    Atomic spectrometry update : a review of advances in environmental analysis

    This is the 31st annual review of the application of atomic spectrometry to the chemical analysis of environmental samples. This update refers to papers published approximately between August 2014 and July 2015 and continues the series of Atomic Spectrometry Updates (ASUs) in Environmental Analysis that should be read in conjunction with other related ASUs in the series, namely: clinical and biological materials, foods and beverages; advances in atomic spectrometry and related techniques; elemental speciation; X-ray spectrometry; and metals, chemicals and functional materials. In the field of air analysis, highlights within this review period included: the development of a new laser fluorescence instrument for the ultratrace determination of mercury vapour; single particle ICP-MS studies and the coupling of elemental analysers to mass spectrometers for the improved characterisation of carbonaceous aerosols. In the arena of water analysis, methods continue to be developed: for the extraction and preconcentration of elements, As, Cr, Hg and Sb species and determination of elemental constituents in colloidal and NP fractions. Emerging elements of interest include Gd derived from MRI agents discharged at low level from medical facilities in water courses. Instrumental developments reported included the use of MC-ICP-MS for isotopic tracer studies and a review of TXRF techniques and associated preconcentration procedures for trace element analysis. In the period covered by this update several articles have explored the analysis of soil extracts for geochemical prospecting. There has been widening interest in the use of CS-AAS and in the application of techniques capable of direct sample analysis such as slurry sampling ETAAS and ETV-ICP-AES. Portable XRF instrumentation is now being used in many disciplines to quantify trace elements in soils – bringing a need for better transfer of analytical knowledge to non-specialist users – and the growing use of portable XRF in proximal sensing is also noteworthy. Recent research indicates that geological applications still drive many of the instrumental and methodological advances in LA-ICP-MS. Fundamental studies continued to shed light on the processes involved and hence ways of improving the analysis of laser-produced aerosols and to minimise matrix and fractionation effects. A new technique LA-DOF-MS (distance of flight) was described. The utility of LIBS and portable XRF for in situ survey work continues to show promise but issues such as appropriate calibration regimes and data processing protocols will still need to be addressed

This update refers to papers published approximately between August 2016 and June 2017 and continues the series of Atomic Spectrometry Updates (ASUs) in environmental analysis that should be read in conjunction with other related ASUs in the series, namely: clinical and biological materials, foods and beverages; advances in atomic spectrometry and related techniques; elemental speciation; X-ray spectrometry; and metals, chemicals and functional materials. In the field of air analysis, highlights within this review period included the fabrication of new air samplers using 3D printer technology, development of a portable aerosol concentrator unit based upon electrostatic precipitation and instrumental developments such as a prototype portable spark emission spectrometer to quantify metal particles in workplace air. The advent of ICP-MS/MS systems has enabled analysts to develop improved methods for the determination of PGEs and radioactive elements present in airborne particles. With such instruments, the capacity to eliminate or minimise many isobaric interferences now enables analysts to forego the use of many onerous sample clean-up procedures. Improvements in the capabilities of aerosol mass spectrometers were noted as were developments in other complimentary measurement techniques such as Raman. In the arena of water analysis there are growing concerns regarding engineered NPs e.g. Ag NPs, entering water courses resulting in the development and optimisation of new methods based upon FFF and sp-ICP-MS techniques to measure such inputs. Similar concerns exist for MRI contrasting agents e.g. Gd-based compounds and here improved methodologies that involve the use of sample preconcentration using chelating columns and ICP-MS analysis have been proposed. In the field of plant and soil analysis, similar to developments in the water sector, there has been increased interest in the measurement of NPs. Many comparisons of sample digestion or extraction methods have been reported but a key issue rarely addressed is transferability, i.e. whether methods preferred by one group of researchers using particular apparatus are also optimal in a different laboratory using different apparatus. New sample preconcentration methods continued to appear although – as in previous years – the CRMs selected for method validation often failed to reflect the nature of the intended sample(s). A noteworthy advance is the use of HR-CS-ETMAS for elemental analysis. Developments in LIBS included greater use of TEA CO2 lasers in place of Nd:YAG lasers and increased use of stand-off measurement. The past year has also seen a rise in proximal sensing using LIBS and pXRFS. In the field of geological analysis, the quest continues for well-characterised matrix-matched materials suitable for the calibration of elemental and, particularly, isotopic measurements by microanalytical techniques. Increasing interest in stable isotope analysis by SIMS is reflected by the number of matrix-matched RMs developed specifically for this technique. Much work continues on ways of improving isotope ratio measurements by ICP-MS and TIMS for a wide range of different isotope systems relevant to geochemical studies. High spatial resolution analysis by LIBS, LA-ICP-MS and SIMS to obtain data on chemical and isotopic variations in minerals and biogenic materials in two and three dimensions are the foundation for many new insights in geoscientific research.     Surgical need among the ageing population of Uganda

