    Fair Access to Scarce Medical Capacity for Non-Covid-19 Patients: A Role for Reserves

    As hospitals in the US and elsewhere fill again with patients with covid-19, discussions about how to fairly allocate scarce medical resources have come to the fore once again. One frequently voiced concern is that non-covid-19 patients with urgent health needs are facing indefinitely postponed surgeries, long-distance hospital transfers, or even are unable to access medical treatment. In our view, a reserve or categorised priority system could help. It could be used to fairly distribute scarce medical capacity—such as staffing, physical space, and medical treatments—between covid-19 and non-covid-19 patients, just as it has been used or proposed to allocate covid-19 therapeutics and vaccines

    Work-related injuries sustained by emergency medical technicians and paramedics in Turkey

    BACKGROUND: Evaluated in the present study were locations, descriptions, and results of work-related injuries (WRIs) sustained by emergency medical technicians (EMTs) and paramedics in Turkey’s most crowded city, İstanbul. METHODS: After the present study had been accepted by the urban health authority, a questionnaire was emailed to the healthcare personnel of İstanbul’s 195 ambulance stations. RESULTS: Included in the present study were the responses of 901 members of staff (660 EMTs and 241 paramedics), with a mean age of 29.5±6.1 (min: 18; max: 61). The majority of participants (94.9%) had encountered verbal abuse from the public, and 39.8% had encountered physical violence from patients’ relatives. Levels of satisfaction with work in emergency medical services (EMS) was also evaluated, and 510 participants (57.6%) were unhappy. Regarding gender, female employees were more likely to be verbally attacked (p=0.01), while males were more likely to be physically attacked (p=0.001). It was reported that motor vehicle accidents (MVAs) were the most common cause of WRIs (81.4%), followed by needle-stick injuries (52.2%), ocular exposure to blood and other fluids (30.9%), and sharp injuries (22.5%). Only 10.5% (n=95) of WRIs were reported to authorities; 488 (54.2%) of participants just attended to the practice to prevent possible WRIs. CONCLUSION: For paramedics and EMTs, risk of WRI is obviously high. Strategies to decrease and prevent verbal and physical violence should be developed. © 2016 TJTES

    cis-Acting Complex-Trait-Associated lincRNA Expression Correlates with Modulation of Chromosomal Architecture.

    Intergenic long noncoding RNAs (lincRNAs) are the largest class of transcripts in the human genome. Although many have recently been linked to complex human traits, the underlying mechanisms for most of these transcripts remain undetermined. We investigated the regulatory roles of a high-confidence and reproducible set of 69 trait-relevant lincRNAs (TR-lincRNAs) in human lymphoblastoid cells whose biological relevance is supported by their evolutionary conservation during recent human history and genetic interactions with other trait-associated loci. Their enrichment in enhancer-like chromatin signatures, interactions with nearby trait-relevant protein-coding loci, and preferential location at topologically associated domain (TAD) boundaries provide evidence that TR-lincRNAs likely regulate proximal trait-relevant gene expression in cis by modulating local chromosomal architecture. This is consistent with the positive and significant correlation found between TR-lincRNA abundance and intra-TAD DNA-DNA contacts. Our results provide insights into the molecular mode of action by which TR-lincRNAs contribute to complex human traits

    This study was conducted to investigate the effects of intratesticular glycerol and ethanol injection on sterilization. Twelve mature male dogs were used. They were allocated randomly to two equal groups called glycerol and ethanol and one ml glycerol (70%) and ethanol (95%) was injected intratesticularly. The animals were observed during two months. The blood and sperm samples obtained once a week in all groups were examined regarding to serum testosterone level and spermatozoon concentration and motility rate. There was a fistula formation and scrotal wounds resulting from orchitis in all glycerol and two ethanol group dogs after the 5th day of injection. The fistular openings closed spontaneously in all cases in due course. After the 2nd month, testicular atrophy was seen to occur. It was determined the presence of marked decreases (p<0.01) in both groups spermatozoon concentration and motility as well as in the level of serum testosterone two weeks after drug injection. The histopathologic examination conducted after the 2nd month revealed degeneration, necrosis and diffuse connective tissue proliferation in the seminiferus contortus tubulus of both groups. It has been concluded that glycerol and ethanol can be effective for sterilization in male dogs

    A framework to evaluate the viability of robotic process automation for business process activities

    Robotic process automation (RPA) is a technology for centralized automation of business processes. RPA automates user interaction with graphical user interfaces, whereby it promises efficiency gains and a reduction of human negligence during process execution. To harness these benefits, organizations face the challenge of classifying process activities as viable automation candidates for RPA. Therefore, this work aims to support practitioners in evaluating RPA automation candidates. We design a framework that consists of thirteen criteria grouped into five perspectives which offer different evaluation aspects. These criteria leverage a profound understanding of the process step.     A novel approach for rapid screening of mitochondrial D310 polymorphism

