    The Soil-Cement Brick on Construction with Structural Masonry – An Alternative in the Fight Against Housing Deficit and Environmental Pollution in the State of Rondônia*

    The Housing Deficit in the State of Rondônia is increasing, not only due to the absence of effective public policies dedicated to the treatment of housing, but also due to the lack of study and application of new technologies in the area of civil construction, which can generate great savings and remarkable rapidity in the construction of housing units. This deficit is characterized by the lack of housing and the existence of those considered inadequate to human occupation with quality and safety, due to its operationality, physical characteristics and location. However, there is a need for greater economy and productivity in the construction of housing, in order to be able to supply part of this demand, without letting the quality levels drop. In the light of Sustainable Development, it will be proposed and directed the correction of this paradigm that tends to hold more weight in the "social" dimension. The application of the Structural Masonry System with the use of the Cement Brick, popularly known as "ecological brick", is the main basis of this study, where it is intended to demonstrate that its option for housing construction can be an excellent response to the housing deficit and environmental pollution, generated by the rubbish of Civil Construction in the State of Rondônia

    The purpose of this article is to describe the amendments made in the new version of DSM (fifth edition) as to understand not only main aspects of certain diseases and/or personality disorders, but also whether or not changes shifted focus to dimensional instead of categorical system or if in some aspects both are considered. Moving forward from a general overview of the handbook, the effective changes will be examined in detail. Attention will also be given to Personality Diseases, taking account of various studies on this subject and the most recent knowledge on "Borderline Disease" from a diagnostic standpoint. Considering the complex construction of such disease, will be firstly illustrated the assumption of first studies so as to cover the most recent clinical and theoretical contributions, together with a brief overview of some treatment approach such as psychodynamic, cognitive-behavioral and systemic-relational, principles of medical treatment, a summary on differential diagnosis and a representation of some clinical case as well. The article will be concluded by a brief discussion of future prospects able to provide the clinician with even more effective and safe operational strategies as to lead professionals towards more specific and less misleading assumptions for a better approach in any scope (therapeutic, forensic, etc.) and giving comfort to those who receive support as well. Everyone is a very unique individual, even if similar to others. That is why it is extremely important to clearly identify a disease and ensure an adequate treatment. RiassuntoL'obiettivo del presente lavoro, è quello di descrivere le principali modifiche apportate nella nuova versione del DSM (quinta ed.) rispetto a quella precedente, per cercare di comprendere non solo gli aspetti più significativi di determinate Patologie e/o Disturbi della Personalità, ma anche se nei suindicati cambiamenti, l'ottica si è spostata o meno ad un sistema dimensionale piuttosto che categoriale o se in alcuni aspetti, vengono considerati entrambi, partendo quindi da una panoramica generale sul Manuale, per poi affrontare nel dettaglio le reali modifiche. Verrà inoltre approfondita la tematica relativa ai Disturbi della Personalità, tenendo presenti i vari studi e ricerche sull'argomento e quindi le acquisizioni più recenti in termini di strumenti operativi utili da un punto di vista diagnostico-clinico, con particolare riferimento al "Disturbo Borderline", decisamente complesso nel suo costrutto, partendo dall'assunto relativo ai primi studi sul tema, fino a giungere ai vari contributi teorico-clinici più recenti, unitamente ai cenni su alcuni approcci terapeutici quali ad esempio, psicodinamico, cognitivo-comportamentale e sistemico-relazionale, principi sulla farmacoterapia ed una sintesi sulla diagnosi differenziale, nonché la rappresentazione di alcuni Casi clinici. Concludendo con un sintetico excursus su quelle che potrebbero essere le prospettive future in grado di fornire strategie operative sempre più fertili e sicure per il clinico, sia a conforto dello stesso che di chi usufruisce dell'aiuto, guidando gli operatori verso ipotesi più specifiche e meno devianti, per un migliore approccio in qualsiasi ambito di applicazione (terapeutico, forense ecc..). Ogni soggetto è un individuo a se stante, pur evidenziando caratteristiche simili. Per questo è estremamente importante per quanto più è possibile, riuscire nell'individuazione di una precisa patologia su cui va applicato un mirato processo trattamentale. ResumenEl propósito de este artículo es describir las enmiendas hechas en la nueva versión del DSM (quinta edición) para comprender no solo los aspectos principales de ciertas enfermedades y / o trastornos de la personalidad, sino también si los cambios cambiaron el enfoque a dimensional en lugar de categórico sistema o si en algunos aspectos se consideran ambos. Avanzando desde una descripción general del manual, los cambios efectivos serán examinados en detalle. También se prestará atención a las enfermedades de la personalidad, teniendo en cuenta varios estudios sobre este tema y el conocimiento más reciente sobre "Enfermedad límite" desde un punto de vista diagnóstico. Teniendo en cuenta la compleja construcción de dicha enfermedad, se ilustrará en primer lugar la suposición de los primeros estudios para cubrir las contribuciones clínicas y teóricas más recientes, junto con una breve descripción de algunos enfoques de tratamiento, tales como psicodinámico, cognitivo-conductual y sistémico-relacional, principios del tratamiento médico, un resumen sobre diagnóstico diferencial y una representación de algunos casos clínicos también. El artículo concluirá con una breve discusión de las perspectivas futuras capaces de proporcionar al clínico estrategias operativas aún más efectivas y seguras para guiar a los profesionales hacia suposiciones más específicas y menos engañosas para un mejor enfoque en cualquier ámbito (terapéutico, forense, etc.). y dar consuelo a quienes también reciben apoyo.     Detection of potential enteric pathogens in children with severe acute gastroenteritis using the filmarray: Results from a three - years hospital-based survey in Northern Italy

