    Car-following techniques: reconsidering the role of the human factor

    Keeping correct distance between vehicles is a fundamental tenet in road traffic. New road signs and markings appearing on motorways aid drivers in determining this distance. However, the ‘Nagoya experiment’ (Sugiyama et al., 2008) revealed correct distance made following safe while also eventually destabilizing traffic flow. When traffic becomes dense, most drivers keep the minimum safety distance and brake when the vehicle ahead decelerates. The resultant chain reaction along the entire line of closely following vehicles causes for no apparent reason a traffic stoppage, known as a ‘phantom’ or ‘shockwave’ jam. The car-following models of Sugiyama et al. found certain speeds, traffic densities, and inter-vehicular distances combined to congest traffic. Drawing upon these and other phenomena (e.g., wave movement in Nature), car following by Driving to keep Inertia (DI) was conceived by us as an alternative to Driving to keep Distance (DD). Three studies explored possible prevention of ‘phantom’ jams by adopting DI. Using a driving simulator, affective and behavioural measures were taken (N=113). The results comparing the efficiency of DI vs. DD are summarized. DI promoted a more stable driver trajectory, in cognitive-affective and behavioural terms, and lowered fuel consumption by about 20%

    Spanish validation of the Internet Gaming Disorder-20 (IGD-20) Test

    In recent years, problematic and addictive gaming has been a phenomenon of growing concern worldwide. In light of the increasing awareness about this issue, the latest (fifth) edition of the American Psychiatric Association’s Diagnostic and Statistical Manual of Mental Disorders (DSM-5) included Internet Gaming Disorder (IGD) as an area in need of more empirical research. The Internet Gaming Disorder Test (IGD-20 Test) was developed as a valid and reliable tool to assess IGD. The aim of the present study was to validate the Spanish version of the IGD-20 Test, and analyze the different profiles found among a sample of 1,074 Spanish-speaking gamers. A confirmatory factor analysis showed the validity of the Spanish version of the IGD-20 Test and its six factor structure (i.e., salience, mood modification, tolerance, withdrawal, conflict and relapse). The latent profile analysis (LPA) showed five different gamer classes. The 'disordered gamers’ class comprised 2.6% of the participants. Based on this class, sensitivity and specificity analyses showed an adequate empirical cut-off point of 75 (out of 100). It is concluded that the Spanish version of the IGD-20 Test is valid and reliable and can be used in research into IGD among Spanish speaking populations

    Can disordered mobile phone use be considered a behavioral addiction? An update on current evidence and a comprehensive model for future research

    Despite the many positive outcomes, excessive mobile phone use is now often associated with potentially harmful and/or disturbing behaviors (e.g., symptoms of deregulated use, negative impact on various aspects of daily life such as relationship problems, and work intrusion). Problematic mobile phone use (PMPU) has generally been considered as a behavioral addiction that shares many features with more established drug addictions. In light of the most recent data, the current paper reviews the validity of the behavioral addiction model when applied to PMPU. On the whole, it is argued that the evidence supporting PMPU as an addictive behavior is scarce. In particular, it lacks studies that definitively show behavioral and neurobiological similarities between mobile phone addiction and other types of legitimate addictive behaviors. Given this context, an integrative pathway model is proposed that aims to provide a theoretical framework to guide future research in the field of PMPU.     El desarrollo de ascitis moderada o severa es infrecuente tras una enfermedad inflamatoria pélvica por Chlamydia trachomatis, una de las principales causas de infección de transmisión sexual a nivel mundial. Caso clínico: Paciente de 29 años que tras aborto diferido (gestación tras inseminación artificial) que inicia a las seis semanas con cuadro de dolor abdominal inespecífico y ascitis de predominio linfocitario. El diagnostico se realizo mediante PCR (Werfen®) tanto el liquido ascítico como en exudado endocervical. La paciente recibió tratamiento antibiótico con doxiciclina. Conclusión: Las enfermedades de transmisión sexual deben ser consideradas cuando se realiza un diagnóstico diferencial de una mujer sexualmente activa con dolor abdominal y ascitis, instaurar tratamiento antibiótico y evitar pruebas e intervenciones quirúrgicas innecesarias. The development of moderate or severe ascites is infrequent after a pelvic inflammatory disease from Chlamydia trachomatis, one of the main causes of sexually transmitted infection worldwide. Clinical case: A 29-year-old patient who, after a delayed abortion (gestation after artificial insemination), started at six weeks with symptoms of non-specific abdominal pain and predominantly lymphocytic ascites. The diagnosis is made by PCR (Werfen®) both the ascitic fluid and the endocervical exudate. The patient received antibiotic treatment with doxycycline. Conclusion: Sexually transmitted diseases should be considered when making a differential diagnosis of a sexually activated woman with abdominal pain and ascites. Establishing antibiotic treatment, and avoiding unnecessary tests and surgical treatments