    BACKGROUND: Mutations in the mitochondrial DNA (mtDNA) have been reported in a wide variety of human neoplasms. A polynucleotide tract extending from 303 to 315 nucleotide positions (D310) within the non-coding region of mtDNA has been identified as a mutational hotspot of primary tumors. This region consists of two polycytosine stretches interrupted by a thymidine nucleotide. The number of cytosines at the first and second stretches are 7 and 5 respectively, according to the GeneBank sequence. The first stretch exhibits a polymorphic length variation (6-C to 9-C) among individuals and has been investigated in many cancer types. Large-scale studies are needed to clarify the relationship between cytosine number and cancer development/progression. However, time and money consuming methods such as radioactivity-based gel electrophoresis and sequencing, are not appropriate for the determination of this polymorphism for large case-control studies. In this study, we conducted a rapid RFLP analysis using a restriction enzyme, BsaXI, for the single step simple determination of 7-C carriers at the first stretch in D310 region. METHODS: 25 colorectal cancer patients, 25 breast cancer patients and 41 healthy individuals were enrolled into the study. PCR amplification followed by restriction enzyme digestion of D310 region was performed for RFLP analysis. Digestion products were analysed by agarose gel electrophoresis. Sequencing was also applied to samples in order to confirm the RFLP data. RESULTS: Samples containing 7-C at first stretch of D310 region were successfully determined by the BsaXI RFLP method. Heteroplasmy and homoplasmy for 7-C content was also determined as evidenced by direct sequencing. Forty-one percent of the studied samples were found to be BsaXI positive. Furthermore, BsaXI status of colorectal cancer samples were significantly different from that of healthy individuals. CONCLUSION: In conclusion, BsaXI RFLP analysis is a simple and rapid approach for the single step determination of D310 polymorphism of mitochondrial DNA. This method allows the evaluation of a significant proportion of samples without the need for sequencing- and/or radioactivity-based techniques

    On the Study of Hyperbolic Triangles and Circles by Hyperbolic Barycentric Coordinates in Relativistic Hyperbolic Geometry

    Barycentric coordinates are commonly used in Euclidean geometry. Following the adaptation of barycentric coordinates for use in hyperbolic geometry in recently published books on analytic hyperbolic geometry, known and novel results concerning triangles and circles in the hyperbolic geometry of Lobachevsky and Bolyai are discovered. Among the novel results are the hyperbolic counterparts of important theorems in Euclidean geometry. These are: (1) the Inscribed Gyroangle Theorem, (ii) the Gyrotangent-Gyrosecant Theorem, (iii) the Intersecting Gyrosecants Theorem, and (iv) the Intersecting Gyrochord Theorem. Here in gyrolanguage, the language of analytic hyperbolic geometry, we prefix a gyro to any term that describes a concept in Euclidean geometry and in associative algebra to mean the analogous concept in hyperbolic geometry and nonassociative algebra. Outstanding examples are {\it gyrogroups} and {\it gyrovector spaces}, and Einstein addition being both {\it gyrocommutative} and {\it gyroassociative}. The prefix "gyro" stems from "gyration", which is the mathematical abstraction of the special relativistic effect known as "Thomas precession".Comment: 78 pages, 26 figure

    The validation service of the hydrological SAF geostationary and polar satellite precipitation products

    Abstract. The development phase (DP) of the EUMETSAT Satellite Application Facility for Support to Operational Hydrology and Water Management (H-SAF) led to the design and implementation of several precipitation products, after 5 yr (2005–2010) of activity. Presently, five precipitation estimation algorithms based on data from passive microwave and infrared sensors, on board geostationary and sun-synchronous platforms, function in operational mode at the H-SAF hosting institute to provide near real-time precipitation products at different spatial and temporal resolutions. In order to evaluate the precipitation product accuracy, a validation activity has been established since the beginning of the project. A Precipitation Product Validation Group (PPVG) works in parallel with the development of the estimation algorithms with two aims: to provide the algorithm developers with indications to refine algorithms and products, and to evaluate the error structure to be associated with the operational products. In this paper, the framework of the PPVG is presented: (a) the characteristics of the ground reference data available to H-SAF (i.e. radar and rain gauge networks), (b) the agreed upon validation strategy settled among the eight European countries participating in the PPVG, and (c) the steps of the validation procedures. The quality of the reference data is discussed, and the efforts for its improvement are outlined, with special emphasis on the definition of a ground radar quality map and on the implementation of a suitable rain gauge interpolation algorithm. The work done during the H-SAF development phase has led the PPVG to converge into a common validation procedure among the members, taking advantage of the experience acquired by each one of them in the validation of H-SAF products. The methodology is presented here, indicating the main steps of the validation procedure (ground data quality control, spatial interpolation, up-scaling of radar data vs. satellite grid, statistical score evaluation, case study analysis). Finally, an overview of the results is presented, focusing on the monthly statistical indicators, referred to the satellite product performances over different seasons and areas
