    Preliminaries for a new mathematical framework for modelling tumour growth using stress state decomposition technique

    The main goal of the present paper is to present a mathematical framework for modelling tumour growth based on stress state decomposition technique (SSDT). This is a straightforward extension of the model for multi-phase non-saturated soil consolidation with pollutant transport presented by the authors and may be regarded as an alternative to classical frameworks based on TCAT theory. In this preliminary work, the Representative Volume Element (RVE) for tumour is proposed along with its comparison with the corresponding one for soils modelling developed formerly by the authors. Equations standing for tumour phase are flawlessly brought into correspondence with those of gaseous phase in the soil problem showing that a similar task may be carried out for the remainders phases taking part in both RVEs. Furthermore, stresses induced by nonlinear saturation and permeability dependence on suction for soil interstitial fluids transport finds its counterpart on the contact between the cancer cell membrane and interstitial fluids rendering a higher primary variables coupling degree than what was attained in TCAT theory. From these preliminaries assessments, it may be put forward that likewise the stress state decomposition procedure stands for an alternative for modelling multi-phase nonsaturated soil consolidation with pollutant transport; it does for modelling cancer as well.Fil: Di Rado, Hector Ariel.     A selective technique for metaheuristic optimization of 3D truss structures

    In this paper, a multiobjective optimization tool based on genetic algorithms (GA) is proposed in order to analyze the structure design of 3D trussed. In this regard, an open source code (SciLab) GA-based was developed. The main aim of this code is the simultaneous optimization of three different objective functions based on a linear combination method, i.e. multiobjective optimization. The first function is the weight of the structure (which is directly related to its cost) and the second is the maximum displacement in the most unfavorable section. The remaining objective function corresponds to the minimization of the Euclidean norm of the stress state in order to get a homogeneous field of internal stresses in all structural sections. Before the evolutionary optimization process of the GA, a sensitivity analysis of the main variables is performed, in order to focus the study on the most sensitive variables.Fil: Mroginski, Javier Luis.     Digoxina: alternativas a la pauta de descanso

    Introduction: Digoxin is characterized by narrow therapeutic margin which makes its dosing difficult and monitoring of its serum levels necessary. This becomes more complex in patients with chronic nephropathy who require a greater dose adjustment. Development of the experience: We present the case of an 88-year-old woman admitted to our unit under treatment with oral digoxin for chronic heart failure with a regimen of 1 tablet daily with rest at weekends, who presented poor symptom control and subtherapeutic serum digoxin levels in controls. Changing the formulation to pediatric oral syrup (Lanacordin® 0.05mg/mL) resulted in better drug levels and symptom control. Conclusions: The use of digoxin in oral syrup may be an alternative to tablet formulation, especially in patients in whom drug level control may be complex due to their comorbidities.Introducción: La digoxina se caracteriza por estrecho margen terapéutico que hace dificultosa su dosificación y sea necesaria una monitorización de sus niveles séricos. Esto se hace más complejo en pacientes con nefropatía crónica que precisan de un mayor ajuste de dosis. Desarrollo de la experiencia: Presentamos el caso de una mujer de 88 años admitida en nuestra unidad en tratamiento con digoxina oral por insuficiencia cardiaca crónica con pauta de 1 comprimido diario con descanso los fines de semana, que presenta mal control de síntomas y niveles séricos de digoxina infraterapéuticos en controles. Al cambiar la formulación a jarabe oral pediátrico (Lanacordin® 0,05mg/mL) se consiguió un mejor nivel de los niveles del fármaco y del control de síntomas. Conclusiones: La utilización de la digoxina en jarabe oral puede ser una alternativa a la formulación con comprimidos, sobre todo en pacientes en los que el control de los niveles del fármaco puede resultar complejo por sus comorbilidades

    Spanish Paediatricians’ Knowledge, Attitudes and Practices Regarding Oral Health of Children under 6 Years of Age: A Cross-Sectional Study

