    Holter STAT-ON™ against other tools for detecting MF in advanced Parkinson’s disease: an observational study

    BackgroundDifferent screening tools to identify advanced Parkinson’s disease (APD) have emerged in recent years. Among them, wearable medical devices, such as STAT-ON™, have been proposed to help to objectively detect APD.ObjectivesTo analyze the correlation between STAT-ON™ reports and other assessment tools to identify APD and to assess the accuracy of screening tools in APD patients, using the STAT-ON™ as the gold standard.MethodsIn this retrospective, observational study, data from the University Hospital Complex of Pontevedra database on 44 patients with potential APD who wore STAT-ON™ were extracted. Data were collected according to different sources of tools for identifying APD: (1) STAT-ON™, (2) information provided by the patient, (3) questionnaire for advanced Parkinson’s disease (CDEPA), (4) 5-2-1 Criteria, and (5) Making Informed Decisions to Aid Timely Management of Parkinson’s Disease (MANAGE-PD). Considering STAT-ON™ recordings as a reference, the sensitivity, specificity, and positive and negative predictive values for each tool were calculated. The kappa index assessed the degree of agreement between the gold standard and the other instruments.ResultsAlthough no statistically significant association was found between STAT-ON™ recordings and any screening methods evaluated, the CDEPA questionnaire demonstrated the highest sensitivity and VPN values to detect patients with APD candidates for second-line therapy (SLT). According to the correlation analyses, MANAGE-PD demonstrated the highest degree of concordance with STAT-ON™ recordings to identify the SLT indication and to predict the SLT decision.ConclusionSTAT-ON™ device may be a helpful tool to detect APD and to guide treatment decisions

    Sequential oral antibiotic in uncomplicated Staphylococcus aureus bacteraemia: a propensity-matched cohort analysis

    Objectives: We aimed to analyse the efficacy and safety of oral sequential therapy (OST) in uncomplicated Staphylococcus aureus bacteraemia (SAB). Methods: Single-centre observational cohort at a tertiary hospital in Spain, including all patients with the first SAB episode from January 2015 to December 2020. We excluded patients with complicated SAB and those who died during the first week. Patients were classified into the OST group (patients who received oral therapy after initial intravenous antibiotic therapy [IVT]), and IVT group (patients who received exclusively IVT). We performed a propensity-score matching to balance baseline differences. The primary composite endpoint was 90-day mortality or microbiological failure. Secondary endpoints included 90-day SAB relapse. Results: Out of 407 SAB first episodes, 230 (56.5%) were included. Of these, 112 (n = 48.7%) received OST and 118 (51.3%) IVT exclusively. Transition to oral therapy was performed after 7 days (interquartile range, 4–11). The primary endpoint occurred in 10.7% (11/112) in OST vs. 30.5% (36/118) in IVT (p < 0.001). SAB relapses occurred in 3.6% (4/112) vs. 1.7% (2/118) (p 0.436). None of the deaths in OST were related to SAB or its complications. After propensity-score matching, the primary endpoint was not more frequent in the OST group (relative risk, 0.42; 95% CI, 0.22–0.79). Ninety-day relapses occurred similarly in both groups (relative risk, 1.35; 95% CI, 0.75–2.39). Discussion: After an initial intravenous antibiotic, patients with uncomplicated SAB can safely be switched to oral antibiotics without apparent adverse outcomes. This strategy could save costs and complications of prolonged hospital stays. Prospective randomized studies are needed

