    Important features of home-based support services for older Australians and their informal carers

    This author accepted manuscript (post print) is made available in accordance with the publisher copyright policy following 24 month embargo from the date of publication (8 February 2015) in accordance with publishers copyright policy.In Australia, newly initiated, publicly subsidised ‘Home-Care Packages’ designed to assist older people (≥65 years of age) living in their own home must now be offered on a ‘consumer-directed care’ (CDC) basis by service providers. However, CDC models have largely developed in the absence of evidence on users’ views and preferences. The aim of this study was to determine what features (attributes) of consumer-directed, home-based support services are important to older people and their informal carers to inform the design of a discrete choice experiment (DCE). Semi-structured, face-to-face interviews were conducted in December 2012–November 2013 with 17 older people receiving home-based support services and 10 informal carers from 5 providers located in South Australia and New South Wales. Salient service characteristics important to participants were determined using thematic and constant comparative analysis and formulated into attributes and attribute levels for presentation within a DCE. Initially, eight broad themes were identified: information and knowledge, choice and control, self-managed continuum, effective co-ordination, effective communication, responsiveness and flexibility, continuity and planning. Attributes were formulated for the DCE by combining overlapping themes such as effective communication and co-ordination, and the self-managed continuum and planning into single attributes. Six salient service features that characterise consumer preferences for the provision of home-based support service models were identified: choice of provider, choice of support worker, flexibility in care activities provided, contact with the service co-ordinator, managing the budget and saving unspent funds. Best practice indicates that qualitative research with individuals who represent the population of interest should guide attribute selection for a DCE and this is the first study to employ such methods in aged care service provision. Further development of services could incorporate methods of consumer engagement such as DCEs which facilitate the identification and quantification of users’ views and preferences on alternative models of delivery

    FIDES: Enhancing Trust in Reconfigurable Based Hardware Systems

    Extensive use of third party IP cores (e.g., HDL, netlist) and open source tools in the FPGA application design and development process in conjunction with the inadequate bitstream protection measures have raised crucial security concerns in the past for reconfigurable hardware systems. Designing high fidelity and secure methodologies for FPGAs are still infancy and in particular, there are almost no concrete methods/techniques that can ensure trust in FPGA applications not entirely designed and/or developed in a trusted environment. This work strongly suggests the need for an anomaly detection capability within the FPGAs that can continuously monitor the behavior of the underlying FPGA IP cores and the communication activities of IP cores with other IP cores or peripherals for any abnormalities. To capture this need, we propose a technique called FIDelity Enhancing Security (FIDES) methodology for FPGAs that uses a combination of access control policies and behavior learning techniques for anomaly detection. FIDES essentially comprises of two components: (i) {\em Trusted Wrappers}, a layer of monitors with sensing capabilities distributed across the FPGA fabric; these wrappers embed the output of each IP core ii with a tag τi\tau_i according to the pre-defined security policy Π\Pi and also verifies the embeddings of each input to the IP core to detect any violation of policies. The use of tagging and tracking enables us to capture the normal interactions of each IP core with its environment (e.g., other IP cores, memory, OS or I/O ports). {\em Trusted Wrappers} also monitors the statistical properties exhibited by each IP core module on execution such as power consumption, number of clock cycles and timing variations to detect any anomalous operations; (ii) a {\em Trusted Anchor} that monitors the communication between the IP cores and the peripherals with regard to the centralized security policies Ψ\Psi as well as the statistical properties produced by the peripherals. We target FIDES architecture on a Xilinx Zynq 7020 device implemented with a red-black system comprising of sensitive and non-sensitive IP cores. Our results show that FIDES implementation leads to only 1-2\% overhead in terms of the logic resources per wrapper and incurs minimal latency per wrapper for tag verification and embedding. On the other hand, as compared to the baseline implementation, when all the communications within the system are routed to the Trusted Anchor for centralized policy checking and verification, a latency of 1.5X clock cycles is observed; this clearly manifests the advantage of using distributed wrappers as opposed to centralized policy checking

    Inverse problem of photoelastic fringe mapping using neural networks

    This paper presents an enhanced technique for inverse analysis of photoelastic fringes using neural networks to determine the applied load. The technique may be useful in whole-field analysis of photoelastic images obtained due to external loading, which may find application in a variety of specialized areas including robotics and biomedical engineering. The presented technique is easy to implement, does not require much computation and can cope well within slight experimental variations. The technique requires image acquisition, filtering and data extraction, which is then fed to the neural network to provide load as output. This technique can be efficiently implemented for determining the applied load in applications where repeated loading is one of the main considerations. The results presented in this paper demonstrate the novelty of this technique to solve the inverse problem from direct image data. It has been shown that the presented technique offers better result for the inverse photoelastic problems than previously published works

