    Drought Monitoring and Assessment: Remote Sensing and Modeling Approaches for the Famine Early Warning Systems Network

    Drought monitoring is an essential component of drought risk management. It is usually carried out using drought indices/indicators that are continuous functions of rainfall and other hydrometeorological variables. This chapter presents a few examples of how remote sensing and hydrologic modeling techniques are being used to generate a suite of drought monitoring indicators at dekadal (10-day), monthly, seasonal, and annual time scales for several selected regions around the world. Satellite-based rainfall estimates are being used to produce drought indicators such as standardized precipitation index, dryness indicators, and start of season analysis. The Normalized Difference Vegetation Index is being used to monitor vegetation condition. Several satellite data products are combined using agrohydrologic models to produce multiple short- and long-term indicators of droughts. All the data sets are being produced and updated in near-real time to provide information about the onset, progression, extent, and intensity of drought conditions. The data and products produced are available for download from the Famine Early Warning Systems Network (FEWS NET) data portal at http:// earlywarning.usgs.gov. The availability of timely information and products support the decision-making processes in drought-related hazard assessment, monitoring, and management with the FEWS NET. The drought-hazard monitoring approach perfected by the U.S. Geological Survey for FEWS NET through the integration of satellite data and hydrologic modeling can form the basis for similar decision support systems. Such systems can operationally produce reliable and useful regional information that is relevant for local, district-level decision making

    Coronary calcium scores on dual-source photon-counting computed tomography:an adapted Agatston methodology aimed at radiation dose reduction

    OBJECTIVES: The aim of this study was to determine mono-energetic (monoE) level-specific photon-counting CT (PCCT) Agatston thresholds, to yield monoE level independent Agatston scores validated with a dynamic cardiac phantom. Also, we examined the potential of dose reduction for PCCT coronary artery calcium (CAC) studies, when reconstructed at low monoE levels. METHODS: Theoretical CAC monoE thresholds were calculated with data from the National Institute of Standards and Technology (NIST) database. Artificial CAC with three densities were moved in an anthropomorphic thorax phantom at 0 and 60-75 bpm, and scanned at full and 50% dose on a first-generation dual-source PCCT. For all densities, Agatston scores and maximum CT numbers were determined. Agatston scores were compared with the reference at full dose and 70 keV monoE level; deviations (95% confidence interval) < 10% were deemed to be clinically not-relevant. RESULTS: Averaged over all monoE levels, measured CT numbers deviated from theoretical CT numbers by 6%, 13%, and - 4% for low-, medium-, and high-density CAC, respectively. At 50% reduced dose and 60-75 bpm, Agatston score deviations were non-relevant for 60 to 100 keV and 60 to 120 keV for medium- and high-density CAC, respectively. CONCLUSION: MonoE level-specific Agatston score thresholds resulted in similar scores as in standard reconstructions at 70 keV. PCCT allows for a potential dose reduction of 50% for CAC scoring using low monoE reconstructions for medium- and high-density CAC. KEY POINTS: • Mono-energy level-specific Agatston thresholds allow for reproducible coronary artery calcium quantification on mono-energetic images. • Increased calcium contrast-to-noise ratio at reduced mono-energy levels allows for coronary artery calcium quantification at 50% reduced radiation dose for medium- and high-density calcifications

    Endocarditis is een ernstig ziektebeeld met een hoge mortaliteit en morbiditeit. In de klinische praktijk proberen we de behandeling van deze patiënten te verbeteren door goede en snelle diagnostiek en door tijdig adequate antibiotische en zo nodig chirurgische therapie te starten. In de nieuwste richtlijnen wordt het begrip ‘endocarditisteam’ geïntroduceerd als cruciaal onderdeel in de verbetering van de zorg voor patiënten met (een verdenking op) endocarditis. Er wordt gesteld dat endocarditis een multidisciplinaire aanpak vraagt omdat het een ziekte is met een grote variatie in presentatie, waarvoor expertise nodig is van verschillende specialisaties, en ook omdat patiënten in een vroege fase dienen te worden besproken in een chirurgisch team. Observationele studies tonen een belangrijke reductie in de mortaliteit van endocarditispatiënten die zijn besproken in een endocarditisteam.Dit artikel bespreekt de ervaringen met het opzetten van een endocarditisteam in twee verschillende regio’s in Nederland (Rotterdam-​Rijnmond en Groningen). Wat is belangrijk als het gaat om de structuur en functie van een endocarditisteam? Het opzetten van een endocarditisteam kan lastig zijn. Daarom geven we enkele praktische tips. Ten slotte wordt de toegevoegde waarde van een operationeel endocarditisteam geïllustreerd aan de hand van een casus.Endocarditis is a life-​threatening disease with high mortality and morbidity. In clinical practice, we try to improve the outcome of patients with endocarditis by implementing a better and faster diagnostic workup, a timely start of antimicrobial therapy and an early surgical intervention if required. In the most recent update of the guidelines for the management of patients with endocarditis, an Endocarditis team is put forward as crucial part in the improvement of care for patients with (suspected) endocarditis. They state that endocarditis requires a multidisciplinary approach since patients present with highly variable signs and symptoms, need a high-​standard of care from several medical specialists, and need to be discussed in a surgical team early in the course of the disease. Observational studies support this implementation by showing a marked decrease in mortality after dicussing endocarditis patients in an Endocarditis team. This article discusses the experience with the implementation of an Endocarditis team in two different regions of the Netherlands (Rotterdam-​Rijnmond and Groningen). Which aspects are important for the structure and function of an Endocarditis team? The setting up of an Endocarditis team can be difficult, therefore we provide some practical advice. Finally, an illustrative case is presented
