    La histiocitosis de células de Langerhans (HCL), anteriormente conocida como histiocitosis X, es una enfermedad poco frecuente caracterizada por la acumulación y proliferación de histiocitos, eosinófilos y células de Langerhans, con inclusión de gránulos de Birbeck detectables por microscopia electrónica, afectando órganos y sistemas de forma aislada o múltiple. El diagnóstico se realiza mediante biopsia de la lesión y la confirmación de la presencia de CD1a y/o CD207 en la misma. Las distintas formas de presentación producen distinto enfoque terapéutico y pronóstico, desde formas benignas, autolimitadas, con resolución espontánea hasta otras de curso tórpido o maligno.Langerhans’ cell histiocytosis (LCH), previously known as histiocytosis X, is a rare disease. It is characterized by the accumulation and proliferation of histiocytes, eosinophils and Langerhans’ cells with Birbeck granules detected by electron microscopy. It involves single organs or systems or can present as a multisystem disease. The diagnosis is made by biopsy of the lesion and confirming the presence of CD1a and / or CD207 on it.     The competitor release effect applied to carnivore species: how red foxes can increase in numbers when persecuted

    The objective of our study was to numerically simulate the population dynamics of a hypothetical community of three species of small to medium–sized carnivores subjected to non–selective control within the context of the competitor release effect (CRE). We applied the CRE to three carnivore species, linking interspecific competition with predator control efforts. We predicted the population response of European badger, the red fox and the pine marten to this wildlife management tool by means of numerical simulations. The theoretical responses differed depending on the intrinsic rate of growth (r), although modulated by the competition coefficients. The red fox, showing the highest r value, can increase its populations despite predator control efforts if control intensity is moderate. Populations of the other two species, however, decreased with control efforts, even reaching extinction. Three additional theoretical predictions were obtained. The conclusions from the simulations were: 1) predator control can play a role in altering the carnivore communities; 2) red fox numbers can increase due to control; and 3) predator control programs should evaluate the potential of unintended effects on ecosystems

    Minimizing the social impact of construction work on mobility: a decision-making method

    Minimising the impacts of construction work on mobility, especially in urban areas, is a major issue for local authorities and construction planners that has not been sufficiently studied. This paper proposes a deterministic decision-making method for quantifying the impacts of construction work on mobility, including emergency vehicles, mass transit, individual transport, bicycles, and pedestrians. The method is based on multi-attribute utility theory, interviews with experts representing various stakeholders in construction, and a review of the literature and legislation. The practical use is illustrated with a real case study in which two shaft-construction processes (diaphragm wall excavated using a hydromill and vertical shaft sinking machine) are compared and ranked. The sensitivity analysis shows the robustness of the results. The resulting Mobility Impact Index can easily be integrated with other social, economic, and environmental criteria, thereby enabling the evaluation of alternatives from a multi-criteria perspective, e.g., in tender processes. The method could be useful to public authorities and design and construction companies and is being piloted in construction projects of the city of Barcelona. It has implications for corporate social responsibility, social/sustainable procurement, and social/sustainable impact assessment in construction

    The use of caspase inhibitors in pulsed-field gel electrophoresis may improve the estimation of radiation-induced DNA repair and apoptosis

    Background: Radiation-induced DNA double-strand break (DSB) repair can be tested by using pulsed-field gel electrophoresis (PFGE) in agarose-encapsulated cells. However, previous studies have reported that this assay is impaired by the spontaneous DNA breakage in this medium. We investigated the mechanisms of this fragmentation with the principal aim of eliminating it in order to improve the estimation of radiation-induced DNA repair. Methods: Samples from cancer cell cultures or xenografted tumours were encapsulated in agarose plugs. The cell plugs were then irradiated, incubated to allow them to repair, and evaluated by PFGE, caspase-3, and histone H2AX activation (gamma H2AX). In addition, apoptosis inhibition was evaluated through chemical caspase inhibitors. Results: We confirmed that spontaneous DNA fragmentation was associated with the process of encapsulation, regardless of whether cells were irradiated or not. This DNA fragmentation was also correlated to apoptosis activation in a fraction of the cells encapsulated in agarose, while non-apoptotic cell fraction could rejoin DNA fragments as was measured by gamma H2AX decrease and PFGE data. We were able to eliminate interference of apoptosis by applying specific caspase inhibitors, and improve the estimation of DNA repair, and apoptosis itself. Conclusions: The estimation of radiation-induced DNA repair by PFGE may be improved by the use of apoptosis inhibitors. The ability to simultaneously determine DNA repair and apoptosis, which are involved in cell fate, provides new insights for using the PFGE methodology as functional assay