    Acute gastroenteritis (AGE) are leading causes of morbidity and mortality in children. Therefore, rapid pathogens identification is needed. The AGE aetiology was investigated from 2018 to 2020 in 2,066 children in Parma (Italy) by FilmArray Gastrointestinal Panel and Enterovirus-targeting RT-PCR. Pathogens were detected in 1,162 (56.2%) stool samples from as many children; 798 (68.7%) were single and 364 (31.3%) mixed infections (68.7% vs 31.3%, P < 0.0001). Children aged 0–5 years showed the highest infection incidence (66.1%). The most frequent pathogens were Enteropathogenic Escherichia coli (EPEC; 19.14%), Clostridioides difficile (10.42%), Norovirus (10.36%), Enterovirus (9.44%), and Campylobacter (9.21%). EPEC, Campylobacter, enteroaggregative E. coli, Norovirus, and Rotavirus showed seasonality. The incidence of pathogens detected decreased between 2018 and 2020 (42.7% vs 20.8%, P < 0.0001), seemingly for the preventive measures imposed by the severe acute respiratory syndrome coronavirus-2 pandemic.     Estudo multicêntrico de avaliação perioperatória para operações não cardíacas (EMAPO)

    INTRODUCTION: The accuracy of perioperative evaluation methods available is better than chance, but their performance is not ideal. OBJECTIVES: To compare a new evaluation method (EMAPO) to the American College of Physicians method for determining the risk of cardiovascular complications in noncardiac surgeries and to look for new influencing variables. METHODS: Evaluations through EMAPO and the American College of Physicians method were employed for 700 patients. Cardiac events and deaths were recorded, the risk variables related to the occurrence of complications were verified, and the models were compared by analyzing the areas under the receiver operating characteristic curves. RESULTS: Mortality rate was 3.4%, and the incidence of cardiovascular complications was 5.3%. Renal failure (P = 0.01), major surgery (P = 0.004), and emergency surgery (P = 0.003) were independently related to the occurrence of cardiovascular complications. The two methods produced similar results. CONCLUSION: EMAPO is as good as the American College of Physicians method in determining the risk of cardiovascular complications in noncardiac surgeries. New variables related to surgical risk were identified.INTRODUÇÃO: A precisão dos métodos de avaliação perioperatória disponíveis é melhor que o acaso, porém está longe do ideal. OBJETIVOS: Comparar um novo método de avaliação perioperatória (EMAPO) ao método do American College of Physicians para determinar o risco cardíaco em cirurgias não cardíacas e buscar novas variáveis envolvidas na determinação deste risco. MÉTODOS: O EMAPO e o método do American College of Physicians foram aplicados em 700 pacientes. A ocorrência de eventos cardíacos e de mortes foi documentada, a relação entre as variáveis de risco e as complicações foi estabelecida e os métodos foram comparados analisando as áreas sob a curva ROC. RESULTADOS: A mortalidade foi 3.4% e a incidência de complicações cardiovasculares 5.3%. A presença de insuficiência renal (p=0.01), cirurgia de grande porte (p=0.004) e cirurgia de emergência (p=0.003) se correlacionaram com a ocorrência de complicações cardiovasculares na análise multivariada. Não houve diferença entre os dois métodos. CONCLUSÕES: O EMAPO é tão eficaz quanto o método do American College of Physicians para determinar o risco de complicações cardiovasculares em cirurgias não cardíacas.     Stalking of social healthcare workers: Preliminary results of a study conducted in Sardinia, Italy

