    Parametric mapping of contrasted ovarian transvaginal sonography.

    The purpose of this study was to assess the accuracy of parametric analysis of transvaginal contrast-enhanced ultrasound (TV-CEUS) for distinguishing benign versus malignant ovarian masses. A total of 48 ovarian masses (37 benign and 11 borderline/malignant) were examined with TV-CEUS (Definity; Lantheus, North Billerica, MA; Philips iU22; Philips Medical Systems, Bothell, WA). Parametric images were created offline with a quantification software (Bracco Suisse SA, Geneva, Switzerland) with map color scales adjusted such that abnormal hemodynamics were represented by the color red and the presence of any red color could be used to differentiate benign and malignant tumors. Using these map color scales, low values of the perfusion parameter were coded in blue, and intermediate values of the perfusion parameter were coded in yellow. Additionally, for each individual color (red, blue, or yellow), a darker shade of that color indicated a higher intensity value. Our study found that the parametric mapping method was considerably more sensitive than standard region of interest (ROI) analysis for the detection of malignant tumors but was also less specific than standard ROI analysis. Parametric mapping allows for stricter cutoff criteria, as hemodynamics are visualized on a finer scale than ROI analyses, and as such, parametric maps are a useful addition to TV-CEUS analysis by allowing ROIs to be limited to areas of the highest malignant potential

    Cardiovascular Risk Behavior among Sedentary Female Smokers and Smoking Cessation Outcomes

    Background We examined female sedentary smokers\u27 additional cardiovascular disease (CVD) risk behaviors and their associations to smoking cessation. Methods This study was part of a randomized controlled trial testing the effectiveness of exercise and nicotine gum in smoking cessation. Included in the analyses were 148 participants. Dietary habits and alcohol consumption were measured as additional CVD risk behaviors. High-fat diet and heavy alcohol use were considered those risk behaviors. Nicotine dependence, length of the longest quit attempt, depressive symptoms, self-efficacy, and education were examined as other baseline variables. Abstinence from tobacco was recorded through 12 months. Results Diet was related to depressive symptoms at baseline. Alcohol use was related to nicotine dependence and education level. Heavy alcohol use alone and accumulation of two added risk behaviors predicted poorer smoking cessation outcome. Although diet alone was not associated with cessation outcome the high-fat diet interacted with depressive symptoms, such that the depressed women with high-fat diet were significantly more likely to relapse in their quit attempt compared to other subgroups. Conclusion Non-moderate alcohol use alone and accumulation of multiple CVD risk behaviors seem to be associated with lower success in smoking cessation

    This article presents a framework, transition management, for managing complex societal systems. The principal contribution of this article is to articulate the relationship between transition management and complex systems theory. A better understanding of the dynamics of complex, adaptive systems provides insight into the opportunities, limitations, and conditions under which it is possible to influence such systems. Transition management is based on key notions of complex systems theory, such as variation and selection, emergence, coevolution, and self-organization. It involves a cyclical process of phases at various scale levels: stimulating niche development at the micro level, finding new attractors at the macro level by developing a sustainability vision, creating diversity by setting out experiments, and selecting successful experiments that can be scaled up

    Doxycycline for Malaria Chemoprophylaxis and Treatment: Report from the CDC Expert Meeting on Malaria Chemoprophylaxis

    Doxycycline, a synthetically derived tetracycline, is a partially efficacious causal prophylactic (liver stage of Plasmodium) drug and a slow acting blood schizontocidal agent highly effective for the prevention of malaria. When used in conjunction with a fast acting schizontocidal agent, it is also highly effective for malaria treatment. Doxycycline is especially useful as a prophylaxis in areas with chloroquine and multidrug-resistant Plasmodium falciparum malaria. Although not recommended for pregnant women and children < 8 years of age, severe adverse events are rarely reported for doxycycline. This report examines the evidence behind current recommendations for the use of doxycycline for malaria and summarizes the available literature on its safety and tolerability

    Self-oligomerization regulates stability of survival motor neuron protein isoforms by sequestering an SCF<sup>Slmb</sup> degron

    Spinal muscular atrophy (SMA) is caused by homozygous mutations in human SMN1. Expression of a duplicate gene (SMN2) primarily results in skipping of exon 7 and production of an unstable protein isoform, SMNΔ7. Although SMN2 exon skipping is the principal contributor to SMA severity, mechanisms governing stability of survival motor neuron (SMN) isoforms are poorly understood. We used a Drosophila model system and label-free proteomics to identify the SCFSlmb ubiquitin E3 ligase complex as a novel SMN binding partner. SCFSlmb interacts with a phosphor degron embedded within the human and fruitfly SMN YG-box oligomerization domains. Substitution of a conserved serine (S270A) interferes with SCFSlmb binding and stabilizes SMNΔ7. SMA-causing missense mutations that block multimerization of full-length SMN are also stabilized in the degron mutant background. Overexpression of SMNΔ7S270A, but not wild-type (WT) SMNΔ7, provides a protective effect in SMA model mice and human motor neuron cell culture systems. Our findings support a model wherein the degron is exposed when SMN is monomeric and sequestered when SMN forms higher-order multimers

    Strong symmetry of shocks allows for the formation of a monetary union with low costs due to losing monetary sovereignty. I employ vector autoregression to identify structural shocks and study their symmetry through time. I find that the underlying structural shocks have not changed significantly and remain rather asymmetric, particularly demand shocks. Copyright (c) 2009 The Author(s).     Synthetic computed tomography based dose calculation in prostate cancer patients with hip prostheses for magnetic resonance imaging-only radiotherapy

    Background and purpose: Metallic hip prostheses cause substantial artefacts in both computed tomography (CT) and magnetic resonance (MR) images used in radiotherapy treatment planning (RTP) for prostate cancer patients. The aim of this study was to evaluate the dose calculation accuracy of a synthetic CT (sCT) generation workflow and the improvement in implant visibility using metal artefact reduction sequences. Materials and methods: The study included 23 patients with prostate cancer who had hip prostheses, of which 10 patients had bilateral hip implants. An in-house protocol was applied to create sCT images for dose calculation comparison. The study compared prostheses volumes and resulting avoidance sectors against planning target volume (PTV) dose uniformity and organs at risk (OAR) sparing. Results: Median PTV dose difference between sCT and CT-based dose calculation among all patients was 0.1 % (−0.4 to 0.4%) (median(range)). Bladder and rectum differences (V50Gy) were 0.2 % (−0.3 to 1.1%) and 0.1 % (−0.9 to 0.5%). The median 3D local gamma pass rate for partial arc cases using a Dixon MR sequence was Γ20%2mm/2% = 99.9%. For the bilateral full arc cases, using a metal artefact reconstruction sequence, the pass rate was Γ20%2mm/2% = 99.0%. Conclusions: An in-house protocol for generating sCT images for dose calculation provided clinically feasible dose calculation accuracy for prostate cancer patients with hip implants. PTV median dose difference for uni- and bilateral patients with avoidance sectors remained <0.4%. The Outphase images enhanced implant visibility resulting in smaller avoidance sectors, better OAR sparing, and improved PTV uniformity