    A Case of Hemorrhagic Myositis Associated With Prophylactic Heparin Use in Dermatomyositis

    Dermatomyositis (DM) is a rare systemic autoimmune disease that is associated with inflammation of the skin and muscles. It typically presents with weakness of the proximal muscles along with characteristic skin lesions such as Gottron\u27s papules and heliotrope rash. One of the most feared complications of this disease is the appearance of spontaneous hemorrhagic myositis, as most reported cases are fatal. The mechanism or risk factors of this condition have not been elucidated; however, prophylactic anticoagulation has been correlated with it in previous case reports, although idiopathic hemorrhagic myositis may also be present. We present a case of spontaneous intramuscular hemorrhage (SIH) in a recently diagnosed DM patient. A 59-year-old Hispanic male with a medical history of recently diagnosed prostate cancer and DM presented to the emergency department (ED) due to worsening anemia. His previous hemoglobin (Hgb) was 9 g/dl, but repeated laboratory tests revealed a level of 6.5 g/dl and later 5.5 g/dl at the ED. On admission, the patient was afebrile, tachycardic, and normotensive, with no overt sign of gastrointestinal bleeding. The physical exam revealed an ecchymosis on the right medial aspect of the thigh, and a digital rectal exam was negative. Computer tomography (CT) of the abdomen and pelvis without contrast was ordered due to suspicion of a retroperitoneal hematoma, revealing an interval development of a right groin complex fluid collection of up to 6 cm, concerning a possible hematoma. The patient did not have any previous vascular procedures in the area but was exposed to deep vein thrombosis (DVT) prophylaxis during the previous admission. Vascular surgery was consulted, and the recommendation was made to proceed with conservative management. On the third day, the patient developed new-onset, left-sided pleuritic chest pain. Upon examination, significant swelling and tenderness were noted in his left pectoral region, which was not present on admission. A CT chest without contrast was ordered due to concerns of underlying hematomas, revealing bilateral thickening of the pectoralis muscles, more on the right side, with a fluid collection of 2.5 cm × 1.3 cm. In addition, there was thickening of the right lateral chest wall muscles in the posterior right trapezius or supraspinatus muscles, most likely from intramuscular hemorrhage. The patient was transferred to the step-down unit for close monitoring. Conservative management was continued with as-needed transfusions for three days until hemoglobin stabilized at 9.8 mg/dL. Once stable, the patient was resumed on steroids and immunosuppressive therapy with posterior resolution of the SIH. SIH has been reported in DM, particularly more prominent in those with anti-MDA-5 antibodies. A case series and literature review showed 60.9% mortality within six months in those presenting with SIH, with a poorer prognosis (80% mortality) in those with deep muscle bleeding versus superficial (25%). There is currently no consensus on the treatment approach, and arterial embolization has not been proven effective. In our patient, conservative management with close surveillance and frequent transfusions helped achieve hemodynamic stability. Clinicians should be more aware of these rare but life-threatening complications in patients presenting with DM

    Antiangiogenic effect of gemcitabine following metronomic administration in a pancreas cancer model

