    Outsourcing VDC Workflows

    In an increasingly demanding world for infrastructure, technological methods have been found to assist construction project\u27s. As innovative construction methods are constantly increasing, so is the demand for technological needs within the architecture, engineering, and construction (AEC) industry. To ensure technological adoption, many countries have started to mandate higher levels of building information modeling (BIM) documentation beyond the 2D construction documents traditionally required throughout construction review and permitting. Some companies have capitalized on this change to the extent of making BIM their primary business. These companies focus solely on virtual design and construction (VDC) and determine whether clients lacking this specialty should outsource their technological needs or invest in their own VDC department. Through attitudinal research, three interviews were conducted to understand contractors’ response to the implementation of a platform that links contractors to freelance modelers specializing in VDC. This research aims to better understand if the use of a third-party platform is effective for contractors that are unwilling to adapt to mandated BIM requirements

    Common Carrier Regulation of Telecommunications Contracts and the Private Carrier Alternative

    The Communications Act of 1934 requires, among other things, that telephone companies as common carriers make their services available to the general public at affordable rates. The Federal Communications Commission (FCC) has the authority to classify telephone services as common carriers as well as the ability to remove common carrier regulation to promote competition, satisfy consumer demand for individually tailored offerings, and avoid unnecessary regulatory costs. The Authors of this Article believe that the FCC should remove the common carrier regulation from certain long-distance service contracts and that such regulation is consistent with the deregulatory aims of the recent Telecommunications Act of 1996. The Article first reviews the judicial development of the common carrier definition which finds that a communications service is acting as a carrier if it either (1) actually holds out its service indiscriminately to the public or (2) is required to hold itself out because the public interest requires it. The Authors discuss the relatively broad application of this definition to new and existing telephone services in a series of cases. Part I concludes with an examination of recent instances where the FCC has used this definition to reclassify various telecommunications services as private or noncommon carriers/carriage. These instances fall into five categories: (1) satellite transponders, (2) broadcast-related services, (3) private land mobile services, (4) private microwave services, and (5) certain communications services, such as enhanced services and inside wiring. In Part II the Authors describe the growth and importance of individually negotiated telecommunications service contracts to the business operations of many customers. The Article then discusses the application of the private carrier alternative to these contracts. The Authors argue that there is nothing inherent in these contracts which requires that they be designated as common carrier services. The Authors offer several reasons why the FCC should no longer require that the contract services indiscriminately be held out to the public, including some of the potential benefits of removing this regulation. The Authors feel that classifying the service contracts as private would promote competition and innovation, as well as reduce regulatory costs

    Pharmacological treatment of focal epilepsy in adults: an evidence based approach.

    INTRODUCTION: Focal seizures represent the most common seizure type and focal epilepsies the most common epilepsy type. Anti-seizure medications (ASMs) still represent the main form of treatment for epilepsy. AREAS COVERED: The aim of this review article is to provide an overview of available evidence about current and upcoming pharmacological options and strategies for adults with focal epilepsy focusing on the last 5 years. EXPERT OPINION: Seventeen drugs are currently approved for the treatment of focal seizures including cenobamate as the very latest option. Ten of these drugs are also licensed for monotherapy. Level A evidence for initial monotherapy is available for seven drugs with no robust data supporting that one drug is superior to the other. Safety, tolerability as well as pharmacoeconomic reasons would then drive treatment decisions. Data on adjunctive treatment are available for 13 ASMs showing again no obvious difference in terms of efficacy. Evidence on specific drug combinations is almost non-existent and the final decision of combining specific drugs is based on the experience of the individual clinician rather than on robust evidence. Current outcome measures do not consider number of previously failed drugs and the observation period is often too short

    Autonomous in situ calibration of ion‐sensitive field effect transistor pH sensors

