    A study of composite material damage induced by laser shock waves

    A laser shock wave technique has been used to study the damage tolerance of T800/M21 CFRP (Carbon Fiber Reinforced Polymer) composite material with different lay_ups. Different levels of damage have been created according to various laser irradiation conditions. Several characterization methods such as Optical Microscopy, X-ray Radiography, or Interferometric Confocal Microscopy have been used to quantify these defects. The nature of the defects induced by the shock wave propagation has been studied. The sensitivity of the composite material damage to the shock conditions has been shown and quantified. Moreover, the experimental results gathered with each technique have been compared to each other and it leads to a better understanding of the CFRP behavior under high dynamic loading. These original results have enabled the definition of a specific damage criterion for CFRP under dynamic loading

    Making H-ARQ suitable for a mobile TCP receiver over LEO satellite constellations

    This paper investigates strategies to carry out delay tolerant services over LEO satellite constellations for mobile receiver. In this context, LEO constellations are characterized by important delay variations where propagation impairments are mostly localized on the Land Mobile Satellite (LMS) channel (i.e. on the last hop). To cope with this issue, distinct reliability schemes can be introduced at the physical or link layers. Although their capacity to cope with transmission errors has been demonstrated, these recovery schemes may induce a high jitter that could severely damage TCP's internal timers and reliability schemes. As a matter of fact, transport and link layers’ reliability schemes exhibit a clear discrepancy. Following temporal traces representing the delay between a mobile terminal and the last hop satellite from a LEO constellation, we assess how HARQ mechanisms impact on the RTO based retransmission and the duplicate acknowledgments of TCP. Based on ns-2 simulations, we propose a layer-2 buffer that let both link and transport layers to conjointly perform. Our evaluations show an end-to-end data rate increase and more generally illustrate the benefit of re-ordering packets at the link layer when link-layer erasure coding recovery mechanisms are used conjointly with TCP

    Development and validation of a knowledge-based score to predict Fried's frailty phenotype across multiple settings using one-year hospital discharge data: The electronic frailty score

    Background: Most claims-based frailty instruments have been designed for group stratification of older populations according to the risk of adverse health outcomes and not frailty itself. We aimed to develop and validate a tool based on one-year hospital discharge data for stratification on Fried's frailty phenotype (FP). Methods: We used a three-stage development/validation approach. First, we created a clinical knowledge-driven electronic frailty score (eFS) calculated as the number of deficient organs/systems among 18 critical ones identified from the International Statistical Classification of Diseases and Related Problems, 10th Revision (ICD-10) diagnoses coded in the year before FP assessment. Second, for eFS development and internal validation, we linked individual records from the Lc65+ cohort database to inpatient discharge data from Lausanne University Hospital (CHUV) for the period 2004-2015. The development/internal validation sample included community-dwelling, non-institutionalised residents of Lausanne (Switzerland) recruited in the Lc65+ cohort in three waves (2004, 2009, and 2014), aged 65-70 years at enrolment, and hospitalised at the CHUV at least once in the year preceding the FP assessment. Using this sample, we selected the best performing model for predicting the dichotomised FP, with the eFS or ICD-10-based variables as predictors. Third, we conducted an external validation using 2016 Swiss nationwide hospital discharge data and compared the performance of the eFS model in predicting 13 adverse outcomes to three models relying on well-designed and validated claims-based scores (Claims-based Frailty Index, Hospital Frailty Risk Score, Dr Foster Global Frailty Score). Findings: In the development/internal validation sample (n = 469), 14·3% of participants (n = 67) were frail. Among 34 models tested, the best-subsets logistic regression model with four predictors (age and sex at FP assessment, time since last hospital discharge, eFS) performed best in predicting the dichotomised FP (area under the curve=0·71; F1 score=0·39) and one-year adverse health outcomes. On the external validation sample (n = 54,815; 153 acute care hospitals), the eFS model demonstrated a similar performance to the three other claims-based scoring models. According to the eFS model, the external validation sample showed an estimated prevalence of 56·8% (n = 31,135) of frail older inpatients at admission. Interpretation: The eFS model is an inexpensive, transportable and valid tool allowing reliable group stratification and individual prioritisation for comprehensive frailty assessment and may be applied to both hospitalised and community-dwelling older adults.     Development of a frailty score based on hospital discharge data linked to cohort data

    Introduction Frailty is strongly associated with adverse health outcomes and health care costs in elders. However, we have almost no idea of the prevalence of frail older inpatients in Swiss hospitals. Hospital discharge data could contribute to predicting frailty in these patients, and eventually improving SwissDRGs system or casemix-adjustment. Objectives and Approach The HFrailty project aimed to develop a predictive model of Fried’s Frailty Phenotype (FFP) based on hospital discharge data. We linked Lausanne University Hospital (CHUV) discharge data to clinical data from the Lausanne cohort study (Lc65+) over the period 2004-2015. The Lc65+ is a longitudinal population-based cohort comprising three random samples of approximately 1500 Lausanne residents aged 65 to 70, born respectively before, during and after World War II. With stepwise and lasso penalized logistic regression, random forest and neural networks, we identified the best-performing model for predicting FFP using CHUV’s data recorded within 12 months prior to frailty assessments. Results Among Lc65+ participants, 1649 were assessed for frailty and hospitalized at least once during the follow-up period, resulting in 3499 FFP assessments of which 544 were preceded by at least one hospitalization within 12 months.  In total, 45.7% of the participants were men and 9.4% were frail (FFP score ≥ 3). As expected, prevalence of frailty increased with age from 4.1% in the 66-70 age group, to 5.3% and 10.5% in the 71-75 and 76-80 groups, respectively. Logistic regression with lasso penalty was finally the best model regarding both performance and complexity. It had an area under receiver operating curve of 0.67 to predict FFP based on detailed diagnosis and procedure codes. Conclusion/Implications Hospital discharge data may be used to identify frail and non-frail individuals and estimate their prevalence in the Swiss non-institutionalized population. Our predictive model showed limited performance and could be improved. We are currently testing groups of diagnosis and procedure codes, as predictors, instead of detailed ones