    Gemcitabine shows a marked antitumor effect as a result of its cytotoxic action toward proliferative cells. In this article, we aim to investigate the potential antitumor and antiangiogenic effect of gemcitabine following a metronomic schedule that involves the regular administration of cytotoxic drugs at doses lower than standard treatment. In vitro results showed that human endothelial cells are more sensitive to gemcitabine (IC50 3 nmol/L) than pancreatic tumor cells (IC50 20 nmol/L). For in vivo studies, we used an orthotopic implantation model of human pancreatic carcinoma in nude mice. Gemcitabine was administered i.p. following a low-dose schedule (1 mg/kg/d for a month) and compared with the conventional schedule (100 mg/kg days 0, 3, 6, and 9 postimplantation). Metronomic treatment effect on established tumor was equivalent to standard administration. The measure of CD31 endothelial marked area allowed us to show an in vivo antiangiogenic effect of this drug that was further enhanced by using metronomic administration. This effect correlated with an induction of thrombospondin-1, a natural inhibitor of angiogenesis. Our results allow us to hypothesize that, in addition to a direct antiproliferative or cytotoxic antiendothelial cell effect, a secondary effect involving thrombospondin-1 induction might provide an explanation for the specificity of the effects of metronomic gemcitabine treatment

    Blind Audio-Visual Source Separation Using Sparse Redundant Representations

    This report presents a new method to confront the Blind Audio Source Separation (BASS) problem, by means of audio and visual information. In a given mixture, we are able to locate the video sources first and, posteriorly, recover each source signal, only with one microphone and the associated video. The proposed model is based on the Matching Pursuit (MP) [18] decomposition of both audio and video signals into meaningful structures. Frequency components are extracted from the soundtrack, with the consequent information about energy content in the time-frequency plane of a sound. Moreover, the MP decomposition of the audio is robust in front of noise, because of its plain characteristic in this plane. Concerning the video, the temporal displacement of geometric features means movement in the image. If temporally close to an audio event, this feature points out the video structure which has generated this sound. The method we present links audio and visual structures (atoms) according to their temporal proximity, building audiovisual relationships. Video sources are identified and located in the image exploiting these connections, using a clustering algorithm that rewards video features most frequently related to audio in the whole sequence. The goal of BASS is also achieved considering the audiovisual relationships. First, the video structures close to a source are classified as belonging to it. Then, our method assigns the audio atoms according to the source of the video features related. At this point, the separation performed with the audio reconstruction is still limited, with problems when sources are active exactly at the same time. This procedure allows us to discover temporal periods of activity of each source. However, with a temporal analysis alone it is not possible to separate audio features of different sources precisely synchronous. The goal, now, is to learn the sources frequency behavior when only each one of them is active to predict those moments when they overlap. Applying a simple frequency association, results improve considerably with separated soundtracks of a better audible quality.     Elämyksiä ja kohtaamisia Hopeatien palvelutalon aistihuoneessa

