Les infections bactériennes, fongiques ou virales sont des complications fréquentes au cours des procédures de greffes de cellules souches hématopoïétiques, aussi bien au cours des autogreffes qu’au cours des allogreffes en hématologie. Ces infections, en particulier les sepsis ou les infections fongiques, telles que les aspergilloses invasives,sont responsables d’un taux élevé de décès durant les phases d’aplasie post conditionnement, en particulier myéloablatif. Afin de diminuer la morbi-mortalité des infections, l’introduction du traitement préventif des infections a permis de réduire de façon significative le taux de mortalité lié à la procédure.
Par ailleurs, le groupe coopératif européen des greffes EBMT a mis en place un organisme qui permet d’accréditer les activités des centres de greffe au vu des protocoles et des procédures qu’ils ont mis en place

Gemcitabine and treatment of diffuse large B-cell lymphoma in relapsed or refractory elderly patients: A prospective randomized trial in Algeria
Context: Support for non-Hodgkin\u2032s lymphoma (NHL) with large
cells that is refractory or relapsed after first-line chemotherapy
poses a greater therapeutic problem with bone marrow transplant therapy
or when old age is a contra-indication for high-dose chemotherapy,
especially among developing countries such as Algeria. Aim: To show
that the regimen, including gemcitabine, could be more effective in
treating elderly patients with diffuse large B-cell lymphoma (DLBCL) in
relapse / refractory, without complete remission, when compared with
the ESHAP (etoposide, cisplatine, solumedrol, aracytine) regimen.
Materials and Methods: Ninety-six patients in the age group of 60-70
years were volunteers for a prospective randomized single-blind study,
carried out for three years. Patients were divided into two groups by
the drawing of lots. The first group (GA, n = 48, relapse; n = 27
[56.3%], refractory; n = 21 [43.7%]) received treatment with ESHAP
protocol and the second one (GB, n = 48, relapse; n = 28 [58%],
refractory; n = 20 [42%]) with GPD (gemcitabine, dexamethasone,
cisplatine) protocol. Results: The overall response rates and mean
survival at three years were significantly higher among patients
subjected to GPD treatment compared with those subjected to ESHAP
treatment (63% vs. 55%, P = 0.01 and 20.5% [95% CI 16.5-24.5] vs. 11.8%
[8.9-14.6], respectively). Additionally, three-year progression-free
and event-free survival rates were 20.5% (16.3-24) and 19.7%
(15.9-23.5), respectively, for the GPD regimen and 10.9% (8.2-13.7) and
11.1% (95% CI 8.5-13.7), respectively, for the ESHAP regimen. Moreover,
the GPD regimen was associated with improving overall survival
(RR=2.02, 95% CI 1.59-2.56; P = 0.000), event-free survival (2.03,
1.64-2.52; P < 0.001) and progression-free survival (1.86,
1.46-2.37; P < 0.001). Conclusion: In cases of contra-indication for
high-dose chemotherapy for elderly patients with DLBCL, without
complete remission, the Gemcitabine-based therapy protocol represents a
more effective and less toxic than that of ESHAP
The incidence of acute myeloid leukemia in Calgary, Alberta, Canada: a retrospective cohort study
Abstract Background The incidence rate of acute myeloid leukemia (AML) was determined in the Calgary Metropolitan Area, a major Canadian city. Methods Data from all patients diagnosed with AML between January 1, 2011 and December 31, 2015 were retrieved from a single, centralized cancer cytogenetics laboratory for bone marrow samples, the sole diagnostic facility of its kind in Southern Alberta. Results The calculated incidence rate was 2.79 cases per 100,000 person-years with a median age of 60, slightly lower than previously published data. The age-standardized incidence rate for Canada was 3.46 cases per 100,000 person-years. The higher value is reflective of Calgary’s younger population compared to the rest of Canada. Higher male incidence and greatest incidence occurring at approximately the age of 85 is similar to data from other developed countries. The lower incidence rates and median age of diagnosis, in comparison with that of other high-income nations, may be due to differences in the proportion of aging citizens in the population. Conclusion This is the first published incidence rate of acute myeloid leukemia (AML) in Canada across all age groups

