    This comparative study develop a theoretical-methodological discussionon the concrete conditions for social change, specifically for change in the capital/work relationship. Priorizing a regional perspective which analyses internalas well as external hegemonies and peripheries, the author examines the relationshipbetween the State and industry, defining the State as a new product ofclass relations and, at the same time, a producer of change.     The renal and hepatic distribution of Bence Jones proteins depends on glycosylation: A scintigraphic study in rats

    The aim of the present study was to evaluate renal and liver distribution of two monoclonal immunoglobulin light chains. the chains were purified individually from the urine of patients with multiple myeloma and characterized as lambda light chains with a molecular mass of 28 kDa. They were named BJg (high amount of galactose residues exposed) and BJs (sialic acid residues exposed) on the basis of carbohydrate content. A scintigraphic study was performed on male Wistar rats weighing 250 g for 60 min after iv administration of 1 mg of each protein (7.4 MBq), as the intact proteins and also after carbohydrate oxidation. Images were obtained with a Siemens gamma camera with a high-resolution collimator and processed with a MicroDelta system. Hepatic and renal distribution were established and are reported as percent of injected dose. Liver uptake of BJg was significantly higher than liver uptake of BJs (94.3 vs 81.4%) (P<0.05). This contributed to its greater removal from the intravascular compartment, and consequently lower kidney accumulation of BJg in comparison to BJs (5.7 vs 18.6%) (P<0.05). After carbohydrate oxidation, there was a decrease in hepatic accumulation of both proteins and consequently a higher renal overload. the tissue distribution of periodate-treated BJg was similar to that of native BJs: 82.7 vs 81.4% in the liver and 17.3 vs 18.6% in the kidneys.     Anuloplastia mitral com tira de pericárdio bovino resultados de 18 anos

    PURPOSE: Prosthetic annuloplasty rings are currently used in mitral reconstruction. Posterior annuloplasty with a bovine pericardial strip is a technique largely used in the Heart Institute of University of São Paulo Medical School. The purpose of the study was to analyze the late results of mitral valve repair with posterior annuloplasty using a bovine pericardial strip. METHODS: Between January 1984 and December 2002, 273 patients underwent mitral valve repair with posterior pericardial annuloplasty in the Heart Institute of University of São Paulo Medical School. One hundred and forty four (52.7%) were women and ages ranged between 1 and 76 years (38.3 ± 21.1). Rheumatic fever was present in 52.0% of the patients. Associated techniques were employed in 26.0% of the patients, and the most frequent was chordal shortening (9.2%). RESULTS: Hospital mortality was 3.3% (9 patients), with the major cause being low cardiac output (6 patients). Actuarial survival was 55.1% ± 16.8% in 18 years. During the 18-year follow-up: patients were free from the following: reoperation (59.1% ±± 13.9%, (percent +/- Standard Error), thromboembolism (97.4% ± 2.3%), hemolysis (99.2% ± 0.2%), and endocarditis (99.6% ± 1.0%). In the late follow-up period, 83.9% were classified as New York Heart Association functional class I. CONCLUSIONS: Late results with mitral valve repair with posterior annuloplasty using a bovine pericardial strip were satisfactory. The technique is feasible, reproducible, and cost effective.OBJETIVO: Anéis protéticos para anuloplastia são usados nas plásticas da valva mitral de forma rotineira. A anuloplastia posterior com tira de pericárdio bovino é uma técnica largamente utilizada no Instituto do Coração - HC - FMUSP. O objetivo deste estudo foi analisar os resultados tardios da plástica da valva mitral com a utilização desta técnica na nossa instituição. MÉTODOS: Entre janeiro de 1984 e dezembro de 2002, 273 pacientes foram submetidos a plástica da valva mitral com anuloplastia posterior no Instituto do Coração. Foram excluídos os pacientes submetidos a outros procedimentos no anel posterior. Cento e quarenta e quatro (52,7%) eram do sexo feminino e a idade variou entre 1 e 76 anos (média: 38,3 ± 21,1 anos). Em 52% dos pacientes, havia história de febre reumática. Técnicas associadas para reconstrução valvar foram empregadas em 26% dos casos, sendo a mais utilizada, o encurtamento de cordas (9,2%). RESULTADOS: A mortalidade hospitalar foi de 3,3% (9 casos). A principal causa de óbito foi baixo débito cardíaco (6 pacientes). A sobrevida actuarial foi de 55,1 ± 16,8% (percent +/- Standard Error) em 18 anos, sobrevida livre de reoperação, tromboembolismo, hemólise e endocardite foram respectivamente de 59,1 ± 13,9%, 97,4 ± 2,3%, 99,2 ± 0,2%, 99,6 ± 1,0% em 18 anos. No seguimento tardio, 83,9% dos pacientes se encontravam em classe funcional I (NYHA). CONCLUSÕES: Concluímos que os resultados tardios foram satisfatórios.     3D microsimulation of milkruns and pickers in warehouses using SIMIO

