    Gartland type III supracondylar humerus fractures: outcome and complications as related to operative timing and pin configuration

    Purpose Supracondylar fractures of the humerus are the most common fracture of theelbow in children. The pur- pose of this study was to evaluate, in terms of outcomes andcomplications, Gartland type III pediatric supracondy- lar humerus fractures treated at apediatric level-one trauma center over a 7-year period, specifically addressing the- impact of time to surgery on the incidence of complications and conversion to openreduction. Methods We retrospectively reviewed 297 pediatric patients that sustained a closedGartland type III supra- condylar humerus fracture treated between December 2004 andDecember 2011. The time to the operating room was calculated from the medical recordsfor each patient. The outcome measures evaluated were operative time, conver- sion toopen procedure, and perioperative and postoperative complications. Results In our study, there were 30 complications in 25 children (8.4%). Conversion to open reduction occurred in 28 children (9.4%). The time from the emergency depart- ment to the operating room was not significantly correlated with increased complications, increased operative time, or conversion to open reduction ( p > 0.05). Crossed pinning resulted in an increased risk of overall complications [odds ratio (OR) = 2.6] and iatrogenic nerve injuries (OR = 9.3). Complications also occurred more commonly in boys (OR = 3.3) and in older patients ( p = 0.0069) Conclusions We found no significant correlation between the time to surgery andcomplications, operative time, or need for open reduction. These findings support thetrend of treating Gartland type III supracondylar humerus fractures in a less urgentmanner. In addition, our study supports the concept that cross pinning leads to morecomplications than lateral pinning, including an 8-fold increase in iatrogenic nerveinjur

    Hydrographic and nutrient data from R/V Knorr cruise 73, leg 2 : February to March, 1978 : off the coast of Peru

    In February and March of 1978 a major cruise was undertaken on the R/V KNORR off the Peruvian coast near 15°S in order to investigate the organic biogeochemical processes associated with upwelling areas. The purpose of this report is to collate the large amount of hydrographic, nutrient, and plankton data generated from various investigators on this cruise and use the report as a standard for the cruise participants.     Federated Learning Based Proactive Content Caching in Edge Computing

    This is the author accepted manuscript. the final version is available from IEEE via the DOI in this recordContent caching is a promising approach in edge computing to cope with the explosive growth of mobile data on 5G networks, where contents are typically placed on local caches for fast and repetitive data access. Due to the capacity limit of caches, it is essential to predict the popularity of files and cache those popular ones. However, the fluctuated popularity of files makes the prediction a highly challenging task. To tackle this challenge, many recent works propose learning based approaches which gather the users' data centrally for training, but they bring a significant issue: users may not trust the central server and thus hesitate to upload their private data. In order to address this issue, we propose a Federated learning based Proactive Content Caching (FPCC) scheme, which does not require to gather users' data centrally for training. The FPCC is based on a hierarchical architecture in which the server aggregates the users' updates using federated averaging, and each user performs training on its local data using hybrid filtering on stacked autoencoders.     Development of a Coupled Ocean–Atmosphere–Wave–Sediment Transport (COAWST) Modeling System

The current scientific understanding of coastal sediment transport and geology suggests that examining coastal processes at regional scales can lead to significant insight into how the coastal zone evolves. To better identify the significant processes affecting our coastlines and how those processes create coastal change we developed a Coupled Ocean–Atmosphere–Wave–Sediment Transport (COAWST) Modeling System, which is comprised of the Model Coupling Toolkit to exchange data fields between the ocean model ROMS, the atmosphere model WRF, the wave model SWAN, and the sediment capabilities of the Community Sediment Transport Model. This formulation builds upon previous developments by coupling the atmospheric model to the ocean and wave models, providing one-way grid refinement in the ocean model, one-way grid refinement in the wave model, and coupling on refined levels. Herein we describe the modeling components and the data fields exchanged. The modeling system is used to identify model sensitivity by exchanging prognostic variable fields between different model components during an application to simulate Hurricane Isabel during September 2003. Results identify that hurricane intensity is extremely sensitive to sea surface temperature. Intensity is reduced when coupled to the ocean model although the coupling provides a more realistic simulation of the sea surface temperature. Coupling of the ocean to the atmosphere also results in decreased boundary layer stress and coupling of the waves to the atmosphere results in increased bottom stress. Wave results are sensitive to both ocean and atmospheric coupling due to wave–current interactions with the ocean and wave growth from the atmosphere wind stress.     Bibliometric analysis of authorship trends and collaboration dynamics over the past three decades of BONE's publication history

    The existence of a gender gap in academia has been a hotly debated topic over the past several decades. It has been argued that due to the gender gap, it is more difficult for women to obtain higher positions. Manuscripts serve as an important measurement of one's accomplishments within a particular field of academia. Here, we analyzed, over the past 3 decades, authorship and other trends in manuscripts published in BONE, one of the premier journals in the field of bone and mineral metabolism. For this study, one complete year of manuscripts was evaluated (e.g. 1985, 1995, 2005, 2015) for each decade. A bibliometric analysis was then performed of authorship trends for those manuscripts. Analyzed fields included: average number of authors per manuscript, numerical position of the corresponding author, number of institutions collaborating on each manuscript, number of countries involved with each manuscript, number of references, and number of citations per manuscript. Each of these fields increased significantly over the 30-year time frame (p < 10− 6). The gender of both the first and corresponding authors was identified and analyzed over time and by region. There was a significant increase in the percentage of female first authors from 23.4% in 1985 to 47.8% in 2015 (p = 0.001). The percentage of female corresponding authors also increased from 21.2% in 1985 to 35.4% in 2015 although it was not significant (p = 0.07). With such a substantial emphasis being placed on publishing in academic medicine, it is crucial to comprehend the changes in publishing characteristics over time and geographical region. These findings highlight authorship trends in BONE over time as well as by region. Importantly, these findings also highlight where challenges still exist

    Favorable outcome of early treatment of new onset child and adolescent migraine-implications for disease modification.

    There is evidence that the prevalence of migraine in children and adolescents may be increasing. Current theories of migraine pathophysiology in adults suggest activation of central cortical and brainstem pathways in conjunction with the peripheral trigeminovascular system, which ultimately results in release of neuropeptides, facilitation of central pain pathways, neurogenic inflammation surrounding peripheral vessels, and vasodilatation. Although several risk factors for frequent episodic, chronic, and refractory migraine have been identified, the causes of migraine progression are not known. Migraine pathophysiology has not been fully evaluated in children. In this review, we will first discuss the evidence that early therapeutic interventions in the child or adolescent new onset migraineur, may halt or limit progression and disability. We will then review the evidence suggesting that many adults with chronic or refractory migraine developed their migraine as children or adolescents and may not have been treated adequately with migraine-specific therapy. Finally, we will show that early, appropriate and optimal treatment of migraine during childhood and adolescence may result in disease modification and prevent progression of this disease