The results of the present prospective, noninterventional observational study revealed great diversity in the treatment regimens used to treat MM. Our results also provide data to inform health economic, pharmacoepidemiologic, and outcomes research, providing a framework for the design of protocols to improve the outcomes of patients with MM. Background: The present prospective, multinational, noninterventional study aimed to document and describe real-world treatment regimens and disease progression in multiple myeloma (MM) patients. Patients and Methods: Adult patients initiating any new MM therapy from October 2010 to October 2012 were eligible. A multistage patient/site recruitment model was applied to minimize the selection bias; enrollment was stratified by country, region, and practice type. The patient medical and disease features, treatment history, and remission status were recorded at baseline, and prospective data on treatment, efficacy, and safety were collected electronically every 3 months. Results: A total of 2358 patients were enrolled. Of these patients, 775 and 1583 did and did not undergo stem cell transplantation (SCT) at any time during treatment, respectively. Of the patients in the SCT and non-SCT groups, 49%, 21%, 14%, and 15% and 57%, 20%, 12% and 10% were enrolled at treatment line 1, 2, 3, and ≥ 4, respectively. In the SCT and non-SCT groups, 45% and 54% of the patients had received bortezomib-based therapy without thalidomide/lenalidomide, 12% and 18% had received thalidomide/lenalidomide-based therapy without bortezomib, and 30% and 4% had received bortezomib plus thalidomide/lenalidomide-based therapy as frontline treatment, respectively. The corresponding proportions of SCT and non-SCT patients in lines 2, 3, and ≥ 4 were 45% and 37%, 30% and 37%, and 12% and 3%, 33% and 27%, 35% and 32%, and 8% and 2%, and 27% and 27%, 27% and 23%, and 6% and 4%, respectively. In the SCT and non-SCT patients, the overall response rate was 86% to 97% and 64% to 85% in line 1, 74% to 78% and 59% to 68% in line 2, 55% to 83% and 48% to 60% in line 3, and 49% to 65% and 36% and 45% in line 4, respectively, for regimens that included bortezomib and/or thalidomide/lenalidomide. Conclusion: The results of our prospective study have revealed great diversity in the treatment regimens used to manage MM in real-life practice. This diversity was linked to factors such as novel agent accessibility and evolving treatment recommendations. Bone marrow graft versus peripheral blood graft in haploidentical hematopoietic stem cells transplantation: a retrospective analysis in1344 patients of SFGM-TC registry.
peer reviewedThe use of peripheral blood (PB) or bone marrow (BM) stem cells graft in haploidentical hematopoietic stem cell transplantation with post-transplant cyclophosphamide (PTCy) for graft-versus-host disease (GVHD) prophylaxis remains controversial. Moreover, the value of adding anti-thymoglobulin (ATG) to PTCy is unknown. A total of 1344 adult patients received an unmanipulated haploidentical transplant at 37 centers from 2012 to 2019 for hematologic malignancy. We compared the outcomes of patients according to the type of graft, using a propensity score analysis. In total population, grade II-IV and III-IV acute GVHD (aGVHD) were lower with BM than with PB. Grade III-IV aGVHD was lower with BM than with PB + ATG. All outcomes were similar in PB and PB + ATG groups. Then, in total population, adding ATG does not benefit the procedure. In acute leukemia, myelodysplastic syndrome and myeloproliferative syndrome (AL-MDS-MPS) subgroup receiving non-myeloablative conditioning, risk of relapse was twice greater with BM than with PB (51 vs. 22%, respectively). Conversely, risk of aGVHD was greater with PB (38% for aGVHD II-IV; 16% for aGVHD III-IV) than with BM (28% for aGVHD II-IV; 8% for aGVHD III-IV). In this subgroup with intensified conditioning regimen, risk of relapse became similar with PB and BM but risk of aGVHD III-IV remained higher with PB than with BM graft (HR = 2.0; range [1.17-3.43], p = 0.012)

Narrowing the gap for hematopoietic stem cell transplantation in the East-Mediterranean/African region: comparison with global HSCT indications and trends.
Hematopoietic Stem Cell Transplantation (HSCT) activity was evaluated in the African (AFR)/EMRO region and compared to the global activity for the years 2006-2013. Data were obtained from 1570 teams in the 6 WHO continental regions. Of these, 29 (1.85%) of all teams were active in 12 of the 68 AFR/EMRO countries. They reported 2.331 (3.3%) of the worldwide 71.036 HSCT, and a transplant rate of 32.8 (TR; HSCT/10 million inhabitants; worldwide 128.5). This reflects still the lowest regional TR despite an increase of 90% since 2006. HSCT activity in AFR/EMRO countries was characterized by a higher use of allogeneic compared to autologous HSCT, an almost exclusive use of family donors, including haploidentical family donors. These findings contrast with the prevalence of autologous over allogeneic HSCT, and a higher frequency of unrelated HSCT in other parts of the world. Of note, the increase by 200% in HSCT for hemoglobinopathies from 2006 to 2013 (72 per year) in the AFR/EMRO region. This reflects the specific role of HSCT for these disease categories with high prevalence and incidence in the AFR/EMRO region. This report provides information for the competent authorities to foster adequate infrastructure. It urges transplant organization to optimize their cooperation