    Background: Early Childhood Caries (ECC) is a prevalent chronic pathology, and it has a negative impact on the oral and general health of the child patient. Aim: To evaluate the knowledge, attitudes and practices of Spanish paediatricians regarding early childhood caries according to the professional’s years of experience. Material and Methods: A cross-sectional questionnaire was conducted by Spanish paediatricians via WhatsApp and e-mails from January to April 2021. Data were analysed using Chi-squared test, Fisher’s exact test and Cramer’s V test. Results: There were a total of 359 participants. Most respondents were women (81.3%) with up to 10 years of professional experience (31.2%) in primary health care and public health. In most cases, participants had an excellent knowledge of primary dentition (90.8%), but they ignored (56%) when the first visit to the dentist should occur. Regarding the aetiological factors of caries, oral hygiene and prevention, a lower rate of knowledge was observed. The majority of participants (80.8%) were not able to identify white spot lesions and enamel defects (76%). They considered that their knowledge in oral health was deficient, highlighting the need to increase their training. Less experienced paediatricians were found to have higher success rates. Conclusions: The level of knowledge and attitudes regarding early childhood caries of the evaluated paediatricians should be improved. Paediatricians had difficulties in identifying early caries lesions and enamel defects. Nevertheless, a higher level of knowledge and positive attitudes towards dental caries has been detected among paediatricians with fewer years of professional experience

    La supervisión en Trabajo Social es un proceso teórico-práctico fuertemente imbricado en el desarrollo la disciplina y que persigue incrementar las habilidades profesionales, las buenas prácticas y la calidad del trabajo con usuarios/as de los servicios de ayuda. Asimismo es una parte fundamental en el desarrollo de las prácticas pre-profesionales de los y las estudiantes de Trabajo Social que debe ayudarles no sólo a integrar conocimientos teóricos para comprender a la persona que precisa ayuda, sino que debe servir para desarrollar habilidades y competencias para ser más eficaces en la intervención. Paralelamente deben ayudarles desde la auto-reflexión a desvelar sus potencialidades y debilidades como personas en la construcción de la relación de ayuda. La supervisión académica de las prácticas pre-profesionales es un instrumento eficaz que contribuye a la eficacia de las prácticas de los y las estudiantes. Desde estas páginas proponemos un modelo de supervisión como proceso de ayuda que tiene como sujetos y actores no sólo a las y los estudiantes, sino a las personas usuarias de los servicios de ayuda

    La Escuela Politécnica Superior de la Universidad de Alicante, oferta un variado programa de formación de postgrado que se materializa en trece titulaciones de másteres (todas ellas relacionadas con la ingeniería y la arquitectura), que permiten al alumnado que finaliza sus estudios de Grado, completar su formación universitaria. En la actualidad, las titulaciones universitarias tienen que someterse a un exigente Sistema de Garantía de Calidad que es controlado y revisado de forma periódica para conseguir una mejora continuada en dichas titulaciones. Por este motivo, es necesaria la realización de diversos informes de seguimiento (que serán posteriormente evaluados) y en los cuáles se analizan diversas “evidencias” para determinar los niveles de calidad de la titulación. Una de dichas “evidencias” es la Tasa de abandono en la titulación. Para obtener unos buenos resultados, se persigue disponer de información y herramientas que contribuyan a disminuir significativamente las tasas de abandono en los estudios de másteres, hecho que repercutirá positivamente en la renovación de la acreditación del estudio de máster correspondiente. El objetivo, por tanto consistirá en realizar un diagnóstico de la situación e identificar las causas del abandono para la posterior realización de propuestas de mejora

    Delivery of Health Care by Spanish Dental Hygienists in Private and Public Dental Services during the COVID-19 De-Escalation Phase (June 2020): A Cross-Sectional Study

    Background: The first wave of the COVID-19 pandemic in Spain posed a major challenge for Spanish dental professionals. The objective of this work is to describe the dental hygienists’ work status and employment patterns during the de-escalation phase in order to analyse the standards of knowledge, compliance with official recommendations, and dental activities both in the public health service and in the private sector. Material and Methods: A cross-sectional questionnaire was answered by Spanish dental hygienists via WhatsApp, Facebook, and Instagram. The questionnaire was piloted before it was distributed and carried out during June 2020. Results: Here, 517 dental hygienists were surveyed, of which 86.2% followed the official recommendations to avoid contagion and 63.8% agreed with the gradual return to work by limiting the use of aerosols. Private dental hygienists identified more with returning to work without restrictions (14.5%) versus those working for the public service (1.2%) (p < 0.005). Conclusions: Dental hygienists’ return to work has involved different strategies, aimed at controlling infection and guaranteeing the safety of patients and the rest of the dental team. The availability of personal protective equipment, the adaptation of clinical infrastructure, and patient care management have differed between professionals working in the private and public sectors
