El resultado final fue satisfactorio en el primer caso, teniendo que realizarse una artrodesis posterior C1-C2 en el segundo.     Análisis del plano sagital en las escoliosis idiopáticas tratadas con instrumentación Cotrel-Dubousset

    Se analiza la evolución del plano sagital en 50 enfermos con escoliosis idiopática intervenidos mediante artrodesis posterior con instrumentación de Cotrel-Dubousset con un seguimiento medio de 72 meses. Se recogieron los valores evolutivos de cifosis torácica medida entre T5 y T12 y de lordosis lumbar medida entre Ll y L5. Las curvas escolióticas fueron clasificadas según King et al. Los resultados obtenidos mostraron que no existía una variación global significativa de la cifosis torácica tras la cirugía. Sólo en aquellos enfermos que previamente existía una hipocifosis torácica existía una mejoría estadísticamente significativa de la misma con la intervención. Se evidenció una clara tendencia al incremento de la cifosis torácica con el tiempo. La lordosis lumbar se mantuvo, en la mayoría de los casos, dentro de límites fisiológicos. La instrumentación de Cotrel-Dubousset ofrece ventajas evidentes respecto a instrumentaciones previas en lo que respecta a la obtención de una curva más fisiológica en el plano sagital. Permite corregir la hipocifosis torácica severa y mantener la lordosis lumbar dentro de límites normales, incluso en instrumentaciones por debajo de L3.Sagittal plane analysis of 50 patients with idiopathic scoliosis treated with Cotrel- Dubousset instrumentation was performed. Mean follow-up was 72 months. The values of thoracic kyphosis from T5 to T12 and lumbar lordosis from L1 to L5 were retrospectively reviewed. Scoliotic curves were classified according to King et al. The results showed that there was not any significant change in thoracic kyphosis after surgery. Only those patients with preoperative thoracic hypokyphosis achieved a significant realignment. There was an obvious increase of thoracic kyphosis with time. Lumbar lordosis was maintained within normal limits in the mayority of patients. Cotrel-Dubousset instrumentation achieves better sagittal alignment than previous systems. It corrects thoracic hypokyphosis and preserves lumbar lordosis, even when the instrumentation is carried out lower than L3

    Valoración del efecto de la calcitonina en la consolidación de la fractura de Colles

    Se estudian cuarenta pacientes en que se ha producido una Fractura de Epífisis Distal de Radio (Fractura de POUTEAU-COLLES) en pacientes mayores de cincuenta años, con signos de osteoroporosis. Se han hecho dos grupos aleatorios de veinte pacientes. El grupo A se ha tratado con un programa de calcitonina y calcio que cubre los tres primeros meses. El grupo B es tratado exclusivamente con calcio, por igual período. En ambos la reducción e inmovilización sigue los mismos criterios. Se evalúan los resultados, observando que el grupo A presenta menos secuelas dolorosas.The authors have studied 40 patients with fracture of distal epiphysis of radius (POUTEAU-COLLES' fracture) all of them fifty years old with signs of osteoporosis. They have made two groups of patients (20 every one). The group A has been treated with a program of calcitonin and calcium during three months. The group B has been treated only with calcium, three months, as well as the other group. Both groups have the same criterion of reduction and inmovilization. The evaluation of results in the first group showed less painful consequences

    Accelerated inbreeding depression suggests synergistic epistasis for deleterious mutations in Drosophila melanogaster

    Epistasis may have important consequences for a number of issues in quantitative genetics and evolutionary biology. In particular, synergistic epistasis for deleterious alleles is relevant to the mutation load paradox and the evolution of sex and recombination. Some studies have shown evidence of synergistic epistasis for spontaneous or induced deleterious mutations appearing in mutation-accumulation experiments. However, many newly arising mutations may not actually be segregating in natural populations because of the erasing action of natural selection. A demonstration of synergistic epistasis for naturally segregating alleles can be achieved by means of inbreeding depression studies, as deleterious recessive allelic effects are exposed in inbred lines. Nevertheless, evidence of epistasis from these studies is scarce and controversial. In this paper, we report the results of two independent inbreeding experiments carried out with two different populations of Drosophila melanogaster. The results show a consistent accelerated inbreeding depression for fitness, suggesting synergistic epistasis among deleterious alleles. We also performed computer simulations assuming different possible models of epistasis and mutational parameters for fitness, finding some of them to be compatible with the results observed.     MMP-8 Deficiency Increases TLR/RAGE Ligands S100A8 and S100A9 and Exacerbates Lung Inflammation during Endotoxemia

    Matrix metalloproteinase-8, released mainly from neutrophils, is a critical regulator of the inflammatory response by its ability to cleave multiple mediators. Herein, we report the results of a model of endotoxemia after intraperitoneal LPS injection in mice lacking MMP-8 and their wildtype counterparts. Control, saline-treated animals showed no differences between genotypes. However, there was an increased lung inflammatory response, with a prominent neutrophilic infiltration in mutant animals after LPS treatment. Using a proteomic approach, we identify alarmins S100A8 and S100A9 as two of the main differences between genotypes. Mice lacking MMP-8 showed a significant increase in these two molecules in lung homogenates, but not in spleen and serum. Mice lacking MMP-8 also showed an increase in MIP-1α levels and a marked activation of the non-canonical NF-κB pathway, with no differences in CXC-chemokines such as MIP-2 or LIX. These results show that MMP-8 can modulate the levels of S100A8 and S100A9 and its absence promotes the lung inflammatory response during endotoxemia
