The Deacon: Ministry through Words of Faith and Acts of Love
From the moment the 1996 General Conference of The United Methodist Church reordered its ministry and adopted the new Order of Deacon, there has been much speculation on the meaning of the new Order, how it should be developed, and what difference it should make. (Some saw no reason for the new Order or could not grasp the reason for such an Order, and others thought they knew all the answers and have judged the establishment of the deacon on their understandings.)
The Section of Deacons and Diaconal Ministries in the Division of Ordained Ministry has the responsibility of resourcing the church in the establishment of the deacon and was very conscious of these varied responses to the new Order. They decided that an appropriate way to assist the church would be to develop a vision statement and then lead toward that vision. After much study and struggle, the following vision statement was adopted as a work in progress.
The ministry of the deacon (diaconate) distinctively embodies and gives leadership to servant ministry of compassion, mercy, and justice. Laity and clergy are transformed through worship which connects faith and daily life so that the people of God serve a hurting world as faithful disciples.
The section was aware that if the new Order of Deacon was to develop appropriately, enhanced understandings of how the Order connected to church traditions were needed. Much study, work, and reflection on the purpose and work of the deacon must take place.
It was realized that one of the most important groups of teachers would be the present ordained deacons in full connection. Their work and the fact that each one is appointed to a local church makes them crucial as interpreters and teachers on the new order.
The Deacon: Ministry Through Words of Faith and Acts of Love, is a resource for the deacon and for the church. It will assist with the dialogue and reflection on the Order and help the church be more effective as a lay-centered, lay-led body that is missional at its very core.
Carefully studied, the work will help the reader reflect on biblical images related to the ministry of the deacon. The relationship of worship and service, so distinctly linked to this new Order, is prominent in the scripture study. In addition, the authors have amplified the images with models from the lives and ministries of deacons at work today.
Paul Van Buren and Ben Hartley have produced a superb work by providing insights on how the United Methodist deacon can relate to a world in need and provide a needed flexibility in ministry similar to that of the early days of Methodism on the frontier of the United States.
I believe one of the most significant elements of this booklet is the scriptural visions of the deacon. The writers have effectively enhanced the church’s dialogue by sharing scriptures that reflect servant leadership.
This book will be useful if deacons, elders, and all church leaders will study it for understandings that will shape the day-to-day work and relationships of the deacon. That study and dialogue will assist the church in solidifying a paradigm of leadership
Interview with Paul Van Buren Hatten
In his January 19, 2004 interview with Lee Q. Miller, Paul Van Buren Hatten explains how he entered the service during WWII and became a ball gunner for the Air Force. Hatten recalls basic training, his first mission, and the times he was “concerned” for his safety. Hatten provides detail about his crew members and how their confidence and expertise made him feel safe. Case-Control Study of Risk Factors Associated with Feline and Canine Chronic Kidney Disease
An age-matched case-control study was initiated to determine the major risk factors associated with CKD in cats and dogs and to determine what clinical signs cat and dog owners observed before their veterinarian diagnosed their pet with CKD. When compared to controls, the feline cases were more likely to have had polydipsia and polyuria in the year before the owners' cats were diagnosed with CKD. In the dogs, increased water intake, increased urination, small size and a recent history of weight loss and bad breath were noticed by the dog owners before veterinary CKD diagnosis. Dog owners recognized abnormal drinking and urination behavior over half a year before their pet's veterinary diagnosis with CKD, and they recognized weight loss almost 4 months before CKD diagnosis. Bad breath was noticed 1.2 years before recognition of CKD by a veterinarian. Given that earlier CKD diagnosis should have been possible in most cases, clinical trials should proceed to measure the efficacy of early interventions
An IR Search for Extinguished Supernovae in Starburst Galaxies
IR and Radio band observations of heavily extinguished regions in starburst
galaxies suggest a very high SN rate associated with such regions. Optically
measured supernova (SN) rates may therefore underestimate the total SN rate by
factors of up to 10, due to the high extinction to SNe in starburst regions.
The IR/radio SN rates come from a variety of indirect means, however, which
suffer from model dependence and other problems.
