Five-membered iminocyclitol a-glucosidase inhibitors: Synthetic,biological screening and in silico studies
The design and synthesis of a small library of pyrrolidine iminocyclitol inhibitors with a structural similarity
to 1,4-dideoxy-1,4-imino-D-arabitol (DAB-1) is reported. This library was specifically designed to
gain a better insight into the mechanism of inhibition of glycosidases by polyhydroxylated pyrrolidines
or iminocyclitols. Pyrrolidine-3,4-diol 15a and pyrrolidine-3,4-diol diacetate 15b had emerged as the
most potent a-glucosidase inhibitors in the series. Docking studies performed with an homology model
of a-glucosidase disclosed binding poses for compounds 15a, 15b, 16a, and 16a0 occupying the same
region as the NH group of the terminal ring of acarbose and suggest a closer and stronger binding of compound
15a and 15b with the enzyme active site residues. Our studies indicate that 2 or 5-hydroxyl substituents
appear to be vital for high inhibitory activity

Contactless Monitoring System Versus Gold Standard for Respiratory Rate Monitoring in Emergency Department Patients: Pilot Comparison Study
Background: Respiratory rate is a crucial indicator of disease severity yet is the most neglected vital sign. Subtle changes in respiratory rate may be the first sign of clinical deterioration in a variety of disease states. Current methods of respiratory rate monitoring are labor-intensive and sensitive to motion artifacts, which often leads to inaccurate readings or underreporting; therefore, new methods of respiratory monitoring are needed. The PulsON 440 (P440; TSDR Ultra Wideband Radios and Radars) radar module is a contactless sensor that uses an ultrawideband impulse radar to detect respiratory rate. It has previously demonstrated accuracy in a laboratory setting and may be a useful alternative for contactless respiratory monitoring in clinical settings; however, it has not yet been validated in a clinical setting.
Objective: The goal of this study was to (1) compare the P440 radar module to gold standard manual respiratory rate monitoring and standard of care telemetry respiratory monitoring through transthoracic impedance plethysmography and (2) compare the P440 radar to gold standard measurements of respiratory rate in subgroups based on sex and disease state.
Methods: This was a pilot study of adults aged 18 years or older being monitored in the emergency department. Participants were monitored with the P440 radar module for 2 hours and had gold standard (manual respiratory counting) and standard of care (telemetry) respiratory rates recorded at 15-minute intervals during that time. Respiratory rates between the P440, gold standard, and standard telemetry were compared using Bland-Altman plots and intraclass correlation coefficients.
Results: A total of 14 participants were enrolled in the study. The P440 and gold standard Bland-Altman analysis showed a bias of –0.76 (–11.16 to 9.65) and an intraclass correlation coefficient of 0.38 (95% CI 0.06-0.60). The P440 and gold standard had the best agreement at normal physiologic respiratory rates. There was no change in agreement between the P440 and the gold standard when grouped by admitting diagnosis or sex.
Conclusions: Although the P440 did not have statistically significant agreement with gold standard respiratory rate monitoring, it did show a trend of increased agreement in the normal physiologic range, overestimating at low respiratory rates, and underestimating at high respiratory rates. This trend is important for adjusting future models to be able to accurately detect respiratory rates. Once validated, the contactless respiratory monitor provides a unique solution for monitoring patients in a variety of settings

Food Allergy in Children - Data from an Allergy Outpatient Clinic
Estima-se que a prevalĂŞncia de alergia alimentar nos paĂses Ocidentais seja de cerca de 2 % na população geral e atĂ© 8% em crianças, nĂŁo existindo dados concretos
no que diz respeito a Portugal.
Objectivos: Avaliar a prevalência de alergia alimentar e identificar os alergenos alimentares principais numa população de crianças observadas na Consulta
de Imunoalergologia do Hospital de Dona Estefânia num perĂodo de 12 meses.
MĂ©todos: Foi feita uma revisĂŁo de registos clĂnicos dos 4879 doentes com idade igual ou inferior a 18 anos observados na Consulta durante o ano de 1998. O diagnĂłstico baseou-se na histĂłria clĂnica, testes cutâneos por prick e prova de provocação oral. Foram
incluĂdos os casos de alergia alimentar clinicamente relevante nos Ăşltimos trĂŞs anos de vida.
Resultados: Foi identificada uma prevalência de alergia alimentar de 8,5% (414 casos, correspondendo a 477 quadros de alergia alimentar), sendo o alimento alergénico mais importante o leite, seguido por ovo e
peixe. No subgrupo de crianças com idade superior a 12 anos o padrão foi bastante diferente, surgindo os crustáceos, o peixe, o amendoim, os frutos frescos e secos como principais alergenos. A maioria das crianças
(83%) apresentou sensibilização a apenas um alimento.
Clinicamente, predominaram os quadros de urticária e angioedema, seguidos por vómitos, diarreia e agravamento
de dermite atĂłpica.
Conclusões: A população estudada apresenta uma prevalência de alergia alimentar de 8,5%, sendo de prever que seja inferior na população geral pediátrica.
Será interessante complementar este estudo com dados obtidos noutros grupos etários, visando uma melhor identificação da prevalência de alergia alimentar e dos
alergenos alimentares major no nosso paĂs. É

