    A randomized controlled trial on the comparative effectiveness of mindfulness-based cognitive therapy and health qigong-based cognitive therapy among Chinese people with depression and anxiety disorders

    Background: The goal of this study was to investigate treatment outcome and related intervention processes of mindfulness-based cognitive therapy versus health qigong-based cognitive therapy versus waitlist control among individuals with mood disorders. Methods: A total of 187 individuals with mood disorders were randomized and allocated into mindfulness-based cognitive therapy, health qigong-based cognitive therapy, or waitlist control groups. All participants were assessed at three time points with regard to depressive and anxiety symptoms, physical and mental health status, perceived stress, sleep quality, and self-efficacy. Linear mixed models analysis was used to test the individual growth model by studying the longitudinal data. Results: Mindfulness-based cognitive therapy and health qigong-based cognitive therapy both produced greater improvements on all outcome measures as compared with waitlist control. Relatively, more reductions of mood symptoms were observed in the health qigong-based cognitive therapy group as compared with the mindfulness-based cognitive therapy group. Health qigong-based cognitive therapy is more conducive to physical health status whereas mindfulness-based cognitive therapy has more favorable mental health outcomes. Individual growth curve models indicated that alterations in perceived stress was the common predictor of mood changes in both intervention groups. Conclusions: The predominant emphasis on physical health in health qigong-based cognitive therapy makes it more acceptable and effective than mindfulness-based cognitive therapy as applied in Chinese individuals with mood disorders.     Influenza H5N1 and H1N1 Virus Replication and Innate Immune Responses in Bronchial Epithelial Cells Are Influenced by the State of Differentiation

    Influenza H5N1 virus continues to be enzootic in poultry and transmits zoonotically to humans. Although a swine-origin H1N1 virus has emerged to become pandemic, its virulence for humans remains modest in comparison to that seen in zoonotic H5N1 disease. As human respiratory epithelium is the primary target cells for influenza viruses, elucidating the viral tropism and host innate immune responses of influenza H5N1 virus in human bronchial epithelium may help to understand the pathogenesis. Here we established primary culture of undifferentiated and well differentiated normal human bronchial epithelial (NHBE) cells and infected with highly pathogenic influenza H5N1 virus (A/Vietnam/3046/2004) and a seasonal influenza H1N1 virus (A/Hong Kong/54/1998), the viral replication kinetics and cytokine and chemokine responses were compared by qPCR and ELISA. We found that the in vitro culture of the well differentiated NHBE cells acquired the physiological properties of normal human bronchi tissue which express high level of α2-6-linked sialic acid receptors and human airway trypsin-like (HAT) protease, in contrast to the low expression in the non-differentiated NHBE cells. When compared to H1N1 virus, the H5N1 virus replicated more efficiently and induced a stronger type I interferon response in the undifferentiated NHBE cells. In contrast, in well differentiated cultures, H5N1 virus replication was less efficient and elicited a lower interferon-beta response in comparison with H1N1 virus. Our data suggest that the differentiation of bronchial epithelial cells has a major influence in cells' permissiveness to human H1N1 and avian H5N1 viruses and the host innate immune responses. The reduced virus replication efficiency partially accounts for the lower interferon-beta responses in influenza H5N1 virus infected well differentiated NHBE cells. Since influenza infection in the bronchial epithelium will lead to tissue damage and associate with the epithelium regeneration, the data generated from the undifferentiated NHBE cultures may also be relevant to disease pathogenesis

    Implementing an eleven year through-train model to complete Primary and Secondary Education , is it possible? Why Not? : some challenges and principles

    As learning and teaching is moving away from imparting student with mere "knowledge", the simple one-size-fits-all solution of a prescribed years of schooling with some rigid and prescriptive subject syllabuses designated for different year-levels in primary and secondary schools will no longer meet the developmental needs of students. Logos Academy of Hong Kong has started an "Eleven-year Through-train Program in September 2002, to re-define the different key stages in primary, junior and senior secondary levels to provide a broad and balanced curriculum which maintains seamless continuity. The eleven-year program consists of three stages, each with its particular characteristics: Foundation Stage: (FS1- FS3); Developmental Stage: (DS1 - DS5) and Mastery Stage (MS1 - MS3). We have achieved some pleasing outcome so far and we believe that this re-definition of Key Learning Stages is forward looking and keeping abreast of global trends. If this "Eleven-year Through-Train Schooling System" model is proved to be successful, it will throw some light on a new schooling structure - which will have significant implications on the government's funding and planning policie

    In educational transformation, Logos Academy of Hong Kong has started to create space in two aspects: to accommodate for new learning areas, and to use the most updated technologies for learning. In different Learning Stages, new learning areas like "Family Life Education", "Analytical study of Current Issues", Mind-mapping, MegaSkills and Media Education are introduced. The teachers will design different level- and age-appropriate activities and assignments that encourage the mastery of basic concepts and development of aesthetic appreciation, family life education, character formation, physique building and inquiry/research skills. Moreover, integrated tasks and projects intertwining with different study skills are mounted to enable the children to experiment creative designs and try out increasingly complex investigations. To facilitate learning and teaching, Logos Academy also creates new platforms to use the newest technologies for pre-lesson use, for lesson use, and for post-lesson use. It is reviewed that with the aid of some updated technologies, our teachers are committed to facilitate change, reflect on current practices, explore further improvements in new learning areas and to use the new technologies effectively - which will in turn enhance the effectiveness of integrated study skills, self-directed learning, team work and social interaction of the students

