    Deceleration area and fetal acidemia

    Aims: To compare the predictive ability for neonatal acidemia of individual components of intrapartum cardiotocography (CTG) described by National Institute of Child Health and Human Development (NICHD) system and deceleration area. Design: Case-control study. Setting: Spanish tertiary obstetrical hospital. Population: CTG patterns of 102 acidemic fetus (umbilical arterial cord gas pH =7.10, base deficit (BD>48) and 102 nonacidemic controls (umbilical arterial cord gas pH>7.10). Methods: Two reviewers blind to clinical and outcome data analyzed the last thirty minutes before delivery of 204 fetal heart rate (FHR) tracings, extracting those features defined by NICHD and certain measures of FHR decelerations, including deceleration area, not considered by this system. Outcome measures: The primary outcome was the predictive ability of NICHD features and non-NICHD deceleration measures for fetal acidemia. The secondary outcome was the impact of deceleration area in the last 30 min of labor on gasometry components (pH, BD and lactate). Results: Minimal variability (area under the curve (AUC) 0.74), total number of late (AUC: 0.75) and prolonged decelerations (0.77) were the three NICHD features with the greatest predictive ability for fetal acidemia in the last thirty minutes of labor. Total deceleration area demonstrated the highest discrimination power (AUC: 0.83) of all the analyzed elements. For each cm2 the area increases in the last 30 min of labor, pH decreases 0.08 units, BD increases 0.272 mEq/L and lactate 0.183 mEq/L. Conclusions: Total deceleration area showed the greatest predictive ability for fetal acidemia and its measure could help to estimate intrapartum fetal acid-base status

    Nitrate source identification in the Baltic Sea using its isotopic ratios in combination with a Bayesian isotope mixing model

    Nitrate (NO3-) is the major nutrient responsible for coastal eutrophication worldwide and its production is related to intensive food production and fossil-fuel combustion. In the Baltic Sea NO3- inputs have increased 4-fold over recent decades and now remain constantly high. NO3- source identification is therefore an important consideration in environmental management strategies. In this study focusing on the Baltic Sea, we used a method to estimate the proportional contributions of NO3- from atmospheric deposition, N-2 fixation, and runoff from pristine soils as well as from agricultural land. Our approach combines data on the dual isotopes of NO3- (delta N-15-NO3- and delta O-18-NO3-) in winter surface waters with a Bayesian isotope mixing model (Stable Isotope Analysis in R, SIAR). Based on data gathered from 47 sampling locations over the entire Baltic Sea, the majority of the NO3- in the southern Baltic was shown to derive from runoff from agricultural land (33-100 %), whereas in the northern Baltic, i.e. the Gulf of Bothnia, NO3- originates from nitrification in pristine soils (34-100 %). Atmospheric deposition accounts for only a small percentage of NO3- levels in the Baltic Sea, except for contributions from northern rivers, where the levels of atmospheric NO3- are higher. An additional important source in the central Baltic Sea is N-2 fixation by diazotrophs, which contributes 49-65% of the overall NO3- pool at this site. The results obtained with this method are in good agreement with source estimates based upon delta N-15 values in sediments and a three-dimensional ecosystem model, ERGOM. We suggest that this approach can be easily modified to determine NO3- sources in other marginal seas or larger near-coastal areas where NO3- is abundant in winter surface waters when fractionation processes are minor

    Application of magnetically induced hyperthermia on the model protozoan Crithidia fasciculata as a potential therapy against parasitic infections

    Magnetic hyperthermia is currently an EU-approved clinical therapy against tumor cells that uses magnetic nanoparticles under a time varying magnetic field (TVMF). The same basic principle seems promising against trypanosomatids causing Chagas disease and sleeping sickness, since therapeutic drugs available display severe side effects and drug-resistant strains. However, no applications of this strategy against protozoan-induced diseases have been reported so far. In the present study, Crithidia fasciculata, a widely used model for therapeutic strategies against pathogenic trypanosomatids, was targeted with Fe_{3}O_{4} magnetic nanoparticles (MNPs) in order to remotely provoke cell death using TVMFs. The MNPs with average sizes of d approx. 30 nm were synthesized using a precipitation of FeSO_{4}4 in basic medium. The MNPs were added to Crithidia fasciculata choanomastigotes in exponential phase and incubated overnight. The amount of uploaded MNPs per cell was determined by magnetic measurements. Cell viability using the MTT colorimetric assay and flow cytometry showed that the MNPs were incorporated by the cells with no noticeable cell-toxicity effects. When a TVMF (f = 249 kHz, H = 13 kA/m) was applied to MNP-bearing cells, massive cell death was induced via a non-apoptotic mechanism. No effects were observed by applying a TVMF on control (without loaded MNPs) cells. No macroscopic rise in temperature was observed in the extracellular medium during the experiments. Scanning Electron Microscopy showed morphological changes after TVMF experiments. These data indicate (as a proof of principle) that intracellular hyperthermia is a suitable technology to induce the specific death of protozoan parasites bearing MNPs.     Presiones plantares, en estática bipodal, en niños/adolescentes con síndrome de Down

