. Beryllium is a light, hard metal that is very transparent to x-rays due to its low atomic number. We present quantitative measurements of cavitation vapor distribution conducted over a range of non-dimensional cavitation and Reynolds numbers, up to values typical of gasoline and diesel fuel injectors. A novel aspect of this work is the ability to quantitatively measure the area contraction along the nozzle with high spatial resolution. Analysis of the vapor distribution, area contraction and discharge coefficients are made between the beryllium nozzle and plastic nozzles of the same nominal geometry. When gas is dissolved in the fuel, the vapor distribution can be quite different from that found in plastic nozzles of the same dimensions, although the discharge coefficients are unaffected. In the beryllium nozzle, there were substantially fewer machining defects to act as nucleation sites for the precipitation of bubbles from dissolved gases in the fuel, and as such the effect on the vapor distribution was greatly reduced.Raul Payri was funded by a Fulbright visiting scholar grant in collaboration with the Ministry of Education, Culture and Sports of Spain (reference PRX14/00331) while performing this work.     Breaking the entangling gate speed limit for trapped-ion qubits using a phase-stable standing wave

    All laser-driven entangling operations for trapped-ion qubits have hitherto been performed without control of the optical phase of the light field, which precludes independent tuning of the carrier and motional coupling. By placing 88Sr+ ions in a λ=674  nm standing wave, whose relative position is controlled to ≈λ/100, we suppress the carrier coupling by a factor of 18, while coherently enhancing the spin-motion coupling. We experimentally demonstrate that the off-resonant carrier coupling imposes a speed limit for conventional traveling-wave Mølmer-Sørensen gates; we use the standing wave to surpass this limit and achieve a gate duration of 15  μs, restricted by the available laser power

    Phase I study of TP300 in patients with advanced solid tumors with pharmacokinetic, pharmacogenetic and pharmacodynamic analyses

    Background: A Phase I dose escalation first in man study assessed maximum tolerated dose (MTD), dose-limiting toxicity (DLT) and recommended Phase II dose of TP300, a water soluble prodrug of the Topo-1 inhibitor TP3076, and active metabolite, TP3011. <p/>Methods: Eligible patients with refractory advanced solid tumors, adequate performance status, haematologic, renal, and hepatic function. TP300 was given as a 1-hour i.v. infusion 3-weekly and pharmacokinetic (PK) profiles of TP300, TP3076 and TP3011 were analysed. Polymorphisms in CYP2D6, AOX1 and UGT1A1 were studied and DNA strand-breaks measured in peripheral blood mononuclear cells (PBMCs). <p/>Results: 32 patients received TP300 at 1, 2, 4, 6, 8, 10, 12 mg/m2. MTD was 10 mg/m2; DLTs at 12 (2/4 patients) and 10 mg/m2 (3/12) included thrombocytopenia and febrile neutropenia; diarrhea was uncommon. Six patients (five had received irinotecan), had stable disease for 1.5-5 months. TP3076 showed dose proportionality in AUC and Cmax from 1--10 mg/m2. Genetic polymorphisms had no apparent influence on exposure. DNA strand-breaks were detected after TP300 infusion. <p/>Conclusions: TP300 had predictable hematologic toxicity, and diarrhea was uncommon. AUC at MTD is substantially greater than for SN38. TP3076 and TP3011 are equi-potent with SN38, suggesting a PK advantage
