A Cool and Inflated Progenitor Candidate for the Type Ib Supernova 2019yvr at 2.6 Years Before Explosion
We present Hubble Space Telescope imaging of a pre-explosion counterpart to
SN 2019yvr obtained 2.6 years before its explosion as a type Ib supernova (SN
Ib). Aligning to a post-explosion Gemini-S/GSAOI image, we demonstrate that
there is a single source consistent with being the SN 2019yvr progenitor
system, the second SN Ib progenitor candidate after iPTF13bvn. We also analyzed
pre-explosion Spitzer/IRAC imaging, but we do not detect any counterparts at
the SN location. SN 2019yvr was highly reddened, and comparing its spectra and
photometry to those of other, less extinguished SNe Ib we derive
mag for SN 2019yvr. Correcting photometry
of the pre-explosion source for dust reddening, we determine that this source
is consistent with a and K star. This relatively cool photospheric
temperature implies a radius of 320, much larger
than expectations for SN Ib progenitor stars with trace amounts of hydrogen but
in agreement with previously identified SN IIb progenitor systems. The
photometry of the system is also consistent with binary star models that
undergo common envelope evolution, leading to a primary star hydrogen envelope
mass that is mostly depleted but seemingly in conflict with the SN Ib
classification of SN 2019yvr. SN 2019yvr had signatures of strong circumstellar
interaction in late-time (150 day) spectra and imaging, and so we consider
eruptive mass loss and common envelope evolution scenarios that explain the SN
Ib spectroscopic class, pre-explosion counterpart, and dense circumstellar
material. We also hypothesize that the apparent inflation could be caused by a
quasi-photosphere formed in an extended, low-density envelope or circumstellar

SN2023ixf in Messier 101: the twilight years of the progenitor as seen by Pan-STARRS
The nearby type II supernova, SN2023ixf in M101 exhibits signatures of
early-time interaction with circumstellar material in the first week
post-explosion. This material may be the consequence of prior mass loss
suffered by the progenitor which possibly manifested in the form of a
detectable pre-supernova outburst. We present an analysis of the long-baseline
pre-explosion photometric data in , , , , and filters from
Pan-STARRS as part of the Young Supernova Experiment, spanning 5,000
days. We find no significant detections in the Pan-STARRS pre-explosion light
curve. We train a multilayer perceptron neural network to classify
pre-supernova outbursts. We find no evidence of eruptive pre-supernova activity
to a limiting absolute magnitude of . The limiting magnitudes from the full
set of (average absolute magnitude -8) data are consistent
with previous pre-explosion studies. We use deep photometry from the literature
to constrain the progenitor of SN2023ixf, finding that these data are
consistent with a dusty red supergiant (RSG) progenitor with luminosity
5.12 and temperature 3950K,