    A Two Micron All-Sky Survey View of the Sagittarius Dwarf Galaxy: II. Swope Telescope Spectroscopy of M Giant Stars in the Dynamically Cold Sagittarius Tidal Stream

    We present moderate resolution (~6 km/s) spectroscopy of 284 M giant candidates selected from the Two Micron All Sky Survey photometry. Radial velocities (RVs) are presented for stars mainly in the south, with a number having positions consistent with association to the trailing tidal tail of the Sagittarius (Sgr) dwarf galaxy. The latter show a clear RV trend with orbital longitude, as expected from models of the orbit and destruction of Sgr. A minimum 8 kpc width of the trailing stream about the Sgr orbital midplane is implied by verified RV members. The coldness of this stream (dispersion ~10 km/s) provides upper limits on the combined contributions of stream heating by a lumpy Galactic halo and the intrinsic dispersion of released stars, which is a function of the Sgr core mass. The Sgr trailing arm is consistent with a Galactic halo containing one dominant, LMC-like lump, however some lumpier halos are not ruled out. An upper limit to the total M/L of the Sgr core is 21 in solar units. A second structure that roughly mimics expectations for wrapped, leading Sgr arm debris crosses the trailing arm in the Southern Hemisphere; however, this may also be an unrelated tidal feature. Among the <13 kpc M giants toward the South Galactic Pole are some with large RVs that identify them as halo stars, perhaps part of the Sgr leading arm near the Sun. The positions and RVs of Southern Hemisphere M giants are compared with those of southern globular clusters potentially stripped from the Sgr system and support for association of Pal 2 and Pal 12 with Sgr debris is found. Our discussion includes description of a masked-filtered cross-correlation methodology that achieves better than 1/20 of a resolution element RVs in moderate resolution spectra.Comment: 41 pages, 6 figures, Astronomical Journal, in press (submitted Nov. 24, 2003; tentatively scheduled for July 2004 issue

    Towards Real-World Neurorobotics: Integrated Neuromorphic Visual Attention

Proceedings, Part IIINeuromorphic hardware and cognitive robots seem like an obvious fit, yet progress to date has been frustrated by a lack of tangible progress in achieving useful real-world behaviour. System limitations: the simple and usually proprietary nature of neuromorphic and robotic platforms, have often been the fundamental barrier. Here we present an integration of a mature “neuromimetic” chip, SpiNNaker, with the humanoid iCub robot using a direct AER - address-event representation - interface that overcomes the need for complex proprietary protocols by sending information as UDP-encoded spikes over an Ethernet link. Using an existing neural model devised for visual object selection, we enable the robot to perform a real-world task: fixating attention upon a selected stimulus. Results demonstrate the effectiveness of interface and model in being able to control the robot towards stimulus-specific object selection. Using SpiNNaker as an embeddable neuromorphic device illustrates the importance of two design features in a prospective neurorobot: universal configurability that allows the chip to be conformed to the requirements of the robot rather than the other way ’round, and stan- dard interfaces that eliminate difficult low-level issues of connectors, cabling, signal voltages, and protocols. While this study is only a building block towards that goal, the iCub-SpiNNaker system demonstrates a path towards meaningful behaviour in robots controlled by neural network chips

    Modelling electrified railway signalling misoperations during extreme space weather events in the UK

    Space weather has the potential to impact ground-based technologies on Earth, affecting many systems including railway signalling. This study uses a recently developed model to analyse the impact of geomagnetically induced currents on railway signalling systems in the United Kingdom during the March 1989 and October 2003 geomagnetic storms. The March 1989 storm is also scaled to estimate a 1-in-100 year and a 1-in-200 year extreme storm. Both the Glasgow to Edinburgh line, and the Preston to Lancaster section of the West Coast Main Line are modelled. No “right side” failures (when unoccupied sections appear occupied) are suggested to have occurred during either storm, and the total number of potential “wrong side” failures (when occupied sections appear clear) is low. However, the modelling indicates “right side” and “wrong side” failures are possible on both routes during the 1-in-100 year and 1-in-200 year extreme storms, with the Glasgow to Edinburgh line showing more total misoperations than the Preston to Lancaster section of the West Coast Main Line. A 1-in-100 year or 1-in-200 year extreme storm would result in misoperations over an extended period of time, with most occurring over a duration of 2–3 h either side of the peak of the storm

    Pragmatic cluster randomised cohort cross-over trial to determine the effectiveness of bridging from emergency to regular contraception:the Bridge-It study protocol