    Mastectomía ahorradora de piel y pezón en carcinoma ductal in situ. Seguridad oncológica a 10 años

    Objetivo El objetivo es evaluar la seguridad oncológica a 10 años de la mastectomía ahorradora de piel y pezón (MAP) en pacientes con carcinoma ductal in situ (CDIS). Método Análisis observacional retrospectivo. Se realizaron 35 MAP en pacientes con CDIS durante 2005-2018. Evaluamos resultados histológicos, oncológicos y de morbilidad. Resultados Las indicaciones más frecuentes fueron márgenes afectos tras tumorectomía (31, 5%), multifocalidad/multicéntricidad (22, 8%), tumor >3 cm (8, 6%) correlación desfavorable tamaño tumoral/mama (8, 6%) y decisión de la paciente (8, 6%). La técnica más usada fue incisión lateral externa en 11 pacientes, seguida de técnica de Spira en nueve casos. La presencia de CDIS se confirmó en 22 pacientes y en 11 no se encontró tumor en la pieza de mastectomía. La tasa de complicaciones fue 22, 8%. Tras una mediana de seguimiento de 104 meses (DE 69, 9) no se observó necrosis del pezón. Un 20% de pacientes precisó reintervención a largo plazo. Once pacientes (31, 4%) recibieron tratamiento adyuvante (QT y/o RT). Solamente una paciente presentó recurrencia local (2, 8%) 28 meses tras la intervención. Una paciente presentó metástasis tras 78 meses de SLE. Las tasas de SLE y SG fueron 94, 3% y 97, 22%. El análisis univariante mostró dos factores de riesgo de recurrencia: edad <40 [OR (IC95) 2, 529 (1, 230 - 5, 199)] y márgenes afectos [OR (IC95) 5, 242 (2, 041 - 13, 464)]. Conclusión La MAP es factible y segura en pacientes con CDIS no candidatas a cirugía conservadora. Objective: The aim of this study was to assess the oncological safety of nipple-sparing mastectomy (NSM) in patients with ductal in situ carcinoma (DCIS) over a 10-year period. Method: Retrospective observational analysis. A total of 35 NSM were performed in patients with DCIS from 2005 - 2018. We assessed the histological, oncological and morbidity results. Results: The most frequent indications were margin involvement after lumpectomy (31.5%), multifocality / multicentricity (22.8%), tumour size >3 cm (8.6%) unfavourable tumour / breast size correlation (8.6%) and patient choice (8.6%). The most commonly used technique was external lateral incision in 11 patients, followed by the Spira technique in 9 patients. DCIS was confirmed in 22 patients and no tumour was found in mastectomy specimen in 11 patients. The complication rate was 22.8%. After a median follow-up of 104 months (SD 69.9) there was no nipple necrosis. In all, 20% of the patients required long-term reintervention. Eleven patients (31.4%) underwent adjuvant treatment (chemotherapy and / or radiotherapy). Only one patient showed local recurrence (2.8%) 28 months after the intervention. One patient developed metastases after 78 months of disease-free survival (DFS). DFS and overall survival rates were 94.3% and 97.22%. Univariate analysis showed two risk factors for recurrence: age <40 years [OR (95% CI) 2.529 (1.230-5.199)] and margin involvement [OR (95% CI) 5.242 (2.041 - 13.464)]. Conclusion: NSM is safe and feasible in patients with DCIS who are not candidates for conservative surgery

    Cosmogenic production of tritium in dark matter detectors

    The direct detection of dark matter particles requires ultra-low background conditions at energies below a few tens of keV. Radioactive isotopes are produced via cosmogenic activation in detectors and other materials and those isotopes constitute a background source which has to be under control. In particular, tritium is specially relevant due to its decay properties (very low endpoint energy and long half-life) when induced in the detector medium, and because it can be generated in any material as a spallation product. Quantification of cosmogenic production of tritium is not straightforward, neither experimentally nor by calculations. In this work, a method for the calculation of production rates at sea level has been developed and applied to some of the materials typically used as targets in dark matter detectors (germanium, sodium iodide, argon and neon); it is based on a selected description of tritium production cross sections over the entire energy range of cosmic nucleons. Results have been compared to available data in the literature, either based on other calculations or from measurements. The obtained tritium production rates, ranging from a few tens to a few hundreds of nuclei per kg and per day at sea level, point to a significant contribution to the background in dark matter experiments, requiring the application of specific protocols for target material purification, material storing underground and limiting the time the detector is on surface during the building process in order to minimize the exposure to the most dangerous cosmic ray components

