    International Classification of Diseases – 11th revision: from design to implementation

    The World Health Organization launched in May 2019 the new International Classification of Diseases (ICD), 11th revision. As a contribution to this transition, this article aims to present the main changes of the revised version of the classification and indicate the most pressing challenges. After 30 years of the ICD-10 publication, we identified significant challenges regarding the new classification, which was presented for adoption by several countries and will be in force in January 2022. The purpose of the preview is to allow countries to plan its use and train their professionals. The new version is completely digital, thus reducing notification errors and facilitating the dissemination and consolidation of this new version. The update highlights the advances in scientific understanding, and it demands structural actions and implementation efficiency from governments, so that everyone who deals with assistance can speak the same language, on a global scale.A Organização Mundial da Saúde lançou em maio de 2019 a nova Classificação Internacional de Doenças (CID), 11ª revisão. Como contribuição a essa transição, o objetivo deste texto é apresentar as principais mudanças da versão revisada da classificação e indicar os desafios mais prementes. Após 30 anos da publicação da CID-10, identificam-se desafios importantes quanto à nova classificação, que foi apresentada para adoção dos Estados-membros e entrará em vigor em janeiro de 2022. A finalidade da pré-visualização é permitir aos países planejar o uso e treinar seus profissionais. A nova versão é completamente digital, diminuindo assim os erros de notificação e facilitando a divulgação e consolidação da nova versão. A atualização deixa transparecer os avanços da compreensão cientifica e exige dos governos ações estruturantes e eficiência na implementação, para que todos que tratam da assistência possam se comunicar numa mesma linguagem, em escala mundial

    Analysis of permanent second molar development in children born with cleft lip and palate

    The study of dental development in individuals born with cleft lip and palate (CLP) serves to determine when orthodontic intervention should start. Objective: To evaluate the permanent second molar development in children born with cleft lip and palate according to Demirjian’s and Nolla’s methods. Methodology: Out of a total of 513 digital panoramic radiographs, 113 pairs of children aged 3 to 16 years were selected. The exams were from children born with or without cleft lip and palate, of the same sex, with an age difference of up to 30 days. The images were analyzed by three examiners and reliability was checked through intra-examiner agreement by the Kappa test. The data were analyzed by Wilcoxon's and Mann-Whitney tests according to each dataset. Results: The findings indicated delayed development of the permanent second molars in children with CLP (P<0.001). The development of the right permanent second molar was delayed compared to the left molar in children with CLP. Moreover, mandibular teeth showed significantly earlier development than maxillary teeth in both the case and control groups. There was no significant difference in the development of permanent second molars between sexes. Conclusion: Children with CLP presented delay in the development of permanent second molars

    The Effect of Reciprocating and Rotary Systems on Postoperative Endodontic Pain: A Systematic Review and Meta-analysis

    Introduction: The objective of this study was to evaluate the influence of the instrumentation kinematics on endodontic postoperative pain. Methods and Materials: PubMed, Scopus, Web of Science, Lilacs, Cochrane Library and the System for Information on Gray Literature in Europe were searched electronically without time or language limitations up to June 2020. Subsequently, data extraction, quality assessment and meta-analysis were conducted. The meta-analysis was performed using random-effects inverse-variance methods, and heterogeneity was tested using the I2 index (P<0.05). Results: A total of 318 articles were successfully identified in the search. Sixteen studies were used in qualitative synthesis and fourteen used for quantitative synthesis. Meta-analysis showed that patients treated with reciprocating system had lower risk of pain 48 h after endodontic treatment (Risk ratio [RR]=1.04, 95% Confidence interval [CI]=1.01-1.06, P=0.003) (I2=0%), but the mean postoperative pain for the reciprocating system was greater  24 h post endodontic treatment (Standardized mean difference [SMD]=0.25, 95% CI=0.06 to 0.44, P=0.01) (I2=43%). Other time points presented similar rates of postoperative pain (P>0.05). The certainty of evidence ranges from very low to high. Conclusions: The rate of postoperative endodontic pain was low, and reciprocating systems evoked more pain within the 24 h interval. Overall, the incidence and level of postoperative pain did not vary between reciprocating and rotary systems. There is no consensus if there is a relationship between the kinematics (rotary and reciprocating) and the incidence of postoperative pain

