Familial Ménière's disease: clinical and genetic aspects
ABSTRACT Background and purpose:Mre's disease is not uncommon, with an incidence in Caucasians of about one in 2000. The incidence peaks in the fifth decade. Cases are usually isolated or sporadic, but in perhaps five per cent other family members are affected. We report here the clinical and genetic characteristics of a comprehensive set of familial Mre's disease cases from the UK.Methods:Forty-six affected families were studied. All cases were diagnosed using the American Academy of Otolaryngolog 8211;Head and Neck Surgery committee on hearing and equilibrium 1995, or more stringent, criteria.Outcomes and results:Autosomal dominant inheritance with reduced penetrance was the most likely mode of inheritance overall. Apparent genetic anticipation was observed, but may also be a result of ascertainment bias given the collection strategy. There was also a slight tendency for cases to result from maternal transmission within the families in this set. Failure assessment of lightly reinforced floor slabs. I: Experimental investigation
This paper is concerned with the ultimate behavior of lightly reinforced concrete floor slabs under extreme loading conditions. Particular emphasis is given to examining the failure conditions of idealized composite slabs which become lightly reinforced in a fire situation as a result of the early loss of the steel deck. An experimental study is described which focuses on the response of two-way spanning floor slabs with various materials and geometric configurations. The tests enable direct assessment of the influence of a number of key parameters such as the reinforcement type, properties, and ratio on the ultimate response. The results also permit the development of simplified expressions that capture the influence of salient factors such as bond characteristics and reinforcement properties for predicting the ductility of lightly reinforced floor slabs. The companion paper complements the experimental observations with detailed numerical assessments of the ultimate response and proposes analytical models that predict failure of slab members by either reinforcement fracture or compressive crushing of concrete. © 2011 American Society of Civil Engineers

Atherosusceptible Shear Stress Activates Endoplasmic Reticulum Stress to Promote Endothelial Inflammation.
Atherosclerosis impacts arteries where disturbed blood flow renders the endothelium susceptible to inflammation. Cytokine activation of endothelial cells (EC) upregulates VCAM-1 receptors that target monocyte recruitment to atherosusceptible regions. Endoplasmic reticulum (ER) stress elicits EC dysregulation in metabolic syndrome. We hypothesized that ER plays a central role in mechanosensing of atherosusceptible shear stress (SS) by signaling enhanced inflammation. Aortic EC were stimulated with low-dose TNFα (0.3 ng/ml) in a microfluidic channel that produced a linear SS gradient over a 20mm field ranging from 0-16 dynes/cm2. High-resolution imaging of immunofluorescence along the monolayer provided a continuous spatial metric of EC orientation, markers of ER stress, VCAM-1 and ICAM-1 expression, and monocyte recruitment. VCAM-1 peaked at 2 dynes/cm2 and decreased to below static TNFα-stimulated levels at atheroprotective-SS of 12 dynes/cm2, whereas ICAM-1 rose to a maximum in parallel with SS. ER expansion and activation of the unfolded protein response also peaked at 2 dynes/cm2, where IRF-1-regulated VCAM-1 expression and monocyte recruitment also rose to a maximum. Silencing of PECAM-1 or key ER stress genes abrogated SS regulation of VCAM-1 transcription and monocyte recruitment. Systems study for an Integrated Digital-Electric Aircraft (IDEA)
The results of the Integrated Digital/Electric Aircraft (IDEA) Study are presented. Airplanes with advanced systems were, defined and evaluated, as a means of identifying potential high payoff research tasks. A baseline airplane was defined for comparison, typical of a 1990's airplane with advanced active controls, propulsion, aerodynamics, and structures technology. Trade studies led to definition of an IDEA airplane, with extensive digital systems and electric secondary power distribution. This airplane showed an improvement of 3% in fuel use and 1.8% in DOC relative to the baseline configuration. An alternate configuration, an advanced technology turboprop, was also evaluated, with greater improvement supported by digital electric systems. Recommended research programs were defined for high risk, high payoff areas appropriate for implementation under NASA leadership

Influence of surgery and rehabilitation conditioning on psychophysiological fitness
The purpose of this study was to assess changes in psychophysiological fitness following reconstructive knee surgery and early phase (2.5 months) physical rehabilitation. Nine patients (7 male, 2 female; mean age, 29.9 years) electing to undergo anterior cruciate ligament reconstructive surgery (central third, bone-patella tendon-bone graft) were assessed on four separate assessment occasions post-surgery. Repeated measures ANOVAs showed significant condition (injured/non-injured leg) by test occasion (2 weeks pre-surgery and 6, 8 and 10 weeks post-surgery) interactions for knee ligamentous compliance (anterior tibiofemoral displacement), peak force and electromechanical delay associated with the knee flexors of the injured and noninjured legs (F3,24 = 4.7 to 6.6; p < 0.01), together with individualized emotional profile disturbance scores that were significantly less at 10 weeks post-surgery compared to pre-surgery, 6 weeks and 8 weeks post-surgery (F3,24 = 7.6; p < 0.01). Spearman rank correlation coefficients identified significant relationships between musculoskeletal fitness and emotional profile scores at pre-surgery (r = 0.69–0.72; p < 0.05) and at 8 weeks post-surgery (r = 0.70–0.73; p < 0.05). The 6 Bi-POMS subscales and the 12 ERAIQ responses found inconsistent patterns of response and relationships across the assessment occasions. Overall, the patterning of changes and associations amongst emotional performance profile discrepancy scores in conjunction with those scores from indices of musculoskeletal fitness performance capability offered important support for the efficacy of an approach which integrates self-perceptive and objective measurements of fitness capability during rehabilitation following surgery to a synovial joint
