The five-year progression from early and intermediate AMD to late disease is known to range from 0.4% to 53%. This wide variation explains the particular interest in searching predictive AMD biomarkers. Clinical parameters such as drusen size, presence of pigmentary abnormalities, and fellow eye status were, traditionally, the more important predictive elements. Multimodal retinal assessment (Color Fundus Photography, Optical Coherence Tomography, Optical Coherence Angiography and Fundus Autofluorescence) is providing new and accurate image biomarkers, useful in research and in daily practice. If individual progression risk could be anticipated, then management plans should be adapted accordingly, considering follow-up intervals and therapeutic interventions. Here, we reviewed the most important image progression biomarkers of early and intermediate AMD with relevant interest in clinical practice.publishersversionpublishe

Nutritional and Lifestyle Interventions for Age-Related Macular Degeneration: A Review

    Get PDF
Age-related macular degeneration (AMD) is the leading cause of blindness in the developed world. In this narrative review, we will summarize the nutritional interventions evaluated in numerous observational studies and a few randomized clinical trials. The AREDS and AREDS2 studies demonstrated that supplements including vitamins C and E, beta-carotene, and zinc may reduce the progression to advanced AMD, in some patients, by 25% in five years. This is one of the few nutritional supplements known to have beneficial effects in any eye disease. Lutein/zeaxanthin supplementation may have beneficial effects in some individuals whereas omega-3 fatty acids supplementation needs to be further investigated and supported by more evidence. Genetic factors may explain the different patterns of response and explain differences found among individuals. More importantly, a combination of lifestyle behaviors such as the avoidance of smoking, physical activity, and the adoption of a healthy dietary pattern like the Mediterranean diet was associated with a lower prevalence of AMD. The adoption of these lifestyles may reduce the prevalence of the early stages of AMD and decrease the number of individuals who develop advanced AMD and consequently the onerous and climbing costs associated with the treatment of this disease

Spectral-Domain Optical Coherence Tomography Features of Acute Syphilitic Posterior Placoid Chorioretinitis: The Role of Autoimmune Response in Pathogenesis

Purpose: Syphilis is an infectious disease that can cause a wide variety of ocular signs. One of the rarest manifestations of ocular syphilis is acute syphilitic posterior placoid chorioretinitis (ASPPC). We report on the spectral-domain optical coherence tomography (SD-OCT) features of a case diagnosed with unilateral ASPPC. Methods: A 64-year-old man presented with a sudden loss of visual acuity (VA) in the right eye. His only clinical sign was a large, geographic, yellow-white lesion centered on the right fovea. Our patient was studied with SD-OCT on presentation and during follow-up, as well as with fluorescein and indocyanine green angiography, electrophysiological study, and serologic and autoimmune screening. Results: Laboratory workup revealed positive serology for active syphilis and elevated anti-beta2 glycoprotein I antibodies. SD-OCT showed a marked distortion of both the choroidal and outer retinal architecture. After treatment, best-corrected VA improved to 20/25. Pattern electroretinography displayed a severe reduction of P50 amplitude, which improved in late follow-up. Six months after presentation, VA was 20/25 and anti-beta2 glycoprotein I antibodies returned to normal levels. Conclusions: Our findings are compatible with immunologically mediated temporary physiological impairment of the neuroretina, since the changes seen by SD-OCT could not have normalized if they were due to anatomical injury. The results of our study provide clues to understanding the pathogenesis of this disease and allow us to define a characteristic temporal sequence of events in ASPPC

Proposal for a universal measurement system for school chairs and desks for children from 6 to 10 years old

    Get PDF
In a primary education classroom of any country, children of the same age have very different statures, reaching variations of 200 mm (Goncalves, 2012). However, the school furniture provided is not suitable or adaptable to these differences. Designing school furniture able to respond to these variations is, therefore, a challenge for ergonomics and design in a global market. It is clearly not viable for industries to adapt productions for each country. When competitiveness and limitation of resources are essential for the viability of any product it becomes essential to find a universal system adapted to the requisites of any country. Taking as prescription measure the popliteal height obtained from the data of different countries, a universal measurement system for the school chair and desk set is proposed, combining the ellipse methodology used by Molenbroek et al. (2003) and the (mis)match equations mentioned by Castellucci et al. (2014b). From the results obtained, it can be concluded that only 5 sizes are needed to implement this new measurement system of evolutionary school furniture for the primary education classroom

Human values and religion: evidence from the European Social Survey

https://v2.sherpa.ac.uk/id/publication/22280Values are guiding constructs of social action that connote some actions as desirable, undesirable, acceptable, and unacceptable, containing a normative moral/ethical component, and constituting a guide for actions, attitudes, and objectives for which the human being strives. The role of religion in the development of moral and ideal behaviors is a subject of concern and object of theoretical and empirical debate in various sciences. Analyzing sociodemographic and religious variables, the present work aimed to understand the contribution of religious variables to the explanation of Schwartz’s human values and to identify an explanatory model of second-order values, i.e., self-transcendence, conservation, self-promotion, and openness to change. This study was carried out with a representative sample of the Portuguese population, consisting of 1270 participants from the European Social Survey (ESS), Round 8. Benevolence (as human motivational value) and self-transcendence (as a second-order value) were found to be the most prevalent human values among respondents, with the female gender being the one with the greatest religious identity, the highest frequency of religious practices, and valuing self-transcendence and conservation the most. Older participants had a more frequent practice and a higher religious identity than younger ones, with age negatively correlating with conservation and positively with openness to change. It was concluded that age, religious identity, and an item of religious practice contribute to explain 13.9% of the conservation variance. It was also found that age and religious practice are the variables that significantly contribute to explain 12.2% of the variance of openness to change. Despite the associations between psychological variables (values) and religious ones, it can be concluded that religious variables contribute very moderately to explain human values. The results obtained in this study raised some important issues, namely, if these weakly related themes, i.e., religiosity and human values, are the expression of people belief without belonging.info:eu-repo/semantics/publishedVersio