We describe a direct measurement of the SN rate from a regular patrol of
starburst galaxies done with K' band imaging to minimize the effects of
extinction. A collection of K' measurements of core-collapse SNe near maximum
light is presented. Results of a preliminary SN search using the MIRC camera at
the Wyoming IR Observatory (WIRO), and an improved search using the ORCA optics
are described. A monthly patrol of starburst galaxies within 25 Mpc should
yield 1.6 - 9.6 SNe/year. Our MIRC search with low-resolution (2.2" pixels)
failed to find extinguished SNe, limiting the SN rate outside the nucleus (at >
15" radius) to less than 3.8 Supernova Rate Units (SRU or SNe/century/10^10
L(solar); 90% confidence). The MIRC camera had insufficient resolution to
search nuclear starburst regions, where SN activity is concentrated, explaining
why we found no heavily obscured SNe. We conclude that high-resolution, small
field SN searches in starburst nuclei are more productive than low resolution,
large-field searches, even for our large galaxies. With our ORCA
high-resolution optics, we could limit the total SN rate to < 1.3 SRU at 90%
confidence in 3 years of observations, lower than the most pessimistic

HLA and cross-reactive antigen group matching for cadaver kidney allocation
Background. Allocation of cadaver kidneys by graded human leukocyte antigen (HLA) compatibility scoring arguably has had little effect on overall survival while prejudicing the transplant candidacy of African-American and other hard to match populations. Consequently, matching has been proposed of deduced amino acid residues of the individual HLA molecules shared by cross- reactive antigen groups (CREGs). We have examined the circumstances under which compatibility with either method impacted graft survival. Methods. Using Cox proportional hazards regression modeling, we studied the relationship between levels of conventional HLA mismatch and other donor and recipient factors on primary cadaver kidney survival between 1981 and 1995 at the University of Pittsburgh (n=1,780) and in the United Network for Organ Sharing (UNOS) Scientific Registry during 1991-1995 (n=31,291). The results were compared with those obtained by the matching of amino acid residues that identified CREG-compatible cases with as many as four (but not five and six) HLA mismatches. Results. With more than one HLA mismatch (>85% of patients in both series), most of the survival advantage of a zero mismatch was lost. None of the HLA loci were 'weak.' In the UNOS (but not Pittsburgh) category of one-HLA mismatch (n=1334), a subgroup of CREG-matched recipients (35.3%) had better graft survival than the remaining 64.7%, who were CREG-mismatched. There was no advantage of a CREG match in the two- to four-HLA incompatibility tiers. Better graft survival with tacrolimus was observed in both the Pittsburgh and UNOS series. Conclusions. Obligatory national sharing of cadaver kidneys is justifiable only for zero-HLA-mismatched kidneys. The potential value of CREG matching observed in the one-HLA-mismatched recipients of the UNOS (but not the Pittsburgh) experience deserves further study
Modes of Disintegration of Solid Foods in Simulated Gastric Environment
A model stomach system was used to investigate disintegration of various foods in simulated gastric environment. Food disintegration modes and typical disintegration profiles are summarized in this paper. Mechanisms contributing to the disintegration kinetics of different foods were investigated as related to acidity, temperature, and enzymatic effect on the texture and changes in microstructure. Food disintegration was dominated by either fragmentation or erosion, depending on the physical forces acting on food and the cohesive force within the food matrix. The internal cohesive forces changed during digestion as a result of water penetration and acidic and enzymatic hydrolysis. When erosion was dominant, the disintegration data (weight retention vs. disintegration time) may be expressed with exponential, sigmoidal, and delayed-sigmoidal profiles. The different profiles are the result of competition among the rates of water absorption, texture softening, and erosion. A linear-exponential equation was used to describe the different disintegration curves with good fit. Acidity and temperature of gastric juice showed a synergistic effect on carrot softening, while pepsin was the key factor in disintegrating high-protein foods. A study of the change of carrot microstructure during digestion indicated that degradation of the pectin and cell wall was responsible for texture softening that contributed to the sigmoidal profile of carrot disintegration
Hepatocyte KLF6 expression affects FXR signalling and the clinical course of primary sclerosing cholangitis