    Introduction Oral emergency contraception (EC) can prevent unintended pregnancy but it is important to start a regular method of contraception. Women in the UK usually access EC from a pharmacy but then need a subsequent appointment with a general practitioner or a sexual and reproductive health (SRH) service to access regular contraception. Unintended pregnancies can occur during this time. Methods and analysis Bridge-It is a pragmatic cluster randomised cohort cross-over trial designed to determine whether pharmacist provision of a bridging supply of a progestogen-only pill (POP) plus rapid access to a local SRH clinic, results in increased uptake of effective contraception and prevents more unintended pregnancies than provision of EC alone. Bridge-It involves 31 pharmacies in three UK regions (London, Lothian and Tayside) aiming to recruit 626–737 women. Pharmacies will give EC (levonorgestrel) according to normal practice and recruit women to both intervention and the control phases of the study. In the intervention phase, pharmacists will provide the POP (desogestrel) and offer rapid access to an SRH clinic. In the control phase, pharmacists will advise women to attend a contraceptive provider for contraception (standard care). Women will be asked 4 months later about contraceptive use. Data linkage to abortion registries will provide abortion rates over 12 months. The sample size is calculated on the primary outcome of effective contraception use at 4 months (yes/no) with 90% power and a 5% level of significance. Abortion rates will be an exploratory secondary analysis. Process evaluation includes interviews with pharmacists, SRH clinicians and women. Cost-effectiveness analysis will use a healthcare system perspective and be expressed as incremental cost-effectiveness ratio. Ethics and dissemination Ethical approval was received from South East Scotland REC June 2017. Results will be published in peer-reviewed journals and conference presentations.     Use of effective contraception following provision of the progestogen-only pill for women presenting to community pharmacies for emergency contraception (Bridge-It):a pragmatic cluster-randomised crossover trial

    BACKGROUND:Unless women start effective contraception after oral emergency contraception, they remain at risk of unintended pregnancy. Most women in the UK obtain emergency contraception from community pharmacies. We hypothesised that pharmacist provision of the progestogen-only pill as a bridging interim method of contraception with emergency contraception plus an invitation to a sexual and reproductive health clinic, in which all methods of contraception are available, would result in increased subsequent use of effective contraception. METHODS:We did a pragmatic cluster-randomised crossover trial in 29 UK pharmacies among women receiving levonorgestrel emergency contraception. Women aged 16 years or older, not already using hormonal contraception, not on medication that could interfere with the progestogen-only pill, and willing to give contact details for follow-up were invited to participate. In the intervention group, women received a 3-month supply of the progestogen-only pill (75 μg desogestrel) plus a rapid access card to a participating sexual and reproductive health clinic. In the control group, pharmacists advised women to attend their usual contraceptive provider. The order in which each pharmacy provided the intervention or control was randomly assigned using a computer software algorithm. The primary outcome was the use of effective contraception (hormonal or intrauterine) at 4 months. This study is registered, ISRCTN70616901 (complete). FINDINGS:Between Dec 19, 2017, and June 26, 2019, 636 women were recruited to the intervention group (316 [49·6%], mean age 22·7 years [SD 5·7]) or the control group (320 [50·3%], 22·6 years [5·1]). Three women (one in the intervention group and two in the control group) were excluded after randomisation. 4-month follow-up data were available for 406 (64%) participants, 25 were lost to follow-up, and two participants no longer wanted to participate in the study. The proportion of women using effective contraception was 20·1% greater (95% CI 5·2-35·0) in the intervention group (mean 58·4%, 48·6-68·2), than in the control group (mean 40·5%, 29·7-51·3 [adjusted for recruitment period, treatment group, and centre]; p=0·011).The difference remained significant after adjusting for age, current sexual relationship, and history of effective contraception use, and was robust to the effect of missing data (assuming missingness at random). No serious adverse events occurred. INTERPRETATION:Provision of a supply of the progestogen-only pill with emergency contraception from a community pharmacist, along with an invitation to a sexual and reproductive health clinic, results in a clinically meaningful increase in subsequent use of effective contraception. Widely implemented, this practice could prevent unintended pregnancies after use of emergency contraception.     Why barcode? High-throughput multiplex sequencing of mitochondrial genomes for molecular systematics

    Mitochondrial genome sequences are important markers for phylogenetics but taxon sampling remains sporadic because of the great effort and cost required to acquire full-length sequences. Here, we demonstrate a simple, cost-effective way to sequence the full complement of protein coding mitochondrial genes from pooled samples using the 454/Roche platform. Multiplexing was achieved without the need for expensive indexing tags (‘barcodes’). The method was trialled with a set of long-range polymerase chain reaction (PCR) fragments from 30 species of Coleoptera (beetles) sequenced in a 1/16th sector of a sequencing plate. Long contigs were produced from the pooled sequences with sequencing depths ranging from ∼10 to 100× per contig. Species identity of individual contigs was established via three ‘bait’ sequences matching disparate parts of the mitochondrial genome obtained by conventional PCR and Sanger sequencing. This proved that assembly of contigs from the sequencing pool was correct. Our study produced sequences for 21 nearly complete and seven partial sets of protein coding mitochondrial genes. Combined with existing sequences for 25 taxa, an improved estimate of basal relationships in Coleoptera was obtained.