    To help the Bosch Car Multimedia Portugal in Ferreiros, Braga to reduce its costs (both in time and space) with its warehouse, a micro simulation model is being developed in Simio. Particularly, the tool needs to be able to model pickers riding milkruns to collect containers of products, from a warehouse, to satisfy the needs of the production lines. In this sense, the storage strategy used on the warehouse, the quantity of requests a picker gets per shift, the time between shifts, the number of types of products, the arrival rate of requests, and the number of milkruns and pickers needs to be adjustable. Additionally, to design the corridors of the warehouse in a configurable way, an Add-in in C#, using the API of Simio, is being developed. Thus, this paper intends to document the first part of the simulation model developed, which consists on the pickers receiving requests and riding their milkruns to collect the respective containers from the warehouse. Five different Simio models compose the main simulation model.     Towards an approach to assess Supply Chain Quality Management maturity

    Maturity models have been developed in order to help companies to improve organizational performance. Furthermore, due to the globalization and the growing competition, companies need to increase the competitiveness through operational efficiency, internally and in the entire supply chain.In this paper, an approach for the design and implementation of a Supply Chain Quality Management maturity model is presented.The concept of Supply Chain Quality Management has been developed in order to achieve a good integration between the two approaches: Quality Management and Supply Chain Management, and how such integration can help and support the companies to become more effective and efficient.The proposed approach to assess Supply Chain Quality Management maturity incorporates both quantitative and qualitative information to define several levels corresponding to different integration maturity levels.     Infrared spectroscopy to assess manufacturing procedures of bone artefacts from the chalcolithic settlement of Vila Nova de São Pedro (Portugal)

    Vibrational spectroscopy was applied to study cylindrical engraved bone boxes from the Chalcolithic settlement of Vila Nova de São Pedro (VNSP, Azambuja, Portugal) which has the largest and richest artefact assemblage of Copper Age Western Iberia. The objectives were to reconstitute manufacturing techniques, determine the role of pyrotechnology in the production of cylindrical engraved bone boxes and assess oxygen conditions during burning. Four fragments of cylindrical engraved bone “boxes” from VNSP were used in this research. Anaerobic experimental burn conditions were recreated by using a home-made steel airtight chamber under vacuum. Human bone fragments were burnt at 400–1000 °C for 120–211 min. Fourier-transform infrared spectroscopy analyses were performed on bone powder samples. The resulting spectra and chemometric indices were used as a reference to establish comparisons with the archaeological artefacts. None of these presented spectral features compatible with anaerobic burning. Therefore, aerobic burns were used to achieve the whitish look and were most probably used to attain the darker shade displayed by the artefacts. Artefact manufacturing appears to have relied on bone cutting, bone engraving and maybe polishing, followed by heat treatment.     Characterization of NS5A and NS5B Resistance-Associated Substitutions from Genotype 1 Hepatitis C Virus Infected Patients in a Portuguese Cohort

    This study is focused on the prevalent NS5 coding region resistance-associated substitutions (RASs) in DAA-naive genotype (GT)1 HCV-infected patients and their potential impact on success rates. Plasma RNA from 81 GT1 HCV-infected patients was extracted prior to an in-house nested RT-PCR of the NS5 coding region, which is followed by Sanger population sequencing. NS5A RASs were present in 28.4% (23/81) of all GT1-infected patients with 9.9% (8/81) having the Y93C/H mutation. NS5B RASs showed a prevalence of 14.8% (12/81) and were only detected in GT1b. Overall 38.3% (31/81) of all GT1 HCV-infected patients presented baseline RASs. The obtained data supports the usefulness of resistance testing prior to treatment since a statistically significant association was found between treatment failure and the baseline presence of specific NS5 RASs known as Y93C/H (p = 0.04).publishersversionpublishe