Vitamin D and oestrogen receptor polymorphisms in developmental dysplasia of the hip and primary protrusio acetabuli – A preliminary study
We investigated the association of developmental dysplasia of the hip (DDH) and primary protrusion acetabuli (PPA) with Vitamin D receptor polymorphisms Taq I and Fok I and oestrogen receptor polymorphisms Pvu II and Xba I. 45 patients with DDH and 20 patients with PPA were included in the study. Healthy controls (n = 101) aged 18–60 years were recruited from the same geographical area. The control subjects had a normal acetabular morphology based on a recent pelvic radiograph performed for an unrelated cause. DNA was obtained from all the subjects from peripheral blood. Genotype frequencies were compared in the three groups. The relationship between the genotype and morphology of the hip joint, severity of the disease, age at onset of disease and gender were examined. The oestrogen receptor Xba I wild-type genotype (XX, compared with Xx and xx combined) was more common in the DDH group (55.8%) than controls (37.9%), though this just failed to achieve statistical significance (p = 0.053, odds ratio = 2.1, 95% CI = 0.9–4.6). In the DDH group, homozygosity for the mutant Taq I Vitamin D receptor t allele was associated with higher acetabular index (Mann-Whitney U-test, p = 0.03). Pvu II pp oestrogen receptor genotype was associated with low centre edge angle (p = 0.07). This study suggests a possible correlation between gene polymorphism in the oestrogen and vitamin D receptors and susceptibility to, and severity of DDH. The Taq I vitamin D receptor polymorphisms may be associated with abnormal acetabular morphology leading to DDH while the Xba I oestrogen receptor XX genotype may be associated with increased risk of developing DDH. No such correlations were found in the group with PPA
Effects of fluoxetine on functional outcomes after acute stroke (FOCUS): a pragmatic, double-blind, randomised, controlled trial
Background
Results of small trials indicate that fluoxetine might improve functional outcomes after stroke. The FOCUS trial aimed to provide a precise estimate of these effects.
Methods
FOCUS was a pragmatic, multicentre, parallel group, double-blind, randomised, placebo-controlled trial done at 103 hospitals in the UK. Patients were eligible if they were aged 18 years or older, had a clinical stroke diagnosis, were enrolled and randomly assigned between 2 days and 15 days after onset, and had focal neurological deficits. Patients were randomly allocated fluoxetine 20 mg or matching placebo orally once daily for 6 months via a web-based system by use of a minimisation algorithm. The primary outcome was functional status, measured with the modified Rankin Scale (mRS), at 6 months. Patients, carers, health-care staff, and the trial team were masked to treatment allocation. Functional status was assessed at 6 months and 12 months after randomisation. Patients were analysed according to their treatment allocation. This trial is registered with the ISRCTN registry, number ISRCTN83290762.
Findings
Between Sept 10, 2012, and March 31, 2017, 3127 patients were recruited. 1564 patients were allocated fluoxetine and 1563 allocated placebo. mRS data at 6 months were available for 1553 (99·3%) patients in each treatment group. The distribution across mRS categories at 6 months was similar in the fluoxetine and placebo groups (common odds ratio adjusted for minimisation variables 0·951 [95% CI 0·839–1·079]; p=0·439). Patients allocated fluoxetine were less likely than those allocated placebo to develop new depression by 6 months (210 [13·43%] patients vs 269 [17·21%]; difference 3·78% [95% CI 1·26–6·30]; p=0·0033), but they had more bone fractures (45 [2·88%] vs 23 [1·47%]; difference 1·41% [95% CI 0·38–2·43]; p=0·0070). There were no significant differences in any other event at 6 or 12 months.
Interpretation
Fluoxetine 20 mg given daily for 6 months after acute stroke does not seem to improve functional outcomes. Although the treatment reduced the occurrence of depression, it increased the frequency of bone fractures. These results do not support the routine use of fluoxetine either for the prevention of post-stroke depression or to promote recovery of function.

General anaesthetic and airway management practice for obstetric surgery in England: a prospective, multi-centre observational study
There are no current descriptions of general anaesthesia characteristics for obstetric surgery, despite recent changes to patient baseline characteristics and airway management guidelines. This analysis of data from the direct reporting of awareness in maternity patients' (DREAMY) study of accidental awareness during obstetric anaesthesia aimed to describe practice for obstetric general anaesthesia in England and compare with earlier surveys and best-practice recommendations. Consenting patients who received general anaesthesia for obstetric surgery in 72 hospitals from May 2017 to August 2018 were included. Baseline characteristics, airway management, anaesthetic techniques and major complications were collected. Descriptive analysis, binary logistic regression modelling and comparisons with earlier data were conducted. Data were collected from 3117 procedures, including 2554 (81.9%) caesarean deliveries. Thiopental was the induction drug in 1649 (52.9%) patients, compared with propofol in 1419 (45.5%). Suxamethonium was the neuromuscular blocking drug for tracheal intubation in 2631 (86.1%), compared with rocuronium in 367 (11.8%). Difficult tracheal intubation was reported in 1 in 19 (95%CI 1 in 16-22) and failed intubation in 1 in 312 (95%CI 1 in 169-667). Obese patients were over-represented compared with national baselines and associated with difficult, but not failed intubation. There was more evidence of change in practice for induction drugs (increased use of propofol) than neuromuscular blocking drugs (suxamethonium remains the most popular). There was evidence of improvement in practice, with increased monitoring and reversal of neuromuscular blockade (although this remains suboptimal). Despite a high risk of difficult intubation in this population, videolaryngoscopy was rarely used (1.9%)
Building a Theory of Multi-Media CMC: An Analysis, Critique and Integration of Computer-Mediated Communication Theory and Research
In order to provide directions for future computer-mediated communication (CMC) scholarship, in this article, I analyze, critique and integrate contemporary CMC theory and research. Particularly, based upon an analysis of recent developments in multi-media software and the world wide web, I explore the theoretical implications of increased audio, video and three-dimensionality in cyberspace. In general, in this article, I argue that CMC theory and research has been limited by the `textual bias' of previous scholars. CMC researchers and theorists must begin to reconstruct the communicative, rhetorical and epistemological features of multi-media CMC in order to describe and explain communication in cyberspace.