We assessed their early and long-term dynamics after initiation of antiretroviral treatment (ART).[Methods] Persons with HIV-1 (PWH) aged>18 years starting their first ART in 2015-2021 in a prospective cohort (n=73) were included. IP-10 and MIG plasma levels were quantified using a multiplexed bead-based assay.[Results] IP-10 and MIG plasma levels showed a significant and consistent reduction following ART (80% integrase inhibitor [INSTI]-based) initiation, starting at day 20 and maintained throughout the study period (48 months), paralleling the HIV-1 RNA decay and CD4+ count recovery (p<0·001). At baseline, PWH≥ 50 years, CDC stage C and CD4+ count<350cells/mm3 had higher levels of IP-10 (p=0·022, p=0·001 and p=0·002, respectively) and MIG (p<0·001, p=0·024 and p=0·069, respectively). All of them matched their counterparts several months following ART initiation. MIG levels showed a greater decrease at day 10 in those treated with INSTI (p=0·038). Low-level HIV-1 viremia did not impact MIG or IP-10 levels.[Conclusion] Plasma IP-10 and MIG showed an early significant decline following ART initiation, with greater early declines in MIG levels in INSTI-based regimens.     School coexistence through values training supported by information and communication technologies

    En Colombia existe una preocupación en las instituciones educativas por los altos índices de violencia escolar, por lo cual se hace necesario impartir una formación en valores que permitan al estudiante tomar decisiones asertivas al enfrentarse a una situación conflictiva, fortaleciendo así la convivencia escolar. El estudio busco promover la convivencia escolar mediante la formación en valores apoyada en las tecnologías de la información y comunicación. Está guiado bajo los lineamientos metodológicos de la IEP con un diseño basado en las trayectorias de indagación, la unidad de análisis se conformó por sesenta (60) estudiantes de la institución educativa Thelma Rosa de Arévalo ubicada en el municipio de Zona bananera Magdalena. Los resultados dejaron en evidencia la importancia de implementar nuevas estrategias pedagógicas al aula de clase, observando un impacto positivo en la convivencia escolar de los estudiantes mediante la formación en valores apoyada en el uso de las TIC.In Colombia there is a concern in educational institutions for the high rates of school violence, which is why it is necessary to provide training in values that allow the student to make assertive decisions when facing a conflictive situation, thus strengthening school coexistence. The study sought to promote school coexistence through training in values supported by information and communication technologies. It is guided under the methodological guidelines of the IEP with a design based on the trajectories of inquiry, the analysis unit was formed by sixty (60) students from the educational institution Thelma Rosa de Arévalo located in the municipality of Magdalena banana zone. The results showed the importance of implementing new pedagogical strategies to the classroom, observing a positive impact on students’ school coexistence through the formation of values supported by the use of TIC

    Améliorer l'auto-efficacité et l'activation des patients chez les survivants du cancer grâce à des interventions de santé numérique: résultats du projet PERSIST (préimpression)

    Cancer survivors face various challenges but also demonstrate resilience and find ways to adapt and cope with life after cancer. Self-efficacy and patient activation are two crucial factors that significantly impact the well-being of cancer survivors. These concepts play a vital role in enabling cancer survivors to take control of their health, manage their treatment effectively, and achieve positive long-term outcomes. The aim of this study is to assess the impact of a mobile health system (mHealthApp) to improve the self-efficacy and patient activation of breast cancer and colorectal cancer survivors. This study presents the findings from clinical trials conducted according to the published study protocol of the PERSIST project that was funded by European Commision to support cancer survivors using digital health technologies. The acceptability and usability of the mHealthApp, as well as the perceived self-efficacy and satisfaction with care, were assessed using validated tools such as CASE-cancer, PAM, and SUS. The results indicate that the PERSIST project partially achieved its predefined objectives and hypotheses by enhancing the self-confidence and satisfaction of cancer survivors with healthcare and improving the effectiveness of cancer treatment and follow-up procedures to some extent. The PERSIST project demonstrates the potential to improve clinical outcomes, empower patients, and contribute to broader social goals in cancer survivorship. However, larger studies involving a more diverse patient population and a greater number of clinicians are necessary to establish the effectiveness of digital therapies in cancer survivorship care and to provide additional data and evidence.Patients-centered SurvivorShIp care plan after Cancer treatments based on Big Data and Artificial Intelligence technologie
