    Key issues in rural health: perspectives of health service providers in Queensland

    The Centre for Rural and Remote Area Health (CRRAH) held interactive research workshops in eight towns in Southern Queensland. The purpose of the workshops was to determine what health providers considered were major issues affecting their service and for these results to inform future research strategy of CRRAH. Over 150 organisations identified as either providing health services or having a significant interest in health provision in one or more of the targeted towns were invited to attend. The workshops used the nominal group technique to identify what the participants considered were key health issues in the geographical area in which they worked. These issues were then prioritised by the participants. Thematic analysis of the issues generated a ranking of themes by importance. Results were compared with a similar exercise undertaken in 2003. Participants from organisation directly involved with health care were complemented at the workshops by representatives from local government, the police service and church groups. A total of 85 participants representing 47 services and 41 different organisations attended the eight workshops. Issues generated by the participants were pooled into seventeen themes. Workforce issues were by far the major concern of health providers. Recruitment and retention of health workers were a major concern. The other four highest ranked themes across all workshops were mental health, access to health services, perceptions and expectations of consumers of health services and interagency cooperation. Aged care was an additional theme that generated a lot of concern at several of the workshops. The workshops provided important information to CRRAH for developing research strategy. Additionally, several new alliances among health providers were developed which will support sharing of information and resources. The workshops enabled rural and remote organisations to meet and identify the key health issues and supported research planning. Much need alliances among health providers were forged and collaborative research avenues are being explored. The workshop forum is an excellent means of information exchange

    Calcium responses of chicken trigeminal ganglion neurons to methyl anthranilate and capsaicin

    Using digital fluorescence imaging, we determined the effects of methyl anthranilate (MA), an avian irritant, and capsaicin (CAP), a mammalian irritant, on intracellular calcium ([Ca2+]i) in chicken trigeminal neurons. Concentration–response functions indicated that the threshold for inducing increases in [Ca2+]i was higher for CAP (30·mmol·l–1) than for MA (10·mmol·l–1). The maximum magnitudes of [Ca2+]i in response to MA and CAP were compared after normalization to 40·mmol·l–1 KCl. At equal concentrations (300·mmol·l–1), trigeminal neurons responded with a greater change in [Ca2+]i to MA (78% of KCl) than to CAP (43% of KCl). Furthermore, at 300·mmol·l–1, 48% of neurons responded to MA whereas only 16% responded to CAP. The increases in [Ca2+]i induced by both MA and CAP were dependent upon extracellular calcium. While the calcium responses to MA were also dependent on extracellular sodium, responses to CAP were not. There were separate but overlapping populations of neurons sensitive to MA and CAP. Taken together, the higher threshold concentration of CAP, the higher response magnitude to MA than CAP and the greater number of neurons sensitive to MA than CAP provide a rationale for the observed behavioral differences of birds to these two compounds. Finally, the findings that the calcium responses to MA and CAP have different ion dependencies and that there are separate populations sensitive to these compounds suggest different transduction mechanisms mediating chicken trigeminal responses to MA and CAP

    Martian glaciation and the flow of solid CO2

    The flow law determined experimentally for solid CO2 establishes that a hypothesis of glacial flow of CO2 at the Martian poles is not physically unrealistic. Compression experiments carried out under 1 atm pressure and constant strain rate demonstrate that the strength of CO2 near its sublimation point is considerably less than the strength of water ice near its melting point. The data fit a power law "creep" equation of the form , where is compressive strain rate (sec-1), [sigma] is compressive stress (bars), R is the gas constant in calories per mole, and T is absolute temperature. The exponent of [sigma] of 3.9 contrasts with a value near 3.1 for water ice, and indicates that the strain rate is somewhat more sensitive to stress for CO2 than for water. Likewise, the low activation energy for creep, 12 200 cal mole-1, illustrates that CO2 is not highly sensitive to temperature and is thus likely to flow over a broad range of temperatures below its melting point. Strength values for CO2 are of the order of one-tenth to one-third the strength of ice under equivalent conditions.A plausible glacial model for the Martian polar caps can be constructed and is helpful in explaining the unique character of the polar regions. CO2-rich layers deposited near the pole would have flowed outward laterally to relieve high internal shear stresses. The topography of the polar caps, the uniform layering of the layered deposits, and the general extent of the polar "sediments" could all be explained using this model. Flow of CO2 rather than water ice greatly reduces the problems with Martian glaciation. Nevertheless, problems do remain, in particular the large amounts of CO2 necessary, the need to increase vapor pressure and temperature with depth in the polar deposits, and the lack of good observational evidence of flor features.     Decrease in natural marine hydrocarbon seepage near Coal Oil Point, California, associated with offshore oil production