    In an eleven year "through-train" model, to construct a new road map for learning, Logos Academy of Hong Kong has delineated clearly the roles of "Homeroom Teachers" and "Subject Specialists". In the Foundation Stage (The first three years in Primary Schooling), the "Homeroom Teachers" will no longer teach most of the academic subjects for their respective Homeroom classes. They will undertake mainly pastoral care functions whilst different subject specialists are deployed to teach different subject areas accordingly. Each Subject teacher will teach ALL the classes within a year-band. In some Subjects like English Studies, two or three teachers will share the teaching load according to their specialties. After putting in practice for two years, evidence has shown that with this "Subject specialist across the year band" approach, the curriculum rigor has been strengthened and children have made much more remarkable progress in specific learning areas. Moreover, it has created space and opportunities for co-teaching and joint projects. This has in turn facilitated communication, collaboration and professional development of teachers in their subject specialty. Within the same subject area, the inter-teacher difference between classes of the same year level has been diminished, and the effectiveness of teaching and learning across the whole year-band may be better monitored and evaluated.     A Prospective Study of Aspirin Use and the Risk of Gastrointestinal Bleeding in Men

    Data regarding the influence of dose and duration of aspirin use on risk of gastrointestinal bleeding are conflicting.We conducted a prospective cohort study of 32,989 men enrolled in the Health Professionals Follow-up Study (HPFS) in 1994 who provided biennial aspirin data. We estimated relative risk of major gastrointestinal bleeding requiring hospitalization or a blood transfusion.During 14 years of follow-up, 707 men reported an episode of major gastrointestinal bleeding over 377,231 person-years. After adjusting for risk factors, regular aspirin use (≥2 times/week) had a multivariate relative risk (RR) of gastrointestinal bleeding of 1.32 (95% confidence interval [CI], 1.12-1.55) compared to non-regular use. The association was particularly evident for upper gastrointestinal bleeding (multivariate RR, 1.49; 95% CI, 1.16-1.92). Compared to men who denied any aspirin use, multivariate RRs of upper gastrointestinal bleeding were 1.05 (95% CI 0.71-1.52) for men who used 0.5-1.5 standard tablets/week, 1.31 (95% CI 0.88-1.95) for 2-5 aspirin/week, 1.63 (95% CI, 1.15-2.32) for 6-14 aspirin/week and 2.40 (95% CI, 1.10-5.22) for >14 aspirin/week (P(trend)<0.001). The relative risk also appeared to be dose-dependent among short-term users <5 years; P(trend)<.001) and long-term users (≥5 years; P(trend) = 0.015). In contrast, after controlling for dose, increasing duration of use did not appear to be associated with risk (P(trend) = 0.749).Regular aspirin use increases the risk of gastrointestinal bleeding, especially from the upper tract. However, risk of bleeding appears to be more strongly related to dose than to duration of use. Risk of bleeding should be minimized by using the lowest effective dose among short-term and long-term aspirin users

    Reproducibility of the mfERG between instruments

    Purpose First, to examine both the reproducibility of the multifocal electroretinogram (mfERG) recorded on different versions of the same instrument, and the repeatability of the mfERG recorded on a single instrument using two different amplifiers. Second, to demonstrate a means by which multicenter and longitudinal studies that use more than one recording instrument can compare and combine data effectively. Methods Three different amplifiers and two mfERG setups, one using VERIS™ 4.3 software (mfERG1) and another using VERIS™ Pro 5.2 software (mfERG2), were evaluated. A total of 73 subjects with normal vision were tested in three groups. Group 1 (n = 42) was recorded using two amplifiers in parallel on mfERG1. Group 2 (n = 52) was recorded on mfERG2 using a single amplifier. Group 3 was a subgroup of 21 subjects from groups 1 and 2 that were tested sequentially on both instruments. A fourth group of 26 subjects with diabetes were also recorded using the two parallel amplifiers on mfERG1. P1 implicit times and N1-P1 amplitudes of the 103 local first order mfERGs were measured, and the differences between the instruments and amplifiers were evaluated as raw scores and Z-scores based on normative data. Measurements of individual responses and measurements averaged over the 103 responses were analyzed. Results Simultaneous recordings made on mfERG1 with the two different amplifiers showed differences in implicit times but similar amplitudes. There was a mean implicit time difference of 2.5 ms between the amplifiers but conversion to Z-scores improved their agreement. Recordings made on different days with the two instruments produced similar but more variable results, with amplitudes differing between them more than implicit times. For local response implicit times, the 95% confidence interval of the difference between instruments was approximately ±1 Z-score (±0.9 ms) in either direction. For local response amplitude, it was approximately ±1.6 Z-scores (±0.3 μV). Conclusions Different amplifiers can yield quite different mfERG P1 implicit times, even with identical band-pass settings. However, the reproducibility of mfERG Z-scores across recording instrumentation is relatively high. Comparison of data across systems and laboratories, necessary for multicenter or longitudinal investigations, is facilitated if raw data are converted into Z-scores based on normative data
