    Latinidad y Antirracismo: Fostering Antiracist Conversations in Medical Education

    EDUCATIONAL OBJECTIVES: By the end of this activity, learners will be able to: 1. Develop antiracist healthcare workforce by increasing foundational awareness of health equity issues specific to the Hispanic/Latino/x community. 2. Develop upstanders that choose to intervene in situations where racism, discrimination, and microaggressions are present. 3. Describe concepts associated with antiracism, equity, diversity, and inclusion so that trainees speak a foundational common language. 4. Practice antiracist terminology in Spanish and associate this language with medical Spanish skills. INTRODUCTION: The purpose of the lecture series was to create a space for Hispanic/Latino/x learners to share their experiences and critically examine their own culture. In addition, an open invitation created a space for other minoritized and majority students to learn and engage in conversation. This series engages on a topic that is less discussed, racism in Hispanic/Latino/x communities. METHODS: This project consisted of three (3) virtual interactive sessions. Learners did not require prerequisite knowledge; however, we created a discussion guide, Glosario de Términos Asociados con Racismo, that served as a point of reference for students to use throughout the series. Participants rated their familiarity pre and post each session via survey. RESULTS: In total, 22 responses were received to the post-survey (n = 22). After all the presentations, 100% of the participants reported acquiring new information. Ten out of 22 participants informed that they learned a ‘great deal of new information. Furthermore, 95.45% of participants would recommend participating in the interactive sessions with a colleague. DISCUSSION: Through these sessions, we reflected on what it means to be Hispanic/Latino/x, how discrimination exists within the cultures that fall under this umbrella term, how they fit (and often do not fit) in the U.S. social construct of the term regardless of our geographical location, and how their identities are further complicated by intersectionality. We have created the precedent for future medical students to continue having and expanding on these conversations, to further provide nuance as to what it really means to “be” Hispanic/Latino/x

    Risk of suicide in households threatened with eviction: the role of banks and social support

    Background: One of the greatest effects of the financial crisis in Spain has been the enormous increase in the number of evictions. Several studies have shown the association of evictions with different aspects of the physical and mental health. Furthermore, evictions have been associated with an increased risk of suicide. Our objective was to evaluate the risk of suicide among victims of eviction and investigate whether it is associated with specific characteristics of households and interviewees, the eviction process and social support, and health needs. Results: Almost half of the sample (46.7%) were at low (11.8%), moderate (16.9%), or high suicide risk (17.9%). Household and interviewee features had a limited association with suicide risk. On the contrary, the risk of suicide is greater with a longer exposure to the eviction process. In addition, threatening phone calls from banks increased significantly the risk of suicide, especially among men. Suicide risk was also associated with low social support, especially among women. Interviewees at risk of suicide received more help from nongovernmental organizations than those who were not at risk. In interviewees at risk, the main unmet needs were emotional and psychological help, especially in men. A high percentage of those at risk of suicide declare having large unmeet health needs. Finally, there was a tendency among the evicted at risk of suicide to visit emergency room and primary care more often than those not at risk, especially among women. Conclusions: To our knowledge, this is the first study showing that when banks adopt a threatening attitude, suicide risk increases among the evicted.     Methylprednisolone Pulses Plus Tacrolimus in Addition to Standard of Care vs. Standard of Care Alone in Patients With Severe COVID-19. A Randomized Controlled Trial

    Introduction: Severe lung injury is triggered by both the SARS-CoV-2 infection and the subsequent host-immune response in some COVID-19 patients. Methods: We conducted a randomized, single-center, open-label, phase II trial with the aim to evaluate the efficacy and safety of methylprednisolone pulses and tacrolimus plus standard of care (SoC) vs. SoC alone, in hospitalized patients with severe COVID-19. The primary outcome was time to clinical stability within 56 days after randomization. Results: From April 1 to May 2, 2020, 55 patients were prospectively included for subsequent randomization; 27 were assigned to the experimental group and 28 to the control group. The experimental treatment was not associated with a difference in time to clinical stability (hazard ratio 0.73 [95% CI 0.39-1.37]) nor most secondary outcomes. Median methylprednisolone cumulative doses were significantly lower (360 mg [IQR 360-842] vs. 870 mg [IQR 364-1451]; p = 0.007), and administered for a shorter time (median of 4.00 days [3.00-17.5] vs. 18.5 days [3.00-53.2]; p = 0.011) in the experimental group than in the control group. Although not statistically significant, those receiving the experimental therapy showed a numerically lower all-cause mortality than those receiving SoC, especially at day 10 [2 (7.41%) vs. 5 (17.9%); OR 0.39 (95% CI 0.05-2.1); p = 0.282]. The total number of non-serious adverse events was 42 in each the two groups. Those receiving experimental treatment had a numerically higher rate of non-serious infectious adverse events [16 (38%) vs. 10 (24%)] and serious infectious adverse events [7 (35%) vs. 3 (23%)] than those receiving SoC. Conclusions: The combined use of methylprednisolone pulses plus tacrolimus, in addition to the SoC, did not significantly improve the time to clinical stability or other secondary outcomes compared with the SoC alone in severe COVID-19. Although not statistically significant, patients receiving the experimental therapy had numerically lower all-cause mortality than those receiving SoC, supporting recent non-randomized studies with calcineurin inhibitors. It is noteworthy that the present trial had a limited sample size and several other limitations. Therefore, further RCTs should be done to assess the efficacy and safety of tacrolimus to tackle the inflammatory stages of COVID-19

    Potencial de la gramática del MuSIASEM en la representación del análisis de la sostenibilidad

    Una alta capacidad para representar el análisis de la sostenibilidad es la exigencia central de cualquier metodología que pretenda encontrar en dicho análisis una herramienta para el desarrollo social y un diagnóstico de relación de las sociedades humanas con el entorno. Los requerimientos de dicha capacidad representadora se cifran en un lenguaje lo suficientemente recursivo y sintético en sus rasgos y principios como para ser flexible y en ello ofrecer pertinencia y operacionalidad para diversidad de escenarios y situaciones que, en todo caso, se caracterizan por la complejidad del tejido entre sus múltiples niveles y dimensiones. A través de una gramática el MUSIASEM ha cifrado en ello su potencial analítico y predictivo. El presente artículo argumenta las ventajas y alcances de las gramáticas como alternativa a los modelos en la representación del análisis de la sostenibilidad, explica cómo está fundamentado y construido el caso de la gramática del MUSIASEM y expone aplicaciones ya elaboradas a los temas de alimentos, suelos, energía, construcción y energía
