Optimal design of pipes in series: An explicit approximation
This paper introduces a new methodology for the optimum design of pipes in series, named Optimum Hydraulic Grade Line (OHGL). This methodology is explicit and is based on the knowledge of the series topology and the geometrical distribution of water demands on nodes, i.e. the way in which the pipe in series delivers water mass as function of the distance from the entrance. OHGL consists in the pre-determination of that hydraulic grade line which gives the minimum construction cost, in an explicit way. Once this line has been established, calculation of the pipeâs continuous diameters is direct; after a round up to commercial diameters is developed. To validate the proposed methodology, several pipes in series were designed both using GA and OHGL. Transcription of toll-like receptors 2, 3, 4 and 9, FoxP3 and Th17 cytokines in a susceptible experimental model of canine Leishmania infantum infection
Canine leishmaniosis (CanL) due to Leishmania infantum is a chronic zoonotic systemic disease resulting from complex interactions between protozoa and the canine immune system. Toll-like receptors (TLRs) are essential components of the innate immune system and facilitate the early detection of many infections. However, the role of TLRs in CanL remains unknown and information describing TLR transcription during infection is extremely scarce. The aim of this research project was to investigate the impact of L. infantum infection on canine TLR transcription using a susceptible model. The objectives of this study were to evaluate transcription of TLRs 2, 3, 4 and 9 by means of quantitative reverse transcription polymerase chain reaction (qRT-PCR) in skin, spleen, lymph node and liver in the presence or absence of experimental L. infantum infection in Beagle dogs. These findings were compared with clinical and serological data, parasite densities in infected tissues and transcription of IL-17, IL-22 and FoxP3 in different tissues in non-infected dogs (n = 10), and at six months (n = 24) and 15 months (n = 7) post infection. Results revealed significant down regulation of transcription with disease progression in lymph node samples for TLR3, TLR4, TLR9, IL-17, IL-22 and FoxP3. In spleen samples, significant down regulation of transcription was seen in TLR4 and IL-22 when both infected groups were compared with controls. In liver samples, down regulation of transcription was evident with disease progression for IL-22. In the skin, upregulation was seen only for TLR9 and FoxP3 in the early stages of infection. Subtle changes or down regulation in TLR transcription, Th17 cytokines and FoxP3 are indicative of the silent establishment of infection that Leishmania is renowned for. These observations provide new insights about TLR transcription, Th17 cytokines and Foxp3 in the liver, spleen, lymph node and skin in CanL and highlight possible markers of disease susceptibility in this model

Symptoms and quality of life in late stage Parkinson syndromes: a longitudinal community study of predictive factors
BACKGROUND
Palliative care is increasingly offered earlier in the cancer trajectory but rarely in Idiopathic Parkinson's Disease(IPD), Progressive Supranuclear Palsy(PSP) or Multiple System Atrophy(MSA). There is little longitudinal data of people with late stage disease to understand levels of need. We aimed to determine how symptoms and quality of life of these patients change over time; and what demographic and clinical factors predicted changes.
METHODS
We recruited 82 patients into a longitudinal study, consenting patients with a diagnosis of IPD, MSA or PSP, stages 3-5 Hoehn and Yahr(H&Y). At baseline and then on up to 3 occasions over one year, we collected self-reported demographic, clinical, symptom, palliative and quality of life data, using Parkinson's specific and generic validated scales, including the Palliative care Outcome Scale (POS). We tested for predictors using multivariable analysis, adjusting for confounders.
FINDINGS
Over two thirds of patients had severe disability, over one third being wheelchair-bound/bedridden. Symptoms were highly prevalent in all conditions - mean (SD) of 10.6(4.0) symptoms. More than 50% of the MSA and PSP patients died over the year. Over the year, half of the patients showed either an upward (worsening, 24/60) or fluctuant (8/60) trajectory for POS and symptoms. The strongest predictors of higher levels of symptoms at the end of follow-up were initial scores on POS (AOR 1.30; 95%CI:1.05-1.60) and being male (AOR 5.18; 95% CI 1.17 to 22.92), both were more predictive than initial H&Y scores.
INTERPRETATION
The findings point to profound and complex mix of non-motor and motor symptoms in patients with late stage IPD, MSA and PSP. Symptoms are not resolved and half of the patients deteriorate. Palliative problems are predictive of future symptoms, suggesting that an early palliative assessment might help screen for those in need of earlier intervention
The adjusted International Prognostic Index and beta-2-microglobulin predict the outcome after autologous stem cell transplantation in relapsing/refractory peripheral T-cell lymphoma
BACKGROUND AND OBJECTIVES:
Preliminary data on the use of autologous stem cell transplantation (ASCT) as a salvage therapy for peripheral T-cell lymphoma (PTCL) indicate that the results are similar to those obtained in aggressive B-cell lymphomas. The aim of our study was to analyze outcomes of a large series of patients with PTCL with a prolonged follow-up who received ASCT as salvage therapy.
DESIGN AND METHODS:
Between 1990 and 2004, 123 patients in this situation were registered in the GELTAMO database. The median age at transplantation was 43.5 years; in 91% of patients the disease was chemosensitive.