Background & Aims: Primary sclerosing cholangitis (PSC) is characterized by chronic cholestasis and inflammation, which promotes cirrhosis and an increased risk of cholangiocellular carcinoma (CCA). The transcription factor Krueppel-like-factor-6 (KLF6) is a mediator of liver regeneration, steatosis, and hepatocellular carcinoma (HCC), but no data are yet available on its potential role in cholestasis. Here, we aimed to identify the impact of hepatic KLF6 expression on cholestatic liver injury and PSC and identify potential effects on farnesoid-X-receptor (FXR) signalling. Methods: Hepatocellular KLF6 expression was quantified by immunohistochemistry (IHC) in liver biopsies of PSC patients and correlated with serum parameters and clinical outcome. Liver injury was analysed in hepatocyte-specific Klf6-knockout mice following bile duct ligation (BDL). Chromatin-immunoprecipitation-assays (ChIP) and KLF6-overexpressing HepG2 cells were used to analyse the interaction of KLF6 and FXR target genes such as NR0B2. Results: Based on IHC, PSC patients could be subdivided into two groups showing either low (80%) hepatocellular KLF6 expression. In patients with high KLF6 expression, we observed a superior survival in Kaplan-Meier analysis. Klf6-knockout mice showed reduced hepatic necrosis following BDL when compared to controls. KLF6 suppressed NR0B2 expression in HepG2 cells mediated through binding of KLF6 to the NR0B2 promoter region. Conclusion: Here, we show an association between KLF6 expression and the clinical course and overall survival in PSC patients. Mechanistically, we identified a direct interaction of KLF6 with the FXR target gene NR0B2
Effect of No Prehydration vs Sodium Bicarbonate Prehydration Prior to Contrast-Enhanced Computed Tomography in the Prevention of Postcontrast Acute Kidney Injury in Adults With Chronic Kidney Disease The Kompas Randomized Clinical Trial
Importance Prevention of postcontrast acute kidney injury in patients with stage 3 chronic kidney disease (CKD) by means of prehydration has been standard care for years. However, evidence for the need for prehydration in this group is limited. Objective To assess the renal safety of omitting prophylactic prehydration prior to iodine-based contrast media administration in patients with stage 3 CKD. Design, Setting, and Participants The Kompas trial was a multicenter, noninferiority, randomized clinical trial conducted at 6 hospitals in the Netherlands in which 523 patients with stage 3 CKD were randomized in a 1:1 ratio to receive no prehydration or prehydration with 250 mL of 1.4% sodium bicarbonate administered in a 1-hour infusion before undergoing elective contrast-enhanced computed tomography from April 2013 through September 2016. Final follow-up was completed in September 2017. Data were analyzed from January 2018 to June 2019. Interventions In total, 262 patients were allocated to the no prehydration group and 261 were allocated to receive prehydration. Analysis on the primary end point was available in 505 patients (96.6%). Main Outcomes and Measures The primary end point was the mean relative increase in serum creatinine level 2 to 5 days after contrast administration compared with baseline (noninferiority margin of less than 10% increase in serum creatinine level). Secondary outcomes included the incidence of postcontrast acute kidney injury 2 to 5 days after contrast administration, mean relative increase in creatinine level 7 to 14 days after contrast administration, incidences of acute heart failure and renal failure requiring dialysis, and health care costs. Results Of 554 patients randomized, 523 were included in the intention-to-treat analysis. The median (interquartile range) age was 74 (67-79) years; 336 (64.2%) were men and 187 (35.8%) were women. The mean (SD) relative increase in creatinine level 2 to 5 days after contrast administration compared with baseline was 3.0% (10.5) in the no prehydration group vs 3.5% (10.3) in the prehydration group (mean difference, 0.5; 95% CI, -1.3 to 2.3; P <.001 for noninferiority). Postcontrast acute kidney injury occurred in 11 patients (2.1%), including 7 of 262 (2.7%) in the no prehydration group and 4 of 261 (1.5%) in the prehydration group, which resulted in a relative risk of 1.7 (95% CI, 0.5-5.9; P = .36). None of the patients required dialysis or developed acute heart failure. Subgroup analyses showed no evidence of statistical interactions between treatment arms and predefined subgroups. Mean hydration costs were euro119 (US 0) in the no prehydration group (P <.001). Other health care costs were similar. Conclusions and Relevance Among patients with stage 3 CKD undergoing contrast-enhanced computed tomography, withholding prehydration did not compromise patient safety. The findings of this study support the option of not giving prehydration as a safe and cost-efficient measure
The Kalanchoe genome provides insights into convergent evolution and building blocks of crassulacean acid metabolism
Crassulacean acid metabolism (CAM) is a water-use efficient adaptation of photosynthesis that has evolved independently many times in diverse lineages of flowering plants. We hypothesize that convergent evolution of protein sequence and temporal gene expression underpins the independent emergences of CAM from C3 photosynthesis. To test this hypothesis, we generate a de novo genome assembly and genome-wide transcript expression data for Kalanchoë fedtschenkoi, an obligate CAM species within the core eudicots with a relatively small genome (~260 Mb). Our comparative analyses identify signatures of convergence in protein sequence and re-scheduling of diel transcript expression of genes involved in nocturnal CO2 fixation, stomatal movement, heat tolerance, circadian clock, and carbohydrate metabolism in K. fedtschenkoi and other CAM species in comparison with non-CAM species. These findings provide new insights into molecular convergence and building blocks of CAM and will facilitate CAM-into-C3 photosynthesis engineering to enhance water-use efficiency in crops
