    Treatment with tocilizumab or corticosteroids for COVID-19 patients with hyperinflammatory state: a multicentre cohort study (SAM-COVID-19)

    Objectives: The objective of this study was to estimate the association between tocilizumab or corticosteroids and the risk of intubation or death in patients with coronavirus disease 19 (COVID-19) with a hyperinflammatory state according to clinical and laboratory parameters. Methods: A cohort study was performed in 60 Spanish hospitals including 778 patients with COVID-19 and clinical and laboratory data indicative of a hyperinflammatory state. Treatment was mainly with tocilizumab, an intermediate-high dose of corticosteroids (IHDC), a pulse dose of corticosteroids (PDC), combination therapy, or no treatment. Primary outcome was intubation or death; follow-up was 21 days. Propensity score-adjusted estimations using Cox regression (logistic regression if needed) were calculated. Propensity scores were used as confounders, matching variables and for the inverse probability of treatment weights (IPTWs). Results: In all, 88, 117, 78 and 151 patients treated with tocilizumab, IHDC, PDC, and combination therapy, respectively, were compared with 344 untreated patients. The primary endpoint occurred in 10 (11.4%), 27 (23.1%), 12 (15.4%), 40 (25.6%) and 69 (21.1%), respectively. The IPTW-based hazard ratios (odds ratio for combination therapy) for the primary endpoint were 0.32 (95%CI 0.22-0.47; p < 0.001) for tocilizumab, 0.82 (0.71-1.30; p 0.82) for IHDC, 0.61 (0.43-0.86; p 0.006) for PDC, and 1.17 (0.86-1.58; p 0.30) for combination therapy. Other applications of the propensity score provided similar results, but were not significant for PDC. Tocilizumab was also associated with lower hazard of death alone in IPTW analysis (0.07; 0.02-0.17; p < 0.001).     El Proyecto es continuación de otros concedidos en anteriores convocatorias de Innova-Docencia a miembros del Grupo de Investigación UCM, referencia nº 970603: ISYFDI (Innovación en la selección, formación y práctica de los docentes investigadores). Destacándose aquí los siguientes: Determinación de indicadores de buenas prácticas docentes en la universidad para la inclusión del alumnado con discapacidad o enfermedad crónica, en continuidad con el bachillerato, desarrollado en el curso 2015/2016 y; Necesidades y oportunidades de Aprendizaje-Servicio universitario para el alumnado con necesidades de apoyo educativo, en el tránsito entre el bachillerato y la universidad, desarrollado en el curso 2016/2017.El Proyecto de innovación parte de la necesidad, del profesorado que integra el Equipo, profesores de la Facultad de Educación-Centro de Formación del Profesorado- de la Universidad Complutense de Madrid, de buscar procedimientos, viables y prácticos, para evaluar en el alumnado universitario el grado de adquisición de la competencia para el compromiso con la inclusión y la equidad, puesto que, desde ella, se puede apreciar también las actitudes hacia las personas con necesidades específicas de apoyo, no curricular. El objetivo es elaborar un instrumento para la evaluación de la competencia transversal: compromiso con la inclusión y la equidad, para lo que se elaboró una rúbrica de evaluación, organizada en tres dimensiones: 1) Equidad; 2) Pertenencia y, 3) Participación; y fue validada, utilizándose la técnica Delphi, por 10 expertos quienes ofrecieron sus opiniones sobre las dimensiones, descriptores e indicadores de ejecución de la rúbrica. Entre las conclusiones se destaca la sensibilidad positiva que el profesorado manifestadas en las acciones para la difusión del Proyecto.The Innovation Project is based on the need, of the faculty that integrates the Team, professors of the Faculty of Education-Teacher Training Center of the Complutense University of Madrid, to look for feasible and practical procedures to evaluate in the university student the degree of acquisition of competence for the commitment to inclusion and equity, since, from it, you can also appreciate the attitudes towards people with specific support needs, not curricular. The objective is to elaborate an instrument for the evaluation of transversal competence: commitment to inclusion and equity, for which an evaluation rubric was elaborated, organized in three dimensions: 1) Equity; 2) Membership and, 3) Participation; and it was validated, using the Delphi technique, by 10 experts who offered their opinions on the dimensions, descriptors and indicators of execution of the rubric.