    Objetivo: Determinar las presiones plantares en estática bipodal y la alineación de retropié y rodilla, en niños/adolescentes con Síndrome de Down (SD), comparando los datos con los de un grupo control (GC). Diseño. 32 sujetos con SD y 33 control. Se valoró la superficie plantar, presión media y máxima y su distribución porcentual en 9 zonas de la superficie plantar (plataforma”Footchecker”) en estática bipodal. Asimismo, se valoró la alineación del retropié en el plano frontal y la de la rodilla en los planos frontal y sagital. Resultados. En el grupo SD: superficie plantar mayor (p<0.0001); presión media menor (p<0.01); picos de presión menores en el retropié (zona interna p<0.0001) y mayores en parte interna del mediopié y del antepié (p<0.0001); participación menor del retropié, principalmente en parte interna (GC:23.09%; SD:15.87%; p<0.0001), y mayor de la parte interna del mediopié (GC:4%; SD10%; p<0.0001) y del antepié (GC:12%; SD:17%; p<0.0001). Conclusiones. El grupo SD presentó mayor desviación en valgo de talón y rodilla, tendencia al recurvatum de rodilla y aumento de la superficie de apoyo plantar, relacionado con el pie plano que suelen presentar estos sujetos. Además, presentó tendencia a menores presiones medias y modificación de la distribución porcentual de la carga, con menor participación del retropié y mayor de las zonas internas del mediopié y antepié; es decir, hubo un desplazamiento de la carga hacia la zona ántero-interna.Objective. To determine the plantar pressures in standing and the hindfoot and knee alignment, in children/adolescents with Down syndrome (DS). Material and Methods. The sample comprised two groups of 32 children / adolescents, one with DS and other without pathology (CG). We analyzed in standing she support surface, average and peak plantar pressures and their distribution (by means of a Footchecker platform), hindfoot alignment in the frontal plane and knee alignment in the frontal and sagital planes.Comparison between both groups was carried out. Results. The DS group showed: higher plantar surface (p<0.0001); lower average pressure (p<0.01); lower peak pressure in the hindfoot (internal zone p<0.0001) and a higher one in the internal zone of the midfoot and forefoot (p<0.0001); lower involvement of the hindfoot, specially in the internal part (CG:23.09%, DS:15.87%, p<0.0001) and a higher one of the internal part of the midfoot (CG:4%, DS:10%, p<0.0001) and forefoot (CG: 12%, DS: 17%, p <0.0001). Conclusions. The DS group presented higher valgus deviation of the hindfoot and knee, a tendency to a knee recurvatum and higher support surface, related to the flatfoot they usually had. On the other hand, they presented a tendency to lower average pressures and a variation in the porcentage distribution of load, with a lower involvement of the hindfoot and a higher one of the internal parts of the midfoot and forefoot: that’s to say, there was a load displacement towards the anterior and internal zone.Peer Reviewe

    En este estudio se analiza la actividad electromiográfica de los principales músculos de la extremidad inferior derecha: Glúteos Mayor y Medio, Recto Anterior y Vasto Interno del Cuádriceps, Isquiotibioperoneos, Gemelos y Tibial Anterior, al caminar en un tapiz rodante a distintas velocidades y en una carrera suave. Para dividir el ciclo de la marcha y la carrera en fases se utilizó un sistema de análisis tridimensional con dos cámaras de vídeo. La señal electromiográfica de cada una de estas fases se integró y se expresó en porcentaje de la actividad máxima isométrica de su músculo correspondiente. Los resultados obtenidos muestran que la participación muscular en la marcha lenta y normal es muy similar, sin embargo, en la marcha rápida aparecen aumentos importantes conservando el mismo patrón de actuación. En la carrera, no sólo existen actividades del triple de las halladas en la marcha a velocidad cómoda, sino que se modifica el patrón de actuación, presentando todos los músculos sus picos de actividad durante la fase de apoyo, momento en el que el centro de gravedad se lleva hacia delante sobre el miembro inferior

    Cooperative WebLab in chemical engineering between France and Brazil: Validation of the methodology

    A WebLab is an experiment operated remotely via Internet. Besides the strictly technical aspects of such an experiment, which may contribute to the learning of Chemical Engineering fundamentals, there is also important feedback when teams of students of two different countries are working together: the WebLab becomes an intercultural experience, enhancing the communication skills of the students. A WebLab between Universidade Federal de São Carlos (DEQ/UFSCar) and the Ecole Nationale Supérieure d’Ingénieurs en Arts Chimiques et Technologiques (ENSIACET) is presented in this work. A mass transfer experiment in a bench scale reactor (stirred and aerated) had to be studied by mixed teams, thus emulating challenges that will be common in future working environments. In order to perform the experiment, students in Brazil and in France were put into groups. The students had to make decisions about the procedure for executing the experiments. All the students were able to control the equipment, no matter where they were physically. Students communicated using video conference software. The students' and teachers' opinions of this experience were very positive. This methodology is an important contribution to the education of engineers in a world integrated by modern communication technologies

