Contemporary Occupational Carcinogen Exposure and Bladder Cancer: A Systematic Review and Meta-analysis
Importance: Bladder cancer (BC) is a common disease. Despite manufacturing and legislative changes to workplace hygiene, many BCs still arise through occupational carcinogen exposure.
Objective: To profile contemporary risks of occupational BC.
Data Sources: A systematic review using PubMed, Medline, Embase, and Web of Science was performed in October 2012 (initial review) and May 2014 (final review) and was updated in June 2015.
Study Selection: We identified 263 eligible articles. We excluded reports in which BC or occupation were not the main focus, and those with insufficient case, risk, or confidence interval data. We selected the most recent data from populations with multiple reports.
Data Extraction and Synthesis: Reports were selected by 2 of us independently. We combined odds ratios and risk ratios (RRs) to provide pooled RRs, using maximally adjusted RRs in a random effects model. Heterogeneity and publication bias were assessed using I2 and Begg and Egger tests. Risk estimates were annotated by occupational class using Nordisk Yrkesklassificering, or Nordic Occupational Classification, and International Standard Classifications of Occupations (NYK and ISCO-1958) Codes.
Main Outcomes and Measures: Occupations were profiled by BC incidence and mortality risk over time. After data collection, we detected a sex difference in these profiles and recorded this as a secondary outcome.
Results: Meta-analysis revealed increased BC incidence in 42 of 61 occupational classes and increased BC-specific mortality in 16 of 40 occupational classes. Reduced incidence and mortality were seen in 6 of 61 and 2 of 40 classes, respectively. Risk varied with sex and was greatest in men (standardized incidence ratio, 1.03 [95% CI, 1.02-1.03]; P < .001]). From the 1960s to the 1980s, there was a steady decline in standarized incidence ratio (SIR) for both sexes. This trend reversed from the 1980s, as in the decade 2000 to 2010 the SIR increased to 1.13 (95% CI, 1.07-1.19) for men and 1.27 (95% CI, 1.12-1.43) for women. In contrast, mortality risk declined for both sexes from the 1960s to the 1990s. The overall risk of BC mortality was also greater for men (standardized mortality ratio [SMR], 1.32 [95% CI, 1.18-1.48]) than for women (SMR, 1.14 [95% CI, 0.80-1.63]). Limitations include possible publication bias, that reports stratify workers mostly by job title not task, that not all studies adjusted for smoking, and that the population was mostly derived from Western nations.
Conclusions and Relevance: The profile of contemporary occupations with increased BC risk is broad and differs for incidence and mortality. Currently the incidence seems to be increasing, and this increase is occurring faster in women than men. Improved detection mechanisms and screening are possible reasons for this. Workers with aromatic amine exposure have the highest incidence, while those exposed to polycyclic aromatic hydrocarbons and heavy metals have the greatest mortality

Unsupervised machine learning of integrated health and social care data from the Macmillan Improving the Cancer Journey service in Glasgow
Background: Improving the Cancer Journey (ICJ) was launched in 2014 by Glasgow City Council and Macmillan Cancer Support. As part of routine service, data is collected on ICJ users including demographic and health information, results from holistic needs assessments and quality of life scores as measured by EQ-5D health status. There is also data on the number and type of referrals made and feedback from users on the overall service. By applying artificial intelligence and interactive visualization technologies to this data, we seek to improve service provision and optimize resource allocation.Method: An unsupervised machine-learning algorithm was deployed to cluster the data. The classical k-means algorithm was extended with the k-modes technique for categorical data, and the gap heuristic automatically identified the number of clusters. The resulting clusters are used to summarize complex data sets and produce three-dimensional visualizations of the data landscape. Furthermore, the traits of new ICJ clients are predicted by approximately matching their details to the nearest existing cluster center.Results: Cross-validation showed the model’s effectiveness over a wide range of traits. For example, the model can predict marital status, employment status and housing type with an accuracy between 2.4 to 4.8 times greater than random selection. One of the most interesting preliminary findings is that area deprivation (measured through Scottish Index of Multiple Deprivation-SIMD) is a better predictor of an ICJ client’s needs than primary diagnosis (cancer type).Conclusion: A key strength of this system is its ability to rapidly ingest new data on its own and derive new predictions from those data. This means the model can guide service provision by forecasting demand based on actual or hypothesized data. The aim is to provide intelligent person-centered recommendations. Robotic intracorporeal urinary diversion: practical review of current surgical techniques