    Prolific natural hydrcarbon seepage occurs offshore of Coal Oil Point in the Santa Barbara Channel, California. Within the water column above submarine vents, plumes of hydrocarbon gas bubbles act as acoustic scattering targets. Using 3.5 kHz sonar data, seep distribution offshore of Coal Oil Point was mapped for August 1996, July 1995, and July 1973. Comparison of the seep distributions over time reveals more than 50% decrease in the areal extent of seepage, accompanied by declines in seep emission volume, in a 13 km2 area above a production oil reservoir. Declines in reservoir pressure and depletion of seep hydrocarbon sources associated with oil production are the mechanisms inferred to explain the declines in seep and emission volume

    Purpose: Although elevated cardiovascular disease (CVD) risk factors are associated with a higher risk of developing heart conditions across all ethnic groups, variations exist between groups in the distribution and association of risk factors, and also risk levels. This study assessed the 10-year predicted risk in a multiethnic cohort of women and compared the differences in risk between Asian and Caucasian women. Methods: Information on demographics, medical conditions and treatment, smoking behavior, dietary behavior, and exercise patterns were collected. Physical measurements were also taken. The 10-year risk was calculated using the Framingham model, SCORE (Systematic COronary Risk Evaluation) risk chart for low risk and high risk regions, the general CVD, and simplified general CVD risk score models in 4,354 females aged 20–69 years with no heart disease, diabetes, or stroke at baseline from the third Australian Risk Factor Prevalence Study. Country of birth was used as a surrogate for ethnicity. Nonparametric statistics were used to compare risk levels between ethnic groups. Results: Asian women generally had lower risk of CVD when compared to Caucasian women. The 10-year predicted risk was, however, similar between Asian and Australian women, for some models. These findings were consistent with Australian CVD prevalence. Conclusion: In summary, ethnicity needs to be incorporated into CVD risk assessment. Australian standards used to quantify risk and treat women could be applied to Asians in the interim. The SCORE risk chart for low-risk regions and Framingham risk score model for incidence are recommended. The inclusion of other relevant risk variables such as obesity, poor diet/nutrition, and low levels of physical activity may improve risk estimation

    Incentive-based approaches to sustainable fisheries

    The failures of traditional target-species management have led many to propose an ecosystem approach to fisheries to promote sustainability. The ecosystem approach is necessary, especially to account for fishery-ecosystem interactions, but by itself is not sufficient to address two important factors contributing to unsustainable fisheries — inappropriate incentives bearing on fishers, and the ineffective governance that frequently exists in commercial, developed fisheries managed primarily by total harvest limits and input-controls. We contend that much greater emphasis must be placed on fisher motivation when managing fisheries. Using evidence from more than a dozen ‘natural experiments’ in commercial fisheries, we argue that incentive-based approaches that better specify community, individual harvest, or territorial rights and also price ecosystem services — coupled with public research, monitoring and effective oversight — promote sustainable fisheries.incentives, sustainability, rights, fisheries management

    Using examples from more than a dozen fisheries, we highlight the failures of ‘command control’ management and show that approaches that empower fishers with the incentives and the mandate to be co-custodians of the marine environment can promote sustainability. Evidence is provided that where harvesters share well-defined management responsibilities over fish, and experience both the pain of overexploitation and the gains from conservation, they are much more likely to protect fish stocks and habitat.     Wavelet analysis of epileptic spikes

    Interictal spikes and sharp waves in human EEG are characteristic signatures of epilepsy. These potentials originate as a result of synchronous, pathological discharge of many neurons. The reliable detection of such potentials has been the long standing problem in EEG analysis, especially after long-term monitoring became common in investigation of epileptic patients. The traditional definition of a spike is based on its amplitude, duration, sharpness, and emergence from its background. However, spike detection systems built solely around this definition are not reliable due to the presence of numerous transients and artifacts. We use wavelet transform to analyze the properties of EEG manifestations of epilepsy. We demonstrate that the behavior of wavelet transform of epileptic spikes across scales can constitute the foundation of a relatively simple yet effective detection algorithm.Comment: 4 pages, 3 figure
