    Cuerpo carotídeo y fisiopatología del SAOS: hipertensión

    El cuerpo carotídeo (CC) es un pequeño órgano quimiorreceptor que se activa en situaciones de hipoxia, hipercapnia y acidosis plasmática, originando reflejos ventilatorios y cardiocirculatorios. La apnea obstructiva del sueño es un trastorno frecuente en el que los pacientes padecen de manera crónica interrupciones del flujo aéreo que conducen a modificaciones de los parámetros que activan el CC. Cuando estas modificaciones se prolongan en el tiempo suceden una serie de trastornos cardiocirculatorios, metabólicos y neurocognitivos. Este trabajo tiene como objetivo estudiar la participación del CC en la génesis de la hipertensión que constituye uno de los trastornos fundamentales en el síndrome de apnea obstructiva del sueño. Se ha revisado bibliografía sobre dicha participación y los mecanismos fisiopatológicos que desencadenan la hipertensión y que incluyen la activación del simpático y otras alteraciones vasculares como la disfunción endotelial.Grado en Medicin

    Producción CientíficaThe mortality of trauma patients has improved significantly in recent decades due to a combination of factors: medical care, educational campaigns and structural changes. Generalization of both out-of hospital emergence medical services and the hospital care in specific centers for traumatized has undoubtedly contributed to this decline, but other factors such as periodic campaigns to prevent workplace and traffic accidents, as well as improvements in the road network have played a key role. The challenge now is to contain mortality, for which is essential an analysis of the situation to detect potential areas of improvement. The application of diagnostic or therapeutic actions with scientific evidence is associated with lower mortality, but as in other areas of medicine, the application of scientific evidence in trauma patients is barely 50%. Moreover, nearly 90% of trauma deaths occur in the crash site or during the first 72 h of hospitalization, the vast majority as a result of injuries incompatible with life. In these circumstances it is clear that prevention is the most cost-effective activity. As medical practitioners, our role in prevention is mainly focused on the secondary prevention to avoid recidivism, for which it is necessary to identify the possible risk factors (frequently alcohol, illegal drugs, psychotropic medication, etc.) and implement a brief motivational intervention. This activity can reduce recidivism by nearly 50%. In Spain, the activity in this field is negligible; therefore, measures should be implemented for dissemination of secondary prevention in trauma

    Union bound minimization approach for designing grassmannian constellations

    In this paper, we propose an algorithm for designing unstructured Grassmannian constellations for noncoherent multiple-input multiple-output (MIMO) communications over Rayleigh block-fading channels. Unlike the majority of existing unitary space-time or Grassmannian constellations, which are typically designed to maximize the minimum distance between codewords, in this work we employ the asymptotic pairwise error probability (PEP) union bound (UB) of the constellation as the design criterion. In addition, the proposed criterion allows the design of MIMO Grassmannian constellations specifically optimized for a given number of receiving antennas. A rigorous derivation of the gradient of the asymptotic UB on a Cartesian product of Grassmann manifolds, is the main technical ingredient of the proposed gradient descent algorithm. A simple modification of the proposed cost function, which weighs each pairwise error term in the UB according to the Hamming distance between the binary labels assigned to the respective codewords, allows us to jointly solve the constellation design and the bit labeling problem. Our simulation results show that the constellations designed with the proposed method outperform other structured and unstructured Grassmannian designs in terms of symbol error rate (SER) and bit error rate (BER), for a wide range of scenarios.This work was supported by Huawei Technologies, Sweden under the project GRASSCOM.     A measure preserving mapping for structured Grassmannian constellations in SIMO channels

    In this paper, we propose a new structured Grassmannian constellation for noncoherent communications over single-input multiple-output (SIMO) Rayleigh block-fading channels. The constellation, which we call Grass-Lattice, is based on a measure preserving mapping from the unit hypercube to the Grassmannian of lines. The constellation structure allows for on-the-fly symbol generation, low-complexity decoding, and simple bit-to-symbol Gray coding. Simulation results show that Grass-Lattice has symbol error rate performance close to that of a numerically optimized unstructured constellation, and is more power efficient than other structured constellations proposed in the literature.This work was supported by Huawei Technologies Sweden, under the project GRASSCOM.     [EN] Infrastructure usage discovery, positioning and analysis of behaviors of its users, usually requires a collection of accurate and frequent positioning data. This paper shows how a network of inexpensive and non-intrusive sensors can serve to perform this kind of analysis by detecting devices with wifi connectivity.     Analysis of the Genetic Parameters for Dairy Linear Appraisal and Zoometric Traits: A Tool to Enhance the Applicability of Murciano-Granadina Goats Major Areas Evaluation System

    Selection for zoometrics defines individuals’ productive longevity, endurance, enhanced productive abilities and consequently, their long-term profitability. When zoometric analysis is aimed at large highly selected populations or in those at different levels of selection, linear appraisal systems (LAS) provide a timely response. This study estimates genetic and phenotypic parameters for zoometric/LAS traits in Murciano-Granadina goats, estimating genetic and phenotypic correlations among all traits, and determining whether major area selection would be appropriate or if adaptability strategies may need to be followed. Heritability estimates for the zoometric/LAS traits were low to high, ranging from 0.09 to 0.43, and the accuracy of estimation has improved after decades, rendering standard errors negligible. Scale inversion of specific traits may need to be performed before major areas selection strategies are implemented. Genetic and phenotypic correlations suggests that negative selection against thicker bones and higher rear insertion heights indirectly results in the optimization of selection practices in the rest of the traits, especially those in the structure, capacity and mammary system major areas. The integration and implementation of the strategies proposed within the Murciano-Granadina breeding program maximizes selection opportunities and the sustainable international competitiveness of the Murciano-Granadina goat in the dairy goat breed panorama

    Optimization and Validation of a Linear Appraisal Scoring System for Milk Production-Linked Zoometric Traits in Murciano-Granadina Dairy Goats and Bucks

    Implementing linear appraisal systems (LAS) may reduce time, personnel and resource costs when performing large-scale zoometric collection. However, optimizing complex zoometric variable panels and validating the resulting reduced outputs may still be necessary. The lack of cross-validation may result in the loss of accuracy and value of the practices implemented. Special attention should be paid when zoometric panels are connected to economically-relevant traits such as dairy performance. This methodological proposal aims to optimize and validate LAS in opposition to the traditional measuring protocols routinely implemented in Murciano-Granadina goats. The sample comprises 41,323 LAS and traditional measuring records from 22,727 herdbook-registered primipara does, 17,111 multipara does and 1485 bucks. Each record includes information on 17 linear traits for primipara/multipara does and 10 traits for bucks. All zoometric parameters are scored on a nine-point scale. Cronbach’s alpha values suggest a high internal consistency of the optimized variable panels. Model fit, variability explanation power and predictive power (mean square error (MSE), Akaike (AIC)/corrected Akaike (AICc) and Bayesian information criteria (BIC), respectively) suggest the model comprising zoometric LAS scores performs better than traditional zoometry. Optimized reduced models are able to capture variability for dairy-related zoometric traits without noticeable detrimental effects on model validity properties