    Using an independent geochronology based on palaeomagnetic secular variation (PSV) and atmospheric Pb deposition to date Baltic Sea sediments and infer 14C reservoir age

    Dating of sediment cores from the Baltic Sea has proven to be difficult due to uncertainties surrounding the C-14 reservoir age and a scarcity of macrofossils suitable for dating. Here we present the results of multiple dating methods carried out on cores in the Gotland Deep area of the Baltic Sea. Particular emphasis is placed on the Littorina stage (8 ka ago to the present) of the Baltic Sea and possible changes in the C-14 reservoir age of our dated samples. Three geochronological methods are used. Firstly, palaeomagnetic secular variations (PSV) are reconstructed, whereby ages are transferred to PSV features through comparison with varved lake sediment based PSV records. Secondly, lead (Pb) content and stable isotope analysis are used to identify past peaks in anthropogenic atmospheric Pb pollution. Lastly, C-14 determinations were carried out on benthic foraminifera (Elphidium spec.) samples from the brackish Littorina stage of the Baltic Sea. Determinations carried out on smaller samples (as low as 4 mu g C) employed an experimental, state-of-the-art method involving the direct measurement of CO2 from samples by a gas ion source without the need for a graphitisation step - the first time this method has been performed on foraminifera in an applied study. The PSV chronology, based on the uppermost Littorina stage sediments, produced ten age constraints between 6.29 and 1.29 cal ka BP, and the Pb depositional analysis produced two age constraints associated with the Medieval pollution peak. Analysis of PSV data shows that adequate directional data can be derived from both the present Littorina saline phase muds and Baltic Ice Lake stage varved glacial sediments. Ferrimagnetic iron sulphides, most likely authigenic greigite (Fe3S4), present in the intermediate Ancylus Lake freshwater stage sediments acquire a gyroremanent magnetisation during static alternating field (AF) demagnetisation, preventing the identification of a primary natural remanent magnetisation for these sediments. An inferred marine reservoir age offset (Delta R) is calculated by comparing the foraminifera C-14 determinations to a PSV & Pb age model. This Delta R is found to trend towards younger values upwards in the core, possibly due to a gradual change in hydrographic conditions brought about by a reduction in marine water exchange from the open sea due to continued isostatic rebound.     Double balloon catheter for induction of labour in women with a previous cesarean section, could it be the best choice?

    Introduction: We analysed the efficacy and safety of double-balloon catheter for cervical ripening in women with a previous cesarean section and which were the most important variables associated with an increased risk of repeated cesarean delivery. Materials and methods: We designed an observational retrospective study of 418 women with unfavourable cervices (Bishop Score <5), a prior cesarean delivery, and induction of labour with a double-balloon catheter. Baseline maternal data and perinatal outcomes were recorded for a descriptive, bivariate, and multivariate analysis. A p value <0.05 was considered statistically significant. Results: Most women improved their initial Bishop Score (89.5%) although only a 20.8% of them went into spontaneous active labour. Finally, 51.4% of the women achieved a vaginal delivery. Five cases of intrapartum uterine rupture (1.2%) occurred. After multivariate analysis, main risk factors for repeated cesarean section were dystocia in the previous pregnancy (OR 1.744; CI 95% 1.066–2.846), the absence of previous vaginal delivery (OR 2.590; CI 95% 1.066–6.290), suspected fetal macrosomia (OR 2.410; CI 95% 0.959–6.054), and duration of oxytocin induction period (OR 1.005; CI 95% 1.004–1.006). The area under the curve was 0.789 (p < 0.001). Conclusions: Double-balloon catheter seems to be safe and effective for cervical ripening in women with a previous cesarean delivery and unfavourable cervix. In our study, most women could have a vaginal delivery in spite of their risk factors for cesarean delivery. A multivariate model based on some clinical variables has moderate predictive value for intrapartum cesarean section

    Nanoparticles engineered to bind cellular motors for efficient delivery

    Background: Dynein is a cytoskeletal molecular motor protein that transports cellular cargoes along microtubules. Biomimetic synthetic peptides designed to bind dynein have been shown to acquire dynamic properties such as cell accumulation and active intra- and inter-cellular motion through cell-to-cell contacts and projections to distant cells. On the basis of these properties dynein-binding peptides could be used to functionalize nanoparticles for drug delivery applications. Results: Here, we show that gold nanoparticles modified with dynein-binding delivery sequences become mobile, powered by molecular motor proteins. Modified nanoparticles showed dynamic properties, such as travelling the cytosol, crossing intracellular barriers and shuttling the nuclear membrane. Furthermore, nanoparticles were transported from one cell to another through cell-to-cell contacts and quickly spread to distant cells through cell projections. Conclusions: The capacity of these motor-bound nanoparticles to spread to many cells and increasing cellular retention, thus avoiding losses and allowing lower dosage, could make them candidate carriers for drug delivery