    Soluble Isoforms of Vascular Endothelial Growth Factor Are Predictors of Response to Sunitinib in Metastatic Renal Cell Carcinomas

    Angiogenesis is the target of several agents in the treatment of malignancies, including renal cell carcinoma (RCC). There is a real need for surrogate biomarkers that can predict selection of patients who may benefit from antiangiogenic therapies, prediction of disease outcome and which may improve the knowledge regarding mechanism of action of these treatments. Tyrosine kinase inhibitors (TKI) have proven efficacy in metastatic RCC (mRCC). However, the molecular mechanisms underlying the clinical response to these drugs remain unclear.The present study aimed to identify molecular biomarkers associated with the response to sunitinib, a Tyrosine kinase inhibitor. To evaluate this relationship, primary tumors from 23 metastatic RCC patients treated by sunitinib were analyzed for a panel of 16 biomarkers involved in tumor pathways targeted by sunitinib, using real-time quantitative reverse-transcriptase PCR. Nine of the 23 patients (39%) responded to sunitinib. Among transcripts analyzed, only the levels of vascular endothelial growth factor (VEGF) soluble isoforms (VEGF(121) and VEGF(165)) were associated with the response to sunitinib (P = 0.04 for both). Furthermore, the ratio of VEGF soluble isoforms (VEGF(121)/VEGF(165)) was significantly associated with prognosis (P = 0.02).This preliminary study provides a promising tool that might help in the management of metastatic RCC, and could be extended to other tumors treated by TKI

    Comprehensive evaluation of methods to assess overall and cell-specific immune infiltrates in breast cancer

    Background: Breast cancer (BC) immune infiltrates play a critical role in tumor progression and response to treatment. Besides stromal tumor infiltrating lymphocytes (sTILs) which have recently reached level 1B evidence as a prognostic marker in triple negative BC, a plethora of methods to assess immune infiltration exists, and it is unclear how these compare to each other and if they can be used interchangeably. Methods: Two experienced pathologists scored sTIL, intra-tumoral TIL (itTIL), and 6 immune cell types (CD3+, CD4+, CD8+, CD20+, CD68+, FOXP3+) in the International Cancer Genomics Consortium breast cancer cohort using hematoxylin and eosin-stained (n = 243) and immunohistochemistry-stained tissue microarrays (n = 254) and whole slides (n = 82). The same traits were evaluated using transcriptomic- and methylomic-based deconvolution methods or signatures. Results: The concordance correlation coefficient (CCC) between pathologists for sTIL was very good (0.84) and for cell-specific immune infiltrates slightly lower (0.63-0.66). Comparison between tissue microarray and whole slide pathology scores revealed systematically higher values in whole slides (ratio 2.60-5.98).     NASA's interstellar collector from the Stardust mission captured several particles that are now thought to be of interstellar origin. We analyzed two of these via nanodiffraction at the European Synchrotron Radiation Facility (ESRF) and found them to contain crystalline components. The unit cell of the crystalline material is determined from the diffraction patterns and the most likely mineral components are identified as olivine and spinel

    A new molecular classification to drive precision treatment strategies in primary Sjögren’s syndrome

    There is currently no approved treatment for primary Sjögren's syndrome, a disease that primarily affects adult women. The difficulty in developing effective therapies is -in part- because of the heterogeneity in the clinical manifestation and pathophysiology of the disease. Finding common molecular signatures among patient subgroups could improve our understanding of disease etiology, and facilitate the development of targeted therapeutics. Here, we report, in a cross-sectional cohort, a molecular classification scheme for Sjögren's syndrome patients based on the multi-omic profiling of whole blood samples from a European cohort of over 300 patients, and a similar number of age and gender-matched healthy volunteers. Using transcriptomic, genomic, epigenetic, cytokine expression and flow cytometry data, combined with clinical parameters, we identify four groups of patients with distinct patterns of immune dysregulation. The biomarkers we identify can be used by machine learning classifiers to sort future patients into subgroups, allowing the re-evaluation of response to treatments in clinical trials

    Mineralogy and petrology of comet 81P/wild 2 nucleus samples

    The bulk of the comet 81P/Wild 2 (hereafter Wild 2) samples returned to Earth by the Stardust spacecraft appear to be weakly constructed mixtures of nanometer-scale grains, with occasional much larger (over 1 micrometer) ferromagnesian silicates, Fe-Ni sulfides, Fe-Ni metal, and accessory phases. The very wide range of olivine and low-Ca pyroxene compositions in comet Wild 2 requires a wide range of formation conditions, probably reflecting very different formation locations in the protoplanetary disk. The restricted compositional ranges of Fe-Ni sulfides, the wide range for silicates, and the absence of hydrous phases indicate that comet Wild 2 experienced little or no aqueous alteration. Less abundant Wild 2 materials include a refractory particle, whose presence appears to require radial transport in the early protoplanetary disk