    Opinnäytetyön tavoitteena oli selvittää ikäihmisten kokemuksia aistihuoneen käytöstä ja kartoittaa, miten he voivat hyötyä aistihuoneesta. Apuvälineyritys Haltija Groupin SHX-aistihuoneen elementit siirrettiin Helsingin kaupungin Hopeatien palvelutaloon testikäyttöön kolmeksi kuukaudeksi. Opinnäytetyö pohjautuu Gary Kielhofnerin inhimillisen toiminnan malliin “Model of Human Occupation (MOHO)” ja sen keskeiset käsitteet ohjasivat tutkimusaineiston tiedon keruuta sekä sen analysointia. Opinnäytetyön muu teoreettinen perusta muodostui ikääntymisen tarkastelusta toimintakyvyn ja aistien näkökulmasta sekä multisensorisen toiminnan perusteista. Opinnäytetyö perustui laadullisen tutkimuksen periaatteisiin ja siinä oli kaksi työelämäkumppania: Helsingin kaupungin Hopeatien palvelutalo ja Haltija Group. Tutkimusmenetelmiksi valittiin aistihuoneen käyttäjille suunnattu kyselylomake sekä hoitohenkilökunnalle suunnattu ryhmämuotoinen teemahaastattelu. Kirjallinen raporttiosuus sisältää teoreettisen perustan, opinnäytetyön toteutuksen kuvauksen, aineiston analyysin, johtopäätökset ja pohdinnan. Tässä opinnäytetyössä asiakaslähtöisyys ja aistihuoneen esteettömyys nousivat seikoiksi, jotka vaikuttivat myönteisesti ikääntyneen osallistumiseen aistihuoneessa. Asiakkaan elämänhistorian ja mielenkiinnon kohteiden tunteminen sekä yksilöllisen tahdon ja suorituskyvyn huomioiminen koettiin tärkeiksi. Onnistuneeseen käyntiin aistihuoneessa vaikuttivat orientaatio ja henkilökunnan taito käyttää aistihuoneen tekniikkaa. Asiakkaiden kokemukset aistihuoneesta olivat pääosin miellyttäviä, jopa rentouttavia. Tämän opinnäytetyön tulokset ovat yhtenäisessä linjassa verrattuna aiempiin tutkimuksiin aistihuoneista ja niiden vaikutuksista. Opinnäytetyön perusteella aistihuoneen käyttö ikääntyneiden arjessa voi tarjota positiivisia vaikutuksia ikääntyneen psyykkiseen ja sosiaaliseen toimintakykyyn. Huolellinen suunnittelu, esteettömyys ja vahva asiakastuntemus mahdollistavat asiakkaan osallistumisen terapeuttiseen toimintaan aistihuoneessa. Aistihuoneen vaikuttavuudesta pitkällä aikavälillä tarvitaan lisää tutkimustietoa, jota voisi hyödyntää myös suunniteltaessa tulevaisuuden palveluja ikääntyneille.The object of the Bachelor’s thesis was to provide the elderly in the Hopeatie service house in Helsinki with the SHX multisensory elements for trial use. The purpose of this study was to find out experiences of elderly people in the multisensory room and analyze how they can benefit from the multisensory activities in their rehabilitation. The theory is based on Gary Kielhofner’s Model of Human Occupation (MOHO) and its essential concepts that steer the data gathering and analysis. The fundamental aspects of multisensory activities and ageing accompanied by changes in performance and impaired senses are also observed. This thesis is a qualitive study executed in collaboration with two working life partners: the city of Helsinki and Haltija Group, which is a Finnish company for assistive devices, welfare technology and rehabilitation. The thematic group interview of six employees of the service house and a client questionnaire were chosen as research methods. This thesis consists of theory, description of the implementation, data analysis, results of the study, and conclusions and discussion. The results show that the client-centered approach and the accessibility to the multisensory room had an influence on participation of the elderly in the multisensory room. Successful visits of the multisensory room were enabled by employees’ skills to prepare the client for the visit and their skills to master the technology of the room. The experiences of the elderly in the multisensory room were mainly pleasant, some even relaxing. The results of this study are consistent with previous research on the effects of a multisensory room. This study proves that the use of the multisensory room in the elderly people’s daily lives may have a positive effect on their mental and social performance. Thorough planning of therapy intervention, easy access to the multisensory room and strong client knowledge enable committed client participation. More research is needed for long-term effects of multisensory room in order to benefit from the results for planning services for the elderly in the future

    Modelado de restauración forestal en sistemas de Dehesa sometidos a herbivoría

    Los sistemas forestales de dehesa, presentan en la actualidad un serio problema de regeneración. Por este motivo es necesario diseñar estrategias que faciliten el reclutamiento de plantas jóvenes. En este trabajo desarrollamos un modelo teórico de un sistema con alta presión de herbivoría en el que se simula el efecto de dos medidas de manejo: la repoblación con planta o semilla y la protección de plantones. Se concluye que, según el modelo, resulta más rentable la protección de plantones que la repoblación, pero dado que ambas medidas no son excluyentes en restauración forestal se recomienda una estrategia mixta, siendo la protección de los plantones repoblados muy importante en sistemas con alta presión de herbivoría
