    A Longitudinal Study on Motherhood and Well-Being: Developmental and Clinical Implications

    Pregnancy and puerperium are crucial periods at both the biological and psychological levels. The aim of this longitudinal study was to investigate women's perceived health and illness during pregnancy and puerperium through the assessment of hedonic and eudaimonic components of well-being, symptoms of peripartum depression, and their relationship. Nineteen women recruited at the Unit of Obstetrics and Gy-necology of a university hospital in Northern Italy were enrolled and answered a set of questionnaires at two time points: 20-22 weeks of gestation and 6 months after childbirth. Results showed a substantial lack of correlation between the dimensions of mental illness and mental health. The analysis of the well- and ill-being components showed low levels of depression as well as good levels of mental health, especially concerning the eudaimonic components of well-being, both during and after pregnancy. Overall, findings attested to a general stability of well-being over pregnancy and puerperium. El embarazo y el puerperio son periodos cruciales tanto a nivel biológico como psicológico. El objetivo de este estudio longitudinal fue investigar la percepción de las mujeres sobre su salud y enfermedad durante el embarazo y el puerperio mediante la evaluación de componentes hedónicos y eudaimónicos del bienestar, síntomas de depresión, y sus relaciones. Participaron 19 mujeres contactadas en la Unidad de Obstetricia y Ginecología de un hospital italiano, quienes respondieron a unos cuestionarios a las 20-22 semanas de la gestación y a los 6 meses tras el parto. Los resultados mostraron una sustancial falta de correlación entre dimensiones de salud y de enfermedad mental. El análisis del bienestar y malestar mostraron bajos niveles de depresión así como buenos niveles de salud mental, especialmente en los componentes eudaimónicos del bienestar, tanto durante como después del embarazo. Globalmente nuestros hallazgos indican una estabilidad general del bienestar en el embarazo y el puerperio

    Machine Learning and MADIT methodology for the fake news identification: the persuasion index

    [EN] The phenomenon of fake news has grown concurrently with the rise of social networks that allow people to directly access news without the mediation of reliable sources. Recognizing news as fake is a difficult task for humans, and even tougher for a machine. This proposal aims to redesign the problem: from a check of truthfulness of news content, to the analysis of texts’ persuasion level. That is how information is introduced to the reader, assuming that fake news is aimed at persuading towards the reality of sense they intend to convey. M.A.D.I.T. methodology has been chosen. It is useful to describe how texts are built, overcoming the content/structure analysis level and stressing the study of Discursive Repertories: discursive modalities of reality of sense building, classified into real and fake news categories thanks to the Machine learning application. For the dataset building 7,387 news have been analysed.     Leukemia inhibitory factor protects cholangiocarcinoma cells from drug-induced apoptosis via a PI3K/AKT-dependent Mcl-1 activation

    Cholangiocarcinoma is an aggressive, strongly chemoresistant liver malignancy. Leukemia inhibitory factor (LIF), an IL-6 family cytokine, promotes progression of various carcinomas. To investigate the role of LIF in cholangiocarcinoma, we evaluated the expression of LIF and its receptor (LIFR) in human samples. LIF secretion and LIFR expression were assessed in established and primary human cholangiocarcinoma cell lines. In cholangiocarcinoma cells, we tested LIF effects on proliferation, invasion, stem cell-like phenotype, chemotherapy-induced apoptosis (gemcitabine+cisplatin), expression levels of pro-apoptotic (Bax) and anti-apoptotic (Mcl-1) proteins, with/without PI3K inhibition, and of pSTAT3, pERK1/2, pAKT. LIF effect on chemotherapy-induced apoptosis was evaluated after LIFR silencing and Mcl-1 inactivation. Results show that LIF and LIFR expression were higher in neoplastic than in control cholangiocytes; LIF was also expressed by tumor stromal cells. LIF had no effects on cholangiocarcinoma cell proliferation, invasion, and stemness signatures, whilst it counteracted drug-induced apoptosis. Upon LIF stimulation, decreased apoptosis was associated with Mcl-1 and pAKT up-regulation and abolished by PI3K inhibition. LIFR silencing and Mcl-1 blockade restored drug-induced apoptosis. In conclusion, autocrine and paracrine LIF signaling promote chemoresistance in cholangiocarcinoma by up-regulating Mcl-1 via a novel STAT3- and MAPK-independent, PI3K/AKT-dependent pathway. Targeting LIF signaling may increase CCA responsiveness to chemotherapy

