    Simplex space-time meshes in thermally coupled two-phase flow simulations of mold filling

    The quality of plastic parts produced through injection molding depends on many factors. Especially during the filling stage, defects such as weld lines, burrs, or insufficient filling can occur. Numerical methods need to be employed to improve product quality by means of predicting and simulating the injection molding process. In the current work, a highly viscous incompressible non-isothermal two-phase flow is simulated, which takes place during the cavity filling. The injected melt exhibits a shear-thinning behavior, which is described by the Carreau-WLF model. Besides that, a novel discretization method is used in the context of 4D simplex space-time grids [2]. This method allows for local temporal refinement in the vicinity of, e.g., the evolving front of the melt [10]. Utilizing such an adaptive refinement can lead to locally improved numerical accuracy while maintaining the highest possible computational efficiency in the remaining of the domain. For demonstration purposes, a set of 2D and 3D benchmark cases, that involve the filling of various cavities with a distributor, are presented.Comment: 14 pages, 11 Figures, 4 Table

    Prognostic value of immune cell infiltration, tertiary lymphoid structures and PD-L1 expression in Merkel cell carcinomas

    Merkel cell carcinoma (MCC) is an aggressive, virus-associated, neuroendocrine tumor of the skin mainly affecting immunocompromised patients. Higher intratumoral infiltration with CD3 and CD8 positive T-cells is associated with a better prognosis, highlighting the relevance of the immune system for MCC development and progression. In this study 21 primary MCCs were stained with immune cell markers including CD3, CD4, CD8, CD68, CD20, and S100. Furthermore, tumor-infiltrating neutrophils, tertiary lymphoid structures and PD-L1 expression were analyzed and correlated with overall and recurrence free survival. All MCCs were Merkel Cell Polyomavirus positive. Overall and recurrence-free survival did not correlate with intra-and peritumoral CD3 and CD8 T-cell infiltration. In addition, no significant association regarding prognosis was found for tumor-associated neutrophils, tumor-associated macrophages or PD-L1 positivity in MCCs. Interestingly, the presence of tertiary lymphoid structures (TLS) in the tumor microenvironment significantly correlated with recurrence-free survival (P=0.025). In addition, TLS were significantly associated with a higher CD8/CD4 ratio in the tumor periphery (P=0.032), but not in the center of the tumor (P > 0.999). These results demonstrate for the first time that TLS, easily assessed in paraffin-embedded tissue in the tumor periphery of MCCs, may be a valuable prognostic factor indicating prolonged recurrence free survival

    High prevalence of falsely declaring nicotine abstinence in lung transplant candidates

    Tobacco use after lung transplantation is associated with adverse outcome. Therefore, active smoking is regarded as a contraindication for lung transplantation and should be excluded prior to placement on the waiting list. The aim of the study was to compare self-reporting with a systematic cotinine based screening approach to identify patients with active nicotine abuse. Nicotine use was systematically assessed by interviews and cotinine test in all lung transplant candidates at every visit in our center. Patients were classified according to the stage prior to transplantation and cotinine test results were compared to self-reports and retrospectively analyzed until June 2019. Of 620 lung transplant candidates, 92 patients (14.8%) had at least one positive cotinine test. COPD as underlying disease (OR 2.102, CI 1.110-3.981;p = 0.023), number of pack years (OR 1.014, CI 1.000-1.028;p = 0.047) and a time of cessation less than one year (OR 2.413, CI 1.410-4.128;p = 0.001) were associated with a positive cotinine test in multivariable regression analysis. The majority of non-COPD patients (n = 13, 72.2%) with a positive test had a cessation time of less than one year. 78 patients (84.7%) falsely declared not consuming any nicotine-based products prior to the test. Finally, all never smokers were test negative. In conclusion, our data demonstrate that active nicotine use is prevalent in transplant candidates with a high prevalence of falsely declaring nicotine abstinence. COPD was the main diagnosis in affected patients. Short cessation time and a high number of pack years are risk factors for continued nicotine abuse

    Case report: Central venous catheter thrombosis complicated by chronic thromboembolic disease/pulmonary hypertension in two children requiring parenteral nutrition

    Chronic thromboembolic pulmonary hypertension is a rare but life-threatening complication of long-term central venous catheters (CVC) in children. However, evidence in terms of potential treatment strategies and outcome data remains scarce. We describe two cases of CVC-related thrombosis (Hickman-catheter) complicated by recurrent pulmonary emboli. One patient experienced a complete thromboembolic obstruction of the right pulmonary artery with normal pulmonary pressures and the second patient suffered from a central thromboembolic obstruction of both pulmonary arteries associated with severe pulmonary hypertension. Both patients successfully underwent surgical thromboendarterectomy with deep hypothermic circulatory arrest

    Increased expression of AP2 and Sp1 transcription factors in human thyroid tumors: a role in NIS expression regulation?

    BACKGROUND: Sodium/iodide symporter (NIS) is a key protein in iodide transport by thyroid cells and this activity is a prerequisite for effective radioiodide treatment of thyroid cancer. In the majority of thyroid cancers, however, iodide uptake is reduced, probably as a result of decreased NIS protein expression. METHODS: To identify the mechanisms that negatively affect NIS expression in thyroid tumors, we performed electrophoresis mobility shift assays and immunoblot analysis of nuclear protein extracts from normal and tumoral thyroid tissues from 14 unrelated patients. RESULTS: Two proteins closely related to the transcription factors AP2 and Sp1 were identified in the nuclear extracts. Expression of both AP2 and Sp1 in nuclear extracts from thyroid tumors was significantly higher than that observed in corresponding normal tissues. CONCLUSION: These observations raise the possibility that NIS expression, and subsequently iodide transport, are reduced in thyroid tumors at least in part owing to alterations in the binding activity of AP2 and Sp1 transcription factors to NIS promoter

    Earthquakes in Switzerland and surrounding regions during 2013

    This report of the Swiss Seismological Service summarizes the seismic activity in Switzerland and surrounding regions during 2013. During this period, 699 earthquakes and 208 quarry blasts were detected and located in the region under consideration. With a total of 18 events with local magnitudes ML ≥ 2.5, the seismic activity in the year 2013 was slightly below the average over the previous 38years. Most noteworthy were the two earthquake sequences of St. Gallen (SG) in July and Balzers (FL) in December. The former was induced by reservoir stimulation operations at the St. Gallen geothermal project. The maximum local magnitude in the sequence was 3.5, comparable in size with the ML 3.4 event induced by stimulation operations below Basel in 2006. The sequence of Balzers was associated with an ML 4.1 earthquake in the border region to Liechtenstein. More than 30 aftershocks with magnitudes ranging between ML −0.2 and ML 3.7 were detected in the month following the mainshock. The ML 3.5 St. Gallen and the ML 4.1 Balzers earthquakes were widely felt by the public but no reports on damages are known. The maximum intensity for both events was IV