    Background: Uganda’s ageing population (age 50 years and older) will nearly double from 2015 to 2050. HIV/AIDS, diabetes, stroke among other disease processes have been studied in the elderly population. However, the burden of disease from surgi- cally-treatable conditions is unknown. Objectives: To determine the proportion of adults above 50 years with unmet surgical need and deaths attributable to probable surgically-treatable conditions. Methods: A cluster randomized sample representing the national population of Uganda was enumerated. The previously vali- dated Surgeons Overseas assessment of surgical need instrument, a head-to-toe verbal interview, was used to determine any sur- gically-treatable conditions in two randomly-selected living household members. Deaths were detailed by heads of households. Weighted metrics are calculated taking sampling design into consideration and Taylor series linearization was used for sampling error estimation.   Results: The study enumerated 425 individuals above age 50 years. The prevalence proportion of unmet surgical need was 27.8% (95%CI, 22.1-34.3). This extrapolates to 694,722 (95%CI, 552,279-857,157) individuals living with one or more surgically treatable conditions. The North sub-region was observed to have the highest prevalence proportion. Nearly two out of five household deaths (37.9%) were attributed to probable surgically treatable causes.Conclusion: There is disproportionately high need for surgical care among the ageing population of Uganda with approximate- ly 700,000 consultations needed.Keywords: Surgical need, ageing population, Uganda

    The contribution of processing fluency to preference : a comparison with familiarity-based recognition

    There is a great deal of evidence supporting the idea that, when a stimulus is processed fluently, it is more likely to be judged as pleasant. However, this influence of fluency on preference judgement seems to depend on several experimental conditions. So we tried to better understand these conditions via a comparison with recognition and by manipulating some aspects of the procedure (test format) and material (similarity and figure-ground contrast of the stimuli). Two experiments showed that some conditions maximally induce the use of processing fluency in a preference judgement, as in a recognition task.     ProPIG - Organic pig health, welfare and environmental impact across Europe

    Organic production is perceived by consumers as being superior in animal welfare and sustainability and the demand for organic pork products is slowly increasing. Within the past ten years a variety of husbandry and management systems have been developed across the EU, ranging from farms with pigs outdoors all year round using local breeds to farms with housed pigs having concrete outside runs and using conventional breeds (CorePIG, Rousing et al, 2011). So far, mainly clinical parameters have been used to describe the health situation on organic pig farms, identifying some key problems, such as weaning diarrhoea and piglet mortality. Organic pig production is - amongst others - characterised through a holistic approach based on the EU Regulation (EC) No 834/2007 and the IFOAM principles: ‘health, ecology, fairness and care’. This clearly states that health is more than absence of clinical symptoms and, the relation between animals and their environment is identified: ‘Health’ is defined as ‘the wholeness and integrity of living systems. It is not simply the absence of illness, but the maintenance of physical, mental, social and ecological well-being’ (IFOAM; 2006). Concepts of animal welfare include physical and mental welfare as well as the concept of naturalness (Fraser 2003), which is often interpreted as the ability to perform natural behaviour. Verhoog et al (2003) describe three main approaches within organic agriculture’s concept of nature and naturalness: the no-chemicals approach, the agro-ecology approach and the integrity approach. Applying those concepts to organic pig production can highlight potential conflicts: outdoor systems are perceived as the optimal housing system for pigs, as they allow natural behaviour such as rooting. However, this behaviour can cause damage to the grass cover and furthermore the manure fate in outdoor areas needs to be considered. A few studies on outdoor pig production have shown a clear N and P surplus and a high degree of distribution heterogeneity in outdoor areas, increasing the risk of N and P losses (Watson et al. 2003). Robust and competitive organic pig production needs to encompass low environmental impacts and good animal health and welfare. So far few studies have quantified both aspects in different pig husbandry systems. In addition, the theory that improving animal health and welfare reduces environmental impacts through decreased medicine use, improved growth rate and feed conversion efficiency has still to be verified. The aim of the CoreOrganic2 project ProPIG (2011-2014; carried out in eight European countries) is to examine the relationship between health, welfare and environmental impact. On-farm assessment protocols will be carried out on 75 farms in three pig husbandry systems (outdoor, partly outdoor, indoor with concrete outside run). Environmental impact will be assessed using both Life Cycle Assessment and calculations of nutrient balances at farm and outdoor area level. Animal health and welfare will be evaluated from animal based parameters including clinical and selected behavioural parameters. Results will be fed back and used by the farmers to decide farm specific goals and strategies to achieve these goals. As an outcome, all farms will create their individual health, welfare and environmental plan, which will be reviewed after one year to allow continuous development. This will provide the opportunity not only to investigate, but also improve the influence of organic pig farming systems on animal welfare and environmental impact. This fulfils the fourth IFOAM principle of care: ‘Organic Agriculture should be managed in a precautionary and responsible manner to protect the health and well-being of current and future generations and the environment’ (IFOAM, 2006)