RESULTS:
Seventy-three percent of the patients achieved complete remission, 11% partial remission and the procedure failed in 16%. At a median follow-up of 61 months, the 5-year overall and progression-free survival rates were 45% and 34%, respectively. The presence of more than one factor of the adjusted International Prognostic Index (a-IPI) and a high beta2-microglobulin at transplantation were identified as adverse prognostic factors for both overall and progression-free survival and allowed the population to be stratified into three distinct risk groups.
INTERPRETATION AND CONCLUSIONS:
Our data show that approximately one third of patients with PTCL in the salvage setting may enjoy prolonged survival following ASCT, provided they are transplanted in a chemosensitive disease state. The a-IPI and beta2-microglobulin level predict the outcome after ASCT in relapsing/refractory PTCL
Challenges in Estimating Insecticide Selection Pressures from Mosquito Field Data
Insecticide resistance has the potential to compromise the enormous effort put into the control of dengue and malaria vector populations. It is therefore important to quantify the amount of selection acting on resistance alleles, their contributions to fitness in heterozygotes (dominance) and their initial frequencies, as a means to predict the rate of spread of resistance in natural populations. We investigate practical problems of obtaining such estimates, with particular emphasis on Mexican populations of the dengue vector Aedes aegypti. Selection and dominance coefficients can be estimated by fitting genetic models to field data using maximum likelihood (ML) methodology. This methodology, although widely used, makes many assumptions so we investigated how well such models perform when data are sparse or when spatial and temporal heterogeneity occur. As expected, ML methodologies reliably estimated selection and dominance coefficients under idealised conditions but it was difficult to recover the true values when datasets were sparse during the time that resistance alleles increased in frequency, or when spatial and temporal heterogeneity occurred. We analysed published data on pyrethroid resistance in Mexico that consists of the frequency of a Ile1,016 mutation. The estimates for selection coefficient and initial allele frequency on the field dataset were in the expected range, dominance coefficient points to incomplete dominance as observed in the laboratory, although these estimates are accompanied by strong caveats about possible impact of spatial and temporal heterogeneity in selection
Fusion product losses due to fishbone instabilities in deuterium JET plasmas
During development of a high-performance hybrid scenario for future deuteriumâtritium experiments on the Joint European Torus, an increased level of fast ion losses in the MeV energy range was observed during the instability of high-frequency nââ=ââ1 fishbones. The fishbones are excited during deuterium neutral beam injection combined with ion cyclotron heating. The frequency range of the fishbones, 10â25âkHz, indicates that they are driven by a resonant interaction with the NBI-produced deuterium beam ions in the energy rangeâââ€120âkeV. The fast particle losses in a much higher energy range are measured with a fast ion loss detector, and the data show an expulsion of deuterium plasma fusion products, 1 MeV tritons and 3 MeV protons, during the fishbone bursts. An MHD mode analysis with the MISHKA code combined with the nonlinear wave-particle interaction code HAGIS shows that the loss of toroidal symmetry caused by the nââ=ââ1 fishbones affects strongly the confinement of non-resonant high energy fusion-born tritons and protons by perturbing their orbits and expelling them. Biofilm formation at the solid-liquid and air-liquid interfaces by Acinetobacter species
Abstract
Background: The members of the genus Acinetobacter are Gram-negative cocobacilli that are frequently found in
the environment but also in the hospital setting where they have been associated with outbreaks of nosocomial
infections. Among them, Acinetobacter baumannii has emerged as the most common pathogenic species involved
in hospital-acquired infections. One reason for this emergence may be its persistence in the hospital wards, in
particular in the intensive care unit; this persistence could be partially explained by the capacity of these
microorganisms to form biofilm. Therefore, our main objective was to study the prevalence of the two main types
of biofilm formed by the most relevant Acinetobacter species, comparing biofilm formation between the different
species.
Findings: Biofilm formation at the air-liquid and solid-liquid interfaces was investigated in different Acinetobacter
spp. and it appeared to be generally more important at 25°C than at 37°C. The biofilm formation at the solid-liquid
interface by the members of the ACB-complex was at least 3 times higher than the other species (80-91% versus
5-24%). In addition, only the isolates belonging to this complex were able to form biofilm at the air-liquid interface;
between 9% and 36% of the tested isolates formed this type of pellicle. Finally, within the ACB-complex, the
biofilm formed at the air-liquid interface was almost 4 times higher for A. baumannii and Acinetobacter G13TU than
for Acinetobacter G3 (36%, 27% & 9% respectively).
Conclusions: Overall, this study has shown the capacity of the Acinetobacter spp to form two different types of
biofilm: solid-liquid and air-liquid interfaces. This ability was generally higher at 25°C which might contribute to
their persistence in the inanimate hospital environment. Our work has also demonstrated for the first time the
ability of the members of the ACB-complex to form biofilm at the air-liquid interface, a feature that was not
observed in other Acinetobacter species
Azithromycin to Prevent Pertussis in Household Contacts, Catalonia and Navarre, Spain, 2012-2013
We retrospectively assessed the effectiveness of azithromycin in preventing transmission of pertussis to a patient's household contacts. We also considered the duration between symptom onset in the primary patient and azithromycin administration. We categorized contacts into 4 groups: those treated within 21 days after illness onset in the primary patient. We studied 476 primary index patients and their 1,975 household contacts, of whom 4.5% were later identified as having pertussis. When contacts started chemoprophylaxis within 14 days after primary patient's symptom onset was less effective. We recommend that contacts of persons with pertussis begin chemoprophylaxis within <14 days after primary patient's symptom onset