In this practical review, we discuss current surgical techniques reported in the literature to perform Intracorporeal Urinary Diversion (ICUD) after Robotic Radical Cystectomy (RARC), emphasizing criticisms of single approaches and making comparisons with Extracorporeal Urinary Diversion (ECUD). Although almost 97% of all RARCs use an ECUD, ICUD is gaining in popularity, in view of its potential benefits (i.e., decreased bowel exposure, etc.), although there are a few studies comparing ICUD and ECUD. Analysing single experiences and the data from recent metanalyses, we emphasize the current critiques to ICUD, stressing particular technical details which could reduce operative time, lowering the postoperative complications rate, and improving functional outcomes. Only analysis of long-term follow-up data from large-scale homogeneous series can ascertain whether robotic intracorporeal urinary diversion is superior to other approaches
New Zealand Pastoral Systems: A Current Perspective
New Zealand\u27s diverse grassland resource of 13.5 M ha of permanent pasture, tussock or alpine grasslands underpin an intensive pastoral industry worth NZ$13 billion in 2004. The pastoral industry involves 37,000 farmers and a service industry of about 215,000 persons. It produces 27 M prime export lambs and 180,000 t of predominantly crossbred wool from 40 M ewes; 13 B litres of milk from 5.2 M dairy cattle; 800,000 t of beef from 4.7 M beef cattle and 33,000 t of venison from 2 M deer. Pastures and forage crops, mostly non- irrigated, are grazed \u27in situ\u27 by animals through controlled grazing management. The emphasis is on optimising utilisation at each grazing without penalising feed intake, pasture regrowth or persistance. Stocking rate and feed budgeting, along with a flexible stock trading policy are used to match animal demands with seasonal fluctuations in forage supply. The timing and duration of mating are varied to match anticipated seasonal growth. In dairy farming more condensed calving patterns have assisted in extending lactation length
An evaluation of urinary microRNA reveals a high sensitivity for bladder cancer
Background: Urinary biomarkers are needed to improve the care and reduce the cost of managing bladder cancer. Current
biomarkers struggle to identify both high and low-grade cancers due to differing molecular pathways. Changes in microRNA (miR) expression are seen in urothelial carcinogenesis in a phenotype-specific manner. We hypothesised that urinary miRs reflecting low- and high-grade pathways could detect bladder cancers and overcome differences in genetic events seen within the disease.
Methods: We investigated urinary samples (n ÂĽ 121) from patients with bladder cancer (n ÂĽ 68) and age-matched controls (n ÂĽ 53). Fifteen miRs were quantified using real-time PCR.
Results: We found that miR is stable within urinary cells despite adverse handling and detected differential expression of 10 miRs from patients with cancer and controls (miRs 15a/15b/24-1/27b/100/135b/203/212/328/1224, ANOVA Po0.05). Individually, miR-1224-3p had the best individual performance with specificity, positive and negative predictive values and concordance of 83%, 83%, 75% and 77%, respectively. The combination of miRs-135b/15b/1224-3p detected bladder cancer with a high sensitivity (94.1%), sufficient specificity (51%) and was correct in 86% of patients (concordance).
Conclusion: The use of this panel in patients with haematuria would have found 94% of urothelial cell carcinoma, while reducing cystoscopy rates by 26%. However, two invasive cancers (3%) would have been missed
A new approach to the modelling of local defects in crystals: the reduced Hartree-Fock case
This article is concerned with the derivation and the mathematical study of a
new mean-field model for the description of interacting electrons in crystals
with local defects. We work with a reduced Hartree-Fock model, obtained from
the usual Hartree-Fock model by neglecting the exchange term. First, we recall
the definition of the self-consistent Fermi sea of the perfect crystal, which
is obtained as a minimizer of some periodic problem, as was shown by Catto, Le
Bris and Lions. We also prove some of its properties which were not mentioned
before. Then, we define and study in details a nonlinear model for the
electrons of the crystal in the presence of a defect. We use formal analogies
between the Fermi sea of a perturbed crystal and the Dirac sea in Quantum
Electrodynamics in the presence of an external electrostatic field. The latter
was recently studied by Hainzl, Lewin, S\'er\'e and Solovej, based on ideas
from Chaix and Iracane. This enables us to define the ground state of the
self-consistent Fermi sea in the presence of a defect. We end the paper by
proving that our model is in fact the thermodynamic limit of the so-called