    Ion‐sensitive field effect transistor‐based pH sensors have been shown to perform well in high frequency and long‐term ocean sampling regimes. The Honeywell Durafet is widely used due to its stability, fast response, and characterization over a large range of oceanic conditions. However, potentiometric pH monitoring is inherently complicated by the fact that the sensors require careful calibration. Offsets in calibration coefficients have been observed when comparing laboratory to field‐based calibrations and prior work has led to the recommendation that an in situ calibration be performed based on comparison to discrete samples. Here, we describe our work toward a self‐calibration apparatus integrated into a SeapHOx pH, dissolved oxygen, and CTD sensor package. This Self‐Calibrating SeapHOx is capable of autonomously recording calibration values from a high quality, traceable, primary reference standard: equimolar tris buffer. The Self‐Calibrating SeapHOx\u27s functionality was demonstrated in a 6‐d test in a seawater tank at Scripps Institution of Oceanography (La Jolla, California, U.S.A.) and was successfully deployed for 2 weeks on a shallow, coral reef flat (Lizard Island, Australia). During the latter deployment, the tris‐based self‐calibration using 15 on‐board samples exhibited superior reproducibility to the standard spectrophotometric pH‐based calibration using \u3e 100 discrete samples. Standard deviations of calibration pH using tris ranged from 0.002 to 0.005 whereas they ranged from 0.006 to 0.009 for the standard spectrophotometric pH‐based method; the two independent calibration methods resulted in a mean pH difference of 0.008. We anticipate that the Self‐Calibrating SeapHOx will be capable of autonomously providing climate quality pH data, directly linked to a primary seawater pH standard, and with improvements over standard calibration techniques

    Lacosamide add-on therapy for focal epilepsy

    BACKGROUND: This is an updated version of the Cochrane review published in 2015. Around half of people with epilepsy will not achieve seizure freedom on their first antiepileptic drug; many will require add‐on therapy. Around a third of people fail to achieve complete seizure freedom despite multiple antiepileptic drugs. Lacosamide has been licenced as an add‐on therapy for drug‐resistant focal epilepsy. OBJECTIVES: To evaluate the efficacy and tolerability of lacosamide as an add‐on therapy for children and adults with drug‐resistant focal epilepsy. SEARCH METHODS: We searched the following databases (22 August 2019): the Cochrane Register of Studies (CRS Web), including the Cochrane Epilepsy Group Specialized Register and the Cochrane Central Register of Controlled Trials (CENTRAL), MEDLINE (Ovid, 1946 to 20 August 2019), ClinicalTrials.gov, and the WHO International Clinical Trials Registry Platform (ICTRP), with no language restrictions. We contacted UCB Pharma (sponsors of lacosamide). SELECTION CRITERIA: Randomised controlled trials of add‐on lacosamide in people with drug‐resistant focal epilepsy. DATA COLLECTION AND ANALYSIS: We used standard Cochrane methodology, assessing the following outcomes: 50% or greater reduction in seizure frequency; seizure freedom; treatment withdrawal; adverse events; quality of life; and cognitive changes. The primary analyses were intention‐to‐treat. We estimated summary risk ratios (RR) for each outcome presented with 99% confidence intervals (CI), except for 50% or greater seizure reduction, seizure freedom and treatment withdrawal which were presented with 95% CIs. We performed subgroup analyses according to lacosamide dose and sensitivity analyses according to population age, whereby data from children were excluded from the meta‐analysis. MAIN RESULTS: We included five trials (2199 participants). The risk of bias for all studies was low to unclear. All studies were placebo‐controlled and assessed doses from 200 mg to 600 mg per day. One study evaluated lacosamide in children; all other studies were in adults. Trial duration ranged from 24 to 26 weeks. All studies used adequate methods of randomisation and were double‐blind. Overall, the certainty of the evidence for the outcomes was judged as moderate to high, with the exception of seizure freedom which was low. The RR for a 50% or greater reduction in seizure frequency for all doses of lacosamide compared with placebo was 1.79 (95% CI 1.55 to 2.08; 5 studies; 2199 participants; high‐certainty evidence). The RR for seizure freedom for all doses of lacosamide compared with placebo was 2.27 (95% CI 1.35 to 3.83; 5 studies; 2199 participants; low‐certainty evidence). The RR for treatment withdrawal for all doses of lacosamide compared with placebo was 1.57 (95% CI 1.24 to 1.98; 5 studies; 2199 participants; moderate‐certainty evidence). The estimated effect size for most outcomes did not change considerably following sensitivity analysis. For seizure freedom, however, the RR nearly doubled upon the exclusion of data from children (RR 4.04, 95% CI 1.52 to 10.73). Adverse events associated with lacosamide included: abnormal co‐ordination (RR 6.12, 99% CI 1.35 to 27.77), blurred vision (RR 4.65, 99% CI 1.24 to 17.37), diplopia (RR 5.59, 99% CI 2.27 to 13.79), dizziness (RR 2.96, 99% CI 2.09 to 4.20), nausea (RR 2.35, 99% CI 1.37 to 4.02), somnolence (RR 2.04, 99% CI 1.22 to 3.41), vomiting (RR 2.94, 99% CI 1.54 to 5.64), and number of participants experiencing one or more adverse events (RR 1.12, 99% CI 1.01 to 1.24). Adverse events that were not significant were: vertigo (RR 3.71, 99% CI 0.86 to 15.95), rash (RR 0.58, 99% CI 0.17 to 1.89), nasopharyngitis (RR 1.41, 99% CI 0.87 to 2.28), headache (RR 1.34, 99% CI 0.90 to 1.98), fatigue (RR 2.11, 99% CI 0.92 to 4.85), nystagmus (RR 1.47, 99% CI 0.61 to 3.52), and upper respiratory tract infection (RR 0.70, 99% CI 0.43 to 1.15). AUTHORS' CONCLUSIONS: Lacosamide is effective and well‐tolerated in the short term when used as add‐on treatment for drug‐resistant focal epilepsy. Lacosamide increases the number of people with 50% or greater reduction in seizure frequency and may increase seizure freedom, compared to placebo. Higher doses of lacosamide may be associated with higher rates of adverse events and treatment withdrawal. Additional evidence is required assessing the use of lacosamide in children and on longer‐term efficacy and tolerability

