    Immediately restored dental implants for partial-arch applications : a literature update

    This article carries out a literature update on immediately restored dental implants in partially edentulous patients. A search was made in Medline of all articles published between the year 2000 and February 2007, including all articles published in both English and Spanish, in which immediate restoration of implants was made of partially edentulous areas with a minimum of 12 implants and six months follow-up. Certain decisive factors exist for the success of this technique in partially edentulous patients, such as primary stability, a roughened implant surface, and the absence of parafunctional habits in patients acceptable for this type of treatment. Following the analysis of these studies of immediate restoration of teeth in partially edentulous areas, a weighted mean survival of 95.39% was observed. In spite of the high success rate, major controversy still exists on this subject resulting in few studies and short follow-up periods, making the routine use of this technique questionable

    The post-endodontic periapical lesion : histologic and etiopathogenic aspects

    Apical periodontitis is produced in the majority of cases by intraradicular infection. Treatment consists in the elimination of the infectious agents by endodontia. Even when carrying out a correct cleansing and filling of canals, it is possible that periapical periodontitis will persist in the form of an asymptomatic radiolucency, giving rise to the post-endodontic periapical lesion. The chronic inflammatory periapical lesion is the most common pathology found in relation to alveolar bone of the jaw. From the histological point of view, it can be classified as chronic periapical periodontitis (periapical granuloma), radicular cyst, and as scar tissue. The most frequent is the periapical granuloma, constituted by a mass of chronic inflammatory tissue, in which isolated nests of epithelium can be found. The radicular cyst is characterized by the presence of a cavity, partially or wholly lined by epithelium. Scar tissue is a reparative response by the body, producing fibrous connective tissue.     Subcutaneous emphysema resulting from surgical extraction without elevation of a mucoperiosteal skin flap

    Subcutaneous emphysema, is a rare occurrence in dental practice. It is usually benign and self-limiting; nevertheless, severe consequences can result from surgical treatment. Emphysema occurs when air is injected into the subcutaneous layer of the tissue, this may come from either an air turbine handpiece or air syringe. Due to the danger of developing emphysema, procedures using compressed air are not recommended in dental extractions involving the raising of a skin flap, bone sectioning or exeresis. Two determining factors are always involved with subcutaneous emphysema. On the one hand, a compressed air procedure (air turbine handpiece, air-water syringe), and on the other, a communication between the oral cavity and deeper tissue producing dissection. Early diagnosis and treatment are critical to prevent the trapped air leaking into other areas. The aim is to present a case of subcutaneous emphysema related to dental extraction, without raising a mucoperiosteal flap, possibly caused by close contact between the air turbine handpiece and the tooth during sectioning which forced air into the subcutaneous tissue

    Transcrestal sinus lift and implant placement using the sinus balloon technique

    Objective: A description is made of transcrestal sinus lift using the sinus balloon technique, evaluating the bone height achieved and implant success one year after prosthetic loading. Material and method: Between January and July 2007, transcrestal sinus lift using the sinus balloon technique for dental implant placement was carried out in 6 patients. A panoramic X-ray study and maxillary computed tomography scan were carried out before the operation, in order to discard possible sinus pathology. During the intervention, the integrity of the sinus membrane was evaluated using a Medi Pack Pal endoscope (Farol Store and Co., Tuttlingen, Germany), and the intraoperative complications were analyzed. The dental implants were placed in the same surgical step in the presence of 3 mm or more of residual bone. Following the operation, panoramic X-rays were used to assess the bone height gained. One year after prosthetic loading, the implant success rate was determined based on the criteria of Buser. Results: One patient was excluded due to Schneider's membrane perforation as confirmed by endoscopy. Trans-crestal sinus lift was carried out in 5 males with a mean age of 41.6 years (range 27-51), without antecedents of sinus disease. There were no intraoperative complications. In four patients the implants were placed simultaneous to sinus lift, while in another case implant placement was postponed due to insufficient remaining bone height. The mean gain in height after the operation was 8.7 mm. One year after prosthetic loading, the implant success rate was 100%. Conclusions: Transcrestal sinus lift using the sinus balloon technique is a minimally invasive procedure. In 5 patients the bone height gained proved sufficient to allow implant placement even in the presence of 3 mm of residual bone. © Medicina Oral S. L

