    Analysis of photovoltaic technology development based on technology life cycle approach

    Increasing energy demand has created the challenge of supplying safe, economical, and durable energy with minimal impact on the environment. Therefore, governments have developed and executed several strategies such as increasing efficiency in energy systems in addition to replacing existing sources with renewable energies. One of the most important renewable energy sources that have a competitive advantage compared with other resources is solar energy and its related technologies. However, development of this technology, its related products, and their competitiveness in the market has created a plethora of challenges. In this study, the focus is on the analysis of photovoltaic technology development in the context of different technology generations. The S-shape curve of each generation and sub-technologies of photovoltaic is designed and analyzed. Results show that the first generation of photovoltaic technology is in growth and early maturity stage. The second generation is also in growth stage, but the third generation is mainly in the introduction stage.fi=vertaisarvioitu|en=peerReviewed

    The Effects of Sex Hormones on Liver Regeneration after Liver Trauma in Animal Model

    Background: The surgical management of liver injuries remains a great challenge for the traumatologists and general surgeons. We hypothesized that administration of 17 â-estradiol, a female sex hormone, improves hepatocellular healing after liver trauma.Methods: In an experimental model, 60 rats were divided into six subgroups: A (male control), B (male and estradiol), C (castrated male and estradiol), D (female control), F (female and estradiol), and G (oopherectomized female). After inducing liver trauma, estradiol subgroups received 3 doses of intravenous 17 â-estradiol (1 mg/kg) every 8 hours. 2 weeks post trauma, animals were sacrificed and hepatocellular regeneration was measured with the help of stereologic parameters of regeneration. Hepatocellular healing was compared between previous left lobe samples and the new post-traumatic right lobe samples.Results: Stereological parameters of rats receiving 17 b-estradiol after trauma was much better regarding mean angiogenesis point counting and volume density, compared with non-receiver groups after 2 weeks of trauma (P < 0.005). There was no significant difference for hepatocyte nucleus, hepatocyte point counting and volume density between estradiol receiver and non-receiver groups. In a comparison between subgroups, female sex had the same effect as giving estradiol. Oopherectomized female rats had more fibrogenesis but less angiogenesis (P < 0.005). Fibrogenesis was more in groups that were estradiol non-receiver (P < 0.005). In an explicit comparison of control females and males, estradiol infused males and females, and castrated male or oopherectomized female groups showed that stereological parameters of hepatocyte and hepatocyte nucleus were lower in female subgroups, but angiogenesis was better for female groups except for oopherectomized females.Conclusions: This study did support the administration of exogenic female hormone as an approach to augment the angiogenesis as a good index of regeneration for traumatic liver in rats

    Predictors of appropriate ICD therapy in patients with implantable cardioverter-defibrillator

    BACKGROUND: Understanding the predictors of appropriate implantable cardioverter defibrillator (ICD) therapy could help to better identify candidates for ICD implantation. METHODS: One hundred and sixty two patients with ICD (111 with coronary artery disease [CAD] and 51 with dilated cardiomyopathy [DCM]) were included in the study. Clinical, electrocardiographic, and ICD stored data and electrograms were collected. RESULTS: During mean follow up of 15±11 months 54 patients (33%) received ≥ 1 appropriate ICD therapy (AICDT). We used binary logistic regression analysis with forward selection method to find the potential predictors of appropriate ICD therapy after device implantation. Male gender (odds ratio [OR] = 2.76, 95% confidence interval [CI] = 1.1 – 7.1, P=0.021), DCM as underlying heart disease (OR = 4.2, 95% CI = 1.9 – 9.5, P=0.001), and QRS width > 100 ms (OR = 2.58, 95% CI = 1.2 – 5.4, P=0.010) were correlated with increased likelihood of AICDT during the follow up period. In subgroup analysis of the patients with CAD and DCM, QRS duration > 100 ms was correlated with the probability of ≥ 1 AICDT. In our patients indication of ICD implantation (primary versus secondary prevention) did not influence probability of ≥ 1 AICDT (adjusted OR = 1.66, 95% CI = 0.7 – 4.0, Mantel-Haenszel P value P=0.355.) CONCLUSION: QRS width could be used as an additional simple risk stratifier beyond EF to identify potential candidates who would benefit more from ICD implantation. This may have practical implications for patient selection especially in developing countries. Indication of ICD implantation (primary versus secondary prevention) did not affect the probability of ≥ 1 AICDT during the follow up period

