A dabigatran-antidótum, az intravénás idarucizumab első hazai alkalmazása | The first use of iv. idarucizumab for dabigatran reversal in Hungary
Absztrakt:
Az új típusú, direkt orális antikoagulánsok közül jelenleg egyedül a direkt
trombingátló dabigatran rendelkezik hatékony antidótummal. Az idarucizumab egy
humanizált, monoklonális dabigatrant megkötő antitestfragmentum, amely a
gyógyszer szelektív véralvadásgátló hatását azonnal, tartósan, biztonságosan
felfüggeszti. A szerzők a dabigatran iv. antidótuma, az idarucizumab első hazai
alkalmazását (2016. május 23.) ismertetik egy nonvalvularis paroxysmalis
pitvarfibrilláció miatt dabigatrant (2 × 110 mg/nap) szedő, akut
cholecystectomiára szoruló, magas stroke-rizikójú, csökkent vesefunkciós idős
nőbetegen. A beteg a műtét előtt két órával antidótumot (idarucizumab 2 × 2,5
g/50 ml iv.) kapott, a sebészeti beavatkozás kapcsán kontrollálhatatlan vérzés
nem volt. Az antikoaguláns hatásra utaló műtét előtti megnyúlt aktivált
parciális tromboplasztinidő az antidótum beadása után normalizálódott. Az
idarucizumab iv. beadása kapcsán mellékhatás, szövődmény nem volt. A beteg

Abstract:
At present, the direct thrombin inhibitor dabigatran is the only one amongst the
new direct anticoagulants which has an effective, specific reversal agent. The
novel agent idarucizumab is a humanized, monoclonal antibody fragment binds to
dabigatran within minutes thereby offers an opportunity to induce a safe,
long-lasting reverse of the anticoagulant effects of dabigatran. The authors
describe the first use of idarucizumab in Hungary (23. 05. 2016) in an old
female patient with non-valvular paroxysmal atrial fibrillation of high stroke
risk-score and renal dysfunction who was taking dabigatran (2 x 110 mg/day) when
an acute abdomen developed requiring emergency cholecystectomy. Patient received
the antidote (idarucizumab 2 x 2.5 g/50 ml iv.) two hours before the surgical
intervention, and she did not have any uncontrollable, life-threatening bleeding
during the surgery. The high activated partial thromboplastin time relating to
anticoagulative influence before the surgery normalized completely after
administration of the antidote. Antagonizing dabigatran with idarucizumab was
feasible and safe without any side effects. The patient received dabigatran

Autogenous Bone Versus β-Tricalcium Phosphate Graft Alone for Bilateral Sinus Elevations (2- and 3-Dimensional Computed Tomographic, Histologic, and Histomorphometric Evaluations): Preliminary Results
The aim of this research was to compare 2 different graft materials, β-tricalcium phosphate (Cerasorb) and autogenous bone, used in the same patient. Bilateral sinus grafting was performed on 4 selected patients; Cerasorb only was used on the experimental side, and autogenous bone only was used on the control side. In all 4 patients, the maxilla was atrophied to such an extent that the reconstruction included not only sinus grafting but also onlay plasty. The procedure was followed by implant placement 6 months later. In addition to routine panoramic radiographs, 2- and 3-dimensional computed tomographic (CT) examinations were performed pre- and postoperatively and after implantation. Information from CTs is necessary when alveolar bone atrophy is extensive, complications appear probable, and in difficult cases, when exact documentation is important. A total of 16 bone biopsies were taken at the time of implant placement. The histologic and histomorphometric results indicated that when the formation of new bone was slow, it was slow on both sides; when it was fast, then it was fast on both sides. Individual patient factors strongly influenced the fates of the various graft materials in the organism. Comparisons of the present results with the findings of other investigators demonstrated that β-tricalcium phosphate is a satisfactory graft material, even without autogenous bone