    Percepção da identidade alimentar de consumidores de um mercado público do Nordeste Brasileiro / Perception of food identity of consumers in a public market in Northeastern Brazil

    A alimentação está envolta por diversos significados, questões socioculturais, experiências pessoais. O estudo objetivou descrever o perfil do indivíduo que consome alimentos habitualmente em um mercado público da capital teresinense e conhecer as principais motivações que os levam a fazer suas refeições nesse espaço. Trata-se de uma pesquisa de campo transversal, de natureza quantitativa, com aspectos qualitativos. O mercado é um local frequentado por um público diversificado, representado em maior parte por adultos jovens, em maioria do sexo masculino, casados ou solteiros, pardos, com pelo menos o ensino médio e com prevalência de baixa renda. Sabor, tempo e preço foram valores destacados pelos entrevistados, mostrando que a gastronomia local contribui para a motivação da escolha alimentar. Ficou evidenciado que a comida do mercado é dotada de simbolismo e o espaço é um replicador da cultura local, uma vez que fornece à população produtos e pratos típicos da região

    Social capital and dental pain in Brazilian northeast: a multilevel cross-sectional study.

    <p>Abstract</p> <p>Background</p> <p>There is limited evidence on possible associations between social determinants and dental pain. This study investigated the relationship of neighborhood and individual social capital with dental pain in adolescents, adults and the elderly.</p> <p>Methods</p> <p>A population-based multilevel study was conducted involving 624 subjects from 3 age groups: 15–19, 35–44 and 65–74 years. They were randomly selected from 30 census tracts in three cities in the State of Paraíba, Brazil. A two-stage cluster sampling was used considering census tracts and households as sampling units. The outcome of study was the presence of dental pain in the last 6 months. Information on dental pain, demographic, socio-economic, health-related behaviors, use of dental services, self-perceived oral health and social capital measures was collected through interviews. Participants underwent a clinical examination for assessment of dental caries. Neighborhood social capital was evaluated using aggregated measures of social trust, social control, empowerment, political efficacy and neighborhood safety. Individual social capital assessment included bonding and bridging social capital. Multilevel logistic regression was used to test the relationship of neighborhood and individual social capital with dental pain after sequential adjustment for covariates.</p> <p>Results</p> <p>Individuals living in neighborhoods with high social capital were 52% less likely to report dental pain than those living in neighborhoods with low social capital (OR = 0.48, 95% CI = 0.27-0.85). Bonding social capital (positive interaction) was independently associated with dental pain (OR = 0.88, 95% CI = 0.80-0.91). Last dental visit, self-perceived oral health and number of decayed teeth were also significantly associated with dental pain.</p> <p>Conclusions</p> <p>Our findings suggest that contextual and individual social capital are independently associated with dental pain.</p

    Atualmente, com a era da antibioticoterapia e demais meios terapêuticos, o abscesso pulmonar decaiu em termos de morbimortalidade, mas ainda permanece como um desafio em termos diagnósticos e manejo clínico. O abscesso pulmonar corresponde a uma cavidade com pus no pulmão, envolvido por tecido inflamado e geralmente oriunda de uma infecção. O artigo objetivou descrever de modo narrativo a evolução clínica do portador de abscesso pulmonar, ressaltando os principais dados para a compreensão deste fenômeno. Um abscesso pulmonar é causado principalmente por bactérias existentes na boca ou garganta, a qual são aspiradas até os pulmões. A sintomatologia é inespecífica, abordando fadiga, inapetência, sudorese noturna, febre, perda ponderal e tosse com expectoração. O quadro clínico geralmente necessita do complemento de exames de imagem, principalmente a radiografia torácica para diagnóstic
