    Five year retrospective study on Syphilis in the Sexual Transmitted Disease (STD) centre of the teaching hospital Umberto I in Rome

    Objectives: A retrospective study describing syphilis epidemiological and clinical features in patients referring to an infectious diseases centre in Rome, Italy. Methods: Between January 2011 and December 2015 demographic, behavioral and clinical data were collected from all adult patients attending the Sexual Transmitted Disease Centre of the Teaching Hospital Umberto I in Rome. Results: Overall 723 patients, 495 males and 228 females, with syphilis infection diagnosis were included. Average age 39.6 ± 13.6 years (median 38) was higher in men than women (41.1 ± 13.6 vs. 36.3 ± 13.1; p<0.001). Patients were from Italy (486 or 67.2%), EU (90 or 12.4%), rest of Europe (38 or 5.3 %), Americas (46 or 6.4%), Africa (36 or 5.0%) and Asia (27 or 3.7%). One-hundred-twenty-three (17.0%) presented primary syphilis, 43 (5.9%) secondary syphilis, 8 (1.1%) tertiary syphilis, 246 (34.0%) serological syphilis, 80 (11.1%) preceding syphilis, 56 (7.7%) gravidic syphilis and 167 (23.1%) came to the STD to control a preceding syphilis treatment. Fifty-six (24.6%) women were diagnosed with syphilis during their pregnancies. Among Chinese female patients, those pregnant represented 87.5%. There were 100 subjects (13.8%) simultaneously HIV+ and 623 (86.2%) HIV- patients. HIV co-infection affected more frequently men (RR 5.30; CI 2.62 – 10.72; p<0.001). In males HIV co-infection affected more frequently homosexuals (RR 11.72; CI 6.72 – 20.45; p<0.001). Overall HIV co-infection affected more frequently foreign patients, specially from the Americas (26.1%), Africa (25.7%) and Asia (22.2%). Conclusions: A serious problem of “gravidic syphilis” suggests the need for Public Health preventive action. Also an early diagnosis of both syphilis and HIV infection should be reinforced

    Abnormal temporal coupling of tactile perception and motor action in Parkinson's disease

    Evidence shows altered somatosensory temporal discrimination threshold (STDT) in Parkinson's disease in comparison to normal subjects. In healthy subjects, movement execution modulates STDT values through mechanisms of sensory gating. We investigated whether STDT modulation during movement execution in patients with Parkinson's disease differs from that in healthy subjects. In 24 patients with Parkinson's disease and 20 healthy subjects, we tested STDT at baseline and during index finger abductions (at movement onset "0", 100, and 200 ms thereafter). We also recorded kinematic features of index finger abductions. Fifteen out of the 24 patients were also tested ON medication. In healthy subjects, STDT increased significantly at 0, 100, and 200 ms after movement onset, whereas in patients with Parkinson's disease in OFF therapy, it increased significantly at 0 and 100 ms but returned to baseline values at 200 ms. When patients were tested ON therapy, STDT during index finger abductions increased significantly, with a time course similar to that of healthy subjects. Differently from healthy subjects, in patients with Parkinson's disease, the mean velocity of the finger abductions decreased according to the time lapse between movement onset and the delivery of the paired electrical stimuli for testing somatosensory temporal discrimination. In conclusion, patients with Parkinson's disease show abnormalities in the temporal coupling between tactile information and motor outflow. Our study provides first evidence that altered temporal processing of sensory information play a role in the pathophysiology of motor symptoms in Parkinson's disease

    Electronic cigarette: a threat or an opportunity for public health? State of the art and future perspectives

    The e-cigarette, also known as e-cig, represents an emerging issue of great concern for public health. The aim of the present report was to explore the scientific literature about the use of electronic cigarette (e-cig), with a particular reference to the features of “toxicological safety”, “effectiveness in overcoming the addiction to smoking the traditional cigarette” and “necessary research agenda”. The efficacy of e-cig for smoking cessation is uncertain: some authors found that it can be a valid support, but long-term cessation rate has not be assessed. Other studies evidenced that e-cig is often used not for quitting smoking but to avoid smoking ban for traditional cigarettes and, even, some researches evidenced that it appears to contribute to nicotine addiction. E-cig smoking seems to be less dangerous of conventional cigarettes, but its use is not risk-free. Besides, cases of accidental or intentional poisoning with liquid solutions of e-cig have been reported. Also, the smoke of e-cig decreases indoor air quality, releasing particulate matter and other toxics that can persist on surfaces for days and generating passive exposure. These phenomena are similar to environmental tobacco smoke produced by conventional smoking, that is the sum of second- and third-hand smoke. We propose to call them “environmental electronic smoke”, “electronic-second- hand smoke” and “electronicthird-hand smoke”, respectively. Uncertainties relating to e-cig features determined the sequence, in the short term, of warnings and regulations approved and then replaced. In conclusion, although in recent years many researches were performed, evidences is limited and there is a need to study in deep all these issues