    DRY CUPPING IN CHILDREN WITH FUNCTIONAL CONSTIPATION: A RANDOMIZED OPEN LABEL CLINICAL TRIAL

    Background: As a common disease in pediatrics, constipation has poses a high burden to the community. In this study, we aimed to investigate the efficacy of dry cupping therapy (an Eastern traditional manipulative therapy) in children with functional constipation. Materials and Methods: One hundred and twenty children (4-18 years old) diagnosed as functional constipation according to ROME III criteria were assigned to receive a traditional dry cupping protocol on the abdominal wall for 8 minutes every other day or standard laxative therapy (Polyethylene glycol (PEG) 40% solution without electrolyte), 0.4 g/kg once daily) for 4 weeks, in an open label randomized controlled clinical trial using a parallel design with a 1:1 allocation ratio. Patients were evaluated prior to and following 2, 4, 8 and 12 weeks of the intervention commencement in terms of the ROME III criteria for functional constipation. Results: There were no significant differences between the two arms regarding demographic and clinical basic characteristics. After two weeks of the intervention, there was a significant better result in most of the items of ROME III criteria of patients in PEG group. In contrast, after four weeks of the intervention the result was significantly better in the cupping group. There was no significant difference in the number of patients with constipation after 4 and 8 weeks of the follow-up period. Conclusion: This study showed that dry cupping of the abdominal wall, as a traditional manipulative therapy, can be as effective as standard laxative therapy in children with functional constipation

    Concomitant Transurethral and Transvaginal-Periurethral Injection of Autologous Adipose Derived Stem Cells for Treatment of Female Stress Urinary Incontinence: A Phase One Clinical Trial

    Stress urinary incontinence is a common medical problem among women. The urethral closure complex and/or the supportive mechanisms are responsible for incontinence in the majority of patients. Several surgical procedures with different degrees of invasiveness and outcomes have been reported to treat the problem. Although most of these procedures are reasonably effective, a general trend towards the study of natural and biocompatible tissues is emerging over popular synthetic materials. Here we report our experience of autologous adipose-derived stem cells transplantation into the periurethral region as a new method of stress urinary incontinence treatment. Ten women with symptoms of stress urinary incontinence were treated by injections of autologous adipose-derived stem cells into the periurethral region via transurethral and transvaginal approach under urethroscopic observation. This report presents the short-term outcome of the patients. The outcome measured by pad test results, ICIQ-SF scores, and Qmax. The mean age of the participants was 45.8±8.7 years. Urinary incontinence significantly decreased through the first two, 6 and 24 weeks after the injection therapy. The difference was significant in pad test results (P<0.001) and ICIQ-SF scores (P<0.001), especially comparing results between 2 and 6 weeks and among 6 and 24 weeks, but not for 2 and 6 weeks compared to each other. Surprisingly, Qmax showed improvement after the study period (means 32.6 vs. 35.7; P=0.002). This study showed that injection of the autologous adipose-derived stem cells to the periurethral region is a safe, yet short-term effective treatment option for stress urinary incontinence. Further studies with longer follow up are needed to confirm its long term efficacy

    Pneumorrhachis (PR), which involves the entrapment of air or gas within the spinal canal, is a rare clinical entity. The pathogenesis and etiology of this uncommon entity vary and may present a diagnostic challenge. Air in the spinal canal can be divided into primary and secondary PR, extra- or intradural PR and etiologically classified as iatrogenic, traumatic and non-traumatic. PR is typically asymptomatic but can be symptomatic–by itself or by its underlying pathology. The latter, although often severe, might be concealed and must be carefully examined to facilitate adequate patient treatment. Here we report a rare case of traumatic PR in a 28-year old man following a motor vehicle accident in northern Iran, who sustained paraplegia and sphincter dysfunction without any laceration and wound in the spinal area