    Qualidade de vida de idosos hipertensos e diabéticos em um serviço ambulatorial

    Modelo do estudo: Estudo populacional descritivo, de corte transversal. Objetivo: Avaliar a qualidade de vida de pacientes idosos hipertensos e diabéticos em um ambulatório de um hospital universitário no interior do Estado de São Paulo, Brasil. Metodologia: Estudo realizado em um ambulatório de Geriatria, em São José do Rio Preto, envolvendo 62 pacientes idosos com pressão arterial elevada e/ou diabetes mellitus, atendidos no período de dezembro de 2009 a fevereiro de 2010. Os dados foram coletados com o WHOQOL-OLD, analisados com o Statistical Package for Social Sciences (SPSS) software, com testes Mann-Whitney e Kruskal-Wallis não paramétricos e correlação de Pearson, o nível de significância foi ajustado para um valor de p&lt;0,05. Resultados: 38 (61,3%) idosos eram hipertensos e 23 (37,1%) diabéticos. A idade dos idosos foi de 67±7 (média, DP). Houve prevalência do sexo feminino (58 %), ensino fundamental (87,1%), casados (56,5%), profissões ligadas ao setor de serviços (56,4%), renda de 1 salário mínimo (66,1%) e morando com esposo(a) (58,1%). O maior escore mediano (75,0) foi encontrado nas facetas "Atividades passadas, presentes e futuras", "Participação social", "Morte e morrer" e "Intimidade". A faceta "Autonomia" apresentou o menor escore, correspondendo a 62,5. Os idosos hipertensos tiveram escore inferior aos diabéticos na faceta do "Funcionamento Sensório" (62,2 vs. 73,6, respectivamente). Conclusão: Os idosos apresentaram qualidade de vida inferior na faceta "Autonomia" e melhor qualidade para as facetas "Atividades passadas, presentes e futuras", "Participa- ção social", "Morte e morrer" e "Intimidade". Não houve diferença na qualidade de vida entre diabéticos ou hipertensos, exceto por tendência a menor "Funcionamento do sensório" entre os hipertensosModel of study: The study design was a populational, cross-sectional and descriptive. Objective: To evaluate the quality of life of high blood pressure and diabetic elderly outpatients of a teaching hospital in the interior of Sao Paulo State, Brazil. Methodology: The study was done in a Geriatrics Outpatient Service in São José do Rio Preto, involved 62 elderly patients with high blood pressure and/or diabetes mellitus treated in the period from December 2009 to February 2010. The data were collected with the WHOQOL-OLD, analyzed with the Statistical Package for Social Sciences (SPSS) software, with tests Mann-Whitney, Kruskal-Wallis non-parametric and Pearson's correlation; the level of significance was set to a value of p&lt;0.05. Results: A total of 38 elderly patients studied (61.3%) have high blood pressure, and 23 (37.1%) were diabetics. The age was 67±7 (mean, SD). Characteristics of the sample studied were: female (58.1%), elementary school (87.1%), married status (56.4%), professions linked to the sector of services (56.4%), the income of 1 minimum wage (66.1%) and living with the spouse (58.1%). The highest median score (75.0) was found in the facets "Past, present and future" "Activities", "Social participation", "Death and dying" and "Intimacy". The facet "Autonomy" showed the lowest scoring of 62.5. The elderly with high blood pressure had a lower score compared to patients with diabetes in "Sensory Functioning" facet (62.17 versus 73.64, respectively). Conclusion: The elderly showed a lower quality of life in the "Autonomy" and best quality for facets: "Past, present and future", "Activities", "Social participation", "Death and dying" and "Intimacy". There was not difference in the quality of life among elderly with diabetes and high blood pressure patients, except for a tendency to lower "Sensory Functioning" facet in high blood pressure patient