    Informal Caregivers of Patients with Disorders of Consciousness: a Qualitative Study of Communication Experiences and Information Needs with Physicians

    Due to improvements in medicine, the figures of patients with disorders of consciousness (DoC) are increasing. Diagnostics of DoC and prognostication of rehabilitation outcome is challenging but necessary to evaluate recovery potential and to decide on treatment options. Such decisions should be made by doctors and patients’ surrogates based on medico-ethical principles. Meeting information needs and communicating effectively with caregivers as the patients´ most common surrogate-decision makers is crucial, and challenging when novel tech-nologies are introduced. This qualitative study aims to explore information needs of informal DoC caregivers, how they manage the obtained information and their perceptions and experiences with caregiver-physician communication in facilities that implemented innovative neurodiagnostics studies. In 2021, we conducted semi-structured interviews with nine caregivers of clinically stable DoC patients in two rehabilitation centers in Italy and Germany. Participants were selected based on consecutive purposeful sampling. Caregivers were recruited at the facilities after written informed consent. All interviews were recorded, transcribed verbatim and translated. For analysis, we used reflexive thematic analysis according to Braun & Clarke (2006). Caregivers experienced the conversations emotionally, generally based on the value of the information provided. They reported to seek positive information, comfort and empathy with-in the communication of results of examinations. They needed detailed information to gain a deep understanding and a clear picture of their loved-one’s condition. The results suggest a mismatch between the perspectives of caregivers and the perspectives of medical profession-als, and stress the need for more elaborate approaches to the communication of results of neu-rodiagnostics studies.     KIF5A and ALS2 Variants in a Family With Hereditary Spastic Paraplegia and Amyotrophic Lateral Sclerosis

    This paper describes the clinical evolution and the novel genetic findings in a KIF5A mutated family previously reported as affected by spastic paraparesis only. The additional evidence we report here, a homozygous ALS2 mutation detected in the proband, and the clinical evolution observed in the affected members of the family, are in line with the evidence of an overlap between Hereditary Spastic Paraplegias and Amyotrophic Lateral Sclerosis associated with variants in these genes. The proband, a 14-years-old boy, started manifesting a pure form of HSP at age 14 months. The disease rapidly progressed to a juvenile form of ALS. This boy carries a heterozygous missense variant in KIF5A p.(Glu755Lys), inherited from the father, and a homozygous missense variant in the alsin protein encoded by the ALS2 gene p.(Pro192Leu). The father shows a family history of ALS. In the last few years, he has been developing signs and symptoms of both upper and lower motor neuron degeneration, with mild bulbar motor involvement and emotional lability. The patients described in this family, confirm the continuum and partial overlap of the two clinical entities, HSP and ALS, historically viewed as distinct entities. The genetic findings in this family further substantiate the genetic bases underlying the overlap, broadening the clinical spectrum associated with KIF5A mutations

    Managing the Consequences of Oncological Major Surgery: A Short- and Medium-Term Skills Assessment Proposal for Patient and Caregiver through M.A.D.I.T. Methodology

    The effects of cancer surgery and treatment harm patients' life and working ability: major causes of this can be intensified by the postoperative symptoms. This study, the first part of the HEAGIS project (Health and Employment after Gastrointestinal Surgery), proposes a method to assess patients and caregivers' competences in dealing with postoperative course and the related needs to improve the adequate competences. In this observational study, an ad hoc structured interview was conducted with 47 patients and 15 caregivers between the third and fifteenth postoperative day. Oesophageal (38%), esophagogastric junction (13%), gastric (30%), colon (8%) and rectum (11%) cancer patients were considered. Computerized textual data analysis methodology was used to identify levels of competences. Text analysis highlighted three different levels (low, medium and high) of four specific types of patients and caregivers' competences. In particular, the overall trend of the preview of future scenarios and use of resource competences was low. Less critical were situation evaluation and preview repercussion of own actions' competences. Caregivers' trends were similar. The Kruskal-Wallis test did not distinguish any differences in the level of competences related to the characteristics of the participants. Patients and caregivers are not accurate in planning the future after surgery, using personal beliefs rather than referring to physicians, and not recognizing adequate resources. The medium-low competences' trend leads to unexpected critical situations, and patients could not deal with them in a maximally effective way. Both patients and caregivers should be taken over by healthcare professionals to improve patients' competences and make the curative surgery effective in daily life

