    Complications associated with submental liposuction:a scoping review

    Liposuction is one of the most commonly performed cosmetic procedures worldwide. Complications associated with submental liposuction are rare. However, when they occur they are significant and can cause disFiguring consequences. The objective of this study was evaluated complications from submentual liposuction in literature and description of clinical experience of complication after submentual liposuction. At first, a scoping review was carried out online search with no time restrictions for complications after submental liposuction was performed in the databases Medline / PubMed, Embase, and Web of Science. The variables analyzed were: age, sex, type of esthetic procedure, anesthesia, complications, time after Procedure, treatment, follow-up care, and sequelae. Then, a case of a patient with submental hematoma after an aesthetic procedure for submental liposuction was described. Firstly, 539 articles were selected, after application of the inclusion criteria, 4 studies were included. Most cases were female (8:1), with a mean age of 55.77 years. Postoperative complications were found, such as submental depression, submental edema, hypertrophic scar formation, scar contracture, cervical necrotizing fasciitis, Cervico-facial dystonia and transient facial nerve paralysis. The follow-up period for cases ranged from 3 to 12 months. The clinical case presented there was no sequelae. Submental liposuction requires the surgeon's attention. Anatomical knowledge, correct clinical and surgical management, diagnosis, and immediate approach to adverse situations are points that must be respected in this type of esthetic procedure to avoid more serious complications

    Association Of Anticardiolipin Antibody And C677t In Methylenetetrahydrofolate Reductase Mutation In Women With Recurrent Spontaneous Abortions: A New Path To Thrombophilia?

    Recurrent spontaneous abortion (RSA) has been associated with venous thrombosis in the mother. Acquired and inherited thrombophilia factors are possible causes. To evaluate the association between thrombogenic factors and recurrent spontaneous abortion. Case-control study. Centro de Atenção Integral à Saúde da Mulher, Universidade Estadual de Campinas. 40 ml of blood was collected from 88 women attending an RSA clinic and 88 fertile women attending a family planning clinic, to evaluate the presence of acquired and inherited thrombophilia factors. Anticardiolipin antibodies (ACA), lupus anticoagulant and deficiencies of proteins C and S and antithrombin III were evaluated by enzyme-linked immunosorbent assay (ELISA), dilute Russell Viper Venom time (dRVVT), coagulometric and chromogenic methods. DNA was amplified by the polymerase chain reaction (PCR) to study factor V Leiden and G20210A mutations in the prothrombin gene and C677T mutation in the methylenetetrahydrofolate reductase (MTHFR) gene. Data were analyzed using odds ratios and a regression model for age adjustment. Fishers exact test was used to evaluate statistical relationships between associated factors and RSA. ACA was detected in 11 women with RSA and one fertile woman. Heterozygous C677T was detected in 59 women with RSA and 35 fertile women. Concomitant presence of ACA and C677T was found in eight women with RSA and no fertile women (p < 0.01). The meaning of the association between C677T mutation in the MTHFR gene and ACA is still not clear. It is possible that an inherited factor that alone would not strongly predispose a woman to thrombosis could, when associated with an acquired factor, start the process and increase the likelihood of thrombosis expression. ACA and C677T in the MTHFR gene are statistically associated with RSA. The association of these two conditions is a new finding in thrombogenic factors and RSA.12315-2

    Cervical bacterial colonization in women with preterm labor or premature rupture of membranes

    PURPOSE: to study cervical colonization in women with preterm labor or premature rupture of membranes. METHODS: two hundred and twelve pregnant women with preterm labor or premature rupture of membranes were studied. Two cervical samples from each woman were collected and bacterioscopy and culture were performed. Association of cervical microorganisms and urinary tract infection, chorioamnionitis, fetal stress, antibiotic use, prematurity, neonatal infection, and neonatal death were evaluated. RESULTS: the prevalence of endocervical colonization was 14.2% (CI95%=9.5-18.9%), with similar results in preterm labor or premature rupture of membranes. Group B streptococcus was the most prevalent organism (9.4%). Other organisms isolated were Candida sp, Streptococcus sp, Streptococcus pneumoniae, Escherichia coli and Enterococcus sp. The most common findings of bacterioscopy were a reduced number of lactobacilli and a great number of leukocytes. Endocervical colonization was associated with a higher occurrence of urinary tract infection (23.8 versus 5.4%; p<0.01), early-onset neonatal infection (25.0 versus 7.3%; p<0.01) and neonatal mortality (two cases in colonized women; p<0.02) when compared with a negative culture of endocervical mucus. CONCLUSIONS: this study showed high prevalence of endocervical colonization despite the use of a nonselective culture media. The main microorganism isolated was group B streptococcus, but other organisms were present in one third of the studied population. More studies are needed to evaluate the influence of endocervical colonization on obstetrical outcome and on neonatal infection and mortality.OBJETIVO: estudar a colonização bacteriana do canal cervical em gestantes com trabalho de parto prematuro ou com ruptura prematura de membranas. MÉTODOS: foram avaliadas 212 gestantes com trabalho de parto prematuro ou ruptura prematura de membranas. Na admissão hospitalar foram coletadas duas amostras do conteúdo endocervical e realizadas bacterioscopia e cultura em meios ágar sangue e ágar chocolate. Foram analisadas associações da colonização endocervical com infecção do trato urinário materno, corioamnionite, utilização de antibióticos, sofrimento fetal, prematuridade e infecção e óbito neonatais. RESULTADOS: a prevalência de colonização endocervical foi 14,2% (IC95%=9,5-18,9%), com resultados similares entre os casos com trabalho de parto prematuro ou ruptura prematura de membranas. O microorganismo mais prevalente na população estudada foi o estreptococo do grupo B (9,4%), sendo também isolados Candida sp, Streptococcus sp, Streptococcus pneumoniae, Escherichia coli e Enterococcus sp. Das bacterioscopias analisadas, os achados mais freqüentes foram baixa prevalência de bacilos de Döderlein e elevado número de leucócitos. Em mulheres colonizadas, houve maior prevalência de infecção do trato urinário (23,8 versus 5,4%; p<0,01), infecção neonatal (25,0 versus 7,3%; p<0,01) e óbito neonatal (dois casos entre as colonizadas; p<0,02), quando comparadas às não colonizadas. CONCLUSÕES: observou-se alta prevalência de colonização endocervical, mesmo sem a utilização de meios de cultura seletivos. O estreptococo do grupo B foi o principal microorganismo isolado, reforçando a necessidade de triagem deste agente na gestação. Um terço das culturas positivas ocorreram por outros agentes. Estudos complementares são necessários para esclarecer a importância destes achados bacteriológicos no canal endocervical e sua associação com complicações gestacionais, sepse e mortalidade neonatais.39339