    Optimizing Care for Ugandans with Untreated Abdominal Surgical Conditions

    Background: Abdominal operations account for a majority of surgical volume in low-income countries, yet population-level prevalence data on surgically treatable abdominal conditions are scarce. Objective: In this study, our objective was to quantify the burden of surgically treatable abdominal conditions in Uganda. Methods: In 2014, we administered a two-stage cluster-randomized Surgeons OverSeas Assessment of Surgical Need survey to 4,248 individuals in 105 randomly selected clusters (representing the national population of Uganda). Findings: Of the 4,248 respondents, 185 reported at least one surgically treatable abdominal condition in their lifetime, giving an estimated lifetime prevalence of 3.7% (95% CI: 3.0 to 4.6%). Of those 185 respondents, 76 reported an untreated condition, giving an untreated prevalence of 1.7% (95% CI: 1.3 to 2.3%). Obstructed labor (52.9%) accounted for most of the 238 abdominal conditions reported and was untreated in only 5.6% of reported conditions. In contrast, 73.3% of reported abdominal masses were untreated. Conclusions: Individuals in Uganda with nonobstetric abdominal surgical conditions are disproportionately undertreated. Major health system investments in obstetric surgical capacity have been beneficial, but our data suggest that further investments should aim at matching overall surgical care capacity with surgical need, rather than focusing on a single operation for obstructed labor

    Preventing disease through opportunistic, rapid engagement by primary care teams using behaviour change counselling (PRE-EMPT): protocol for a general practice-based cluster randomised trial

    BACKGROUND: Smoking, excessive alcohol consumption, lack of exercise and an unhealthy diet are the key modifiable factors contributing to premature morbidity and mortality in the developed world. Brief interventions in health care consultations can be effective in changing single health behaviours. General Practice holds considerable potential for primary prevention through modifying patients' multiple risk behaviours, but feasible, acceptable and effective interventions are poorly developed, and uptake by practitioners is low. Through a process of theoretical development, modeling and exploratory trials, we have developed an intervention called Behaviour Change Counselling (BCC) derived from Motivational Interviewing (MI). This paper describes the protocol for an evaluation of a training intervention (the Talking Lifestyles Programme) which will enable practitioners to routinely use BCC during consultations for the above four risk behaviours. METHODS/DESIGN: This cluster randomised controlled efficacy trial (RCT) will evaluate the outcomes and costs of this training intervention for General Practitioners (GPs) and nurses. Training methods will include: a practice-based seminar, online self-directed learning, and reflecting on video recorded and simulated consultations. The intervention will be evaluated in 29 practices in Wales, UK; two clinicians will take part (one GP and one nurse) from each practice. In intervention practices both clinicians will receive training. The aim is to recruit 2000 patients into the study with an expected 30% drop out. The primary outcome will be the proportion of patients making changes in one or more of the four behaviours at three months. Results will be compared for patients seeing clinicians trained in BCC with patients seeing non-BCC trained clinicians. Economic and process evaluations will also be conducted. DISCUSSION: Opportunistic engagement by health professionals potentially represents a cost effective medical intervention. This study integrates an existing, innovative intervention method with an innovative training model to enable clinicians to routinely use BCC, providing them with new tools to encourage and support people to make healthier choices.     Digital Cranial Endocast of Hyopsodus (Mammalia, “Condylarthra”): A Case of Paleogene Terrestrial Echolocation?

    We here describe the endocranial cast of the Eocene archaic ungulate Hyopsodus lepidus AMNH 143783 (Bridgerian, North America) reconstructed from X-ray computed microtomography data. This represents the first complete cranial endocast known for Hyopsodontinae. The Hyopsodus endocast is compared to other known “condylarthran” endocasts, i. e. those of Pleuraspidotherium (Pleuraspidotheriidae), Arctocyon (Arctocyonidae), Meniscotherium (Meniscotheriidae), Phenacodus (Phenacodontidae), as well as to basal perissodactyls (Hyracotherium) and artiodactyls (Cebochoerus, Homacodon). Hyopsodus presents one of the highest encephalization quotients of archaic ungulates and shows an “advanced version” of the basal ungulate brain pattern, with a mosaic of archaic characters such as large olfactory bulbs, weak ventral expansion of the neopallium, and absence of neopallium fissuration, as well as more specialized ones such as the relative reduction of the cerebellum compared to cerebrum or the enlargement of the inferior colliculus. As in other archaic ungulates, Hyopsodus midbrain exposure is important, but it exhibits a dorsally protruding largely developed inferior colliculus, a feature unique among “Condylarthra”. A potential correlation between the development of the inferior colliculus in Hyopsodus and the use of terrestrial echolocation as observed in extant tenrecs and shrews is discussed. The detailed analysis of the overall morphology of the postcranial skeleton of Hyopsodus indicates a nimble, fast moving animal that likely lived in burrows. This would be compatible with terrestrial echolocation used by the animal to investigate subterranean habitat and/or to minimize predation during nocturnal exploration of the environment