    Mid-Holocene relative sea-level changes along Atlantic Patagonia: new data from Camarones, Chubut, Argentina

    This paper concerns the relative sea-level changes associated with the Atlantic Patagonian coast derived from sea-level index points whose elevation was determined by a differential global position system (DGPS). Bio encrustations from outcrops located near Camarones, Chubut, Argentina, consist of autochthonous deposits characterized by Austromegabalanus psittacus (Molina, 1782), encrusting acer vulinid foraminifera, coralline red algae and bryozoans. The association of the different organisms is interpreted as being associated with an intertidal environment, and they have been used as index points to establish the relative sea-level position. The main conclusion is that the relative sea-level between c. 7000 and 5300 cal. yr BP was in the range ofc. 2?4 m a.s.l., with a mean value of c. 3.5 m a.s.l. Our data seem to support the existence of different rates of relative sea-level fall in different sectors of Atlantic Patagonia during the Holocene and highlight the importance of a more precise and accurate relative sea-level estimation by producing new data and revisiting the indicative meaning of most of the indicators so far used in the area.Facultad de Ciencias Naturales y Muse

    Promoting well-being in old age : the psychological benefits of two training programs of adapted physical activity

    In the last few decades, the relationship between physical conditions and mental health has increasingly attracted the interest of researchers and professionals across disciplines. This relationship is especially relevant in old age, as the challenges posed by aging at various levels represent crucial concerns for policy makers. Due to the remarkable increase in life expectancy across countries, sustainable prevention strategies are needed to help individuals preserve psychophysical well-being in old age. In particular, the regular practice of a moderately intense physical activity is recommended by the World Health Organization to enhance balance, prevent falls, strengthen muscles, and promote psychophysical well-being. Daily physical exercise represents a beneficial and low-cost strategy, easily accessible to the general population and potentially customizable to specific needs through brief training programs. Based on these premises, the present research aimed at longitudinally evaluating mental well-being among 58 Italian people aged 67-85, who were involved in two Adapted Physical Activity (APA) training programs. Inclusion criteria for participation comprised high autonomy levels in daily activities, no cognitive impairment, sedentary habits or only occasional performance of moderate physical activity. Based on physical and functional assessment, 39 participants joined a program of adapted motor activity (PoliFit; Study 1), while 19 participants attended a variant program specifically designed for people with osteoporosis (OsteoFit; Study 2). Well-being dimensions were assessed through the Mental Health Continuum-Short Form, the Emotion Regulation Questionnaire and the Satisfaction with Life Scale. Physical functioning were evaluated before and after the programs through the Short Physical Performance Battery and the Handgrip Dynamometer Jamar Test. Findings highlighted that, besides physical benefits, participants reported significantly more adaptive emotion regulation strategies after both training programs; in addition, participants attending OsteoFit reported significantly higher levels of emotional well-being. Results suggest the potential of moderate physical activity in promoting mental health, emphasizing the additional role of training programs as cost-effective opportunities for elderly people to socialize and improve emotional functioning. Overall, the findings support the view of old age as a stage of competence development and adaptive adjustment, rather than a phase of mere psychophysical decline

    GESTÃO DE RESÍDUOS SÓLIDOS URBANOS EM CAPITAIS BRASILEIRAS ALTERNATIVAS PARA UM MODELO DE GESTÃO

    O presente artigo tem como principal objetivo comparar a gestão dosresíduos sólidos de dez capitais brasileiras, destacando seus aspectosnegativos e positivos. Muitos municípios brasileiros possuem dificuldadesem fazer uma gestão adequada dos resíduos sólidos urbanos devido avários fatores, como: falta de capacidade financeira e administrativa, poucacapacidade técnica na gestão dos serviços de limpeza pública, além dotema não ser uma prioridade em muitos casos para a gestão pública. Tratasede uma pesquisa qualitativa e de análise de conteúdo, que buscacompreender melhor a forma de gestão dos resíduos sólidos desde oplanejamento até a aplicação. Com os resultados observam-se que amaioria dessas capitais possui uma gestão que contempla a coleta seletiva,tratamento, pontos de transbordo, cooperativas de catadores de papel,além da parceria da prefeitura com programas e projetos específicosenvolvendo os resíduos sólidos.