Nobody Wants to Be Narcan\u27d: A Pilot Qualitative Analysis of Drug Users\u27 Perspectives on Naloxone
INTRODUCTION: Bystander naloxone distribution is an important component of public health initiatives to decrease opioid-related deaths. While there is evidence supporting naloxone distribution programs, the effects of increasing naloxone availability on the behavior of people who use drugs have not been adequately delineated. In this study we sought to 1) evaluate whether individuals\u27 drug use patterns have changed due to naloxone availability; and 2) explore individuals\u27 knowledge of, access to, experiences with, and perceptions of naloxone.
METHODS: We conducted a pilot study of adults presenting to the emergency department whose medical history included non-medical opioid use. Semi-structured interviews were conducted with participants and thematic analysis was used to code and analyze interview transcripts.
RESULTS: Ten participants completed the study. All were aware of naloxone by brand name (Narcan) and had been trained in its use, and all but one had either currently or previously possessed a kit. Barriers to naloxone administration included fear of legal repercussions, not having it available, and a desire to avoid interrupting another user\u27s high. Of the eight participants who reported being revived with naloxone at least once during their lifetime, all described experiencing a noxious physical response and expressed a desire to avoid receiving it again. Furthermore, participants did not report increasing their use of opioids when naloxone was available.
CONCLUSIONS: Participants were accepting of and knowledgeable about naloxone, and were willing to administer naloxone to save a life. Participants tended to use opioids more cautiously when naloxone was present due to fears of experiencing precipitated withdrawal. This study provides preliminary evidence countering the unsubstantiated narrative that increased naloxone availability begets more high-risk opioid use and further supports increasing naloxone access

We investigate the structural and magnetic properties of two molecule-based magnets synthesized from the same starting components. Their different structural motifs promote contrasting exchange pathways and consequently lead to markedly different magnetic ground states. Through examination of their structural and magnetic properties we show that [Cu(pyz)(H 2 O)(gly) 2 ](ClO 4 ) 2 may be considered a quasi-one-dimensional quantum Heisenberg antiferromagnet whereas the related compound [Cu(pyz)(gly)](ClO 4 ) , which is formed from dimers of antiferromagnetically interacting Cu 2+ spins, remains disordered down to at least 0.03 K in zero field but shows a field-temperature phase diagram reminiscent of that seen in materials showing a Bose-Einstein condensation of magnons

Cross-Cultural Validation of the Portuguese from Portugal Version of the Test for Respiratory and Asthma Control in Kids (TRACK) Questionnaire
Introduction. The Test for Respiratory and Asthma Control in Kids (TRACK) is a tool to assess asthma control in preschool children. This study aims to validate the Portuguese from Portugal version of the TRACK questionnaire. Methods. A prospective cohort study was carried out to assess their psychometric characteristics. Caregivers of 141 children under age 5 with asthma symptoms were enrolled. Results. Internal reliability was close to 0.70 (Cronbach's α). The test-retest reliability was 0.87. TRACK scores were different between well, partially, and non-controlled asthma groups (p less than 0.001). Patients rated as having better control showed an increase in TRACK scores. Conclusions. The Portuguese version of the TRACK questionnaire is accurate and reliable for monitoring asthma control. Factors Associated With Asthma Control in 121 Preschool Children
Background: Data on risk factors for uncontrolled asthma in preschool children are controversial.
Objective: This study aims to explore the association between clinical and functional parameters and the lack of asthma control in preschool children.
Methods: Children aged 3-5 years with asthma and healthy controls were recruited. A questionnaire was used to identify potential risk factors for uncontrolled asthma, as defined by the Global INitiative for Asthma criteria. Lung function and bronchial reversibility were evaluated through impulse oscillometry and spirometry. Adjusted odds ratios were estimated based on multivariable generalized additive regression models. The discriminative ability of the models was measured by the area under the receiver operating characteristic curve (AUC).
Results: The study population comprised 121 children (107 with asthma and 14 healthy controls). Fifty-three patients (50%) had uncontrolled asthma. After adjustment, the variables associated with an increased risk of lack of control were as follows: “More than 3 flare-ups in the last 12 months”, “Moderate to severe rhinitis”, and “Relative variation in postbronchodilator FVC and FEV1”. The AUC of the final models that included variation in FVC or FEV1 were 0.82 and 0.81, respectively. The R5-20, R5-20%, and AX z-score values of the healthy
group were lower than those of children with asthma.
Conclusion: In preschool children, clinical and functional parameters are associated with uncontrolled asthma. More studies are needed to confirm the usefulness of impulse oscillometry.info:eu-repo/semantics/publishedVersio
Different goals, different appropriations? The use of Facebook by bottom-up initiatives of digital democracy in Brazil
O trabalho analisa como iniciativas civis de democracia digital brasileiras utilizam o Facebook nas suas atividades. Objetiva-se verificar como diferentes tipos de projetos se apropriam da ferramenta e se o seu uso condiz com o seu propĂłsito de ação polĂtica. Durante seis meses, foi analisada a atuação do Meu Rio, do Cidade Democrática e da Rede Nossa SĂŁo Paulo. No total, 951 posts foram categorizados quanto ao seu recorte temático. Ao final, verificou-se que há um predomĂnio de postagens do tipo informativas.Palavras-chave: Facebook, redes sociais online, democracia digital.This article analyzes how Brazilian bottom- up initiatives of digital democracy use Facebook in their activities. It aims to see how the tool is appropriated by the different types of projects, and if its use is consistent with its purpose of political action. Over six months, we analyzed the performance of Meu Rio, Cidade Democrática and Rede Nossa SĂŁo Paulo. In total, 951 posts were categorized according to their thematic focus.