    [beta Thalassemia Major And Pregnancy During Adolescence: Report Of Two Cases].

    Beta thalassemia major is a rare hereditary blood disease in which impaired synthesis of beta globin chains causes severe anemia. Medical treatment consists of chronic blood transfusions and iron chelation. We describe two cases of adolescents with beta thalassemia major with unplanned pregnancies and late onset of prenatal care. One had worsening of anemia with increased transfusional requirement, fetal growth restriction, and placental senescence. The other was also diagnosed with hypothyroidism and low maternal weight, and was admitted twice during pregnancy due to dengue shock syndrome and influenza H1N1-associated respiratory infection. She also developed fetal growth restriction and underwent vaginal delivery at term complicated by uterine hypotonia. Both patients required blood transfusions after birth and chose medroxyprogesterone as a contraceptive method afterwards. This report highlights the importance of medical advice on contraceptive methods for these women and the role of a specialized prenatal follow-up in association with a hematologist.37291-29

    Traditional alimentary products: purchase habits and honey consumption

    Este estudo tem como objectivos conhecer o perfil do consumidor do mel e descrever os seus hábitos de consumo e compra. Foi elaborado um estudo quantitativo que teve como base a construção de um questionário, que foi aplicado a 172 indivíduos do Distrito de Bragança. Através da análise descritiva dos dados faz-se a caracterização da amostra e descrevem-se os hábitos de consumo e compra do mel. Participaram neste estudo 172 indivíduos, dos quais 41,9% são do sexo masculino e 58,1% são do sexo feminino. Verificou-se que a esmagadora maioria é consumidor de mel (94,8%), prefere o mel da região porque está mais disponível, é mais saboroso e tem qualidade (70,9%). A principal forma de utilização do mel é combinado com outros alimentos (60,5%), o local preferido para a sua compra é junto do produtor (51,7%) e o factor que mais pesa na decisão de compra é o paladar (52,3%). To determine the honey consumer’s profile and to describe your purchase and consume habits are the objectives of this study. A cross-sectional survey was carried out in a probabilistic sample including 172 individuals of the Braganza region population. After, data descriptive analyses is done characterization of the sample and are described purchase and consume habits of the inquiry. In this study participated 172 individuals, of which 41,9% are male and 58,1% are female. It was verified that the smashing majority is honey consumer (94,8%), they prefer the national honey because it is more available, it is tastier, it is quality and it is trustworthy (70,9%). The main form honey use is mixed with other foods (60,5%), the favorite place for the honey purchase is near the producer (51,7%) and the factor that more it weighs in the purchase decision this product is the palate (52,3%)

    Calcified abdominal pregnancy with eighteen years of evolution: case report

    CONTEXT: The lithopedion (calcified abdominal pregnancy) is a rare phenomenon and there are less than 300 cases reported in the medical literature. CASE REPORT: In this case, a 40 year-old patient had had her only pregnancy 18 years earlier, without medical assistance since then. She came to our hospital with pain and tumoral mass of approximately 20 centimeters in diameter. Complementary examinations (abdominal X-ray, ultrasonography and computerized tomography) demonstrated an extra-uterine abdominal 31-week pregnancy with calcification areas. Exploratory laparotomy was performed, with extirpation of a well-conserved fetus with partially calcified ovular membranes.CONTEXTO: O Litopédio (gestação abdominal calcificada) é um fenômeno raro, com menos de 300 casos descritos na literatura. RELATO DE CASO: Uma paciente de 40 anos, teve uma única gestação há 18 anos e não procurou serviço médico desde então. Chegou com dor em baixo ventre e massa tumoral de aproximadamente 20 centímetros. Os exames complementares (radiografia abdominal, ecografia e tomografia computadorizada) evidenciaram gestação de aproximadamente 31 semanas extra-uterina e com áreas de calcificação. Foi realizada laparotomia com extração do feto em bom estado de conservação, envolvido pela membrana parcialmente calcificada.19219