    Immunohistochemical study of Langerhans cells in periapical lesions : correlation with inflammatory cell infiltration and epithelial cell proliferation

    Aim: The aim of this study is to determine the presence and distribution of Langerhans cells in periapical lesions, and correlate this with inflammatory cell infiltration and epithelial cell proliferation. Material and Methods: Seventy chronic dental periradicular lesions, obtained during periapical surgery from 70 patients, were included in this study, including: 46 granulomas, 18 scar tissue and 6 periradicular cysts. Immunohistochemical staining was performed using the following markers: CD3 to analyze the inflammatory infiltrate, CD1a to determine the presence of Langerhans cells and Ki67 to analyze the epithelial cell proliferation. The CD1a immunostaining density was established following Cincura (2007) criteria, being classified ranging from intense (3), moderate (2), discrete (1) or no (0) immunostaining. CD3 and Ki67 staining was evaluated following the Liapatas et al. scale, as: 0) no cells stained; 1) weak stain or few cells stained (11-25%); 2) moderate staining or some cells stained (26-75%); 3) intense staining or many cells stained (more than 76%). Results: Langerhans cells were found in 32.8% of the periapical lesions being more intense in the epithelialized lesions. CD3 immunohistochemical staining was found in all lesions, but with different values in relation to histological subtypes. Ki67 was positive in all epithelialized lesions, although with a moderate staining. Conclusions: Langerhans cells appeared to be associated with T-lymphocyte infiltration and the proliferative potential of the epithelial tissue in periapical lesion

    Oral mucocele: review of the literature

    Mucocele is a common lesion of the oral mucosa that results from an alteration of minor salivary glands due to a mucous accumulation. Mucocele involves mucin accumulation causing limited swelling. Two histological types exist - extravasation and retention. Mucoceles can appear at any site of the oral mucosa where minor salivary glands are present. Diagnosis is principally clinical; therefore, the anamnesis should be carried out correctly, looking for previous trauma. The most common location of the extravasation mucocele is the lower lip, while retention mu- coceles can be found at any other site. Mucoceles can affect the general population, but most commonly young patients (20-30 years old). Clinically they consist of a soft, bluish and transparent cystic swelling which normally resolves spontaneously. Treatment frequently involves surgical removal. Never-theless micro marsupialization, cryosurgery, steroid injections and CO2 laser are also described. Mucocele is a common lesion and affects the general population. For this reason we felt it would be interesting review the clinical characteristics of mucoceles, and their treatment and evolution in order to aid decision-making in daily clinical practice

    Conocimiento, conducta y actitud ante el Código europeo contra el cáncer por parte de los profesionales sanitarios de atención primaria

    Objetivo: Determinar si los profesionales de atención primaria (AP) conocen el Código europeo contra el cáncer (CECC) y si ello se relaciona con que practiquen sus recomendaciones. Diseño: Observacional descriptivo. Emplazamiento: Centros de AP españoles. Participantes: Profesionales de AP. Intervenciones: Cuestionario autoadministrado. Mediciones principales: Factores de riesgo y protectores del cáncer, pruebas de cribado y conocimiento del CECC. Resultados: Respondieron 1.734 profesionales (10, 5%), edad media de 48, 5 años (DT: 9, 2), el 71, 5% mujeres. El 50, 0% desconoce el CECC; los tutores (OR = 1, 61), adscritos al PAPPS (OR = 1, 51) y que llevan más tiempo trabajando en AP (OR = 2, 62) son los que más lo conocen. El 7, 2% fuma y el 79, 1% bebe alcohol. El 64, 1% presenta normopeso y el 19% realiza actividad física. Evita exponerse al sol el 52, 7% y usa protectores el 53, 8%. El 85, 2% nunca se ha realizado un cribado colorrectal, el 11% nunca una citología y el 12% una mamografía. Existe relación entre conocer el CECC y el tabaquismo (p < 0, 001), sobrepeso/obesidad (p = 0, 024), actividad física (p = 0, 003), ingesta de carnes (p < 0, 001), mamografía (p < 0, 001), citología (p = 0, 022), PSA (p = 0, 045), así como el fomento de sus consejos entre los pacientes. Conclusiones: El CECC es desconocido por la mitad de los profesionales de AP. La práctica de sus recomendaciones es desigual, siendo la prevalencia de tabaquismo baja, y la protección solar una conducta poco extendida. Los que lo conocen son los que más practican sus recomendaciones. Objective: To decide if Primary Care (PC) professionals know the European Code against Cancer (CECC) and if this relates to practice the recommendations with themselves and with their patients. Design: descriptive, observational study. Setting: Spanish Health Centers. Participants: Health professionals. Interventions: self-administered questionnaire. Main measurements: cancer protective and risk factors, screening for colorectal, breast, cervix and prostate cancer, level of knowledge of the ECAC. Results: 1734 participants (10.5%), mean age 47.4 years (SD: 8.6), 71.5% female. 50.0% do not know the CECC; tutors (OR = 1.61), assigned to the PAPPS (OR = 1.51) and who have been working in AP for more time (OR = 2.62) are the ones who know it most. 7.2% smoke and 79.1% drink alcohol. 64.1% presented normal weight and 19% performed physical activity. They avoid exposing themselves to the sun 52.7% and use protectors 53.8%. 85.2% have never had a colorectal screening, 11% never had a cytology, and 12% had a mammogram. There is a relationship between knowing the CECC and smoking (P <.001), overweight/obesity (P =.024), physical activity (P =.003), meat intake (P <.001), mammography (P <.001), cytology (P =.022), PSA (P =.045), as well as the promotion of their advice among patients. Conclusions: The CECC is unknown by half of the AP professionals. The practice of its recommendations is uneven, with the prevalence of low smoking, and sun protection a little extended behavior. Those who know him are the ones who most practice his recommendations