    Comparison of Two Methods for Measuring Sea Surface Temperature When Surfing

    Nearshore coastal waters are among the most dynamic regions on the planet and difficult to sample from conventional oceanographic platforms. It has been suggested that environmental sampling of the nearshore could be improved by mobilising vast numbers of citizens who partake in marine recreational sports, like surfing. In this paper, we compared two approaches for measuring sea surface temperature (SST), an Essential Climate Variable, when surfing. One technique involved attaching a commercially-available miniature temperature logger (Onset UTBI-001 TidbiT v2) to the leash of the surfboard (tether connecting surfer and surfboard) and the second, attaching a surfboard fin (Smartfin) that contained an environmental sensor package. Between July 2017 and July 2018, 148 surfing sessions took place, 90 in the southwest UK and 58 in San Diego, California, USA. During these sessions, both Smartfin and leash sensors were deployed simultaneously. On the leash, two TidbiT v2 sensors were attached, one with (denoted LP) and one without (denoted LU) a protective boot, designed to shield the sensor from sunlight. The median temperature from each technique, during each surfing session, was extracted and compared along with independent water temperature data from a nearby pier and benthic logger, and matched with photosynthetically available radiation (PAR) data from satellite observations (used as a proxy for solar radiation during each surf). Results indicate a mean difference ( Ύ ) of 0.13 °C and mean absolute difference ( ϔ ) of 0.14 °C between Smartfin and LU, and a Ύ of 0.04 °C and an ϔ of 0.06 °C between Smartfin and LP. For UK measurements, we observed better agreement between methods ( Ύ=0.07 °C and ϔ=0.08 °C between Smartfin and LU, and Ύ=0.00 °C and ϔ=0.03 °C between Smartfin and LP) when compared with measurements in San Diego ( Ύ=0.22 °C and ϔ=0.23 °C between Smartfin and LU, and Ύ=0.08 °C and ϔ=0.11 °C between Smartfin and LP). Surfing SST data were found to agree well, in general, with independent temperature data from a nearby pier and benthic logger. Differences in SST between leash and Smartfin were found to correlate with PAR, both for the unprotected (LU) and protected (LP) TidbiT v2 sensors, explaining the regional differences in the comparison (PAR generally higher during US surfing sessions than UK sessions). Considering that the Smartfin is sheltered from ambient light by the surfboard, unlike the leash, results indicate the leash TidbiT v2 sensors warm with exposure to sunlight biasing the SST data positively, a result consistent with published tests on similar sensors in shallow waters. We matched all LU data collected prior to this study with satellite PAR products and corrected for solar heating. Results highlight the need to design temperature sensor packages that minimise exposure from solar heating when towed in the surface ocean

    Expanding Aquatic Observations through Recreation

    Accurate observations of the Earth system are required to understand how our planet is changing and to help manage its resources. The aquatic environment—including lakes, rivers, wetlands, estuaries, coastal and open oceans—is a fundamental component of the Earth system controlling key physical, biological, and chemical processes that allow life to flourish. Yet, this environment is critically undersampled in both time and space. New and cost-effective sampling solutions are urgently needed. Here, we highlight the potential to improve aquatic sampling by tapping into recreation. We draw attention to the vast number of participants that engage in aquatic recreational activities and argue, based on current technological developments and recent research, that the time is right to employ recreational citizens to improve large-scale aquatic sampling efforts. We discuss the challenges that need to be addressed for this strategy to be successful (e.g., sensor integration, data quality, and citizen motivation), the steps needed to realize its potential, and additional societal benefits that arise when engaging citizens in scientific sampling