    Biomechanics in crutch assisted walking

    Crutch-assisted walking is very common among patients with a temporary or permanent impairment affecting lower limb biomechanics. Correct crutches’ handling is the way to avoid undesired side effects in lower limbs recovery or, in chronic users, upper limbs joints diseases. Active exoskeletons for spinal cord injured patients are commonly crutch assisted. In such cases, in which upper limbs must be preserved, specific training in crutch use is mandatory. A walking test setup was prepared to monitor healthy volunteers during crunch use as a first step. Measurements were performed by using both a motion capture system and instrumented crutches measuring load distribution. In this paper, we present preliminary tests results based on different subjects - having a variety of anthropometrical characteristics - during walking with parallel or alternate crutches, the so-called three and two-points strategies. Tests results present inter and intra subject variabilities and, as a first goal, influencing factors affecting crutch loads have been identified. In the future we aim to address crutch use errors that could lead to delayed recovery or upper limbs suffering in patients, giving valuable information to physicians and therapists to improve user’s training

    Effect of boulder shape on the response of compound meshes subject to dynamic impacts

    Rockfall is a type of natural hazard associated with the detachment of one or several boulders in steep slopes. Passive risk mitigation strategies are based on intercepting these blocks during their movement, using rigid barriers, embankments, and flexible protection systems. In recent years, the advancement of remote sensing techniques based on discrete fracture networks allows the characterisation of the shape and size of these boulders even before their detachment [13]. However, physical and numerical modelling of the impact on flexible protection system typically considers spheres [4] and truncated cubes [3] as boulder shape. In this work, the local, i.e. bullet effect [5] and full-scale effect of the aspect ratio of the block is investigated during its impact with a full-scale barrier model. The barrier is characterised by a compound mesh, formed by interweaved double-twisted hexagons and strand ropes stretched between two fence posts. The mesh geometry is reproduced within the Discrete Element framework, using the remote contact interaction approach and the fast mesh generation technique described in [11]

    MINERALOGICAL STUDIES OF THE W-Sn VEIN SKARNS OF MONTE TAMARA (NUXIS, SULCIS DISTRICT): INSIGHTS FOR STRATEGIC MINERALS EXPLORATION IN SW SARDINIA (ITALY).