    Several studies have highlighted that particular social and professional categories are at higher risk of being victim of stalking. Physicians and healthcare workers in general are at risk of being harassed while practising their profession, especially by their patients; this risk has been shown to be higher for psychiatrists and psychotherapists. According to a recent study, 21% of psychiatrists has been a victim of stalking; length of stalking behaviour on healthcare workers varies from a few weeks to several years. Aim of the present work is to gain a better understanding and a highlight on the frequency with which stalking occurs toward social and healthcare workers of public health facilities in Sardinia. After a review of literature, an anonymous questionnaire, prepared ad-hoc, has been administered to a sample of 265 healthcare workers in Sardinia: out of the 210 who have answered, we have taken into consideration 187 healthcare workers, have excluded 23 operators (psychiatric rehabilitators, professional educators, social workers) and chosen to analyze victimization suffered by psychiatrists, psychologists, nurses, social healthcare workers. In our sample the subjects who reported harassments comparable to the crime of stalking are represented by 30 healthcare workers out of the total sample of 187 (16%). Females are the most frequently represented with a ratio of 23/30 (77%) compared to males 7/30 (23%). The risk of being a victim of harassment was higher for the group of psychiatrists and psychologists than for nurses. Among the victims, 18 were psychiatrists (60%), 3 psychologists (10%), and 9 nurses (30%). Healthcare professionals are at high risk of victimization. The main tool available to the services is the team approach to multi-problematic cases, avoiding isolation of the dyad designated operator-patient. The feeling of impotence and isolation that is frequent in the victims of stalking, as shown also in our sample, is highlighted by several studies in which 10% of the victims interviewed claims to have thoughts of suicide as a way out

    Prevalence of Intestinal Parasitoses in a Non-Endemic Setting during a 10-Year Period (2011–2020): A Focus on Dientamoeba fragilis

    Dientamoeba fragilis is a cosmopolitan and neglected protozoan. Although little is known concerning its pathogenicity and its true prevalence worldwide, its role as enteric pathogen is emerging, as the occurrence of dientamoebiasis has increased also in industrialised countries. This study investigated the occurrence and prevalence of intestinal parasites, focusing on D. fragilis in a 10-year period (2011–2020) in a single tertiary-care hospital located in Northern Italy. A statistical evaluation of the correlation between dientamoebiasis and specific signs other than gastrointestinal-related ones was performed. The laboratory diagnosis was performed on 16,275 cases of suspected intestinal parasitoses. Intestinal parasites were detected in 3254 cases, 606 of which were associated to D. fragilis, which represented 18.6% (606/3254) of all the intestinal parasitoses with a 3.7% (606/16,275) prevalence and an increasing trend during the last five years (2011–2015: 2.8% vs. 2016–2020: 4.8%). D. fragilis was commonly detected in foreigners, especially those from developing countries, as well as in children; prevalence was equal in males and females. With regard to the clinical aspect, the only putative sign statistically related to dientamoebiasis was anal pruritus. Despite the controversial epidemiological knowledges on dientamoebiasis, the prevalence of D. fragilis found in this study highlights the need to consider this parasite in any differential diagnosis of gastrointestinal disease

