Economic and Environmental Assessment of Bacterial Poly(3-Hydroxybutyrate) Production from the Organic Fraction of Municipal Solid Waste
The management of municipal solid waste is a major logistic and environmental problem worldwide. Nonetheless, the organic fraction of municipal solid waste (OFMSW) is a valuable source of nutrients which can be used for a variety of purposes, according to the Circular Economy paradigm. Among the possible applications, the bioproduction of a biodegradable polyester, poly(3-hydroxybutyrate) [P(3HB)], using OFMSW as carbon platform is a promising strategy. Here, an economic and environmental assessment of bacterial P(3HB) production from OFMSW is presented based on previously published results. The SuperPro Designer (R) software was used to simulate P(3HB) production under our experimental parameters. Two scenarios were proposed depending on the fermentation medium: (1) enzymatic hydrolysate of OFMSW supplemented with glucose and plum waste juice; and (2) basal medium supplemented with glucose and plum waste juice. According to our results, both scenarios are not economically feasible under our experimental parameters. In Scenario 1, the low fermentation yield, the cost of the enzymes, the labour cost and the energy consumption are the factors that most contribute to that result. In Scenario 2, the cost of the extraction solvent and the low fermentation yield are the most limiting factors. The possibility of using process waste as raw material for the generation of other products must be investigated to enhance economic feasibility. From an environmental viewpoint, the photochemical oxidation potential (derived from the use of anisole as extraction solvent) and the generation of acid rain and global warming effect (caused by the burning of fuels for power generation) are the most relevant impacts associated to P(3HB) production under our experimental parameters.Jon Kepa Izaguirre was the recipient of a predoctoral fellowship from the Basque Governmen

A neurodevelopmental disorder caused by a dysfunctional CACNA1A allele
P/Q-type Ca2+ flux into nerve terminals via CaV2.1 channels is essential for neurotransmitter release at neuromuscular junctions and nearly all central synapses. Mutations in CACNA1A, the gene encoding CaV2.1, cause a spectrum of pediatric neurological disorders. We have identified a patient harboring an autosomal-dominant de novo frameshift-causing nucleotide duplication in CACNA1A (c.5018dupG). The duplicated guanine precipitated 43 residues of altered amino acid sequence beginning with a glutamine to serine substitution in CaV2.1 at position 1674 ending with a premature stop codon (CaV2.1 p.Gln1674Serfs*43). The patient presented with episodic downbeat vertical nystagmus, hypotonia, ataxia, developmental delay and febrile seizures. In patch-clamp experiments, no Ba2+ current was observed in tsA-201 cells expressing CaV2.1 p.Gln1674Serfs*43 with β4 and α2δ-1 auxiliary subunits. The ablation of divalent flux in response to depolarization was likely attributable to the inability of CaV2.1 p.Gln1674Serfs*43 to form a complete channel pore. Our results suggest that the pathology resulting from this frameshift-inducing nucleotide duplication is a consequence of an effective haploinsufficiency
Estudio clinicoepidemiológico de la enfermedad por Mycobacterium kansasii en el área urbana de Bilbao
Objetivo: Descripción epidemiológica de los individuos con aislamiento de Mycobacterium kansasii en muestras respiratorias y análisis de estos aislamientos mediante técnicas de biología molecular. Material y métodos: Se realizó un estudio retrospectivo, prospectivo y descriptivo de enero de 1994 a abril de 2002 en los hospitales de Basurto y Santa Marina, y de enero de 2000 a abril de 2002 en los hospitales de Cruces, Galdakao y San Eloy. Se aplicaron los criterios de la American Thoracic Society para el diagnóstico de enfermedad y se utilizaron otras definiciones para abarcar todos los casos. Se comparó la enfermedad por M. kansasii en pacientes sin infección por el virus de la inmunodeficiencia humana (VIH) con un grupo control con enfermedad por Mycobacterium tuberculosis. Se aplicó la reacción en cadena de la polimerasa con análisis de RFLP (restriction fragment-length polymorphisms) para diferenciar las especies de micobacterias y subtipificación en genotipos, y la AFLP (amplified fragment-length polymor-phisms) para reconocer clones dentro de cada genotipo. Resultados: Se revisaron 334 