    Skarn deposits are a relevant source of critical raw materials such as W, Sn, and In. Recent studies conducted in South Sardinia pointed out the relationships between various Sn-W-Mo deposits and the early Permian (289-286 Ma) F-bearing, ilmenite-series ferroan granites (e.g., Sulcis pluton). This new evidence triggered a broad re-examination of granite-related deposits including skarn deposits hosted by Cambrian limestones of the low-grade Variscan basement of the Sulcis district (SW Sardinia). With this purpose, field investigations and OM, SEM-EDS, EMPA, and LA-ICP-MS observations, and analyses have been conducted on the skarn ores of Monte Tamara (Nuxis, northern Sulcis) where scheelite has been reported in the old San Pietro and Sinibidraxiu mines. The San Pietro mine exploited a 1-5 m thick and 70 m deep, steeply dipping skarn orebody located at the tectonized contact between early Cambrian sandstones and limestones. The orebody includes layers of Grt-Cpx-Wo, magnetite, and Zn-Pb-Cu-Fe sulfide bands. Prograde and retrograde stages with oxides and sulfides can be recognized. Clinopyroxene is the foremost mineral of the prograde stage; garnets (andradite-grossular) are usually dark green with typical anomalous birefringence and distinctly zoned (Fe-rich cores and Al-rich rims). Hematite turned to mushketovite, and Mo-rich scheelite, followed by In-bearing cassiterite, occasionally occur in the prograde assemblages. Amphiboles and epidotes mark the retrograde stage, together with abundant Zn-Cu-Fe-Pb sulfides and accessory molybdenite, stannite, bismuthinite, and Bi-Ag-Pb sulfosalts. At San Pietro, dominant sphalerite displays highly variable Fe, Mn, and Cd contents. Relictlooking blebs of Fe-Mn-poor Sp are scattered in high-Fe-Mn Sp where Sn EMPA peaks may correlate with cassiterite-stannite micro-inclusions. Galena composition suggests localized intergrowths with micro-inclusions of bismuthinite, Bi-Se, and Bi-Te sulfosalts. The stannite-sphalerite geothermometer provided a temperature range of 325-200°C for the sulfide stage. The Sinibidraxiu old mine exploited a 1,5 m thick and 60 m deep columnar body, hosted in early Cambrian marbles. It consists of a sphalerite-wollastonite assemblage with late sulfides, quartz, and calcite, hosting cm-sized arsenopyrite and scheelite. Scheelite is Mo-poor; Sn-, other Mo-phases and Bi-phases are absent. High-Fe Sp, rimmed by low-Fe Sp and blebby galena, is finely intergrown with wollastonite cockades. The results from this study suggest that a wide range of skarn-related mineralizing phenomena occurred in the Monte Tamara area. Both orebodies resulted from a structurally controlled migration of metasomatic fluids inside the hosting carbonate formation. Mineral zonation and composition of the San Pietro skarn point towards skarn development under varying fO2 conditions, oxidizing then rapidly turning to moderately reducing within the prograde W-Sn skarn stage and into the sulfide stage. The features of the Sinibidraxiu orebody (e.g., Mo-poor, As-devoid scheelite) suggest a formation from reducing metasomatic fluids but S-poor compared to San Pietro, probably at more distal environments (e.g. low Sn-Bi contents). From this point of view, the Monte Tamara area still maintains an economic potential, linked to the possible presence of proximal skarn ores at depth; thereby representing a key area for further exploration for granite-related strategic and critical metals in SW Sardinia

    We examine quantum corrections of time delay arising in the gravitational field of a spinning oblate source. Low-energy quantum effects occurring in Kerr geometry are derived within a framework where general relativity is fully seen as an effective field theory. By employing such a pattern, gravitational radiative modifications of Kerr metric are derived from the energy-momentum tensor of the source, which at lowest order in the fields is modelled as a point mass. Therefore, in order to describe a quantum corrected version of time delay in the case in which the source body has a finite extension, we introduce a hybrid scheme where quantum fluctuations affect only the monopole term occurring in the multipole expansion of the Newtonian potential. The predicted quantum deviation from the corresponding classical value turns out to be too small to be detected in the next future, showing that new models should be examined in order to test low-energy quantum gravity within the solar system

    Neutrophil-dependent pentraxin-3 and reactive oxygen species production modulate endothelial dysfunction in haemodialysis patients.

    BACKGROUND: The aim of this study was to investigate neutrophil activation and its role in long pentraxin-3 (PTX3) release and oxidative stress generation during haemodialysis (HD) and to correlate neutrophil PTX3 and oxidant expression with endothelial dysfunction. METHODS: Forty-seven uraemic patients on stable HD, 12 healthy subjects and 15 patients with congestive heart failure (New York Heart Association classes III and IV) were enrolled. Neutrophil PTX3 protein expression was evaluated by confocal microscopy. l-selectin expression, intracellular PTX3 localization and reactive oxygen species (ROS) generation in human neutrophils were measured by flow cytometry. NADPH-dependent superoxide generation was investigated by chemiluminescence. PTX3 plasma concentrations were measured by ELISA. Endothelial dysfunction was studied by flow-mediated dilation (FMD). RESULTS: The low baseline levels of FMD significantly improved after HD, but worsened by 24 h. A significant up-regulation of PTX3 protein expression, localized within secondary granules, was detected in neutrophils isolated at 30 and 240 min of HD, along with an increase in l-selectin expression. The up-regulation in intracellular PTX3 in neutrophils was associated with a significant increase in PTX3 plasma concentrations at 240 min. HD increased ROS production and NADPH oxidase activity in neutrophils. In a univariate analysis, pre-treatment with FMD was inversely correlated with PTX3 expression and ROS generation in neutrophils. In a multivariate analysis, both circulating pre-HD PTX3 and intracellular ROS generation by neutrophils were independent predictors of abnormal FMD. CONCLUSIONS: Neutrophil overexpression of PTX3 is associated with ROS overproduction and endothelial dysfunction and may represent an emerging marker of vascular damage progression in HD patients. © The Author 2016.