    Modulation of micrornome by human cytomegalovirus and human herpesvirus 6 infection in human dermal fibroblasts: Possible significance in the induction of fibrosis in systemic sclerosis

    Human cytomegalovirus (HCMV) and Human herpesvirus 6 (HHV‐6) have been report-edly suggested as triggers of the onset and/or progression of systemic sclerosis (SSc), a severe autoimmune disorder characterized by multi‐organ fibrosis. The etiology and pathogenesis of SSc are still largely unknown but virological and immunological observations support a role for these beta-herpesviruses, and we recently observed a direct impact of HCMV and HHV‐6 infection on the expression of cell factors associated with fibrosis at the cell level. Since miRNA expression has been found profoundly deregulated at the tissue level, here we aimed to investigate the impact on cell microRNome (miRNome) of HCMV and HHV‐6 infection in in vitro infected primary human dermal fibroblasts, which represent one of the main SSc target cells. The analysis, performed by Taq-man arrays detecting and quantifying 754 microRNAs (miRNAs), showed that both herpesviruses significantly modulated miRNA expression in infected cells, with evident early and late effects and deep modulation (>10 fold) of >40 miRNAs at each time post infection, including those previously recognized for their key function in fibrosis. The correlation between these in vitro results with in vivo observations is strongly suggestive of a role of HCMV and/or HHV‐6 in the multistep patho-genesis of fibrosis in SSc and in the induction of fibrosis‐signaling pathways finally leading to tissue fibrosis. The identification of specific miRNAs may open the way to their use as biomarkers for SSc diagnosis, assessment of disease progression and possible antifibrotic therapies

    Rapid microbial identification and phenotypic antimicrobial susceptibility testing directly from positive blood cultures: a new platform compared to routine laboratory methods

    The Accelerate Pheno™ System (APS), a new platform that combines rapid identification (ID) of bacteria and yeasts and phenotypic antimicrobial susceptibility testing (AST) in a single assay, has been evaluated directly from positive blood cultures in comparison to routine laboratory methods. The APS ID results showed an overall sensitivity and specificity of 92.6% and 99.6%, respectively. With regard to AST results, 31 discrepancies (8 single errors and 23 combined errors) were observed, including 13 major errors (3.3%) and 18 minor errors (4.6%) mainly involving Pseudomonas aeruginosa. No very major error was observed. The APS ID results were obtained in 1.5 h and the AST results were available in 7 h, on average 34.1 h before routine laboratory methods. This reduction in AST time-to-result represents one of the main advantages of this technology, reducing the time to provide to the physician the microbiological report

    Rapid classification of clostridioides difficile strains using maldi-tof ms peak-based assay in comparison with pcr-ribotyping

    Typing methods are needed for epidemiological tracking of new emerging and hyperviru-lent strains because of the growing incidence, severity and mortality of Clostridioides difficile infections (CDI). The aim of this study was the evaluation of a typing Matrix-Assisted Desorption/Ionization-Time of Flight Mass Spectrometry (MALDI-TOF MS (T-MALDI)) method for the rapid classification of the circulating C. difficile strains in comparison with polymerase chain reaction (PCR)-ribotyping results. Among 95 C. difficile strains, 10 ribotypes (PR1–PR10) were identified by PCR-ribotyping. In particular, 93.7% of the isolates (89/95) were grouped in five ribotypes (PR1–PR5). For T-MALDI, two classifying algorithm models (CAM) were tested: the first CAM involved all 10 ribotypes whereas the second one only the PR1–PR5 ribotypes. Better performance was obtained using the second CAM: recognition capability of 100%, cross-validation of 96.6% and agreement of 98.4% (60 correctly typed strains, limited to PR1–PR5 classification, out of 61 examined strains) with PCR-ribotyping results. T-MALDI seems to represent an alternative to PCR-ribotyping in terms of reproducibility, set up time and costs, as well as a useful tool in epidemiological investigation for the detection of C. difficile clusters (either among CAM included ribotypes or out-of-CAM ribotypes) involved in outbreaks
