    Understanding the impact of line-of-sight in the ergodic spectral efficiency of cellular networks

    In this paper we investigate the impact of lineof-sight (LoS) condition in the ergodic spectral efficiency of cellular networks. To achieve this goal, we have considered the kappa-mu shadowed model, which is a general model that provides an excellent fit to a wide set of propagation conditions. To overcome the mathematical complexity of the analysis, we have split the analysis between large and small-scale effects. Building on the proposed framework, we study a number of scenarios that range from heavily-fluctuating LoS to deterministic-LoS.     Comparison of Extracellular Vesicle Isolation Methods for miRNA Sequencing

    MicroRNAs (miRNAs) encapsulated in extracellular vesicles (EVs) are potential diagnostic and prognostic biomarkers. However, discrepancies in miRNA patterns and their validation are still frequent due to differences in sample origin, EV isolation, and miRNA sequencing methods. The aim of the present study is to find a reliable EV isolation method for miRNA sequencing, adequate for clinical application. To this aim, two comparative studies were performed in parallel with the same human plasma sample: (i) isolation and characterization of EVs obtained using three procedures: size exclusion chromatography (SEC), iodixanol gradient (GRAD), and its combination (SEC+GRAD) and (ii) evaluation of the yield of miRNA sequences obtained using NextSeq 500 (Illumina) and three miRNA library preparation protocols: NEBNext, NEXTFlex, and SMARTer smRNA-seq. The conclusion of comparison (i) is that recovery of the largest amount of EVs and reproducibility were attained with SEC, but GRAD and SEC+GRAD yielded purer EV preparations. The conclusion of (ii) is that the NEBNext library showed the highest reproducibility in the number of miRNAs recovered and the highest diversity of miRNAs. These results render the combination of GRAD EV isolation and NEBNext library preparation for miRNA retrieval as adequate for clinical applications using plasma samples

    Three double-dose reinforced hepatitis B revaccination scheme for patients with cirrhosis unresponsive to the standard regimen: an open-label randomised clinical trial

Published by BMJ.[Objective] We aimed to compare the response rates between two different hepatitis B virus vaccination schedules for cirrhotic subjects who were non-responders to the first three 40 µg doses (month 0-1-2), and identify factors associated with the final response.[Design] A total of 120 cirrhotic patients (72.5% decompensated) were randomised at a 1:1 ratio to receive a single 40 µg booster vaccination at month 6 (classical arm) versus an additional round of three new 40 µg doses administered at monthly intervals (experimental arm). The main outcome was the rate of postvaccinal anti-hepatitis B surface antibodies levels ≥10 mIU/mL.[Results] Efficacy by ITT analysis was higher in the experimental arm (46.7%) than in the classical one (25%); OR 2.63, p=0.013. The experimental arm increased response rates compared with the classical one from 31% to 68% (OR 4.72; p=0.007), from 24.4% to 50% (OR 3.09; p=0.012) and from 24.4% to 53.8% (OR 3.62; p=0.007), in Child A, Model for End-Stage Liver Disease (MELD) <15 and MELD-Na<15 patients, respectively. Patients with more advanced liver disease did not benefit from the reinforced scheme. Both regimens showed similar safety profiles. Multivariable analysis showed that the experimental treatment was independently response associated when adjusted across three logistic regression models indicating equivalent cirrhosis severity.[Conclusion] For cirrhotic patients, the revaccination of non-responders to the first three dose cycle, with three additional 40 µg doses, achieved significantly better response rates to those obtained with an isolated 40 µg booster dose.[Trial registration number] NCT01884415.This study project was awarded competitive public funding for Independent Research from the Spanish Government Ministry of Health and Social Politics in 2011 via the Carlos III Health Institute (EC11-452).     Clonal chromosomal mosaicism and loss of chromosome Y in elderly men increase vulnerability for SARS-CoV-2

    The pandemic caused by severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2, COVID-19) had an estimated overall case fatality ratio of 1.38% (pre-vaccination), being 53% higher in males and increasing exponentially with age. Among 9578 individuals diagnosed with COVID-19 in the SCOURGE study, we found 133 cases (1.42%) with detectable clonal mosaicism for chromosome alterations (mCA) and 226 males (5.08%) with acquired loss of chromosome Y (LOY). Individuals with clonal mosaic events (mCA and/or LOY) showed a 54% increase in the risk of COVID-19 lethality. LOY is associated with transcriptomic biomarkers of immune dysfunction, pro-coagulation activity and cardiovascular risk. Interferon-induced genes involved in the initial immune response to SARS-CoV-2 are also down-regulated in LOY. Thus, mCA and LOY underlie at least part of the sex-biased severity and mortality of COVID-19 in aging patients. Given its potential therapeutic and prognostic relevance, evaluation of clonal mosaicism should be implemented as biomarker of COVID-19 severity in elderly people.     Análisis de prestaciones en canales con desvanecimientos de tipo kappa-mu shadowed

    Asimismo, se ha investigado el llamado carácter hiper-Rayleigh (HR) asociado al comportamiento de un canal con mayor fading del que puede representar la distribución de Rayleigh. Se ha propuesto un nuevo marco para definirlo, basado en tres magnitudes: el grado de desvanecimiento, la probabilidad de interrupción asintótica y la capacidad ergódica asintótica. Se ha establecido una métrica de tres condiciones y, en función del número que cumpla el modelo de canal, su característica HR se clasifica con una escala de cuatro niveles, reflejando su aplicabilidad a escenarios con desvanecimientos profundos. Cuando el modelo queda definido por funciones estadísticas que dependen de varios parámetros, puede presentar diferentes niveles, lo que se relaciona con una mayor adaptación a escenarios con particularidades diversas. Para estos casos se propone el denominado ‘mapa de nivel HR’. Este marco se ha aplicado al modelo κ-µ shadowed y a sus distribuciones derivadas: Gaussiana unilateral, Nakagami-m, Hoyt, Rice, Ricean shadowed, κ-µ y η-µ, así como al modelo FTR.Esta tesis se centra en el estudio de parámetros de prestaciones para modelos estadísticos de canal que permiten caracterizar los desvanecimientos en sistemas de comunicaciones inalámbricos SISO, donde se puede asumir comportamiento de banda estrecha, siendo el modelo κ-µ shadowed el eje del trabajo de investigación. Para este modelo se han determinado las expresiones analíticas cerradas de la capacidad ergódica y del grado de desvanecimiento. La capacidad ergódica se expresa como una función Meijer-G de dos variables, no disponible en los paquetes de software matemático populares, y se ha implementado en Mathematica la rutina necesaria. Estas magnitudes se han aplicado sobre escenarios realistas de comunicaciones acústicas subacuáticas a frecuencias ultrasónicas y de comunicaciones radio celulares con alta densidad de estaciones base, para demostrar la versatilidad del modelo κ-µ shadowed y la utilidad de las expresiones halladas