    Otitis Media with Effusion in Children and the Impact of Risk Factors on Serum Cytokine Levels

    Introduction:To evaluate the role of allergic-type and infectious-type cytokines in children with chronic otitis media with effusion (OME) Materials and Methods:We investigated serum levels of interleukins (IL)-4, IL-5, and IL-13, along with interferon-gamma (IFN-γ) and tumor necrosis factor-alpha (TNF-α), by enzyme-linked immunosorbent assay (ELISA) in 35 children with OME and 28 healthy controls. Results:Children with OME had significantly higher levels of IL-5 in comparison with the control group, ranging from 1 pg/ml in cases to 0.04 pg/ml in controls (P=0.009). However, after adjusting for confounding variables, there was no significant difference in serum levels of IL-13, IL-4, IFN-γ, or TNF-α between the two groups (P=0.287, P=0.627, P=0.793, and P=0.217, respectively) Conclusions:The findings of this study suggest that in comparison with the control group, serum IL-5 levels were elevated in OME cases

    Comparison of the effects of the time of home-based cardiac rehabilitation program on the changes in cardiometabolic risk factors in patients with phase-IV myocardial infarction: A randomized controlled trial

    BACKGROUND: It seems that the time of performing cardiac rehabilitation is important in determining the risk of cardiac complications in patients with myocardial infarction (MI). The present study aimed to investigate the effects of a home-based cardiac rehabilitation program (HCRP) conducted in either the morning or evening on cardiometabolic risk factors in phase IV (maintenance) MI patients. METHODS: In this randomized controlled clinical trial, 80 patients with MI were divided into 2 groups of intervention and control (40 individuals per group). Patients in each group were categorized into morning and evening subgroups (20 individuals per subgroup). The therapeutic regimen in the intervention group included HCRP, routine medications, and exercise and walking programs for 8 weeks. Patients in the control group received routine treatments for 8 weeks. Cardiovascular risk factors comprising of cardiac troponin I (cTnI), mean platelet volume (MPV), C-reactive protein (CRP), and cardiometabolic indicators including cholesterol (Cho), high-density lipoprotein (HDL), low-density lipoprotein (LDL), triglyceride (TG), and the maximum rate of oxygen consumption (VO2 max) were measured for all patients before and after the intervention. RESULTS: Our results showed significant reductions in Cho, TG, HDL, LDL, VO2 max, CRP, and MPV (P < 0.05) in the group performing HCRP in the evening compared with the morning group. CONCLUSION: Performing HCRP in the evening, compared with morning, can be significantly more effective in improving the levels of cardiometabolic risk factors in patients with MI. Therefore, it is recommended that rehabilitation programs be implemented in these patients in evening shifts

    Crescentic Glomerulonephritis in A Child With Tea- Colored Urine: A Case Report: Crescentic glomerulonephritis

    A 12-year-old girl was admitted with tea-colored urine for 24 hours. She had normal bloodpressure and no edema with a rapid and progressive increase in serum creatinine, so one session of hemodialysis was done. The condition got worse by a thrombus formation in the heart. &nbsp;Urinalysis showed proteinuria and hematuria. The kidney biopsy revealed crescentic glomerulonephritis. Treatment with 3 pulses of methylprednisolone and cyclophosphamide, and antithrombotic drugs were given with rapid recovery to normal serum creatinine and decrease in proteinuria. C3 was normalized after 2 months, and the clot disappeared, while hematuria and proteinuria continued as 1+ after 5 months. Due to the continuous proteinuria, Myfortic was continued for one year. Then, after stopping Myfortic, proteinuria did not return, and only microscopic hematuria continued.In conclusion, severe crescentic glomerulonephritis may have minimal symptoms and becomplicated by heart thrombosis but with excellent response to medical therapy