historias clínicas de pacientes en los que existía un registro de aislamiento microbiológico de M. kansasii. Consideramos que 220 eran enfermos (criterio de la American Thoracic Society más enfermedad probable de definiciones creadas). La enfermedad era más frecuente en varones (n = 185; 84,1%) y en personas sin infección por el VIH (n = 184; 83,6%). La tasa de incidencia de enfermedad más alta en la comarca de Bizkaia fue en Margen Izquierda-Encartaciones, con un 8,05 por cada 100.000 habitantes, y en el Área de Bilbao, en los distritos de la periferia. Las enfermedades de base fueron: tuberculosis (20,5%), enfermedad pulmonar obstructiva crónica (25,9%), neoplasia de pulmón (7,7%), silicosis (0,9%), hepatopatía crónica (11,4%) y gastrectomía (8,6%). Los síntomas constitucionales más frecuentes fueron: fiebre (39,1%), anorexia (23,2%) y disminución de peso (33,3%). Entre los síntomas respiratorios destacaron la tos (70,9%) y la expectoración (62,3%). Los patrones radiológicos más frecuentes fueron cavitación e infiltrados pulmonares. La pauta de tratamiento más habitual fue rifampicina, isoniacida y etambutol (43,4%), y el tiempo medio de duración fue de 12 meses en las personas sin infección por el VIH. En el estudio de sensibilidad realizado en 56 cepas, el 100% fue resistente a isoniacida y ninguna mostró resistencia a rifampicina. Se compararon 34 casos de enfermedad por M. kansasii con 68 casos de tuberculosis, todos sin infección por el VIH, y se obtuvieron los siguientes resultados: predominio de fumadores, de síntomas respiratorios y de cavitación en los pacientes con enfermedad por M. kansasii. El 98,5% de los aislamientos pertenecieron al genotipo I. Se obtuvieron un total de 8 clones; el clon denominado 1 fue más frecuente en personas con infección por el VIH y el denominado 3 en los que no la presentaban. Conclusiones: Se ha registrado un aumento del número de pacientes con enfermedad por M. kansasii en la provincia de Bizkaia en los últimos años. Dicha enfermedad es más frecuente en varones, personas sin infección por el VIH y zonas urbanas. Asimismo, se han encontrado más síntomas respiratorios como manifestaciones clínicas y mayor presencia de cavitación como hallazgo radiológico en la enfermedad por M. kansasii al compararla con la tuberculosis. El genotipo I es el aislado con más frecuencia, y los clones 1 y 3 afectan al 80% de los individuos enfermos

Intraoperative Microscope-Mounted Spectral Domain Optical Coherence Tomography for Evaluation of Retinal Anatomy during Macular Surgery
To evaluate the use of microscope mounted spectral domain optical coherence tomography (SD-OCT) to detect changes in retinal anatomy during macular surgery.
Retrospective, observational case series.
We included 25 eyes of 24 consecutive patients who underwent SD-OCT during macular surgery.
A retrospective review of operative techniques, outcomes, and imaging for all patients who underwent intraoperative microscope mounted SD-OCT during surgery for macular hole or epiretinal membrane (ERM) from April 2009 to April 2010 was performed. Qualitative and quantitative characteristics of intraoperative and postoperative changes in retinal anatomy were studied.
Intraoperative change in macular hole dimensions and retinal thickness in patients with ERM owing to surgical manipulation measured using SD-OCT.
Intraoperative SD-OCT from 13 eyes of 13 patients undergoing surgery for macular hole was reviewed. Two cases had images of suboptimal quality and were excluded. The remaining 11 eyes were subjected to quantitative analysis, which revealed stability of macular hole height and central hole diameter after internal limiting membrane (ILM) peeling, but an increase in the diameter of subretinal fluid under the macula in ten of 11 eyes (average 87% wider). Intraoperative imaging from 12 eyes of 11 patients undergoing surgery for ERM was analyzed. Quantitative analysis revealed an average increase of retinal thickness after ILM peel of <2%. Ten of 12 eyes developed a new subretinal hyporeflectance, which likely represents shallow detachment of the macula, after uncomplicated membrane peel.
Use of intraoperative SD-OCT has provided new insight into the changes to retinal anatomy during macular surgery and may prove to be a useful tool for vitreoretinal surgery. Further study is warranted to determine whether intraoperative changes such as the creation of shallow retinal detachments during uncomplicated macular surgery affects visual recovery.