    Thermocatalytic degradation of lignin monomer coniferyl aldehyde by aluminum–boron oxide catalysts

    Two aluminum–boron oxide catalysts were produced via a sol–gel method at pH 3 and 4 during the solution mixing step of the synthesis, these materials were employed in thermocatalytic degradation of coniferyl aldehyde (CA), which was used as a probe molecule of the lignin polymeric molecule and is comprised of the repetitive monomers coniferyl, sinapyl, and paracoumaryl. The two synthesized catalysts were mostly amorphous and mesoporous, aiding in permeability and percolation of CA. A commercial catalyst was compared (Pt/alumina at 1 wt%) with both catalysts synthesized in this work by kinetic tests by varying the CA concentration and inlet temperature. Under the same reaction conditions, the commercial catalyst showed higher activity than the aluminum–boron oxide catalysts, but the synthetic catalysts presented a wider variety of organic products than the commercial catalyst. In particular, two high-value products, isomers of eugenol and isoeugenol, were yielded in higher percentages. The experimental reaction rate data was fit to a Langmuir–Hinshelwood model, and kinetic parameters were analyzed, revealing how the adsorbed CA molecules on the catalytic surface had higher mobility with the synthesized catalyst compared with the commercial catalyst, the value of ΔSads0\Delta S_{\mathrm{ads}}^{0} for the synthetic catalysts were -5.48 and -4.31 J/mol-K and for the commercial catalyst -37.17 J/mol-K

    A body weight loss- and health-promoting gut microbiota is established after bariatric surgery in individuals with severe obesity

    Obesity has reached an epidemic level worldwide, and bariatric surgery (BS) has been proven to be the most efficient therapy to reduce severe obesity-related comorbidities. Given that the gut microbiota plays a causal role in obesity development and that surgery may alter the gut environment, investigating the impact of BS on the microbiota in the context of severe obesity is important. Although, alterations at the level of total gut bacteria, total gene content and total metabolite content have started to be disentangled, a clear deficit exists regarding the analysis of the active fraction of the microbiota, which is the fraction that is most reactive to the BS. Here, active gut microbiota and associated metabolic functions were evaluated using shotgun proteomics and metabolomics in 40 severely obese volunteers. Samples from each volunteer were obtained under basal conditions, after a short high protein and calorie-restricted diet, and 1 and 3 months after BS, including laparoscopic surgery through Roux-en-Y Gastric Bypass or Sleeve Gastrectomy. The results revealed for the first time the most active microbes and metabolic flux distribution pre- and post-surgery and deciphered main differences in the way sugars and short-fatty acids are metabolized, demonstrating that less energy-generating and anaerobic metabolism and detoxification mechanisms are promoted post-surgery.
