Diagnosis and non-surgical treatment of peri-implant diseases and maintenance care of patients with dental implants - Consensus report of working group 3

The following consensus report is based on four background reviews. The frequency of maintenance visits is based on patient risk indicators, homecare compliance and prosthetic design. Generally, a 6-month visit interval or shorter is preferred. At these visits, peri-implant probing, assessment of bleeding on probing and, if warranted, a radiographic examination is performed. Diagnosis of peri-implant mucositis requires: (i) bleeding or suppuration on gentle probing with or without increased probing depth compared with previous examinations; and (ii) no bone loss beyond crestal bone level changes resulting from initial bone remodelling. Diagnosis of peri-implantitis requires: (i) bleeding and/or suppuration on gentle probing; (ii) an increased probing depth compared with previous examinations; and (iii) bone loss beyond crestal bone level changes resulting from initial bone remodelling. If diagnosis of disease is established, the inflammation should be resolved. Non-surgical therapy is always the first choice. Access and motivation for optimal oral hygiene are key. The patient should have a course of mechanical therapy and, if a smoker, be encouraged not to smoke. Non-surgical mechanical therapy and oral hygiene reinforcement are useful in treating peri-implant mucositis. Power-driven subgingival air-polishing devices, Er: YAG lasers, metal curettes or ultrasonic curettes with or without plastic sleeves can be used to treat peri-implantitis. Such treatment usually provides clinical improvements such as reduced bleeding tendency, and in some cases a pocket-depth reduction of --1 mm. In advanced cases, however, complete resolution of the disease is unlikely.Peer Reviewedhttps://deepblue.lib.umich.edu/bitstream/2027.42/151295/1/idj12490_am.pdfhttps://deepblue.lib.umich.edu/bitstream/2027.42/151295/2/idj12490.pd

Advances in novel therapeutic approaches for periodontal diseases

Abstract Periodontal diseases are pathological processes resulting from infections and inflammation affecting the periodontium or the tissue surrounding and supporting the teeth. Pathogenic bacteria living in complex biofilms initiate and perpetuate this disease in susceptible hosts. In some cases, broad-spectrum antibiotic therapy has been a treatment of choice to control bacterial infection. However, increasing antibiotic resistance among periodontal pathogens has become a significant challenge when treating periodontal diseases. Thanks to the improved understanding of the pathogenesis of periodontal disease, which involves the host immune response, and the importance of the human microbiome, the primary goal of periodontal therapy has shifted, in recent years, to the restoration of homeostasis in oral microbiota and its harmonious balance with the host periodontal tissues. This shift in therapeutic goals and the drug resistance challenge call for alternative approaches to antibiotic therapy that indiscriminately eliminate harmful or beneficial bacteria. In this review, we summarize the recent advancement of alternative methods and new compounds that offer promising potential for the treatment and prevention of periodontal disease. Agents that target biofilm formation, bacterial quorum-sensing systems and other virulence factors have been reviewed. New and exciting microbiome approaches, such as oral microbiota replacement therapy and probiotic therapy for periodontal disease, are also discussed

Luke\u27s first eschatological discourse: Form, function and contextual integration of Luke 12:1-13:9

This study employs narrative and rhetorical criticism in addition to the standard historical-critical methods to address one section of Luke\u27s extensive account of Jesus\u27 final journey to Jerusalem. A satisfying explanation of the kerygmatic function of this evangelist\u27s unusually lengthy account of Jesus\u27 last journey has never been presented. The portion of the journey under close examination, Luke 12:1-13:9, is shown to have been carefully integrated into an eschatological discourse. Comparison of this section with two later eschatological discourses in the journey account demonstrates that all three discourses were edited in light of one another. Analysis of the contents of this series of discourses suggests that the writer presented a progressive disclosure of Jesus\u27 teachings about eschatological matters in a serial fashion. Each of the discourses is distinguished by its emphasis on certain aspects of eschatology, but one may also recognize a progressively more explicit instruction through the course of the series on themes which all three discourses address in common. The impression of a systematic thematic development in the gospel is bolstered by the recognition that the discourses are found at regular intervals in the gospel. This led to the question of whether the entire journey account gives evidence of a similar structuring pattern, based on a series of themes which are introduced, and developed further when they are revisited. A preliminary survey of the entire journey account suggested that such a line of inquiry was well warranted, and provided a number of observations of thematic patterns which call for more detailed study. A review of Luke\u27s redactional devices which provide narrative continuity intimated that the evangelist\u27s practice of arranging his sources into thematic groupings or patterns was not limited to the journey account, but was conferred upon his account of Jesus\u27 Galilean ministry as well. These observations compelled the conclusion that future commentaries on the Gospel of Luke must include narrative-critical and rhetorical-critical perspectives if they intend to fully disclose its kerygmatic content

The regeneration of an urban corridor, enriching the experience of a highway strip at the city's edge

In The Granite Garden, Anne Spirn envisions a future city where "The paths on which people move into and through the city are designed with care. Every street and highway, and every other transportation corridor, is designed for efficient movement, for pleasure, and as an asset to the neighborhood through which it moves" (Spirn 1984, 270). This practicum provides an examination of one of those "paths on which people move into and through the city", a Winnipeg traffic corridors, Pembina Highway, between the Killarney Ave., Kirkbridge Dr. Intersection, and the Perimeter Highway. The practicum looks at what traffic corridors are, what factors have contributed to their present structure, appearance, and character, and what can be done to improve the experience both of being in the corridor and of moving through it. The focus of the proposal is on community liveability and the pedestrian environment. The practicum begins by reviewing various historic and current factors which have influenced the evolution of cities and streets. The corridor, Pembina Highway and its context are analyzed, and factors which affect the use of the site are examined. The results of a literature review are presented in an overview of current design ideas and issues associated with streets, cities, and neighborhoods. Issues identified by the research are used to generate a functional, cohesive, and aesthetically pleasing design for the site. Plant material, pedestrian walkways, open space, bicycle paths, signage, and lighting are all part of the requirements for an attractive, more human scale street environment that reflects the character and identity of the community, provides opportunities for people to move easily through the area, and is supportive of local businesses. This design presents the Pembina Highway traffic corridor as a meaningful and vital urban place as well as an important gateway to Winnipeg