    Electronic gaming machine characteristics: it's the little things that count

    A range of gamblers, from low-frequency social gamblers through to problem gamblers in treatment, participated in focus groups discussing the characteristics of Electronic Gaming Machines (EGMs) that they found attractive. Analyses of the resulting transcripts resulted in two groups of EGM characteristics being identified as important, one group associated with winning and one with betting. Overall, free spin features were identified in all groups as the most attractive characteristic of EGMS. Beyond that it was smaller win-related characteristics, and low-denomination machines with multiple playable lines that were associated with increased duration and intensity of gambling behaviour. The important characteristics were consistent across different levels of gamblers, with the key behavioural difference being a self-reported ‘expertise’, and ‘strategic’ approach to gambling amongst higher-frequency gamblers and problem gamblers in treatment. The key characteristics all occur frequently and result in more wins and extended gambling sessions. The patterns identified resonated with established behavioural principles, and with models describing the development of problem gambling and addictions more generally

    Mixed emotions to near-miss outcomes: a psychophysiological study with facial electromyography

    Near-misses occur across many forms of gambling and are rated as unpleasant while simultaneously increasing the motivation to continue playing. On slot machines, the icon position relative to the payline moderates the effects of near-misses, with near-misses before the payline increasing motivation, and near-misses after the payline being rated as aversive. Near-misses are also known to increase physiological arousal compared to full-misses, but physiological measures to date have not been able to dissociate positive and negative emotional responses. The present study measured facial electromyography at the corrugator (brow) and zygomaticus (cheek) sites, as well as electrodermal activity (EDA), following gambling outcomes on a two-reel slot machine simulation in 77 novice gamblers. Behavioral data was collected using trial-by-trial ratings of motivation and valence. Wins were rated as more pleasant and increased motivation to continue playing, compared to non-win outcomes. Wins were also accompanied by increased EDA and zygomaticus activity. Near-misses after the payline were rated as more aversive than other non-wins, and this was accompanied by increased EDA and zygomaticus activity. Near-misses before the payline increased motivation to continue playing, and were accompanied by increased EDA. Thus, both subjective and physiological responses to near-misses differ for events falling either side of the payline. The ‘near-miss effect’ is not a unitary phenomenon. Facial EMG has differential sensitivity to positive and negative valence and may be a useful measure for future studies of gambling behavior

    Problematic mobile phone use and smartphone addiction across generations: the roles of psychopathological symptoms and smartphone use

    Contemporary technological advances have led to a significant increase in using mobile technologies. Recent research has pointed to potential problems as a consequence of mobile overuse, including addiction, financial problems, dangerous use (i.e. whilst driving) and prohibited use (i.e. use in forbidden areas). The aim of this study is to extend previous findings regarding the predictive power of psychopathological symptoms (depression, anxiety and stress), mobile phone use (i.e. calls, SMS, time spent on the phone, as well as the engagement in specific smartphone activities) across Generations X and Y on problematic mobile phone use in a sample of 273 adults. Findings revealed prohibited use and dependence were predicted by calls/day, time on the phone and using social media. Only for dependent mobile phone use (rather than prohibited), stress appeared as significant. Using social media and anxiety significantly predicted belonging to Generation Y, with calls per day predicted belonging to Generation X. This finding suggests Generation Y are more likely to use asynchronous social media-based communication, whereas Generation X engage more in synchronous communication. The findings have implications for prevention and awareness-raising efforts of possibly problematic mobile phone use for educators, parents and individuals, particularly including dependence and prohibited use

    First results on the search for chameleons with the KWISP detector at CAST

    We report on a first measurement with a sensitive opto-mechanical force sensor designed for the direct detection of coupling of real chameleons to matter. These dark energy candidates could be produced in the Sun and stream unimpeded to Earth. The KWISP detector installed on the CAST axion search experiment at CERN looks for tiny displacements of a thin membrane caused by the mechanical effect of solar chameleons. The displacements are detected by a Michelson interferometer with a homodyne readout scheme. The sensor benefits from the focusing action of the ABRIXAS X-ray telescope installed at CAST, which increases the chameleon flux on the membrane. A mechanical chopper placed between the telescope output and the detector modulates the incoming chameleon stream. We present the results of the solar chameleon measurements taken at CAST in July 2017, setting an upper bound on the force acting on the membrane of 80pN at 95% confidence level. The detector is sensitive for direct coupling to matter 104 = ßm = 108, where the coupling to photons is locally bound to ß¿ = 1011
